A RADIOLOGIC STUDY OF SALIVARY STONES (타석에 대한 방사선학적 연구)

  • The author examined fifty cases of sialolithiasis diagnosed in the Dept. of Oral Radiology in SNUH by conventional radiography and sialography, and analyzed patient's age, sex, location, radio density, numbers, shapes, and relation with ducts and parenchymas. 1. The results of this study were as follows: 1. The average age of patients was 38.6 years in submandibular sialolithiasis, and 39.2 years in parotid sialolithiasis. 2. There was slightly higher incidence in males (58.0%) than in females (42.0%). 3. Salivary stones were found to be much more in the submandibular gland and duct (82.0%) than in the parotid gland and duct (18.0%). 4. Of 62 submandibular salivary stones, 33 (53.2%) occurred in the main duct, 25 (40.3%) occurred in the hilum, and 4 (6.5%) occurred in the parenchyma. Of 18 parotid salivary stones, 9(50.0%) occurred in the main duct, 5(27.8%) occurred in the parenchyma, and 4(22.2%) occurred in the hilum. 5. Of the submandibular salivary stones, the number of radiopaque type was 45 (75.8%), and the number of radiolucent type was 17 (24.2%). Of the parotid salivary stones, the number of radiopaque type was 12 (66.7%), and the number of radiolucent type was 6(33.3%). 6. The single type was 30 cases (73.2%) in submandibular gland, 6 cases (66.7%) in parotid gland, and the multiple type was 11 cases (26.8%) in submandibular gland, and 3 cases (33.3%) in parotid gland. 7. Round shape was 35 cases (43.8%), ovoid shape was 22 cases (27.5%), irregular shape was 17 cases (21.3%), and cylindrical shape was 6 cases (7.5%).

Multiple transcripts of anoctamin genes expressed in the mouse submandibular salivary gland

  • Purpose: Salivary fluid formation is primarily driven by Ca2+-activated, apical efflux of chloride into the lumen of the salivary acinus. The anoctamin1 protein is an anion channel with properties resembling the endogenous calcium-activated chloride channels. In order to better understand the role of anoctamin proteins in salivary exocrine secretion, the expression of the ten members of the anoctamin gene family in the mouse submandibular gland was studied. Methods: Total RNA extracted from mouse submandibular salivary glands was reverse transcribed using primer pairs to amplify the full-length coding regions of each anoctamin gene and was subcloned into plasmid vectors for DNA sequencing. Alternative splice variants were also screened by polymerase chain reaction using primer pairs that amplified six overlapping regions of the complementary DNA of each anoctamin gene, spanning multiple exons. Results: Multiple anoctamin transcripts were found in the mouse submandibular salivary gland, including full-length transcripts of anoctamin1, anoctamin3, anoctamin4, anoctamin5, anoctamin6, anoctamin9, and anoctamin10. Exon-skipping splicing in the N-terminal exons of the anoctamins1, anoctamin5, and anoctamin6 genes resulted in multiple alternative splice variants. No expression of anoctamin2, anoctamin7, or anoctamin8 was found. Conclusions: The predominant anoctamin transcript expressed in the mouse submandibular gland is anoctamin1ac. The chloride channel protein produced by anoctamin1ac is likely responsible for the $Ca^{2+}$-activated chloride efflux, which is the rate-limiting step in salivary exocrine secretion.

Major Salivary Glands Tumors:A 10-Year Experience (주 타액선 종양에 대한 10년 간의 경험)

  • Background and Objectives : This study reports our clinicopathological experiences of major salivary gland tumors. Materials and Methods : This study included 302 patients with major salivary gland tumors who had got the diagnosis from January 1995 through December 2004. Medical records were reviewed retrospectively. Results : We found 244 benign and 58 malignant major salivary gland tumors. Among 267 parotid tumors, the most common benign parotid tumor was pleomorphic adenoma. Mucoepidermoid carcinoma, adenoid cystic carcinoma, and acinic cell carcinoma were three most common parotid malignancies. Among 33 submandibular gland tumors, 20 cases were benign and 13 were malignant. There were one benign and one malignant sublingual gland tumors. The duration of symptom of submandibular gland tumors was longer than that of parotid tumors. Most patients presented with asymptomatic mass. Seventy-one percent of salivary gland malignancies underwent postoperative radiation therapy. Five-year disease free survival rate of parotid malignant tumors seemed to be higher than that of submandibular one, although there was no statistical significance. Conclusion : Malignancy rate of each salivary gland followed old axiom that it is inversely related with the size of gland. Submandibular gland tumor tends to be delayed to reach diagnosis. Clinicians must be alert about this finding because submandibular gland tumors are known to have poorer prognosis than parotid tumors.

