Central pain is defined as pain associated with lesions of the central nervous system, and is among the most intractable of chronic pain syndromes. A 47 year-old-female, who had right arm and shoulder pain, was diagnosed with syringomyelia of the Arnold Chiari malformation type I and received foramen magnum decompression and a syringo-subarachnoid shunt. After the operation, the evoked pain was improved, but she complained of a continuous burning pain, coupled with cold and tactile allodynia. This symptom failed to fully subside on administration of oral medicine; therefore, brachial plexus block was performed, which relieved her pain transiently. Through repeated trials, a gradual decrease in the pain intensity and frequency was found. However, the way in which brachial plexus block improves spinal central pain is not completely known.
대상포진후 신경통으로 체성신경 차단과 함께 성상신경절 차단을 반복적으로 받아오던 64세 여자환자에서 열세번째 우측 성상신경절 차단후 반대측인 좌측에 호너씨 증후군이 발생되었다. 그 원인은 명확하지 않지만 시술 도중 환자머리의 좌측회전으로 인한 해부학적 위치변화와 술자의 부정확한 지표선정 및 주사바늘의 안쪽방향등으로 약제가 중앙선을 넘어 주입된 것으로 추정할수 있다.
In this paper, low-power design and implementation techniques for DWT(Discrete Wavelet Transform) of the JPEG2000 compression are proposed. In DWT block of the JPEG2000, linear phase 9 tap and 7 tap filters are used. For low-power implementation of those filters, processor technique for DA(Distributed Arithmetic) filter and minimization technique for number of addition in CSD(Canonic Signed Digit) filter are utilized. Proposed filter structure consists of 3 blocks. In the first CSD coefficient block, every possible 4 bit CSD coefficients are calculated and stored. In second processor block, multiplication is done by MUX and addition processor in terms of the binary values of filter coefficient. Finally, in third block, multiplied values are output and stored in flip-flop train. For comparison of the implementation area and power dissipation, proposed and conventional structures are implemented by using Verilog-HDL coding. In simulation, it is shown that 53.1% of the implementation area can be reduced comparison with those of the conventional structure.
A poly(ethylene oxide)-b-poly(L-lactide) diblock copolymer (PEO-b-PLLA) is characterized by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) and a block length distribution map is constructed. Although the MALDI- TOF mass spectrum of PEO-b-PLLA is very complicated, most of the polymer species were identified by isolating the overlapped isotope patterns and by fitting the overlapped peaks to the Schulz-Zimm distribution function. Reconstructed MALDI-TOF MS spectrum was nearly identical to the measured spectrum and this method shows its potential to be developed as an easy and fast analysis method of low molecular weight block copolymers.
pH-sensitive block copolymers were synthesized by coupling reaction of sulfamethazine and amphiphilic diblock copolymer, and their micellization-demicellization behavior was investigated. Sulfamethazine (SM), a derivative of sulfonamide, was introduced as a pH responsive moiety while methoxy poly(ethylene glycol)poly(D,L-lactide) (MPEG-PDLLA) and methoxy poly(ethylene glycol)-poly($D,L-lactide-co-{\varepsilon}-caprolactone$) (MPEG-PCLA) were used as biodegradable amphiphilic diblock copolymers. After the sulfamethazine was carboxylated by the reaction with succinic anhydride, the diblock copolymer was conjugated with sulfamethazine by coupling reaction in the presence of DCC. The critical micelle concentration (CMC) and mean diameter of the micelles were examined at various pH conditions through fluorescence spectroscopy, dynamic light scattering and transmission electron microscopy. For MPEG-PDLLA-SM and MPEG-PCLA-SM solutions, the pH-dependent micellization-demicellization was achieved within a narrow pH band, which was not observed in the MPEG-PDLLA and MPEG-PCLA solutions. The micelle showed a spherical morphology and had a very narrow size distribution. This pH-sensitive block copolymer shows potential as a site-targeted drug carrier.
본 논문에서는 임의의 음성 압축 비트스트림의 구조를 분석하여 음성 신호의 부호화 방식을 추정 및 분류하는 기법을 제안한다. 저 비트율 전송 및 저장을 위하여 다양한 보코더 방식의 음성 압축 기법이 개발되었는데, 이들은 블록 구조를 반드시 포함하고 있다. 각 부호화 방식을 구분하는데 있어, 본 논문에서는 Measure of Inter-Block Correlation (MIBC)를 이용하여 블록 구조의 유무 및 신호 블록의 길이를 파악하고, 블록 길이가 동일한 부호화 방식의 경우 각 부호화 방식마다 압축 스트림 내의 각 비트 위치별로 상관도 분포가 다르다는 점을 이용하여 해당 부호화 방식을 정확하게 추정하는 기법을 제안한다. 실험 결과 제안한 비트스트림 분석 기법은 다양한 음성 신호의 종류, 음성 신호의 길이 및 잡음 환경에 강인한 검출 능력을 나타냄을 보인다.
