Clinical observations were performed on 150 cases of chest trauma, those were admitted and treated at the Department of Surgery, Korea University Woosok Hospital, during the past 8 years period from August 1965 to August 1972. 1. The ratio of male to female patient of chest trauma was 3.4:1 in male predominence and age from 20 to 50 occupied 62% of the total cases. 2. The most common cause of chest trauma was traffic accident in this series. Eighty-one cases[54%] were injured by traffic accident and total cases due to blunt trauma [non-penetrating injury] were 113 cases[75.4%]including the cases with traffic accident, and remaining 37 cases[24.6%] were due to penetrating injury including 25[16.6%] cases of stab wounds. 3. Hemopneumothorax were observed in 645/[96 cases] of the total cases, and etiologic distribution revealed 78.1% due to non-penetrating trauma and 20.8% due to penetrating injury. 4. Rib fracture was found in 50% of cases. Common injuries associated with rib fracture were lung, brain and liver. 5. Most common symptom was chest pain and respiratory difficulty, and common sign associated with chest injury was decreased respiratory sound and subcutaneous emphysema. 6. Conservative non-operative treatment was performed in 94 cases[62.6%] and 56 cases [37.4%] were treated with operative treatment including 31 cases[20.6%] with open thoracotomy. 7. Overall mortality was 9.3%[14 cases] and most common causes of death were due to brain edema, asphyxia and shock.
Objectives and Mtthods : In order to study on the Effect of Strychni Semen aqua-acupuncture on the arthritis induced by injecting Freund's complete adjuvant to the right posterior planta of rat's foot, rats were divided 5 groups equally, one group was not treated(Normal) and the other groups were induced arthritis by injecting Freund's complete adjuvant(Control, ST, SS, SST). Results : The following results have been obtained. 1. In effect of Strychni Semen aqua-acupuncture on plantar edema, SS showed significant decrease compared with control. 2. In effect of Strychni Semen aqua-acupuncture on WBC, SS and SST showed significant decrease compated with control. 3. In effect of Strychni Semen aqua-acupuncture on platelet, SS and SST did not show significant decrease. 4. In effect of Strychni Semen aqua-acupuncture on hematocrit, SST showed significant increase compared with control. 5. In effect of Strychni Semen aqus-acupuncture on the contents quantity of total protein and albumin, SS showed significant increase compared with control: Conclusions : Accoring to this result, Strychni Semen aqua-acupunture inserted into Chok-Samn i(ST36) appeared to have effect on Adjuvant arthritis of rats. Therefore it is oxpected that Strychni Semen aqua-acupunture may be available clinically.
The purpose of this study was to assess the effects of ultrasound on adjuvant-induced arthritis in rats. Adjuvant arthritis was induced in 26 female Sprauge-Dawley rats by the subcutaneous injection of a single dose of .1 mL of Complete Freund's Adjuvant (CFA) (1 mg of Mycobacterium Butyricum suspended in .1 $m{\ell}$ paraffin oil) into the right hind paw. After confirming inflammatory edema and arthritis in the paw 2 weeks later, the arthritic rats were divided into 3 groups, i.e., a control group, a pulsed ultrasound group (Group A), and a continuous ultrasound group (Group B) with 8 rats placed in each group. The rats in Group A were treated with pulsed ultrasound at 1 MHz frequency with .5 $W/cm^2$ intensity in 1 : 4 mode for 3 minutes. The rats in Group B were treated with continuous ultrasound at 1 MHz frequency with 2 $W/cm^2$ intensity in the continuous mode for 3 minutes. The ultrasound treatment was done in the left and right ankles for 2 weeks. Clinical, radiographic and histopathologic findings were then evaluated before and after treatment and yielded the following results. 1. No significant difference was present in body weight between the control group and the treated groups. 2. A statistically significant decrease in the edema of the paw was seen in the rats in Group A that was treated with pulsed ultrasound by 26~29 days after the treatment started (p<.05). 3. According to radiological examination, Group A showed the lowest score in arthritis scale which means it showed a tendency to suppress arthritic inflammation of the left and right hind paws. However, no statistically significant difference was present in the score between the control group, Group A and Group B. 4. According to histopathologic findings, the degree of infiltration by inflammatory cells and hypertrophy of the synovium were less in Group A compared with the control group and Group B. The results of the study show that rats that were treated with the pulsed ultrasound effectively suppressed adjuvant arthritis. However, more effort is needed to objectively prove the effectiveness of ultrasound by developing more sensitive testing methods that could quantitatively evaluate the treatment effects of acute rheumatoid arthritis and by trying out different ultrasound treatment methods.

