Objective : In posttraumatic stress disorder (PTSD), it is essential to evaluate the severity of trauma with a reliable instrument. The combat exposure scale (CES) is one of the most widely used measures for the combat-related trauma. The present study was conducted to test the reliability and validity of the Korean version of CES (CES-K). Methods : One hundred and forty-five male Korean veterans of the Vietnam War participated in this study. CES-K, the structured clinical interview for DSM-III-R (SCID), clinician administered PTSD scale (CAPS), and the Korean version life events checklist (LEC-K) were administered. Results : Cronbach's coefficient of CES-K was .85, and the test-retest reliability was .94. The mean [standard deviation (SD)] scores of CES-K were 20.4 (9.0) in the PTSD group and 12.0 (8.6) in the non-PTSD group (p<.001). CES-K showed a significant correlation with LEC-K (r=.31, p<.001) and CAPS (r=.52, p<.001). Only one factor was revealed by the factor analysis. Conclusion : CES-K showed good reliability and validity for assessing the severity of combat exposure. Further, it demonstrated comparable psychometric properties to the previous study. It is expected that CES-K will be a useful tool for evaluating the severity of combat exposure in Korea.
Objective : The purpose of this study was to investigate differences in anxiety and depression symptoms, comorbidity according to the patterns of temperament and character in patients with posttraumatic stress disorder (PTSD). Methods : The temperament and character inventory (TCI), beck depression inventory (BDI) and beck anxiety inventory (BAI) were administered to 151 PTSD patients classified into four groups of adaptation, vulnerable temperament, immature personality and composite vulnerability according to the results of the Temperament and Character Inventory (TCI). MANOVA and Chi-square tests were conducted to analyze differences in BDI, BAI, temperament and character scores and rate of comorbid disorders between the four groups. Results : The immature character and complex vulnerability group showed the higher rate of comorbid depression disorder. Anxiety and depression severity were significantly different among groups, especially depression severity had higher scores in the immature character and complex vulnerability groups and anxiety severity had higher scores in the complex vulnerability group than adaptive group. The immature character and complex vulnerability groups showed significantly lower score on the temperament scale of reward dependence and persistent. Conclusion : The results demonstrate the significance of adaptive characteristics on anxiety and depression symptoms regardless of vulnerable temperaments, and its consequent role in the management of character factors relative to intervention regarding PTSD.

Objectives: Anxiety disorders are the most common psychiatric disorders in adolescents and seem to occur the earliest among all forms of psychopathology. The aim of this study was to investigate the association of anxiety severity with health risk behaviors and mental health in adolescents. Methods: Data from the 2020 Korean Youth Risk Behavior Web-Based Survey were analyzed. A total of 54948 adolescents responded to the 7-item Generalized Anxiety Disorder Scale (GAD-7) for the assessment of their anxiety severity as well as to the mental health and health risk behavior survey. Logistic regression analysis, t tests, and variance analysis of a complex sample general linear model were used to examine the association of anxiety severity with health behaviors and mental health. Results: After statistical adjustment for sociodemographic characteristics, the subjects in the severe anxiety group were significantly more likely to be current smokers (odds ratio [OR]: 2.08, 95% confidence interval [CI]: 1.72-2.50), current drinkers (OR: 1.91, 95% CI: 1.67-2.19), experience habitual substance use (OR: 10.89, 95% CI: 8.22-14.42), have sexual intercourse (OR: 2.10, 95% CI: 1.76-2.51), and have unprotected intercourse (OR: 2.21, 95% CI: 1.67-2.92) than those in the normal group. Anxiety severity negatively correlated with sleep satisfaction and happiness, but positively correlated with stress perception, loneliness, depressive symptoms, and suicidality. Conclusion: Adolescent anxiety is associated with health risk behaviors and poor mental health. Thus, early screening and intervention for anxiety in adolescents could contribute to the management and coping of youth health risk behaviors in the community.
Purpose: The main purposes of this descriptive survey study were to describe parenting stress and needs of mothers of children with atopic dermatitis. Method: Data were collected from 111 mothers of children under 6 year-old with atopic dermatitis registered at one university affiliated medical center in Seoul. The instruments used for this study were a self-report questionnaire, Parenting Stress Index (PSI) Short Form by Abidin, and an open question to identify nursing needs. Descriptive analysis, Pearson correlation and contents analyses were done. Result: The mean score for parenting stress was 77.25 (range: 12-60), for parent domain 31.50 (range: 12-60), parent-child domain 21.02 (range: 11-55) and child domain was 24.72 (range: 11-51). There was a significant positive relationship between parenting stress and severity of symptom (r=0.192, p<.005). The maternal stressors included extra housework related to symptom management, treatment related to atopic dermatitis, personality and behavior of child, and financial problems. Conclusion: the mothers of children with atopic dermatitis have high parenting stress from various stressors and needs. It is recommended that nursing interventions should include stress management and take into consideration the educational needs of mothers.
