A Case Report of Three Patients with Nontuberculous Mycobacterial Pulmonary Disease Caused by Mycobacterium kansasii (Mycobacterium kansasii에 의한 비결핵성 마이코박테리아 폐질환 3례)

  • Mycobacterium kansasii is the second most common cause of nontuberculous mycobacterial pulmonary disease in Western countries and Japan. The clinical and radiological features of pulmonary disease caused by M. kansasii usually resemble those of pulmonary tuberculosis including cavitary infiltrates with an upper lobe predilection. It is also now apparent that patients with M. kansasii pulmonary disease can present with noncavitary nodular bronchiectatic infiltrates similar to lung diseases of M. avium complex. With rifampin-containing regimens, treatment success rates are almost 100%. Timely diagnosis before the development of extensive disease and effective overall treatment strategies are very important to ensure that patients receive the appropriate medications for a sufficiently long period of time. To our knowledge, there has been no Korean case report of M. kansasii pulmonary disease in the immunocompetent patient until now. We report three cases of M. kansasii pulmonary disease in immunocompetent adult patients.

Radiographic Analysis of Instrumented Posterolateral Fusion Mass Using Mixture of Local Autologous Bone and b-TCP (PolyBone$^{(R)}$) in a Lumbar Spinal Fusion Surgery

  • Objective : Although iliac crest autograft is the gold standard for lumbar fusion, the morbidity of donor site leads us to find an alternatives to replace autologous bone graft. Ceramic-based synthetic bone grafts such as hydroxyapatite (HA) and b-tricalcium phosphate (b-TCP) provide scaffolds similar to those of autologous bone, are plentiful and inexpensive, and are not associated with donor morbidity. The present report describes the use of Polybone$^{(R)}$ (Kyungwon Medical, Korea), a beta-tricalcium phosphate, for lumbar posterolateral fusion and assesses clinical and radiological efficacy as a graft material. Methods : This study retrospectively analyzed data from 32 patients (11 men, 21 women) who underwent posterolateral fusion (PLF) using PolyBone$^{(R)}$ from January to August, 2008. Back and leg pain were assessed using a Numeric Rating Scale (NRS), and clinical outcome was assessed using the Oswestry Disability Index (ODI). Serial radiological X-ray follow up were done at 1, 3, 6 12 month. A computed tomography (CT) scan was done in 12 month. Radiological fusion was assessed using simple anterior-posterior (AP) X-rays and computed tomography (CT). The changes of radiodensity of fusion mass showed on the X-ray image were analyzed into 4 stages to assess PLF status. Results : The mean NRS scores for leg pain and back pain decreased over 12 months postoperatively, from 8.0 to 1.0 and from 6.7 to 1.7, respectively. The mean ODI score also decreased from 60.5 to 17.7. X-rays and CT showed that 25 cases had stage IV fusion bridges at 12 months postoperatively (83.3% success). The radiodensity of fusion mass on X-ray AP image significantly changed at 1 and 6 months. Conclusion: The present results indicate that the use of a mixture of local autologous bone and PolyBone$^{(R)}$ results in fusion rates comparable to those using autologous bone and has the advantage of reduced morbidity. In addition, the graft radiodensity ratio significantly changed at postoperative 1 and 6 months, possibly reflecting the inflammatory response and stabilization.

Research on the Function and Economic Effect of Technology Opportunity Development System (기술기회발굴시스템의 기능 및 경제적 효과에 관한 연구)

  • This research focus on estimating the economic effects of TOD system development from the perspective of new market creation, R&D planning cost reduction and increase of R&D projects' commercialization success rates. The research is conducted through simulation and scenario analysis with assumptions about economic effect parameters. Scenario analysis shows that scenario 1 (the application ratio of the new TOD system to total Korean R&D programs' planning is 1.4%) results in total economic effects, 921.3 billion won in 2011 price with B/C ratio 6.15, that scenario 2 (the application ratio is 1.9%) results in total economic effects, 1,250.3 billion won in 2011 price with B/C ratio 8.34, and that scenario 3 (the application ratio is 0.9%) results in total economic effects, 592.2 billion won in 2011 price with B/C ratio 3.95. The research contributed to the prior evaluation of economic validity of "R&D on Technology Opportunity Development (TOD) system" and to cultivating the new methodology of economic benefit estimation in the area of R&D on system development.

