외상성 기관지 파열 -1례 보고- (Tracheobronchial Rupture following Blunt Chest Trauma -1 case report-)

  • The rupture of tracheobronchial tree caused by non penetrating blunt trauma is being increased in incidence though it is still rarely occurred on now Because this type of injury is uncommon, a high index of suspicion should be maintained in all crush injuries involving the chest. Early diagnosis and primary repair not only restore normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair. We experienced a case of tracheobronchial rupture caused by non penetrating blunt chest trauma without rib fracture. The patient was a 16 year old male who was a high school student. He was compressed on anterior chest by hand ball goal post being failed down on the morning of admission day. After this accident, he was suffered from progressively developing dyspnea and subcutaneous emphysema on face, neck and anterior chest. The diagnosis, tracheal rupture, was made by chest CT and bronchoscopy. After right thoracotomy, the ruptured site was directly closed by using interrupted suture. Post-operative course was uneventful. Thus we report this case of traumatic tracheal rupture with review of literature.

소아 호출기 환자에서 경구 투여된 Formoterol이 Theophylline의 혈중농도에 미치는 영향의 연구 (Effect of Formoterol on the Plasma Levels of Theophylline after the Oral Administration to the Children with Respiratory Diseases)

  • Theophylline and $\beta$-adrenergic agonists are frequently used together in patients with respiratory diseases. However the clinical impact of $\beta_2$-adrenergic agonists on the blood concentration of theophylline is not fully evaluated. Formoterol, a newly available oral ${\beta}_2$-adrenergic agonist is frequently used in pediatric respiratory patients. The objective of this study was to investigate the effect of oral formoterol on theophylline's blood concentration. Randomized prospective study was conducted. Twenty-four children were enrolled on the study. Their age ranged 2 to 73 months (mean 35.8 months). Theophylline group (12 patients) received 10 mg/kg/day of for theophylline orally. Theophylline/formoterol group (12 patients) received 10 mg/kg/day of theophylline and $4\;{\mu}g/kg/day$ of formoterol orally. All medications were administered at least for 5 days starting on admission day. Theophylline's trough concentrations were obtained on days 3 and day 5. Pulse rates were recorded before the study medications were given on admission, and days 3 and day 5. Statistical significance was calculated by two-tailed Student's t-test. Theophylline's levels in children given theophylline and formoterol together were lower an those given theophylline alone ($6.38\pm0.90\;{\mu}g/ml\;vs\;7.43\pm0.77\;{\mu}g/ml$ on day 3(p<0.05), $5.62\pm0.56\;{\mu}g/ml\;vs.\;6.78\pm0.61\;{\mu}g/ml$ on day 5 (p<0.05)). In both groups, theophylline's trough concentration on day 5 were lower than day 3. There was no significant side effects in both groups. In conclusion, the new ${\beta}_2$ selective adrenergic agonist formoterol reduced serum theophylline levels in children with respiratory diseases. Further investigation is needed to clarify the long term effect of this drug interaction.

만화 입시 실기의 문제점과 대안 (Problems and Solutions for Admission to Cartoon Studies)

  • 한국의 입시에 관한 첨예한 문제제기는 어제 오늘의 일은 아니다. 만화 관련 학과에서도 예외가 없다. 창의성 부재와 패턴화, 각 학교별 자율적 실기고사 유형에 따른 문제점, 평가의 문제점이 지적되는데, 이에 대한 대안으로 실기시간의 확충, 창의성 평가를 위한 별도의 실기도입, 그리고 창의성 평가를 공교육이 장기간에 걸쳐 전담해 주는 방식 등이 거론된다. 따라서 가장 중요한 전제는 대학은 학생들에게 대학에서 가르치고자 하는 것이 무엇인가에 대한 설정이 우선되어야 하며, 무엇을 가르칠 것이기 때문에 어떠한 능력을 가진 학생이 선발되도록 입시의 문제가 출제 되어야 한다. 진학 후에도 지속적인 모니터링이 필요하며 예상과 다르게 나타날 경우 평가의 기준을 다시 살펴 볼 수 있어야 한다. 이처럼 대학에 많은 것이 요구 되는 것은 학생선발의 주체가 대학이기 때문에 학생들에게 갖추어야할 능력을 요구하는 것도 대학이고 이를 평가하는 것도 대학이기 때문이며, 이러한 선발과정을 거쳐 좋은 인재를 확보하고 교육을 통해 성장시켜야만 본연의 목적을 달성할 수 있기 때문이다.

