Massive Intracerebral Hemorrhage Following Drainage of Subdural Hygroma

  • Subdural hygromas are easily treated by trephination and drainage. Therefore, most neurosurgeons do not consider subdural hygromas seriously. However, various complications including intracerebral hemorrhage may develop after rapid drainage of subdural hygroma although rare. Postoperative intracerebral hemorrhage presents with a rapid deterioration of consciousness and focal neurological deficits occurring immediately after drainage of the subdural hygroma. The authors present an unfortunate massive intracerebral hemorrhage and pneumocephalus following drainage of the bifrontal subdural hygroma. The patient subsequently died. To prevent this disastrous complication, close neurosurgical observation and gradual drainage under a closed system seem mandatory. Possible pathogenic mechanisms for this unfavorable complication is discussed with a review of pertinent literatures.

Arachnoid Cyst with Spontaneous Intracystic Hemorrhage and Chronic Subdural Hematoma

  • We report a case of a patient with an arachnoid cyst in which subdural hematoma and intracystic hemorrhage developed spontaneously. Usually, arachnoid cysts are asymptomatic, but can become symptomatic because of cyst enlargement or hemorrhage, often after mild head trauma. Although they are sometimes combined with subdural hematoma, intracystic hemorrhage has rarely been observed. Our patient had a simultaneous subdural hematoma and intracystic hemorrhage without evidence of head trauma.

  • Spontaneous spinal subdural hematoma is reported at a rare level of incidence, and is frequently associated with underlying coagulopathy or those receiving anticoagulant or antiplatelet agents; some cases accompany concomitant intracranial hemorrhage. The spontaneous development of spinal subdural hemorrhage (SDH) is a neurological emergency; therefore, early diagnosis, the discontinuation of anticoagulant, and urgent surgical decompression are required to enable neurological recovery. In this report, we present a simultaneous spinal subdural hematoma and cranial subarachnoid hemorrhage, which mimicked an aneurysmal origin in a female patient who had been taking warfarin due to aortic valve replacement surgery.

  • Objectives : This cliniclal study was to evaluate the effect of Cheongsangsahwatang (淸上瀉火湯) treatment on the headache after subdural hemorrhage. Methods : We treated two patients who have headache after subdural hemorrhage by oriental medical therapy, specially Cheongsangsahwatang (淸上瀉火湯). We used VAS(Visual Analog Scale) to investigate the effect of oriental medical therapy, specially Cheongsangsahwatang (淸上瀉火湯). Reasults : After prescription of Cheongsangsahwatang (淸上瀉火湯), VAS scores on headache and accompanying symptoms decreased. Conclusions : This study suggests that Cheongsangsahwatang (淸上瀉火湯) is effective in the treatment of headache after subdural hemorrhage.

Abusive Head Trauma in Infants and Children in Japan

  • Subdural hematoma in infants can be caused by abuse, and is thought to be more likely if subdural hematoma is associated with retinal hemorrhage and cerebral edema. In Japan, few doctors disagree that cases of subdural hematoma with retinal hemorrhage and cerebral edema with multiple findings on the body are more likely to have been caused by abuse rather than by household accident. On the other hand, in cases where there are no other significant physical findings, only subdural hematoma and retinal hemorrhage, there is a difference of opinion as to whether the injury was caused by an accident or abuse. The reason for this is that neurosurgeons in Japan promoted the concept that infants can develop subdural hematomas and retinal hemorrages due to minor trauma at home before the concept of abusive head trauma became known. In addition, the age distribution of subdural hematomas in Japan differs from that in other countries, with peaks at around 8 months, and the reason for this remains unclear. Therefore, the etiology of infant subdural hematoma in Japan needs to be investigated in greater detail.

Rapid Spontaneous Resolution of Contralateral Acute Subdural Hemorrhage Caused by Overdrainage of Chronic Subdural Hemorrhage

  • Background: Since the first report of a rapidly resolved subdural hemorrhage (SDH) in 1986, few additional case reports have been presented in the literature. Case Report: An 82-year-old female patient presented with a SDH over the left convexity. The SDH was removed via catheter drainage through a burr hole trephination. Post-operative computed tomography (CT) following 300 mL drainage from the chronic SDH demonstrated a newly developed SDH along the right convexity. A follow-up CT performed 2 hours later revealed an unexpected significant resolution of the acute SDH. Conclusion: The spontaneous resolution of acute SDH is believed to result from redistribution by washout of the hematoma by cerebrospinal fluid dilution. However, its exact pathophysiology is not well understood. When surgical evacuation is considered in acute SDH, conservative management should also be considered because spontaneous resolution of hemorrhage remains a possibility.

Spontaneously Rapid Resolution of Acute Subdural Hemorrhage with Severe Midline Shift

  • Acute subdural hematoma is usually a neurological emergency that requires hematoma evacuation or close observation. However, spontaneous resolutions of an acute subdural hematoma without surgical interventions have been reported rarely. We report on a case who showed rapid resolution of an acute subdural hematoma with neurological improvement and review the relevant literatures.

  • Subdural hemorrhage (SDH) can manifest various neurologic symptoms. However, SDH presenting with only hand weakness has rarely been reported. We report two SDH cases with only hand weakness mimicking peripheral neuropathy. Since SDH can present with hand weakness only, we suggest the clinicians to do a careful history taking and recommend a CT scan in the elderly patients.

  • We experienced a 71year-old man who had a traumatic subdural hemorrhage as well as delirium, and whose condition was improved through oriental medical treatment. Herbal medicine Hwaeo-jeon is administered three times a day. We did Mini-Mental State Examination-Korean(MMSE-K) and Korean-Dementia Rating Scale(K-DRS). After being treated, the patient showed that symptoms of delirium(diminished or altered state of consciousness, impairment of cognitive abilities or physical function) was improved considerably. This result suggests that Hwaeo-jeon has good effects on delirium with Traumatic Subdural Hemorrhage.

외상성 구개강내 출혈과 동반된 척추경막하 출혈; 요추 전자부 자연흡수 (Traumatic Spinal Subdural Hematoma Accompanying intracranial hematoma: Spontaneous Resolution after Pumbar Puncture)

  • A traumatic spinal subdural hematoma is a rare condition, and only nine cases have been reported until now. We report a rare case of concomitant intracranial hemorrhage and spinal subdural hematoma with a review of the literature. A 45-year-old man was referred to our institute after being stroke by a car. He complained of nausea, headache, back pain, and bilateral sciatica. Brain computed tomography and lumbar spine magnetic resonance images revealed both an intracerbral hemorrhage and a subdural hematoma in the L4 to S1 level. After performing a lumbar spinal puncture and draining the hemorrhagic cerebrospinal fluid (CSF), the intracranial and spinal hematomas were resolved completely without any neruologic deficits.