Objective: To propose a three-dimensional (3D) method for evaluating temporomandibular joint (TMJ) changes during Twin-block treatment. Methods: Seventeen patients with Class II division 1 malocclusion treated using Twin-block and nine untreated patients with a similar malocclusion were included in this research. We collected their cone beam computed tomography (CBCT) data from before and 8 months after treatment. Segmentations were constructed using ITK-SNAP. Condylar volume and superficial area were measured using 3D Slicer. The 3D landmarks were identified on CBCT images by using Dolphin software to assess the condylar positional relationship. 3D models of the mandible and glenoid fossa of the patients were constructed and registered via voxel-based superimposition using 3D Slicer. Thereafter, skeletal changes could be visualized using 3DMeshMetric in any direction of the superimposition on a color-coded map. All the superimpositions were measured using the same scale on the distance color-coded map, in which red color represents overgrowth and blue color represents resorption. Results: Significant differences were observed in condylar volume, superficial area, and condylar position in both groups after 8 months. Compared with the control group (CG), the Twin-block group exhibited more obvious condyle-fossa modifications and joint positional changes. Moreover, on the color-coded map, more obvious condyle-fossa modifications could be observed in the posterior and superior directions in the Twin-block group than in the CG. Conclusions: We successfully established a 3D method for measuring and evaluating TMJ changes caused by Twin-block treatment. The treatment produced a larger condylar size and caused condylar positional changes.
Stellate ganglion block(SGB) improves cutaneous blood flow of the head and neck region and upper extremity. For this reason, SGB has been performed in neural and circulatory disorders. But there is controversy on the cerebral blood flow regulation by sympathetic innervation. We investigated the hypothesis that cerebral blood flow could be affected by blocking ipsilateral sympathetic innervation of cerebral blood flow could be affected by blocking ipsilateral sympathetic innervation of cerebral vasculature. In 10 volunteers, the blood flow velocity and pulsatility index(PI) of middle cerebral artery(MCA) was measured using Transcranial Doppler Flowmeter, before and 15 minutes after SGB, at block side and opposite side. The blood flow velocity of MCA at block side was increased from $62.60{\pm}7.60$ cm/s to $72.80{\pm}8.01$ cm/s(P<0.01) and the PI at block side decreased from $0.75{\pm}0.12$ to $0.60{\pm}0.11$(P<0.05). But the blood flow velocity and PI at opposite side did not change. This study demonstrated that the cerebral blood flow could be increased by SGB, the preganglionic nerve fibers of which synapse with other cervical sympathetic ganglions.
이 논문은 다중참조 및 가변블록 움직임 추정의 연산량을 효율적으로 줄이기 위해 세 가지 참조영상 선택 방법들을 소개한다. 제안된 RSP (Reference Selection Pass) 방법은 참조영상 선택의 부가적인 연산을 최소화 할 수 있고 MFS (Modified Frame Selection) 방법은 참조영상 선택 과정 중 영상의 움직임을 고려하여 참조영상 선택 시 연산 횟수를 기존 방식에 비해 17% 감소시킨다. 또한 TPRFS (Two Pass Reference frame Selection) 방법은 H.264/AVC에서 요구하는 가변블록 움직임 추정을 지원하기 위한 부가적인 연산을 블록 크기에 따라 선택되는 참조영상의 특성을 이용하여 최소화 한다. 실험 결과 제안한 방식은 기존의 방식에 비해 화질의 열화 없이 50% 이상의 움직임 추정의 연산량을 감소시킬 수 있다. 또한 제안한 참조영상 선택 방법은 움직임 추정의 주된 연산인 블록정합 단계와 별개로 수행이 되기 때문에 기존의 어떠한 단일참조 고속 움직임 탐색 방법과도 같이 사용되어 효율적으로 다중참조 및 가변블록 움직임 추정 연산을 지원 할 수 있다.
기계학습의 정확도는 학습용 데이터의 양과 데이터의 품질에 많은 영향을 받는다. 기존의 웹을 기반으로 학습용 데이터를 수집하는 것은 실제 학습과 무관한 데이터가 수집 될 수 있는 위험성이 있으며 데이터의 투명성을 보장할 수가 없다. 본 논문에서는 블록체인구조에서 블록들이 직접 병렬적으로 데이터를 수집하게 하고 각 블록들이 수집한 데이터를 타 블록의 데이터와 비교하여 양질의 데이터만을 선별하는 방안을 제안한다. 제안하는 시스템은 각 블록들은 데이터를 서로 블록체인을 통해 공유하며 All-reduce 구조의 Parallel-SGD를 활용하여 다른 블록들의 데이터와 비교를 통해 양질의 데이터만을 선별하여 학습용 데이터셋을 구성할 수가 있다. 또한 본 논문에서는 제안한 구조의 성능을 확인하기 위해 실험을 통해 기존의 벤치마크용 데이터셋의 이미지를 활용하여 변조된 이미지 사이에서 원본 이미지만을 양질의 데이터로 판별함을 확인하였다.