감각자극이 미숙아의 체중증가에 미치는 영향 (Effects of a Sensory Stimulation on Weight Gain in Premature Infants)

  • This study has been conducted on the nonequivalent control group Pretest-posttest design in quasi experimental basis and newly born premature infants from intensive care unit of G Medical University Hospital in Inchon Metropolitan were selected in two groups of 21 infants each. The first group for experimental and the other for control. Data has been collected form October 30, 1997 to August 29, 1998. For the experimental group tactile and kinesthetic stimulation was applied 2 times a day for 10 days(10:00~ll:00 hours in the morning and 17:00~18:00 in the afternoon). As, a weight weighing instrument, electronic indicator scale(Cas Co. korea) was used. Collected data were analyzed with the SAS program using x²-test, student t-test, repeated measures ANOVA, Pearson correlated coefficient and Stepwise multiple regression. The result were as follow. 1. As for the daily weight gain, the experimental group showed first change in weight and this group also showed higher weight in the average weight than the control group. Statistically, however. there was no significant factor between the two groups. 2. In the correlation between general characteristics and weight gain, gestation Period. apgar score, admission Period before study, N.P.O. period after birth, recovery Period to birth weight showed statistical significance negative correlation with weight gam in the experimental group than control group. 3. In the factor with affected the weight gain by general characteristics showed recovery period to birth weight and head circumference in the experimental group, control group showed recovery period to birth weight, N. P. O. period after birth. admission period before study, feeding amount, weight of study. In conclusion. the sensory stimulation in this study showed a Positive aspect through there was no statistical significance in the weight gam. Tn the correlation of general characteristics and weight gam showed statistical significance negative correlation The main factor which affected the weight gain by general characteristics showed recover period to birth weight.

시스템사고를 통한 사교육비경감정책 평가: 노무현 정부와 이명박 정부를 중심으로 (Evaluation of Policy for Reduction of Private Tutoring Expenditure based on Systems Thinking: Focusing on Roh and Lee Governments)

  • The purpose of this paper is to evaluate the policies for reducing private tutoring expenditure in Roh Mu Hyun and Lee Myeong Bak Government using Causal Loop analysis based on the Systems Thinking perspective. The results are as follows. First, high educational achievers receive more private tutoring than lowers and children who have rich parents have better chance to take private tutoring than the others. It reflects the social characteristics which emphasize the academic ability and educational background. Second, two governments implemented educational policies to control the private tutoring expenditure as balancing loops ; strengthening public education, providing after school programs and EBS KSAT teaching and improving the entrance exam of university. Third, they overlooked the unintended feedback loops coming from 1) incongruity between causes and countermeasures of shadow education 2) wrong perception of substitutional relationship between public education and shadow education 3) side effect of the policy increasing the weight of student record 4) problems of diversifying high schools 5) dilemma of easing the burden of testing through admission officer system. The conclusion is that policies of reducing the private education expenses have failed because two governments don't consider unintended Feedback Loops in the process of making education policies. So we have to make policies based on Systems Thinking and reducing private education expenses should not be the purpose of strengthening the public education.

Personality and Learning Behavioral Characteristics as Predictors of Academic Achievement of Medical Students

