A case report of giant right ventricular myxoma (거대 우심실 점액종 1례 보)

  • Myxoma of the heart can now be successfully treated but the success is obviously dependent upon the correct diagnosis and treatment prior to development of catastrophic complications such as sudden death, embolism or cardiac failure. The right ventricular myxoma is very rare, and we treated successfully a case of giant right ventricular myxoma, sessile tumor originated from ventricular apex, weighed 175 gm. The tumor base was broad, about 3 cm x 3cm, and it was hardly adhesed to the tricuspid valvular structures, but could remove with the preservation of adequate residual ventricular chamber size and the maintenance of functional tricuspid valve anatomy. The postoperative course was excellent and uneventful.

A Literature Study of the Effect of Hirudo, Lumbricus, Scolopendra, and Scorpio on Apoplexy (중풍치료(中風治療)에서 충류약(蟲類藥)의 활용(活用)에 관(關)한 연구(硏究))

  • Apoplexy is a disease of a morbid condition manifested as sudden syncope, unconciousness, distortion of face, hemiplegia and dysphasia, usually seen in the middle-aged. The symptoms and signs before sudden onset are headache, dizziness, numbness of extremities, palpitation, etc. This study was performed to investige causes of disease, therapies and prescriptions by insect medicine through the successive medical literatures, recent chinese medical literatures and chinese medical journals. The results are as follows; 1. The treatment of apoplexy is divided into stage of attack and sequela. In stage of attack, the principal therapies of apoplexy are calming the liver, suppressed Yang, dissipate phlegm and elimination weatness. In sequela, the principal therapies of apoplexy are invigorating Qi, promote blood circulation and dredge collateral. 2. Insects medicine have more strong effect than herbal medicine, because apoplexy is a kind of critically desease. 3. Insects medicine is effective in a convalscent stage and sequela of apoplexy. The proper dosage for stage of attack is a small dose of insects medicine(about 2-4g), increse gradually. In convalscent stage, about 4g, in sequela, patients need a large dose of insects medicine(about 8g). 4. Hirudo used to remove stagnated blood and to disperse swelling for the treatment of severe cases of blood stasis, such as cerebral infarction, sequela of cerebrovascular accident, contused wounds. Lumbricus used to for the treatment of convulsions due to high fever, and for hemiplegia and hypertension. Scolopendra used to subdue the endogenous wind for the treatment of various kinds of tics, convulsions and tetanus, and it's character is strong because it will be effective Sthenia-Syndrome of apoplexy. Scorpio used to subdue the endogenous wind for the treatment of various kind of tics, convulsions, tetanus and sequela of cerebrovascular accident.

Case Report of Autonomic Dysreflexia in a Pressure Sore Patient (욕창 환자에서 자율신경성 반사부전증의 경험례)

  • Purpose: Plastic surgeons are responsible for the management of spinal cord injury patients with upper and lower extremity reconstruction, pressure sore, and wounds. Derailment of autonomic nervous systems caused by injury to the spinal cord may result in fatal autonomic dysreflexia. Autonomic dysreflexia is a syndrome of massive imbalance of reflex sympathetic discharge occurring in patients with spinal cord lesion above the splanchnic outflow(T6). It is characterized by a sudden onset and severe increase in blood pressure and is potentially life threatening. The other classic symptoms are headache, chest pain, sweating, and bradycardia. In order to lower the blood pressure, it is important to remove the noxious stimulus for autonomic dysreflexia. If such symptoms last for more than 15 minutes despite conservative interventions, antihypertension drugs are recommended. Methods: In this case study, we report an autonomic dysreflexia case that developed in a 45 year-old tetraplegia patient with sacral pressure sore. When he got bladder irrigation, his blood pressure went up very high and his mentality became stuporous. He was sent to ICU for his blood pressure and mental care. ICU care made his vital sign stabilized and his mentality alert. Results: After the patient underwent proper treatment like inotropic agent, he was transferred to the general ward and his pressure sore on sacral area was coveraged with gluteus maximus myocutaneous advancement flap. Conclusion: If treatment is not effective, the patients have to undergo sudden, severe hypertension, which can cause stroke or death. To provide safe and effective care, plastic surgeons should be able to recognize and treat autonomic dysreflexia.

Intramedullary Spinal Cord Metastasis of Choriocarcinoma

  • The authors describe a case of choriocarcinoma that metastasized to the cerebral cortex, vertebral body, and intramedullary spinal cord. A 21-year-old woman presented with sudden headache, vomiting and a visual field defect. Brain computed tomography and magnetic resonance examinations revealed an intracranial hemorrhage in the left temporo-parietal lobe and two enhancing nodules in the left temporal and right frontal lobe. After several days, the size of the hemorrhage increased, and a new hemorrhage was identified in the right frontal lobe. The hematoma and enhancing mass in the left temporo-parietal lobe were surgically removed. Choriocarcinoma was diagnosed after histological examination. At 6 days after the operation, her consciousness had worsened and she was in a state of stupor. The size of the hematoma in the right frontal lobe was enlarged. We performed an emergency operation to remove the hematoma and enhancing mass. Her mental status recovered slowly. Two months thereafter, she complained of paraplegia with sensory loss below the nipples. Whole spine magnetic resonance imaging revealed a well-enhancing mass in the thoracic intramedullary spinal cord and L2 vertebral body. Despite chemotherapy and radiotherapy, the patient died 13 months after the diagnosis.

