Lung Cancer Presented as Painful Swelling of Lower Legs (하지의 통증 및 부종을 주소로 내원한 환자에서 발견된 폐암1예)

  • Trousseau's syndrome comsists of migratory thrombophlebitis and thromboembolic disorders of the venous and arterial systems in a malignancy or occult cancer. The overall incidence has been reported to vary from 1 to 11%. Pancreatic, lung, prostate, and stomach cancer is associated with the greatest risk of thromboembolic events. We encountered a 49-year-old man who presented with painful swelling of his lower legs. The chest radiograph showed increased opacity of the Left middle lung fields and Doppler sonography showed a thrombus in the left superficial femoral vein. Chest Computed Tomography showed a 5cm sized left hilar mass invading the pericardium with lymphadenopathy. The bronchoscope biopsy demonstrated an adenocarcinoma of the lung. Platinum based chemotherapy and anticoagulant therapy with warfarin was carried out. The patient was later discharged with an improvement in the painful swelling of his lower legs.

A Clinical Review on 315 Cases of Major Salivary Gland Tumor (주 타액선 종양 315예의 임상적 고찰)

  • Objectives: Major salivary gland tumor mainly develops in the parotid gland and pleomorphic adenoma is a large percentage. The aim of this study is to get clinicopathologic characteristics of overall major salivary gland tumors and suggestions regarding surgical management through collective review of 315 cases. Materials and Methods: This is a clinicopathologic review of 315 cases of major salivary gland tumor who were treated surgically at Department of Surgery, Head and Neck Clinic, Pusan Paik Hospital, Inje University during the period of 18 years from 1980 to 1997. Analysis was performed regarding the incidence, classification, surgery and its complications, and survival rate of salivary gland cancer. Results : 1) Parotid gland was the most prevalent site of salivary gland tumor(78%) and submandibular gland(21%) was next in order. Benign tumors were 257cases(81%) and malignant tumors were 58 cases(19%). 2) Male to female sex ratio was 1:1.2, the most prevalent age group was 3rd decade and the second group was 4th decade. 3) Histopathologically, the most common benign salivary gland tumor was pleomorphic adenoma. Warthin's tumor was next common. Among the malignant tumors, mucoepidermoid carcinoma was most common, and the next were adenoid cystic carcinoma and acinic cell carcinoma. 4) In pleomorphic adenoma, superficial parotidectomy was performed in 129 cases, and extracapsular tumorectomy was performed in 3 cases. In non-pleomorphic benign tumor, tumorectomy was performed in 21 cases. In 40 cases of deep lobe tumor, total parotidectomy was performed in only 2 cases and deep parotidectomy was performed in 38 cases. 5) Surgical complications were facial nerve injury 19 cases, Frey syndrome 13 cases, and salivary fistula 3 cases. 6) Overall 5-year survival rate of salivary gland cancer was 63%. Conclusion: Postoperative recurrence rate is low in benign tumor, but high in cancer of salivary gland tumor. Surgical procedure should not be aggressive in benign tumor, especially in parotid tumor, but should be aggressive in malignant salivary gland tumors.

Dose coefficients of mesh-type ICRP reference computational phantoms for idealized external exposures of photons and electrons

  • In the present study, we established a comprehensive dataset of dose coefficients (DCs) of the new meshtype ICRP reference computational phantoms (MRCPs) for idealized external exposures of photons and electrons with the Geant4 code. Subsequently, the DCs for the nine organs/tissues, calculated for their thin radiosensitive target regions, were compared with the values calculated by averaging the absorbed doses over the entire organ/tissue regions to observe the influence of the thin sensitive regions on dose calculations. The result showed that the influences for both photons and electrons were generally insignificant for the majority of organs/tissues, but very large for the skin and eye lens, especially for electrons. Furthermore, the large influence for the skin eventually affected the effective dose calculations for electrons. The DCs of the MRCPs also were compared with the current ICRP-116 values produced with the current ICRP-110 reference phantoms. The result showed that the DCs for the majority of organs/ tissues and effective dose were generally similar to the ICRP-116 values for photons, except for very low energies; however, for electrons, significant differences from the ICRP-116 values were found in the DCs, particularly for superficial organs/tissues and skeletal tissues, and also for effective dose.