  • This study investigates whether personality characteristics and learning behaviors can predict medical students' academic achievement in Korea, specifically in terms of successfully completing medical school without delays or achieving a high grade point average (GPA) in their final year. In May 2018, 316 medical students took the Multi-Dimensional Learning Strategy Test, 2nd edition, which provided data on their personality and learning behavioral characteristics. Their final year's GPA and any delays in completing medical school were ascertained by reviewing all electronic academic records of each semester they had been enrolled. The combination of personality and learning behavioral characteristics was significantly associated with completing medical school without delays, even after adjusting for sex and admission path. A multiple logistic regression analysis showed that the adjusted odds ratios and 95% confidence intervals for completing medical school without delays were 1.52 (95% confidence interval [CI], 0.83-2.78) and 3.64 (95% CI, 1.70-7.82) for "others" and "both high" categories, respectively, when compared with the "both low" category. For 235 students who completed medical school without delays, their learning behavioral characteristics (scores) were significantly associated with their final year's GPA even after adjusting for sex, admission path, and personality characteristics (scores) as determined by the multiple linear regression analysis. This study suggests that individual personality and learning behavior characteristics are predictors of medical students' academic achievement. Therefore, interventions such as personalized counseling programs should be provided in consideration of such student characteristics.

Stakeholders' Opinion on the Desired Characteristics of Nursing School Graduates and Factors Concerning Nursing Curriculum Development in Thailand

  • Effective higher educational management in undergraduate nursing programs is an important issue from the viewpoint of stakeholders. This qualitative research aimed to examine the characteristics of nursing students and curriculum development of undergraduate nursing education from the opinions of Boromarajonani College of Nursing Saraburi, Thailand stakeholders. The population included 4 groups: 1) the alumni who have graduated within the past 5 years and currently work in primary, secondary, and tertiary care units, 2) the supervisors and colleagues of the alumni, 3) nursing lecturers, and 4) the current nursing students. The respondents who are the alumni, nursing lecturers, and current nursing student were selected using a purposive sampling, for the supervisors and colleagues were selected using snowball techniques. Semi-structured interview questions were used for data collection. Group discussions were conducted until saturation on 55 key informants. The qualitative data was analyzed using content analysis. Results showed the viewpoints of stakeholders on the characteristics of future nurse graduates were comprised of four elements: knowledge that meets standards; essential skills for self-development and lifelong learning process; good morals and professional ethics in providing nursing care; and nurse competencies in teamwork, communication, language, research, management, IT, life skills, and global literacy. The viewpoints on the development of the nursing curriculum focus on four elements: the learner, teaching and learning, course content, and instructor tasks. For learners, the admission criteria should include a minimum not only of knowledge, but also positive attitude, science, and art skills, since the nursing profession is both a science and the art of caring. Teaching and learning elements should be authentic, including exposure to real situations, an integrated network, and activities that improve nursing care. Course content was comprised of an updated curriculum, humanized nursing care, student center, theory and practice with moral integration, case-based study, critical thinking, multidisciplinary work, and love for the nursing profession. Instructor tasks are to elicit student ideas, provide opportunities to learn, support infrastructure, support technology use, and extra-curricular activities to develop the competencies of nursing students. Recommendations were that the curriculum administration should review the selection process of student candidates and instructional management to achieve expected outcomes of nursing characteristics in the future. The nurse lecturer should provide authentic and integrated instruction, decrease lecturing, cultivate a lifelong learning process, and sustain the nursing characteristics.

연관관계 규칙을 이용한 학생 유지율 관리 방안 연구 (A Study on Management of Student Retention Rate Using Association Rule Mining)

  • 최근 학령인구 감소에 따라 많은 문제점들이 나타나고 있다. 우리나라는 인구대비 가장 많은 대학을 보유하고 있기 때문에 각 대학의 생존에 필요한 최소한의 학생 유지율 관리가 점점 더 중요해 지고 있다. 따라서 본 연구는 계속되는 학력인구의 감소에 따라 각 대학들이 생존 방안으로 학생 유지율의 적절한 관리 방안을 모색한다. 이를 위하여 특정 대학에 입학한 학생들을 대상으로 성별, 출신고, 출신지역, 성적, 졸업여부 등의 데이타를 분석하여, 학생들이 입학에서 졸업에 이르기까지 지속적으로 유지될 수 있는 학생 유지율을 관리하기 위한 기본적인 방향이 어떤 것인지 알아본다. 또한, 최적의 입력 변수를 파악하고, 최적의 입력 파라메터를 기초로 apriori 알고리즘을 이용하여 연관 분석을 실행하여 유지율 관리에 가장 적합한 자료를 수집할 수 있도록 한다. 이를 바탕으로 각 대학들이 학생들을 모집하고 유지하는데 도움이 되도록 가장 효율이 높은 딥러닝(Deep Learning) 모듈을 개발하기 위한 기초 자료로 만들고자 한다. 의사결정트리를 활용하여 졸업여부를 측정한 결과는 딥러닝의 정확도 보다 낮은 75%로 나타났다. 의사결정트리에서 졸업여부를 결정하는 요인은 일반고를 졸업하고, 도시지역에 거주하면서 여성이면서 성적이 높은 학생들이 졸업확율이 높은 것으로 나타났으며 결과적으로 의사결정트리 보다는 개발된 딥러닝듈이 더 효율적으로 학생들의 졸업여부를 평가할 수 있는 모델로 나타났다.