Optimum Concentration of Alizarin Complexone (ALC) for Otolith Marking of Pacific Cod Gadus macrocephalus (대구 Gadus macrocephalus 이석 표지를 위한 Alizarin Complexone의 최적 농도)

  • Optimum concentration of alizarine complexone (ALC) for staining of otoliths for Pacific cod was determined by examination of stained quality, growth and survival rates. Small juveniles ($34.1{\pm}1.8mm$ TL, $mean{\pm}SD$) were immersed in four different concentrations (20, 40, 60, 80 mg/L) and large ones ($73.8{\pm}1.2mm$ TL, $mean{\pm}SD$) in five different concentrations (1, 5, 10, 20, 40 mg/L) of ALC for 24 hrs. The fish were then reared and observed for 14 days. The stained quality of otoliths was evaluated by dividing them into four grades by the presence of fluorescent stains on the otoliths under G-excited UV light. The small juveniles stained with the different ALC concentrations had 100% staining success, and their growth rate and body weight did not differ significantly between treatments. Optimal ALC concentration for the smaller juveniles was 20 mg/L in the present experiment. On the other hand, distinct fluorescent rings were confirmed on otoliths of larger juveniles in the 10 mg/L treatment with high survival rate. These results suggest that ALC staining at concentrations of 10 mg/L with larger juveniles will be helpful for release-recapture experiments for stock enhancement of Pacific cod.

Molecular Identification and Distribution Pattern of Fish Eggs Collected around Jejudo Island (제주 연안해역 어란의 분자동정과 분포양상)

  • Monthly variation of species composition and abundance of fish eggs were examined to know the spawning time and location of the fishes inhabiting the coastal region of Jejudo Island. Samplings had been performed at the four locations (Jeju port, Seongsanpo, Seogwipo port and Chagwido) with a bongo net which was towed monthly at the sea surface from August 2006 to July 2007. The fish eggs were identified based on phylogenetic analyses with the DNA sequences generated through PCR-amplification and sequencing of the mitochondrial cytochrome b gene. Among a total of 43 taxa classified during the study period, 34 were identified to species, 4 to families, and the remaining 5 were unidentified. Of them, 23 taxa were occurred at Jeju port, 21 at Seongsanpo, 19 at Seogwipo port and 18 at Chagwido, whereas 15 taxa were found in September 2006, 12 in June 2007, 6 to 8 in August 2006 and January~May 2007, 5 in each October and November 2006, 3 in each December 2006 and July 2007. Among 34 species of fish eggs, Engraulis japonicus and Callanthias japonicus most frequently appeared at 16 times out of 48 observations in total. When those eggs were collected, the surface seawater temperature ranged $14.0{\sim}28.6^{\circ}C$ for E. japonicus and $14.9{\sim}20.5^{\circ}C$ for C. japonicus. The success rates of PCR-amplification and species identification in this study were 68.3% and 79.1%, respectively.

  • Oromandibular dystonia is a focal neurological movement disorder characterized by involuntary sustained and often painful muscle contraction, usually producing repetitive movements or abnormal positions of the mouth, jaw and.or tongue. Patients suffering from oromandibular dystonia often experience difficulties in chewing, swallowing and speaking, resulting from the impairment of mandibular movements. At present there is no etiologic treatment for oromandibular dystonia, because the pathophysiology of primary and focal dystonia is still incompletely understood. Many treatments such as medication, behavioral therapy, surgery are suggested to decrease the involuntary movements. But these success rates are relatively low and they have a lot of complications. many studies suggested that chemodenervation with botulinum toxin is the most effective treatment for oromandibular dystonia. We reported the 2 cases which were treated oromandibular dystonia with botulinum toxin and reviewed the orofacial movement disorders(especially oromandibular dystonia) and botulinum toxin treatment for oromanfibular dystonia.

Development of a sow voice analysis system for forecasting parturition time (임신돈의 분만시기 예측을 위한 음성 분석 시스템 개발)

  • Pure voice characteristics of sow were analyzed to predict parturition time. These were analyzed by using oscilloscope and Sound Forge and the results showed that the voice frequency and amplitude of sow were in the range of 30~2,500Hz and -35~-75dB. According to the sound analysis results, the frequencies of sound appearance from farrowing sow in the farrowing pen for three days prior to delivery day and eight hours of prior to time were around 85% and 46%, respectively of the total appearance during eight days to delivery. Forecasting of delivery time of farrowing sow using the number of sound occurrences showed a promising result such that those have been increased whenever the delivery time was approached. The forecasting success rates were 100% for both of one day and six hours prior to the actual delivery.

RETROSPECTIVE STUDY OF SHORT ROUGH SURFACE IMPLANTS (짧은 길이의 거친 표면 임프란트에 대한 후향적 연구)

