구강내 접근을 이용한 악하선 타석의 제거 (Removal of Submandibular Stones via Intraoral approach)

  • Background and Objectives : Traditionally, the excision of submandibular gland (SMG) has been commonly used for treatment of calculi in the proximal duct or gland parenchyma. Over the last 10 years several new minimally invasive techniques including lithotripsy, sialendoscope were introduced in the treatment of sialolithiasis. But these have some limitation on large, infected calculi. The aim of this study is to assess the intraoral treatment of submadibular stones. Subjects and Method : The records of one hundred and seventy-three patients who underwent intraoral removal of submadibular sialolithiasis between June 1, 1989 and July 31, 2006 were retrospectively reviewed. Results : Stone location was distal to the edge of the mylohyoid muscle in 127 patients and proximal to gland in 48 patients (mean size of sialoliths, 7.1mm [range 3.0-25mm]). The complete removal of stones was observed in 170 (97.1%) patients regardless of size and location. Recurrence of lithiasis was found in 8 patients (then treated with intraoral removal in 5 patientsand resection of SMG (submandibular glands) in 3 patients). Acalculous sialadenitis in 9 patients (5.1%) and cyst formation in 2 patients (1.1%) was found. But no evidence of postoperative complications including hemorrhage, fistula, damage to lingual nerve were found. Conclusion : The intraoral removal of submandibular stone is useful in preservation of submandiblar function and effective in palpable stones regardless of location, size.

뒤쥐, Sorex caecutiens 악하선의 미세구조 (Ultrastructure of the Submandibular Gland in the Korean Spider Shrew, Sorex caecutiens)

  • 뒤쥐 Sorex caecutiens의 악하선의 미세구조를 연구하였다. 악하선은 샘포들과 도관들로 구성되었다. 악하선 샘포는 잘 발달된 조면소포체, 미토콘드리아와 많은 전자밀도가 있는 분비과립으로 채워진 장액선 세포와 점액선 세포를 가지는 혼합샘이었다. 장액선 샘포 과립은 명확한 한계막이 없는 타원형으로 다양한 전자밀도를 가지는 거친 알갱이만을 가지고 있었다. 점액선 샘포 과립은 명확한 한계막이 없는 타원형이고 전자밀도가 있는 균질한 기질 내에 몇 개의 연하거나 투명한 띠를 가져 다양한 문양으로 관찰되었다. 따라서 뒤쥐아과(Soricinae)에 속하는 뒤쥐, S. caecutiens의 악하선 샘포 과립은 샘포 과립의 경계막의 부재와 점액 샘포 과립의 특별한 문양으로 땃쥐아과(Crocidurinae)를 포함한 다른 포유류 종들과 구분된다. 과립관세포에서 많은 작은 과립소포와 층으로 된 한계막으로 덮이고 거친 장액성의 분비 과립 혹은 균질한 기질로 채워진 몇 개의 특징적 구조들이 관찰되었다.

타액선 상피-근상피세포암종의 세포학적 소견 - 1예 보고 - (Cytologic Findings of Epithelial - Myoepithelial Carcinoma of the Salivary Gland - A Cese Report -)

  • The report of aspiration cytologic findings of epithelial-myoepithelial carcinoma(EMC) in the salivary gland is extremely rare. We present a case of fine needle aspiration cytology(FNAC) from EMC in the right submandibular gland of a 46 years old male patient. Neck CT scan revealed a confined lesion in the submandibular gland without enlargement of the regional lymph node. FNAC from the tumor showed several three-dimensional cellular clusters with admixed normal acinar cells. They frequently formed blanching tubular structures composed of two type of cells; darker cells haying eosinophilic scanty cytoplasm with round dense nuclei and clear cells having abudant pale cytoplasm with vesicular nuclei at the periphery of clusters. The tumor cells of both types did not show pleomorphism or mitoses. The resected submandibular gland showed an ill-defined whitish firm tumor, measuring $2{\times}1.5{\times}2cm$. The histology revealed an infiltrative tumor showing characteristic two cell types in a duct-like arrangement surrounded by thin basement menbrane. An inner layer of darker cells and outer layer of clear cells were postive for cytokeratin in the former and S-100 protein in the taller on the immunohistochemical stain.

