Anti-inflammatory Effect of Ethyl alcohol Roasted Scutellaria baicalensis Georgi on Croton Oil-induced Mice Ear Edema

  • Scutellaria baicalensis Georgi (SB) has been widely studied to treat inflammatory diseases in east Asia. In the recent years, many studies have focused on modifying herbs to increase the pharmacological effects. Roasting alcohol absorbed SB is one of the traditional methods to increase the therapeutic effects. Currently there are no reports on the pharmacological effects of roasted SB. This study investigated the anti-inflammatory effects of roasted 30% ethyl alcohol absorbed SB extract (SR) on mice ear edema. After intra-gastric injection of dexamethasone (for positive control, 2 mg/kg) and SR (50, 100, 400 mg/kg), ear edema was provoked by croton oil (5% v/v in acetone, 10 ul/ear). Ear thickness was measured with a digital caliper to quantify the change in swelling. For histological study, we made paraffin sections and performed Phloxine-Tartrazine staining and Masson's trichrome staining to observe epidermis, dermis and subcutaneous region and collagen fiber of mice ear tissues. Ear thickness decreased dose-dependent manner in SR treated groups. Histological analysis compared with dexamethasone treated group, SR treated groups demonstrated a similar reduction in hypoplasia of epidermis and influx of inflammatory cells. Increase of subcutaneous layer and decrease of collagen fibers were significantly recovered in SR treated group (400 mg/kg) and dexamethasone treated group. In conclusion, treatment with SR ameliorates auricular inflammation induced with croton oil in mice. Experiments are now underway to understand the molecular mechanisms underlying anti-inflammatory activities of SR.

  • In order to develop a broad-spectrum veterinary anthelmintic, a combined preparation of nitroxynil and levamisole was formulated for subcutaneous injection. The preformulation studies on solubilization, physicochemical stability and toxicity of combined preparation were performed. The combined preparation of nitroxynil-N-ethylglucamine and levamisole base could be solubilized up to 50.3%(w/v) of active ingredient concentration in propylene glycol/water system. Injectable solutions were most stable at $4^{\circ}C$. Local toxicities such as flare and edema were not shown when the usual dose of the combined preparation was injected subcutaneously to the rats.

  • This study was performed to compare the antagonistic effects of intravenous(0.125mg / kg) and subcutaneous(0.25mg /kg) administration of yohimbine on xylazine-induced immobilized(4.5mg/ kg) dog and to investigate the effectiveness of yohimbine compound(0.25mg / kg) in clinical practice. Mean arousal time(MAT), mean walk time(MWT), and time to return to normal electroencephalograms were remarkably decreased in all yohimbine-treated groups compared with the control. In electroencephalograms(A-B$\sub$I/ lead), there were no significant alteration, except RR interval. RR interval was decreased in all yohimbine-treated groups compared with the control. Second-degree heart blocks(41.7%) shown after xylazine administration disappeared within 2 min after yohimbine administration. The frequency of electroencephalograms(RO-RF trace) was recovered faster to normal in yohimbine-treated groups than that of the control. In histopathological changes of ICR mice given yohimbine compound subcutaneously, edema with inflammatory cells of hypodermis was slightly shown on the 1st day, but this findings were not observed on the 5th day. It was considered that no difference in the antagonistic effects of intravenous and subcutaneous administration of yohimbine on xylazine- induced immobilized dog were observed and yohimbine compound was usable in clinical practice for antagonistic agent to the xylazine.

새로운 캅사이신유도체 DA-5018의 피하주사 및 국소도포시 진통효과 (Analgesic Effects of DA-5018, a New Capsaicin Derivative, after Subcutaneous Injection and Topical Application)

  • The analgesic effects of DA-5018, a new caosaucin derivative, were evaluated in various experimental pain models. Drugs were administered subcutaneously or topically. When drugs were administered subcutaneously, 1) the $ED_{50}$ of DA-5018, morphine . HCI, capsaicin and acetaminophen were 0.091-2.0, 0.3-4.3, 1.4-26.5 and 45.4-643 mg/kg, respectively in various pain or inflammatory models including acetic acid writhing, formalin, tail flick, Randall-Selitto, hot plate and crouton oil-induced ear edema test, 2) the AD2 values (the dose for doubling of pain threshold of vehicle control) of DA-5018, capsaicin and ketoprpgin were 1.07 $\pm$ 0. 18, 23.47$\pm$4.46 and 2.97$\pm$0.43 mg/kg in Freund's complete adjuvant (FCA)-induced arthritic pain model. And by topical application, 1) neither DA-5018 0.3% cream nor Zostrix-HP (capsaicin 0.075%) were effective in formalin test, 2) although DA-5018 0.3% cream significantly inhibited the croton oil-induced ear edema being better than Zostrix-HP and Kenofen (ketoprofen 3%). 3) In FCA model, DA-5018 0.3% cream reversed the decreased pain threshold of arthritic rat from 136.4 g (day 0) to 289.0 g (day 5) and 250.1 g (day 10), which was similar to Zostrix-HP. These results suggest that DA-5018 administered subcutaneously has a potent and broad analgesic spectrum than nonsteroidal antiinflammatory drugs against acute and chronic pain, and by topical application it exerts comparable analgesic and antiinglammaatory effects to capsaicin cream.