  • Background: Reduced bone height is one of the major problems faced in restoring tooth loss with implants. By the use of short implants, it is possible to reduce complicated and invasive treatment such as bone graft, allowing more simple surgery. But short implants are generally considered to have lower success rates than that of standard implants. Purpose: To assess the results of short Straumann implants by a retrospective study of short Straumann implants with TPS(titanium plasma-sprayed) and SLA(sandblasted, large grit, acid etched) surfaces. Materials and methods: 173 implants in 106 patients who received short Straumann implant surgery(${\le}8\;mm$) in the department of oral and maxillofacial surgery, Kyunghee Dental Hospital, from February 1996 to October 2006 were selected and studied. All of the implants were followed up after prosthetic rehabilitation. The average follow-up period was 34 months with 119 months as the longest follow up period. The average follow-up period after prosthetic rehabilitation was 31 months. 64 females(60.4%) and 42 males(39.6%) participated in the research with the age range of 19 to 85(mean age 47). 20 patients(18.9%) were under 40, 85 patients(80.2%) were over 40 and under 70, and only one patient(0.9%) was over 70 years old. Results: 27 implants(15.6%) had TPS surface while 146 implants(84.4%) had SLA surface. 9 implants(5.2%) were 3.3, 108 implans(62.4%) were 4.1mm and 56 implants(32.4%) were 4.8mm in diameter. 167 implants(96.5%) were 8mm and 6 implants(3.5%) were 6mm in length. There were 24 implants(13.9%) on the maxillas and 149 implants(68.8%) on the mandibles. 119 implants(68.8%) were rehabilitated with FPD(fixed partial denture), 47 implants(27.2%) with single crowns and 4 implants(2.3%) with overdentures. Among the fixed partial dentures, 30 of them were splinted with short implants only. After over an year of follow-up period, 139 implants(96.5%) out of 144 implants showed marginal bone loss of less than 1mm. 3 out of 173 implants failed showing 98.27% survival rate. Conclusions: The use of short Straumann implants(${\le}8mm$) can be a simple and reliable treatment method in minimal residual bone height.

  • This study is an analysis of distribution of patients who installed Xive implant in Yonsei University Hospital and types of implant site for about 2 years recall check and cumulative survival rate. 41 implant were used in this study. It shows the conclusion below. 1. Patients at the age of 40s and 50s were 60% of all implant cases and average number of implant was 2.4(man), and 1.9(woman). All cases were operated on mandible, 3 implants on anterior region and 38 implants on posterior region. 2. The major cause of tooth loss is dental caries(48.8%), followed by periodontal disease. 3. Most distribution of bone qaulity for mandibular implant site was type II(65.8%) and bone quantity was type B(75.6%). 4. The majority of implants were those of 11, 13mm in length(95%) and regular diameter in width (64%). 5. The 41(19 persons) Xive implants that were placed in the mandibular anterior and posterior region were all survival and showed a 100% 2 year cumulative survival rate. The results provided us with basic data on patient type, implant distribution, bone condition, and survival rate. We wish that our results coupled with other research data helps assist in the further study for better implant success rates, etc.

The Forecasting a Maximum Barbell Weight of Snatch Technique in Weightlifting (역도 인상동작 성공 시 최대 바벨무게 예측)

  • The purpose of this study was to predict the failure or success of the Snatch-lifting trial as a consequence of the stand-up phase simulated in Kane's equation of motion that was effective for the dynamic analysis of multi-segment. This experiment was a case study in which one male athlete (age: 23yrs, height: 154.4cm, weight: 64.5kg) from K University was selected The system of a simulation included a multi-segment system that had one degree of freedom and one generalized coordinate for the shank segment angle. The reference frame was fixed by the Nonlinear Trans formation (NLT) method in order to set up a fixed Cartesian coordinate system in space. A weightlifter lifted a 90kg-barbell that was 75% of subject's maximum lifting capability (120kg). For this study, six cameras (Qualisys Proreflex MCU240s) and two force-plates (Kistler 9286AAs) were used for collecting data. The motion tracks of 11 land markers were attached on the major joints of the body and barbell. The sampling rates of cameras and force-plates were set up 100Hz and 1000Hz, respectively. Data were processed via the Qualisys Track manager (QTM) software. Landmark positions and force-plate amplitudes were simultaneously integrated by Qualisys system The coordinate data were filtered using a fourth-order Butterworth low pass filtering with an estimated optimum cut-off frequency of 9Hz calculated with Andrew & Yu's formula. The input data of the model were derived from experimental data processed in Matlab6.5 and the solution of a model made in Kane's method was solved in Matematica5.0. The conclusions were as follows; 1. The torque motor of the shank with 246Nm from this experiment could lift a maximum barbell weight (158.98kg) which was about 246 times as much as subject's body weight (64.5kg). 2. The torque motor with 166.5 Nm, simulated by angular displacement of the shank matched to the experimental result, could lift a maximum barbell weight (90kg) which was about 1.4 times as much as subject's body weight (64.5kg). 3. Comparing subject's maximum barbell weight (120kg) with a modeling maximum barbell weight (155.51kg) and with an experimental maximum barbell weight (90kg), the differences between these were about +35.7kg and -30kg. These results strongly suggest that if the maximum barbell weight is decided, coaches will be able to provide further knowledge and information to weightlifters for the performance improvement and then prevent injuries from training of weightlifters. It hopes to apply Kane's method to other sports skill as well as weightlifting to simulate its motion in the future study.