일개 의과대학의 학생 상담 유형 분석 (An Analysis of Student Counseling Type in Medical School)

  • 본 연구는 세 명의 연구자가 참여한 상담 유형에 따른 특성에 대한 체계적인 분석을 위해, 가장 많은 사례수의 상담 유형은 무엇인지에 관한 연구이다. 유급/휴학 학생들의 주된 상담 유형을 파악하고, 입학유형별 상담 유형의 특성을 분석하였다. 2018, 2019년 진행된 총 185개의 상담 사례에 대하여 기술통계 분석과 함께 7가지 상담 유형별(정서 및 성격, 학업동기, 학습방법/전략, 가족관계, 동료/이성과의 관계, 진로 및 전공 선택, 자기성장 동기 등) 분석을 통해 학생들의 특성(입학유형, 학년, 성별, 학적변동(유급, 휴학 경험))에 따른 학생 특성별 상담 요구를 확인하였다. '학습방법과 전략'과 '정서 및 성격', '자기 성장 동기'의 순으로 상담이 가장 많은 것으로 확인되었고, 특히 1, 2학년 학생들의 상담 비율이 높은 것을 확인할 수 있었다. 유급/휴학을 경험한 학생들은 '정서 및 성격', '학습방법/전략'의 상담이 대부분이었다. 본 연구를 통해 1, 2학년 학생 대상의 학업 관련 프로그램이 필요하며, 학생 상담 유형별 특성을 고려하여 학교 차원의 상담 시스템 개선 및 구축의 필요성을 확인하였다. 또한, 학적변동 학생들에게 그룹 멘토링 제도 등의 제도적 차원의 지원이 필요성을 제안하였다.

대학입학전형에서 동일계 특별전형 합격생들에 대한 연구 (A study of college students who were granted special admissions for vocational high school students)

  • 1990년 이후 국내 저 출산 붐으로 인한 대학 입시생들의 감소로 인하여 2016년 이후는 대학 정원이 수험생들의 수요보다 많은 상황으로 예측된다. 이로 인해 지방대학들의 학생 충원이 대학의 존폐여부를 좌우할 만큼 학생수급 정도가 심각한 상황으로 부각되고 있다. 따라서 전국 대부분 대학들은 우수한 인재들을 선점하기 위한 특성화 및 전문화된 방식으로 신입생들을 선발하는 다양한 전형방법을 개발하여 시행하고 있다. 본 연구는 지방의 A국립대학교를 중심으로 동일계 특별전형 입학생들에 대한 통계적 분석을 통해 이들을 효과적으로 관리할 수 있는 방법을 찾아보고자 한다. 동일계 특별전형 입학생들을 대상으로 통계적 방법을 통한 재학현황과 학내 적응여부를 분석하고 계열별 성별과의 관계를 파악하여 보다 나은 신입생 선발을 위한 방법에 대해 살펴본다. 저 출산으로 인한 학생 수의 감소로 정원 외로 전형하는 동일계 특별전형을 통해 인재를 확보하는 방안으로 활용할 수 있으며, 이를 토대로 졸업 시까지 학생들을 추수 관리할 수 있다. 또한 다른 전형으로 확대하여 분석을 하고 서로 비교한다면 앞으로의 입시정책에 적합한 전형유형을 개발할 수도 있을 것이다.