A ROENTGENOGRAPHIC STUDY OF NORMAL SUBMANDIBULAR GLANDS USING ISOBARIC SIALOGRAPHY (등압수액선조영술을 이용한 정상성인의 악하선에 관한 방사선학적 연구)

  • The aim of this study was to establish the diagnostic criteria of normal submandibular glands in adults. Materials included 132 sialograms of selected person from 23 to 28 years of age. In subjects each variable was measured and evaluated statistically introducing 18 items respectively. Conclusions from this study were as follows. 1. The mean diameter of main duct was 1.96㎜ in distal end and 2.71㎜ in hilar end, so the diameter of distal end was smaller than that of hilar end in submandibular glands. 2. The mean length of main duct was 42.02㎜ in submandibular glands. 3. The mean angle of main duct was 35.67 degrees with inferior border of mandible and 103.05 degrees at posterior mylohyoid portion. 4. In configurations following shape of main duct, those revealed that 39.39% of cases of the curvilinear type, 16.67% of cases of the linear type, 22.73% of cases of the sigmiod type and 21.21% of cases of the angular type. And in configurations following course of main duct, those revealed that 83.33% of cases of the smooth type and 16.67% of cases of the tortuous type. 5. The mean caliber of submandibular glands was 45.59㎜ in superoinferior height, 43.56㎜ in anteroposterior width, 28.85㎜ in mediolateral width and 33.65㎜ in extent below mandibular angle. 6. Well-delineated acinar parenchyme was observed in 77.28% of cases and well-defined tertiary ducts in 50.06% of cases. 7. There were no differences between right and left submandibular glands in size and shape of main duct and parenchymal portion, but there were great variations in each individual.

Bilateral Plunging Ranula Arising from Accessory Submandibular Gland (부악하선에서 유발된 양측성 몰입성 하마종)

  • Purpose: Whereas oral ranula is relatively common and presents as a cyst in the mouth, the plunging ranula is rare and manifests itself as a mass in the neck with or without an associated oral lesion. The purpose of this study is to examine the clinical characteristics of rare bilateral plunging ranula arising from accessory submandibular gland in order to provide our experience for its correct diagnosis and treatment. Methods: A 13-year-old girl manifests as a slow growing painless, non-mobile swelling in the anterior neck. She underwent surgery via a cervical approach. A pseudocyst was extirpated and adjacent accessory gland tissue and related lymph node were removed. Results: The histologic appearance is characteristically of a cyst, devoiding of epithelium or endothelium, with a vascular fibro-connective tissue wall containing some chronic inflammatory cells and macrophage stuffed with mucin. Pathologic findings represented a form of myxomatous degeneration and lined by condensed connective tissue and granulation tissue. The nature of the accessory gland tissue was same as subligual gland. Although total submandibular or sublingual gland excision was not performed, no recurrence was observed during 6 months follow-up periods. Conclusion: Usually, unilateral plunging ranula develops commonly because of rupture of sublingual gland duct by trauma and extravasation of salivary secretion to the adjacent tissue. But our case developed because of bilateral congenital accessory submandibular gland. This is thought to be a result from a congenital failure of canalization of the terminal end of the duct. Finally, the correct diagnosis is essential for the most effective treatment, which is excision of the ranula and related accessory salivary gland. We performed excision of accessory submandibular gland and plunging ranula and had a good result without recurrence.

Comparison of the diagnostic performance of panoramic and occlusal radiographs in detecting submandibular sialoliths

  • Purpose: The aim of this study was to assess and compare the diagnostic performance of panoramic and occlusal radiographs in detecting submandibular sialoliths. Materials and Methods: A total of 40 patients (20 cases and 20 controls) were included in this retrospective study. Cases were defined as subjects with a submandibular sialolith confirmed by computed tomography (CT), whereas controls did not have any submandibular calcifications. Three observers with different expertise levels assessed panoramic and occlusal radiographs of all subjects for the presence of sialoliths. Intraobserver and interobserver agreement were assessed using the kappa test. Sensitivity, specificity, accuracy, positive and negative predictive values, and the diagnostic odds ratio of panoramic and occlusal radiographs in screening for submandibular sialoliths were calculated for each observer. Results: The sensitivity and specificity values for occlusal and panoramic radiographs all ranged from 80% to 100%. The lowest values of sensitivity and specificity observed among the observers were 82.6% and 80%, respectively (P=0.001). Intraobserver and interobserver agreement were higher for occlusal radiographs than for panoramic radiographs, although panoramic radiographs demonstrated a higher overall accuracy. Conclusion: Both panoramic and occlusal radiographic techniques displayed satisfactory diagnostic performance and should be considered before using a CT scan to detect submandibular sialoliths.

