The development of standards and guidelines of rehabilitation nursing has been the major concerns for providing better nursing to the rehabilitation clients. As the patients with stroke and spinal cord injuries are the most prevalent physical disabilities in Korea, this study focussed on the nursing diagnoses of these two groups of patients. In order to identify the nursing diagnoses frequently used In their practice for the patients with stroke and spinal cord injuries, a survey was done with the questionnaire form developed by the research team. The surveyee were the staff nurses working at rehabilitation wards more than 2 years from 8 general hospitals in Korea, They identified and set the priorities of 13 nursing diagnoses from 79 stroke patients and 10 nursing diagnoses from 35 patients with spinal cord injuries during the periods from March 1 to June 2, 1999. The identified nursing diagnoses for the stroke patients are impaired physical immobility, sensory-perceptual alteration, activity intolerance, self-care deficit, altered defecation, altered urination, risk for injury, unilateral neglect, impaired skin integrity, altered thought processes, pain, altered health maintenance, dysreflexia. The identified nursing diagnoses for spinal cord injuries are altered urination, altered defecation, impaired skin integrity, pain, risk for injury, reflex incontinence, impaired physical immobility, self-care deficit, activity intolerance, knowledge deficit.
Objective: This study focused on subacute stroke patients who were asked to kick a ball while walking on a treadmill. The aim of the study was to determine the effect of a body weight-supported treadmill training (BWSTT) combined with ball-kicking on muscle strength, balance, and gait. Design: Single blind, randomized controlled trial. Methods: Twenty stroke patients who volunteered to participate in this study were randomly assigned to either the BWSTT combined with ball-kicking (BWSTT-BK; 10 participants) group or the BWSTT group (10 participants). Participants in the BWSTTBK group performed treadmill walking combined with simultaneous ball-kicking for 30 minutes daily for 5 weeks. Participants from the BWSTT group performed only treadmill walking. The muscular strength, balance, and gait ability were measured before and after the 5-week training. To assess for muscular strength, a digital muscle tester was used to measure hip flexor, knee extensor, and dorsiflexor strength. To assess for balance, the Berg Balance Scale (BBS) and Timed Up and Go Test (TUG) was used. To assess for gait, the 10 meter walk test (10MWT) and Functional Gait Assessment (FGA) was used. Results: The BWSTT-BK group showed significantly improved muscular strength, balance, and gait according to BBS, TUG, 10MWT, FGA, and digital muscle testing scores compared to the BWSTT group (p<0.05). In addition, within-group comparison showed significant improvement in all variables (p<0.05). Conclusions: These findings suggest that BWSTT-BK results in more favourable outcomes for stroke patients. Therefore, BWSTT-BK may be useful for the recovery of gait ability of stroke patients.

본 연구는 뇌졸중 환자를 대상으로 가상현실 프로그램에 대한 융복합적 효과를 알아보기 위해 메타분석을 실시하였다. PICOS 기준에 따라 국내 문헌을 검색하여 '뇌졸중', '가상현실 프로그램' 관련하여 최종 9편의 연구를 선정하였다. 개별 연구물의 비뚤림 평가는 각 연구의 설계에 따른 평가 도구를 이용하여 실시하였다. CMA 프로그램을 이용하여 메타분석의 효과크기를 산출하였다. 조절효과분석은 하위그룹분석과 메타회귀분석으로 실시하였다. 가상현실 프로그램의 일상생활활동 능력에 대한 전체 효과크기는 Hedges's g=0.302(95% CI: 0.064~0.540)이었다. 따라서 가상현실 프로그램은 뇌졸중 환자의 일상생활활동 능력을 증가시켜 가상현실 산업과 의료 보건 산업을 융복합한 의료산업에 긍정적인 영향을 제공할 수 있다.
Objective: The aim of this study is to investigate the effect of patellar taping on balance and gait abilities in chronic stroke patients. Design: Randomized placebo-controlled trial. Methods: Thirty chronic stroke patients who have been diagnosed at least six months or before were recruited from R hospital. These study subjects were randomized to the experimental group (n=15) or placebo group (n=15). In the experimental group, patellar taping was applied while for the placebo group, placebo taping was applied. The Balance System SD was used for measuring dynamic standing balance in these two groups. In addition, the GAITRite (CIR System Inc.) system was utilized for calculating gait performance in these patients. Results: After application of taping, the patellar taping group showed a significant decrease in dynamic standing balance in their sway area (p<0.05). However, in the placebo group, there was no significant difference in dynamic standing balance ability and gait ability before and after application of taping. Comparison of the patellar taping group and placebo group showed significant differences in dynamic standing balance ability and gait performance (p<0.05). Conclusions: From the results of this study, it appears that application of patellar taping in chronic stroke patients significantly improved dynamic standing balance ability and gait ability in these patients. Based on these results, patellar taping is thought to be useful in real clinical settings where there are many chronic patients who are in need of improvement in their balance and gait ability.