방사선 조사가 백서 악하선조직에 미치는 영향에 관한 실험적 연구 (AN EXPERIMENTAL STUDY OF THE IRRADIATION EFFECTS ON RAT SUBMANDIBULAR GLAND)

  • The purpose of this study was to investigate the effects of irradiation on the rat submandibular gland which composed of the epithelial growth factors. The experimental animals were the Sprague Dawley strain rats, which were the rats as the non-irradiated control group and the rats as the experimental group which were divided into groups as the experimental duration of 1 hour, 3 hours, 6 hours, 12 hours, 1 day, 3 days, 1 week, 2 weeks, 4 weeks. The experimental animals were singly irradiated at a dose of 8 Gray gamma ray to their head and neck region by the Co-60 teletherapy unit and sacrificed after each experimental duration. The specimens were stained with H-E and Azan stain and examined light microscopically. The results of this study were obtained as follows. 1. The all of mucous and serous acini in submandibular gland showed similar pattern of changes in structure according to the lapse of time. 2. The acinic cells started to change after 1 hour, and repaired after 12hours with mitosis and proliferation of the cells between acini. The changes were marked after 1day, and repaired gradually in course of time. 3. The duct were dilatated irregularly, and the outline of the eosinophilic stained ductal cells changed indistinctly.

방사선 조사가 백서 악하선 줄무늬관세포에 미치는 영향에 관한 투과전자현미경적 연구 (AN ELECTRON MICROSCOPIC STUDY OF THE IRRADIATION EFFECTS ON THE STRIATED DUCT CELLS OF THE SUBMANDIBULAR GLAND IN RATS)

  • The purpose of this study was to investigate the effects of irradiation on the striated duct cells of the rat submandibular gland ductal tissues which control the characteristics of saliva. For this study, the experimental group was composed of 36 irradiated Sprague Dawley strain rats divided into 8 subgroups 1 hour, 2 hours, 3 hours, 6 hours, 12 hours, 24 hours, 48 hours, 72 hours after irradiation. 4 non-irradiated rats were used as the control group. The experimental animals were singly irradiated with a dose of 18Gy gamma ray to their head and neck region by the Co-6- teletherapy unit and sacrificed after each experimental duration. The specimens were examined with a light microscope with an H-E stain and with a trans- mission electron microscope. The results of this study were as follows. In the light micrograph, a severe atrophic change occurred in the striated duct cells at 2hours after irradiation and gradual recovery occurred from 6 hours after irradiation. 2. The nuclear chromosomes of the striated duct cells were changed granular at 2 hours after irradiation. Recovery was observed at 6 hours after irradiation. Nuclear bodies were also observed from 3 hours after irradiation. 3. The mitochondria of the striated duct cells had indistinct cristae at 2 hours after irradiation, and were degenerated or swollen at 3 hours after irradiation. They recovered, however, from 6 hours, with an increasing number at 48 hours and a regular arrangement was observed at 72 hours after irradiation. 4. The microvilli showed atrophic changes at 2 hours after irradiation and were almost lost at 3 hours after irradiation. They were observed again from 48 hours after irradiation. 5. The rough endoplasmic reticulum and golgi body were not apparent at 1 hour after irradiation and were dilated with degeneration 2 hours after, but intact rough endoplasmic reticulum were observed from 3 hours after irradiation and developed well at 24 hours after irradiation. By the result of this study, showing a mild change in the functional morphology of the salivary striated duct cells immediately following irradiation, it is considered that the many complications which occur after radiation therapy, will disappear in time with the histological and the functional recovery of the glandular tissues.

  • The author have had a case of salivary stone in the anterior half of Wharton's duct in the left side. 1)The patient was 32-year=old Korean male .2)There was a history of severe pain at mealtime, pus discharging and marked swelling of the left submandibular region.3) The patient was sebmandibular region. 3)The paient was sedated with 100mg .of Seconal and anesthetized with 2% Xylocaine Hydrochloride. The surgical procedure was performed in the usual manner. 4)THhe stone was ellowish-white, single wheat-shaped and 9mm. by 3mm. in size.