In the serum total protein and albumin, HRS group showed a increase with statistical significance as compared with control group. Conclusions : According to the above result, it is concluded that Harpagophyti Radix Aqua-acupuncture showed the therapeutic effect of antiinflammation in adjuvant arthritis and it is suggested that further studies and researchs for clinical use be needed.
Experimental studies on the water content of muscles and the histological changes of muscles, digestive tract and kidney, influenced by forced oral administration of excessive tap water, were performed by using 10 Korean bulls. Experimental animals were divided into two groups, five heads for test group and five for control group. All of the bulls used in this experiment were weighing between 250 kg and 300 kg and 3 and 4 years of age. In each test animal, a total of 140 litters of tap water was passed in the forestomach by means of catheterization within 12 to 18 hours. And each of them was slaughtered immediately after the animals showing symptoms of respiratory distress. In control group, the animals were allowed to drink tap water normally. From test and control animals after slaughter, each 10 gm of M. biceps femoris, M. satorius, M. adductor, M. gluteous supercialis, M. iliocostalis lumborum, and M. transversus costarum were taken from definite parts in order to measure water content. In the histopathological studies tissues of rumen, reticulum, omasum, abomasum, duodenum, colon and kidney were taken as wall as the above mentioned muscles. The results obtained were as follows; 1. Increase of water content in the muscles of test group were 4.6% in M. satorius, 4.24% in M. transversus costarum, 4.14% in M. gluteous supercialis, 4.02% in M. adductor, 3.88% in M. biceps femoris, and 2.46% in M. iliocostalis lumborum respectively. The highest increase was found in M. satorius and the lowest in M. iliocostalis lumborum. 2. In test animal, average increasing value of water content in muscles was 3.9% and shown highly significant (p<0.01). 3. On the microscopical findings of carcass, marked watery edema was observed all of the subcutaneous tissues and intermuscular connective tissues. 4. Microscopically, the epimysium, perimysium and endomysium were widened, and the muscle fibers were lacerated. The cells of stratum lucidum in the epithelium of rumen and reticulum were shown marked vacuolization. In the kidney, dilatation of Bowman's spaces and proximal tubles was observed.

Objective : Based on the immunological dysfunction in RA and the immunological feature of Clematis mandshurica Maxim. herbal acupuncture solution (CM-HAS), this study examined whether subcutaneous (s.c.) administration of CM-HAS has anti-inflammatory effects on adjuvant arthritis (AA) in the rat. Methods : Complete Freund's adjuvant was used to induce AA in rats. Secondary paw swelling of AA rats was measured, and polyarthritis index was scored. The administration of CM-HAS (2, 5, 10 mg/kg) to the Sinsu-hyul (BL 23) acupuncture point subcutaneously (s.c.) inhibited the inflammatory response and restored the weight of body and immune organs of AA rats. In this study. inhibitory effect of CM-HAS on cyclooxygenase-2 (COX-2) was evaluated. The plant material selected for this study has been used in Korean medicine for the treatment of various diseases that are considered as inflammatory in nature, e.g. asthma, arthritis, rheumatism, fever, edema, infections, snakebite and related inflammatory diseases. Results : There were significant secondary inflammatory reactions in AA rats, accompanied with the simultaneous decrease of body and immune organs weight. The administration of CM-HAS (2, 5, 10 mg/kg, s.c.) improved the above changes significantly. CM-HAS showed inhibitory activity against COX-2, which supports their traditional uses. In this study, aspirin and indomethacin were used to contrast with CM-HAS as the COX-2 inhibitors. Conclusion: The findings of this study may explain at least in part why CM-HAS has been traditionally used for the treatment of inflammatory conditions in traditional Korean and Chinese medicine.