PURPOSE: This study sought to investigate the effects of treadmill gait training combined with a thoracic mobility exercise on gait and balance in patients with stroke. METHODS: In this single-blinded, randomized, controlled, comparative study, a total of 20 patients at a rehabilitation hospital who had suffered a hemiplegic stroke were randomly assigned to the experimental group (treadmill gait training combined with a thoracic mobility exercise, n = 11) or control group (treadmill gait training without the thoracic mobility exercise, n = 9). All the participants underwent comprehensive rehabilitation therapy (5 × /week for 4 weeks). Additionally, the experimental group underwent 20 min of treadmill gait training combined with 10 min of a thoracic mobility exercise (3 × / week for 4 weeks) and the control group underwent the former but not the latter. Gait and balance were measured before and after the 4-week training. RESULTS: Significant improvements were observed in the 10-m walking test (10 MWT), timed up-and-go (TUG) test, center of pressure (COP) velocity, and COP length in the experimental group (p < .05). This group also showed a larger decrease in the 10 MWT and COP velocity than the control group (10 MWT, -3.02 sec vs. -1.68 sec, p < .05; COP velocity, -.07 mm/sec vs. .08 mm/sec, p < .05). CONCLUSION: Treadmill gait training, combined with the thoracic mobility exercise, could be effective in improving the gait and balance of stroke patients. It could also be more effective in improving walking speed and static balance than the treadmill gait training alone.
Recently rotary-arc, thermal expansion and their composite interrupters are widely used in the distribution power system because they have lots of advantages in making the larger interrupting capacity, the smaller size, the lighter weight and the less surge. A model interrupter of rotary-arc type, which has constant stroke and thermal expansion volume, was studied by varying the design parameters, i.e. the number of turns of the driving coil, the inner diameter of the moving contact, the gas pressure and the shape of the fixed contact for this project. Short cicuit current interrupting tests were conducted to the model interrupters by varying the requirements from 42% to 175% of the test voltage, interrupting current and transient recovery voltage for the test duty No.4 of 7.2kV 12.5kA single phase test. The pressure rise, minimum and maximum arcing times were analyzed for each model interrupter. All types of model interrupters showed good interrupting performances and sufficient design margins for the ratings.
만성 뇌졸중 환자를 대상으로 메타분석 기법을 활용하여 트레드밀 훈련의 융복합적 효과성을 평가하고자 한다. PICOS 기준에 의거하여 문헌을 검색한 후 총 22편의 국내 '뇌졸중', '트레드밀' 관련 연구물을 선정하였다. 메타분석을 위해 CMA 프로그램을 이용하여 효과크기를 산출하였으며, 국내 트레드밀 훈련의 전체 효과크기는 0.661(95%CI: 0.456~0.865)로 중간크기로 유의한 차이가 있었다(p<.05). 트레드밀 훈련의 효과를 족저압(1.147), 하지 근 기능(0.875), 균형(0.664)으로 나누어 효과크기를 비교한 결과, 족저압의 효과크기가 가장 크게 나타났다. 추가로 균형은 하위항목들을 나누어 분석하였고, 균형은 TUG(0.553), BBS(0.760), 정적균형지수(0.654)의 효과크기가 나타났다. 따라서 트레드밀 훈련이 만성 뇌졸중 환자의 삶의 질 향상에 긍정적인 영향을 제공한다. 트레드밀 훈련에 대한 메타분석 연구는 정보, 통신 및 의료기술의 헬스케어 융합을 향한 업계 패러다임의 전환으로 이어질 수 있을 것이다.
For the past years, higher power rating 2 stroke super long stroke diesel engines having more than 8 cylinders and larger cylinder bore are installed mainly on very large containerships to save on fuel consumption. However, these engines are prone to X-mode vibration due to 2nd node torsional vibration or the X-type moment, particularly because of the increase in total length and height. Recently, cases of excessive X-mode vibration often occurred on engine's major components. This vibration is manifested also as secondary vibration causing failure in engine-mount large structures. This study investigated the excitations caused by the 2nd node propulsion shafting torsional vibration that influence X-mode vibration of the main engine and practical countermeasures are proposed. An 8RT-flex82T 8 cylinder engine and 11S90ME-C 11 cylinder engine for a container ship was used as research model.
Diesel engine has many advantages such as high thermal efficiency, low fuel consumption and low emission of CO2. However, the diesel engine faced with strengthened emission regulation about NOx and PM. To suppress NOx emission, after-treatment systems such as Lean NOx Trap (LNT), Selective Catalytic Reduction (SCR) are considered as a more practical strategy. This paper investigated the performance of Lean NOx trap of the 4 stroke diesel engine which had a LNT catalyst. Characteristic of exhaust emission at NEDC mode was analyzed. From this result, the effect of nozzle attaching degree, injection quantity and gas flow change on NOx conversion performance was clarified.