A CLINICAL STUDY OF SUBMANDIBULAR GLAND EXCISION (악하선 절제술의 임상적 연구)

  • Salivary glands, major and minor, are susceptible to a wide variety of pathologic conditions. Excision of the submandibular gland is a surgical procedure often undertaken. The procedure is the treatment of choice for patients with neoplasm of the submandibular gland and those with non-neoplastic submandibular disorders which are not controlled with conservative medical measures. Extirpation of the submandibular gland may also be undertaken for diagnostic purposes. We evaluated 84 patients who had been admitted to the dept. of oral and maxillofacial surgery of Pusan National University Hospital from January, 1989 to December, 2002 and had been performed submandibular gland excision. The results are as follows : 1. The patients undertaken the excision of the submandibular gland showed an age range of 16 to 71 years. The average was 49.1. 2. They consisted of 60 males(71.4%) and 24 females(28.6%), having 2.5 : 1 of genda ratio. 3. The most common symptom was swelling in 41 cases(48.8%), followed by the pain in 23 cases(27.4%). Other symptoms included mass, dysphagia, facial abnormaly and neck dyscinesia. 4. 42 cases(50.0%) showed sialadenitis and sialodochitis associated with salivary calculus. 5. According to the histopathologic study, all cases consisted of 17 neoplasmatic conditions(20.3%) and 67 non-neoplasmatic(79.7%). The neoplasmatic cases included 13 benign tumors and 4 primary malignant tumors. Sialadenitis and sialodochitis associated with or without salivary calculus were most marked, found in 50 cases(59.3%), in the non-neoplasmatic conditions. Pleoomorphic adenoma showed the highest frequency of the benign tumor.

Studies on Quantitative Analysis of Salivary Gland using Computed Tomography (전산화단층사진을 이용한 타액선의 정량분석에 관한 연구)

  • Purpose: The purpose of this study was to calculate the size and CT number of both normal parotid and submandibular gland. and evaluate their relation to sex, age and obesity using computed tomography. Materials and Methods: The computed tomography was performed parallel to the Frankfurt plane in 46 subjects with healthy salivary gland. The subjects were divided into the three groups (young, middle. old) according to their ages. The size of salivary gland was determined as maximum cross-sectional area and the CT number of salivary gland was determined as the mean CT number of three ROI's. The body mass index was calculated from weight and height. Results: The mean maximum cross-sectional area was 7.79(±1.25)cm² on parotid gland and 4.12(±0.83) cm² on submandibular gland. The mean CT number was -4.43(±23.87) HU on parotid gland and 50.01(±15.63) HU on submandibular gland. There was decreasing pattern of the maximum cross-sectional area of submandibular gland and the CT number of both parotid and submandibular gland according to age(p<0.05). As the body mass index increased. the maximum cross-sectional area of parotid gland increased and CT number of both parotid and submandibular gland decreased(p<0.05). The maximum cross-sectional area of submandibular gland in male was larger than that in female(p<0.05). As the maximum cross-sectional area and CT number of left salivary gland increased. those of right gland increased(p<0.05). Conclusion : Intra-individual differences in salivary gland size and CT number is considered in the age and individual obesity.

Ultrastructure of Acinar Secretory Granules of Submandibular and Parotid Salivary Gland in the Korean Striped Field Mouse, Apodemus agrarius (Rodentia, Murinae)

  • The ultrastructures of the secretory acinar granules of submandibular and parotid salivary gland were examined in the Korean striped field mouse, Apodemus agraius. The acini of the submandibular salivary gland had serous and mucous acinar cells filled with numerous secretory granules. The serous acinar granules had uniformly fine dense contents and were round typed with a definite boundary between the granules. The mucous acinar granules were relatively coarse, with moderate density, and clustered together as a result of the indistinct boundaries between the granules. The acini of the parotid salivary glands contained only serous cells filled with numerous round-typed serous acinar granules. Serous acinar granules had uniformed dense matrix and definite boundaries. The ultrastructures without substructure in a matrix of serous and mucous acinar granules in the submandibular and parotid salivary glands of A. agraius were similar to those of species of Rodentia but different from those of Soricidae in Korea with a characteristic substructure in a matrix. This ultrastructure and charateristics in secretory acinar granules provide fundamental data for molecular comparisions of genetic relationships and are one of the key methods for classifying A. agraius.

Fine Needle Aspiration Cytology of Salivary Duct Carcinoma with Calcification in Submandibular Gland - A Case Report - (악하선의 석회화를 동반한 타액선 관암종의 세침흡인 세포학적 소견 - 1예 보고 -)

  • Salivary duct carcinoma is an uncommon aggressive malignant epithelial neoplasm with similarity to intraductal carcinoma of the breast. This neoplasm occurs most often in the parotid gland of middle-aged and older males. About 7% of reported tumors occured in the submandibular gland. The report of salivary duct carcinoma with calcification is rare. We report a case of salivary duct carcinoma with calcification in the submandibular gland. The patient was a 73-year-old male with a mass of the right submandibular gland for 1 year. On the fine needle aspiration cytology, the aspirate showed scant cellularity, small clusters of tumor cells, and scattered small calcifications. Nuclei of the tumor cells showed mild pleomorphism and round to oval in shape, and cytoplasm was abundant and finely granular. Nucleoli were indistinct and necrosis was not noted. There were no cribriform or papillary arrangements of tumor cells. Cytologlc findings of salivary duct carcinoma are variable depending on histologic findings, and calcifications could be an additional cytologic findings.