The role of bladder sonography in patients with gross hematuria or microscopic hematuria and follow-up of patients with superficial bladder cancer (방광종물에 대한 방광초음파검사의 진단적 유용성)

  • Ultrasonography is used to examine gross or microscopic hematuria without side effects. It is one of the methods of diagnosing bladder lesions, but in some cases, the lesions are not found. We attempted to identify the problems during the ultrasonic examination by analyzing the symptoms, location of lesion, and medical history of urothelial cancer for cases of undetected bladder lesions. Thirty-three patients who underwent transurethral resection of a bladder tumor from January 1 to May 4, 2018 in one hospital were enrolled in this study. Patients who underwent preoperative ultrasonography and cystoscopy were treated. Ultrasonography did not detect bladder lesions in five patients. The size of the lesion was 0.5~2.5 cm in various locations, such as the side, front, and so on. Ultrasonic examination requires more attention if there is gross hematuria or a history of urothelial cancer, and it is necessary to detect recurrence by conducting cystoscopy at the same time, especially when there are lesions on the anterior wall of the bladder.

USING OF THE TEMPORALIS MUSCLE AND FASCIA FLAP FOR MAXILLA RECONSTURCTION (상악 결손부 수복을 위한 측두피판의 이용)

  • The functional deformity following removal of the maxilla is considerable, especially following maxilla cancer excision. Rehabilitation of oral and maxillofacial region used to be very difficult with prosthesis or traditional flaps. Temporal muscle and fascia flaps have been described for immediate reconstruction following hemi-maxillectomy, but not total maxillectomy. The muscle and superficial fascia of the temporal area differ in their physical characteristics, vascular supply and clinical applications. Both can be employed independently or simultaneously as regional flaps in the reconstruction of a variety of complex craniofacial defects. Four case is presented in which 3 case maxilla cancer, one case non-union after maxilla fracture, and all case showed successful use of this flap. Only one patient developed partial necrosis of the flap ; significant necrosis did not occur in any other patient. This present paper reviews the anatomy, surgical technique and utilisation of temporal muscle and fascia flaps.

  • Temporoparietal fascial flap (TPFF) has been used in the reconstruction of a broad spectrum of complex defect of head & neck it can be used as pedicled flap or free flap. TPFF is extensively is good for reconstruction of auricular defects because it is fascial flap with ease of covering irregular surface. TPFF is supplied by the superficial temporal artery & vein and innervated by zygomatico-temporal branch of facial nerve and auriculotemporal nerve. The flap ranges from 2-4mm in thickness and can be harvested up to 17x14cm wide, it can include calvarian bone and enables primary closure of donor site. We carried out reconstruction of oral cavity defects by means of TPFF with a satisfactory result in two cases of oral cavity cancer.

Melanoma Incidence Mortality Rates and Clinico-Pathological Types in the Siberian Area of the Russian Federation

  • Russian rates for melanoma incidence and mortality are relatively low as compared to some other white populations but the tumor is of increasing importance. In this paper, data are based on a retrospective descriptive analysis of melanoma epidemiology and clinicopathological characteristics in Krasnoyarsk Territory belonging to the Siberian Federal District of the Russian Federation. The age-adjusted incidence and mortality rates for the period 1996-2009 were determined with subsequent retrospective analysis of clinicopathological data of 103 primary melanoma cases. Our results showed that incidence and mortality rates in the region under consideration match the Russian national trends and correspond to epidemiological data of the countries of Eastern Europe. Stratification of melanoma cases by age, sex, clinicopathological state and localization revealed a prevalence of lesions on the trunk and lower extremities. Most melanomas diagnosed were of superficial spreading type and the third Clark's level of tumor invasion and stage II according to AJCC. In spite of comparatively low rates of incidence and mortality the trend to increase of melanoma cases in the region under consideration obviously calls for more attention and further investigation.