Prediction of Tumor Progression During Neoadjuvant Chemotherapy and Survival Outcome in Patients With Triple-Negative Breast Cancer

  • Objective: To investigate the association of clinical, pathologic, and magnetic resonance imaging (MRI) variables with progressive disease (PD) during neoadjuvant chemotherapy (NAC) and distant metastasis-free survival (DMFS) in patients with triple-negative breast cancer (TNBC). Materials and Methods: This single-center retrospective study included 252 women with TNBC who underwent NAC between 2010 and 2019. Clinical, pathologic, and treatment data were collected. Two radiologists analyzed the pre-NAC MRI. After random allocation to the development and validation sets in a 2:1 ratio, we developed models to predict PD and DMFS using logistic regression and Cox proportional hazard regression, respectively, and validated them. Results: Among the 252 patients (age, 48.3 ± 10.7 years; 168 in the development set; 84 in the validation set), PD was occurred in 17 patients and 9 patients in the development and validation sets, respectively. In the clinical-pathologic-MRI model, the metaplastic histology (odds ratio [OR], 8.0; P = 0.032), Ki-67 index (OR, 1.02; P = 0.044), and subcutaneous edema (OR, 30.6; P = 0.004) were independently associated with PD in the development set. The clinical-pathologic-MRI model showed a higher area under the receiver-operating characteristic curve (AUC) than the clinical-pathologic model (AUC: 0.69 vs. 0.54; P = 0.017) for predicting PD in the validation set. Distant metastases occurred in 49 patients and 18 patients in the development and validation sets, respectively. Residual disease in both the breast and lymph nodes (hazard ratio [HR], 6.0; P = 0.005) and the presence of lymphovascular invasion (HR, 3.3; P < 0.001) were independently associated with DMFS. The model consisting of these pathologic variables showed a Harrell's C-index of 0.86 in the validation set. Conclusion: The clinical-pathologic-MRI model, which considered subcutaneous edema observed using MRI, performed better than the clinical-pathologic model for predicting PD. However, MRI did not independently contribute to the prediction of DMFS.

평위산 전탕액의 보관 온도 및 기간에 따른 항염증 효능 비교 (Changes in Anti-inflammatory Effect of Pyungwi-san Decoction According to the Preservation Temperature and Period)

  • Objectives : To provide the information of preservation method for herbal decoction, we evaluated the anti-inflammatory effects according to preservation temperature and period of Pyungwi-san (PWS, Ping wei-san) decoction. Methods : The anti-inflammatory activity of PWS was investigated by carrageenin-induced paw edema in SD rats. At 0 month, PWS extract was administrated 100, 300 and 1000 mg/kg/day orally for seven days prior to induction of edema. Edema was induced by subcutaneous injection of 1% carrageenin into the right hind paw. The paw volume was measured at 4 hr following carrageenin-induced paw edema in rats. At 6 and 12 months, PWS extracts according to preservation temperatures (room temperature, $4^{\circ}C$ and $-20^{\circ}C$) were administrated in rats and then carrageenin-induced paw edema volume was measured. Results : PWS showed inhibitory effect on carrageenin-induced paw edema in rats and the optimal dose was 1000 mg/kg/day at 0 month of storage. At 6 month of storage, PWS storaged at $4^{\circ}C$ and $-20^{\circ}C$ were inhibited paw edema but that storaged at room temperature was not reduced paw edema in rats. Conclusions : These results suggest that PWS decoction pouch has anti-inflammatory activities at both $4^{\circ}C$ and $-20^{\circ}C$ during 6 month storage.