Audio Enhancement Algorithm Using Adaptive Perceptual Filter (적응 지각 필터를 이용한 오디오 음질 개선 알고리즘)

  • In this paper, a new adaptive audio signal enhancement algorithm is proposed. In order to remove a broadband noise from a noisy signal, a filter is designed and applied adaptively to noisy audio signal. The noisy signal is first transformed to frequency domain and divided into bark domain to calculate excitation energy. A filter will be calculated to eliminate the noise by using the excitation energy and noisy energy which is obtained from a silent area. The filter is adaptively adjusted and continuously applied until the threshold point is met. The algorithm also works well even though the noise's energy change all of a sudden. SNR, NMR comparison and MOS Test are performed to show the effectiveness of the proposed algorithm.

  • In order to extract motion features from an input image acquired by a static CCD-camera in a restricted area, we need a robust algorithm to cope with noise sensitivity and condition change. In this paper, we proposed an efficient algorithm to extract and track motion features in a noisy environment or with sudden condition changes. We extract motion features by considering a change of neighborhood pixels when moving objects exist in a current frame with an initial background. To remove noise in moving regions, we used a morphological filter and extracted a motion of each object using 8-connected component labeling. Finally, we provide experimental results and statistical analysis with various conditions and models.

  • PCV(Positive Crankcase Ventilation) system is designed to remove blowby gas. In this system, a PCV valve is attached in a manifold suction tube to control the flow rate of blowby gas which generates various operating conditions of an automotive engine. As this valve plays a crucial role, the demand in its design is high owing to the small size and high velocity. For this reason, a numerical investigation was carried out to understand both the spool dynamic motion and internal fluid flow characteristics. As a result, the spool dynamic characteristics(i.e. displacement, velocity, acting force), increase in direct proportion to the magnitude of the pressure difference and indicate periodic oscillating motions. Moreover, the velocity at the orifice region decreases according to the increase in differential pressure due to energy loss caused by the sudden decrease of flow area at the orifice region and the increase of flow volume in front of the spool head. Finally, the mass flow rate at the outlet decreases with the increase of spool displacement.

THE RENAISSANCE REVISITED: FROM A SILK ROAD PERSPECTIVE

  • The Renaissance is generally said to be the rebirth of the ancient civilizations of Greece and Rome, and was centered around Italy from the 14th to the 16th century. This includes the temporal peculiarity of the Renaissance as a sudden phenomenon after the Medieval Ages, and the spatial peculiarity of what happened only in Europe. However, if we remove the European-centered bias here, the horizon for interpreting the Renaissance becomes much wider. There have been claims that similar cultural phenomena resembling the Renaissance existed in other civilizations at the same time. This paper seeks to investigate two possibilities. The first is the possibility of a spatial expansion of the Renaissance. This suggests that the Renaissance was created by long-term exchanges with the Eastern, Middle and Western Hemispheres. The second is the possibility of a simultaneity of the Renaissance in the 14th and 16th centuries. This suggests that it was a global phenomenon that occurred in different civilizations. The Renaissance, therefore, was a crystallization of a complex of civilizations created by the crossing of various cultures along the Silk Road, and should be referred to as the 'Global Renaissance' instead of the 'Western Renaissance.'

Acute Spontaneous Subdural Hematoma due to Rupture of a Tiny Cortical Arteriovenous Malformation

  • Acute subdural hematoma (SDH) of arterial origin is rare, especially SDH associated with an arteriovenous malformation (AVM) is extremely rare. The authors report a case of acute spontaneous SDH due to rupture of a tiny cortical AVM. A 51-year-old male presented with sudden onset headache and mentality deterioration without a history of trauma. Brain CT revealed a large volume acute SDH compressing the right cerebral hemisphere with subfalcine and tentorial herniation. Emergency decompressive craniectomy was performed to remove the hematoma and during surgery a small (5 mm sized) conglomerated aciniform mass with two surrounding enlarged vessels was identified on the parietal cortex. After warm saline irrigation of the mass, active bleeding developed from a one of the vessel. The bleeding was stopped by coagulation and the vessels were removed. Histopathological examination confirmed the lesion as an AVM. We concluded that a small cortical AVM existed at this area, and that the cortical AVM had caused the acute SDH. Follow up conventional angiography confirmed the absence of remnant AVM or any other vascular abnormality. This report demonstrates rupture of a cortical AVM is worth considering when a patient presents with non-traumatic SDH without intracerebral hemorrhage or subarachnoid hemorrhage.
