타석증 3 예 (SIALOLITHIASIS : THREE CASES REPORTS)

  • The sialolithiasis is one of the most common disease that is found in the submandibular gland and they can be usually observed in the extra glandular area. As the lumen of Wharton's duct is larger and more expandable than that of Stensen's duct, the symptom of them is initially painless. Obstruction of salivary secretion by a sialolith can result in sudden swelling and pain, as well as infection of the gland. It can be observed with clinical exam and the scintigraphy. After removal of sialoliths, the majority of them can get the recovery of function. Our department performed the transoral sialolithotomy. One of three cases was recurred sialolithiasis with sialadenitis and was performed by the re-sialolithotomy and extraoral sialoadenectomy.

악하부에 전이된 편평상피암종에 발생한 승저증의 치험례 (MYIASIS IN A SUBMANDIBULAR METASTATIC SQUAMOUS CELL CARCINOMA ; A CASE REPORT)

  • We report a case of myiasis on the left submandibular area after radiation therapy for squamous cell carcinoma. A 77-year-old man visited our hospital, 7 weeks after complicating a course of radiation therapy, had a 7cm diameter necrotic ulcer on the left submandible. The margin of the lesion were undermined by numerous interlinking passages containing 20 maggots. The maggots were removed by simple extraction and were identified as Calliphoridae Lucilia. The probable origin of the myiasis in this case was attributed to poor environmental condition and general weakness of the patient. Oppotunistic cutaneous myiasis can be prevented by protecting open wounds from flies, wound should not be left without a covering capable of excluding flies, especially in the summer month.

  • The review of the literature determines that large osteomas of the mandible are relatively rare. We present a case of a 60-year-old man with painless swelling of the left submandibular area and mild difficulty in breathing. The patient complained that the mass have been progressing slowly for at least 20 years. Radiographic studies showed a giant osteoma that attached to the lingual surface of the left mandibular angle and extended from submandibular space to infratemporal space. After the excision of the tumor the patient made a full recovery.

  • Cystic hygroma is a special form of lymphangioma. It most often occurs in the neck of children. We observed a 14-year-old male whose chief complaint was a abnormal swelling of the right mandibular angle area. The computerized tomography showed the well-defined nonenhancing cystic mass on right submandibular gland area and anterior triangle of the neck. The histopathologic fidings exhibited the multiloculatedfluid-filled cysts, which are lined a flattened layer of endothelial cells with foci of lymphocytes found lying adjacent to the lining. After evaluation of above findings, the mass was diagnosed as a cystic hygroma in the neck.

이하선에 발생된 양측성 Warthin 종양의 증례 (Bilateral Warthin tumor of the parotid gland: A case report)

  • The Warthin tumor is a benign neoplasm that occurs mostly in the parotid gland. The tumor frequently occurs in the tail of the parotid gland. A 75-year-old man was referred to Wonkwang dental hospital with a chief complaint of swelling on the right submandibular area. Numerous salivary stones were observed in the right submandibular gland on computed tomography (CT). And the two tumorous lesions were incidentally found in the parotid gland bilaterally. The tumorous lesions showed homogeneous enhancement on the CT and intermediate signal intensity on both T1- and T2-weighted magnetic resonance (MR) images. This tumorous lesions also showed contrast enhancement on fat suppressed T1 weighted MR images. We report common CT and MR features of this case of Warthin tumor in the parotid gland with literature review.

불소가 발육중인 백서악하선의 Catecholamine에 미치는 영향

  • Catecholamine levels were determined in the submandibular glands of new born rats treated with physiologieal saline solution or large doses of sodium fluoride (10mg/kg) for verying periods of time. In control group, catecholamine levels were gradually increased from fifth day of life until twenty-fifth day of life. When fluoride treatment was started on the fifth day after birth and continued daily to the thirty-fifthday of life, decreased catecholamine levels were obsrved on the fifth, through all experiments, and thirth-fifth day. Daily fluoride treatment from the fifteenth through the thirty-fifth day after birth induced the decreased catecholamine levels which were higher than the levels in submandibular glands of rats treated from the fifth day to the thirty-fifth day after birth with NaF. These experiments suggest that fluoride may be affecting catecholamine biosynthetic mechanisms which were duveloping during the fifteenth to twenty-fifth day of life.

Contralateral Submandibular Retropharyngeal Approach for Recurred High Cervical Chordoma

  • The C2 level is the transition zone between the cranial and cervical spine. Because of its high position and anatomic relationship to vital structures, exposing C2 is challenging and the surgical approach is controversial. We report a of a recurred chordoma in C2 portion, occupying the osseous intraspinal portion. The patient underwent total corpectomy of C3 and gross total removal of tumor by right submandibular approach 3 years previously. We performed a lateral extrapharyngeal approach from contralateral left side with resection C2 central portion followed by gross total removal of mass and placement of graft bone. Although there was transient hypoglossal nerve palsy postoperatively, the patient had full recovery.

Massive lymphangiosarcoma in a dog: a case report

  • A 12-year-old neutered male Golden Retriever presented with a progressively enlarging mass in the submandibular region. Histopathological diagnosis confirmed lymphangiosarcoma with metastasis to the liver and spleen. The pleomorphic neoplastic endothelial cells of the tumor grow directly on bundles of dermal collagen, forming numerous clefts and interconnecting channels that are devoid of conspicuous hematic elements. As lymphangiosarcoma is an uncommon malignant neoplasm, the number of previously reported cases and information of the tumor is limited. The present report describes the clinical history and histopathological diagnosis of a progressive lymphangiosarcoma in the submandibular region with metastases in a dog.