이하선관에 발생한 MALT 림프종 1예 (A Case of MALT Lymphoma in Parotid Gland Duct)

  • Purpose: Primary malignant lymphomas of the salivary glands are uncommon. The parotid gland was most frequently involved, followed by the submandibular gland, minor salivary gland and sublingual gland. The most common subtype is mucosa - associated lymphoid tissue(MALT) lymphoma. We experienced a case of salivary MALT lymphoma involving parotid gland duct, so report a case with a review of the literature. Methods: A 65 year old female presented with a palpable mass on the left side of her cheek. There was no clinical or laboratory evidence of pre - existing autoimmune disease. Preoperative facial and neck CT with contrast showed $2.1{\times}1.7cm$ sized, ill defined, homogeneous low density mass near left masseter muscle, and no evidence of other enlarged lymph nodes. Results: At operation, a yellowish oval shaped mass was found slightly adhered to middle portion of the parotid gland duct, meaduring $2{\times}1.5{\times}0.7cm$. Microscopic finding showed that centrocyte - like cells, monocyte B cells and plasma cells were diffusely infiltrated. Immunophenotyping was preformed on fixed section. The majority of the small cells were immunoreactive for the B cell marker CD20. Based on the typical histological findings supported by immunostaining, the mass was defined as MALT lymphoma. Conclusion: We report that very rare case of MALT lymphoma involving parotid gland duct in 65 year old female patient was experienced with clinical characteristics, histologic features and references.

Streptozotocin유도 당뇨병이 백서 악하선에 미치는 영향에 관한 병리조직학적 연구 (Effect of the Streptozotocin Induced Diabetes in the Rat Submandibular Glands)

  • The purpose of this study was to observe the microscopic change of salivary gland tissues, which is the cause of xerostomia in diabetic condition: for this target the author injected STZ 0.1ml/100gm b.w. on rat to produce diabetes, and than observed microscopic change in submandibular gland through the histopathologic method, obtaining as follows : 1. All of the experimental specimens suffered diabetes after injection of STZ, but the blood glucose level was irregular. 2. There were not interrelationship between the blood glucose level and microscopic change on salivary gland tissues. 3. The salivary gland changed after diabetes initiation in lapse of times; after 14 days,suffered severe destruction, however after 17 days, it was regenerated. 4. Salivary glands showed congested, destructive acini cells, and hyperplastic ductal cells as well as salivary gland duct-like structures. 5. Then were accumulation of fat granules within the cytoplasm of the acini cells on mucous gland in diabetic condition. 6. According to insulin injection, there were no more changes on salivary gland tissues, even in the accumulation of fat granules. 7. Histological changes of the serous gland were obvious more than the mucous gland in this experimental condition.

  • The plunging ranula is a kind of ranula that goes over the mouth floor to the neck and other adjacent tissue. Sublingual gland is gently accepted as origin of plunging ranula. Plunging ranula develops commonly because of rupture of sublingual gland duct by trauma and extravasation of salivary secretion to the adjacent tissue. It is not true cyst so that there is no epithelium. And it consisted with thin connective tissue, inflammation cell infiltration and salivary secretion. Left without treatment, it can grow into the 10 cm more huge lesion. This report is a case of 73 years old female who was diagnosed as plunging ranula with review of literature. She presented 5 cm submandibular swelling at first. When surgery was delayed because of patient's condition, the lesion grew into the l2cm huge size. We performed excision of sublingual gland, submandibular gland and plunging ranula and had a good result without recurrence.

Comparative Ultrastructure of Secretory Granules of the Submandibular Gland in the Korean Spider Shrew, Sorex caecutiens, the Lesser White-toothed Shrew, Crocidura suaveolens and the Big White-toothed Shrew, Crocidura lasiura

  • The ultrastructure of the secretory granules of the submandibular gland was examined in the Korean spider shrew, Sorex caecutiens, lesser white-toothed shrew, Crocidura suaveolens and big white-toothed shrew, C. lasiura. The mucous and serous acinar granules of S. caecutiens with a border of the lucid corona differed from those of C. suaveolens and C. lasiurar with a dense lateral border. The mucous acinar granules of C. lasiura with several bands producing a variety of patterns in the matrix were similar to those of C. suaveolens. The serous acinar granules of C. lasiura had a homogenous pale center surrounded by minute dense specks or had an unusual substructure showing a geometric pattern according to the inclusions in the pale matrix of the granules. This is the first report of an unusual substructure showing a geometric pattern of the serous acinar granules of C. lasiura. The myelin-like body was observed in the granular duct cell of the three species of shrew. The myelin-like body of S. caecutiens with layers of unit membranes was different from that of C. suaveolens and C. lasiura with paired membranes. Therefore, the layers composing of the paired membranes of the myelin-like body might be one of the characteristics of Crocidura.