In the present study, the anti-inflammatory effect of "Jungcheonhwadamgangki-tang ga Antler" water extract was tested in Xylene-Application mouse ear acute inflammation model. The test articles were once dosed before Xylene-Application, and the changes on body weight and ear weights and histopathological observation of induced ear were conducted with ear histomorphometry. The obtained results were as follows. The increases of absolute and relative ear weight detected in vehicle control compared to that of sham, were significantly and dose-dependently inhibited by Jungcheonhwadamgangki-tang ga Antler in the present study. A classic acute inflammatory histological changes such as subcutaneous edema, thickness and infiltration of inflammatory cells, was detected in vehicle control. However, these histological changes were significantly and dose-dependently inhibited by Jungcheonhwadamgangki-tang ga Antler. In addition, the increases of ear thickness half and thickness full detected in the vehicle control were also dose-dependently decreased in the all Jungcheonhwadamgangki-tang ga Antler-dosing groups. Base on these results, it is concluded that Jungcheonhwadamgangki-tang ga Antler extracts has clear anti-inflammatory effect on the acute inflammation such as bronchial asthma.

External volume expansion of the recipient site by suction has been proposed as a way of improving fat graft survival. The objective of this study was to present an innovative and simple intraoperative external expansion system to enhance small-volume autologous fat grafting (40-80 mL) and to discuss its background and its mechanism of action. In this system, expansion is performed using a complete vacuum delivery system known as the Kiwi VAC-6000M with a PalmPump (Clinical Innovations). The recipient site is rapidly expanded intraoperatively 10 times for 30 seconds each with a negative pressure of up to 550 mm Hg before autologous fat injection. During this repetitive stimulation, the tissues become grossly expanded, developing macroscopic swelling that regresses slowly over the course of hours following the cessation of the stimulus. The system sets various mechanisms in motion, including scar release, mechanical stimulation, edema, ischemia, and inflammation, which provide an environment conducive for cell proliferation and angiogenesis. In order to maintain the graft construct in its expansive state, all patients are encouraged postoperatively to use the Kiwi three times daily for one minute per session over the course of three days. The handling of this system is simple for both the patients and the surgeon. Satisfactory clinical outcomes have been achieved without significant complications.
Anti-nociceptive and anti-inflammatory effects of Clematidis Radix (CR)-distillates were investigated in three different pain animal models. Highly purified distillate of CR was injected to Zusanli (ST36) acupoint, called CR herb-acupuncture in the Korean traditional medicine. In tail flick latency test, the CR herb-acupuncture treatment did not show a significant effect of relieving acute phasic pain. To investigate the anti-inflammatory effect of CR herb-acupuncture, inflammatory pain was induced by subcutaneous injection of formalin to the plantar tissue or intra-articular injection of carrageenan to the tibio-tarsal joint in the rats. And the medicinal effect of CR herb-acupuncture was evaluated by analyzing pain behavior such as licking or biting behavior, or by measuring weight distribution ratio between two foot and ankle circumference. In the rat formalin test, the analgesic effect of CR herb-acupuncture was more pronounced in the late phase (for 20 min after the early phase) than in the early phase (for the first 10 min post formalin injection). It also significantly alleviated the carrageenan-induced monoarthritis, in terms of weight distribution ratio and ankle edema. These results revealed that CR herb-acupuncture was effective to treat the inflammatory pain and could be used as an analgesic treatment with an antiinflammatory effect.