관절염 유발 랫드에 대한 생봉독의 치료 효과 (The Therapeutic Effect of Natural Honeybee (Apis mellifera) Venom in Adjuvant-induced Arthritic Rat)

  • This study was performed to assess that clinco-therapeutic effect of natural Italian honeybee (Apis mellifera) venom in adjuvant-induced arthritic rat. Ninety Sprague- Dawley rats of male were injected with complete Freund's adjuvant (CFA). Adjuvant arthritis was produced by a single subcutaneous injection of 1 mg Mycobacterium butyricum suspended in 0.1 ml paraffin oil into the right hindpaw. Righting reflex was uniformly lost and considered to be the point of arthritis development on day 14 after CFA injection. Experimental groups were divided into three groups. When arthritis was developed in the rat hind-paw, tested groups were administrated with prednisolone (10 mg/kg, p.o) and honeybee venom (one bee, s.c) at an interval of two days. Control group was subcutaneously injected with 0.1 ml of physiological saline solution in the rat at an interval of two days. Clinical findings, hematological values and histopathological findings were observed during or after the drugs administration. In tested groups, the development of inflammatory edema and polyarthritis on day 14 after treatment was suppressed. No significant differences of hindpaw edema volume and lameness score between prednisolone and honeybee venom groups were observed during or after therapeutic drugs treatment. WBC counts of prednisolone and honeybee venom treatment groups as compared with the control group were getting remarkably decreased during or after the therapeutic drugs administration(p<0.01). Erosions of articular cartilage and inflammatory cell infiltrations during or after the therapeutic drugs treatment was effectively suppressed in natural honey venom.

상백피(桑白皮)가 대식세포의 NO, $TNF-{\alpha}$$IL-1{\alpha}$ 생산에 미치는 영향 (The Effects of Cortex Mori on NO, $TNF-{\alpha}$ and $IL-1{\alpha}$ production by macrophage)

  • Cortex Mori (Moros alba L.), the root bark of mulberry tree has been used as an autiphlogistic, diuretic and expectorant in herval medicine. Recently, a few papers reported that phenolic extract of Cortex Mori had the hypotensive, hypoglycemic, antiviral and anticancer effects, and hot water extract of Cortex Mori(CM) had inhibitory effect on the degranulation and histamine release from activated mast cells. These previous studies suggest a possibility that CM has an antidotal activity against inflammation which was mediated mainly by macrophage-secreting inflammatory factors. This study was performed to evaluate the influences of CM on carrageenan-induced edema in vivo and release of inflammatory mediators such as NO, TNF and IL-1 by macrophages stimulated with LPS or $IFN-{\gamma}$ in vitro. Subcutaneous injections of carrageenan into the mouse paw rapidly induced local edema by increasing vascular permeability, but single intraperitoneal injection of CM extract at 30 minutes before carrageenan suppressed the development of edema. NO and TNF production from macrophage stimulated by LPS or $IFN-{\gamma}$ were significantly suppressed, especially TNF secretion by up to 3-4 folds. LPS stimulated IL-1 production was also inhibited, but not significantly. Cell viability assay verified that the inhibition was not due to general cell toxicity. These results suggest that reduction of NO, TNF and IL-1 production may be one of the means by which CM prevent inflammation associated diseases.

Periocular Dirofilariasis in a Young Woman: A Case Report

  • A case of periocular dirofilariasis has been diagnosed in Iran. A 27-yr old female referred with a history of edema and redness in the left eye since 2 wk ago. On slit lamp examination, a moving thread-like worm was seen in the subconjunctival area. Two days later, a 126 mm long, live filaria worm was extracted from the lower lid subcutaneous tissue. The worm was diagnosed as, likelihood, Dirofilaria immitis on microscopic examinations. The present case suggests that D. immitis can cause periocular infections.

지연성 후두방사선괴사에 대한 고찰 (Delayed Radionecrosis of the Larynx)

  • Radiation therapy is an effective treatment modality for malignant disease of the head and neck, but it is not without risk and complication. Response of the larynx to radiotherapy varies from mild erythema to severe inflammation with edema and induration. possibly leading to necrosis of cartilage. These changes are due to an inflammatory reaction characterized by infiltration of polymorphonuclear leukocytes, vascular thrombosis, and obliteration of lymphatic channels. Late changes consist of telangiectasia of the skin, alopecia, loss of subcutaneous fat, degenerative changes in the connective tissues. But, radiation necrosis of laryngeal cartilage is an uncommon complication and it is a devastating process for which further necessitates surgical treatment. It is generally agreed that the only treatment for patient not responding to conservative measures is a total laryngectomy. We experienced 4 cases of delayed radionecrosis of the larynx who underwent radiation therapy for glottic cancer and hypopharyngeal cancer. We report these cases with review of literature.