Parotid Gland Tumours in Turkish Population: Analysis of 165 Patients

  • Background: Retrospective data on 165 patients who presented with a parotid mass and underwent surgery in our clinics during 2000-2009 were examined. The obtained data (demographic data, surgical procedures, histopathological diagnoses) were compared to similar studies to make contributions to the literature. Materials and Methods: Patients were classified according to their histopathological diagnosis. Surgical procedures and patient follow-up were clarified. The results are presented as means and standard deviations. Results: Of the 165 masses, 134 (81.3%) were benign and 31 (18.7%) were malignant. Pleomorphic adenoma was the most common benign tumour (79 patients, 59%). Lymphoma and adenoid cystic carcinoma were equally common and were the most common malignant parotid gland tumours (both 6 patients, 19.3%). The most frequent surgical procedure was superficial parotidectomy (92 patients, 55.7%), and the most commonly encountered surgical complication was facial paralysis (12 patients, 7.2%). Conclusions: Our data are generally in line with the literature but lymphoma was more common than in most previous reports. Although the number of cases was low, the high incidence of parotid gland lymphoma was remarkable.

The Clinical Outcomes of Photodynamic Therapy in Early Lung Cancer Patients (조기 폐암환자에서 광역동치료의 효과)

  • Background: Photodynamic therapy (PDT) is effective in managing small superficial early lung cancer patients who were deemed nonsurgical candidates. However, we do not have any previous report on the usefulness of PDT in early lung cancer in South Korea. Thus we report here our experience of PDT in early lung cancer patients. Methods: 10 patients who underwent PDT for managing early lung cancer between June 2006 and July 2010 were analyzed. PDT was carried out 48 hours after photosensitizer injection. Re-bronchoscopy was carried out 48 hours after PDT in order to remove a necrotic tissue from the PDT site. For evaluation of PDT response, bronchoscopy and chest computed tomography (CT) were performed after 3 months. Results: The median age of patients was 69 (49~77) and all patients were male. The smoking history of patients was 48 (20~75) pack-year and the median follow up of patients was 25 (11~52) months. Complete remission was observed in 10 patients and the recurrence of lung cancer was observed in 3 patients. Out of 10 patients, 3 patients died (one case of lung cancer progression and two cases of pneumonia). Conclusion: The PDT is a safe and effective treatment in early lung cancer patients who are not suitable for surgical resection. The PDT in clinical practice is an attractive option in the treatment of early lung cancer.

Expression Pattern of Caspase 2 in Korean Gastric Cancers (한국인 위암에서 Caspase 2 단백 발현 양상)

  • Purpose: Caspase 2, a member of the family of ICE-like proteases, is activated by the Fas pathway and induces apoptosis by triggering the caspase cascade. The purpose of this study was to determine whether the expression pattern of caspase 2 might be associated with gastric cancer development and if so, to determine to which pathologic parameter it is linked. Materials and Methods: For the construction of the gastric cancer tissue microarray, 78 paraffin-embedded tissues containing gastric cancer areas were cored 3 times and transferred to the recipient master block. The expression pattern of caspase 2 was examined on tissue microarray slides by using immunohistochemistry and was compared with pathologic parameters, including histologic type, depth of invasion, lymph node metastasis, and peritoneal dissemination. Results: Caspase 2 was expressed on superficial and foveolar epithelial cells and lymphocytes in the gastric mucosa, mainly in cytoplasm. We found loss of caspase 2 expression in 41 ($52.6\%$) of the 78 gastric cancer tissues. Statistically, histologic type and other pathologic parameters were not related with loss of caspase 2 expression Conclusion: Our findings provide enough evidence that loss of caspase 2 expression may contribute to the development of Korean gastric cancer and that it might be one of the possible escape mechanisms from apoptosis in gastric cancer.