The relationship of stress and psychopathology with subjective or objective tinnitus intensity was investigated in 43 patients with tinnitus. Global assessment of recent stress scale, Beck depression inventory and symptom checklist-90-revision were used to measure stress perception and psychopathology. Subjective tinnitus intensity was assessed by subjective tinnitus severity inventory, whereas objective tinnitus intensity was assessed by tinnitogram. Scores of perceived stress related to interpersonal relationship and sickness or min had significantly positive correlation with subjective tinnitus intensity. In psychopathology, scores of somatization, interpersonal sensitivity, depression, anxiety, phobia, paranoid ideation, psychoticism had significantly positive correlation with subjective tinnitus intensity. However, stress perception and psychopathology did not significantly correlate with objective tinnitus intensity. These findings suggest that tinnitus may be associated with stress perception and multiple psychopathology including anxiety and depression. Thus, it is emphasized that Psychosocial intervention as well as effective consultation-liasion activity is needed for evaluation and treatment of patients with tinnitus.
With advances in neonatal medicine, smaller and sicker infants are surviving premature birth. As these tiny patients battle for their lives, their parents also struggle with psychological consequences of an unexpected role. The birth of a sick infants has long been documented as stressful events for the parents. High stress levels may alter parents' interaction with their baby and impair their communication with health professionals, which can in turn affect parents' participation in care planning and decision making. Nursing interventions aimed at reducing the parental stress and anxiety levels during this crisis may have a positive impact on their ability to form an attatchment to their baby and make prudent decisions about his care. The planning of such interventions would be inhanced if the contributions of various factors to increased parental stress levels were identified. This descriptive study was conducted to understand the contents and degree of parental stress in the NICU during their premature's hospitalization, and to give a baseline data in developing nursing intervention program. Subjects were the 60 mother of hospitalized newborn in NICU of 2 University Hospital in Taejon City from April 1st, 1999 to June 30th, 1999, who agreed to take part in this study. The instrument used in this study were Parental Stressor Scales:NICU(PSS:NICU) developed by Miles et al.. and validated by 3 NICU practitioners and 3 child health nursing faculties. The questionnaire has 4 dimensions and 45 items; sight and sounds of NICU(5 items), babies' appearance and behavior nursing intervention(19 items), parental role alteration and relationship with their baby(10 items), health team communication(11 items). The questionnaire asks parents to rate each item on a five-point Likert type scale that ranges from "not stressful" (1) to "extremely stressfu1"(5). Total scores representing overall stress from the NICU environment are calculated by summing response to each item. A high score indicates high stress. A subscale score is calculated by summing the responses to each item in the subscale. Cronbach's ${\alpha}$ coefficients were .93. The data was analyzed as average, Frequency, Standard deviation, t-test, ANOVA, Pearson correlation coefficient by use of SPSS/PC+. The results of this study is summarized as follows ; 1. The total perceived stress level score of mothers of premature baby was slightly high($3.66{\pm}1.1$). The highest scored dimension was 'relationship with their baby and parental role change'($4.21{\pm}0.9$), and next were 'appearance and behavior of the baby'($3.92{\pm}1.1$), 'communication with health team'($3.32{\pm}1.3$), 'sight and sounds of NICU'($3.30{\pm}1.1$). 2. There were statistically significant correlation noted ; mother's perceived severity of the baby's condition(F=8.0012, P= .0125) and baby's gestational age(r = -.4724, P = .0500). In summary, information about physical environment of NICU, the mother's perceived severity of premature baby's state, maternal role change related variables and the knowledge of characteristics of premature baby must be included in nursing intervention program of mother's of premature baby in reducing the parental stress and anxiety level. It is suggested that each NICU needs to develop a nurse managed supportive care program for parents of premature baby. Also, it is suggested that there need to investigate the coping mechanism of mother of premature baby.
Purpose: This study was to compare and examine the factors influencing burden of primary family caregivers according to the severity of illness of elderly patients admitted in an intensive care unit. Methods: Subjects were the families of elderly patients in intensive care units of K, S and Y hospitals in Seoul. Data were collected from March to October 2007. Subjects were 108 persons over age 65. Data were analyzed by SAS statistics. Results: First, groups 5 and 3 showed higher burden than that of group 4. Second, high correlation was found between stress and burden, stress and anxiety, and burden and anxiety. Third, factors influencing family burden were found to be stress for group 5, stress, anxiety, and monthly income for group 4, and stress and patient age for group 3. Conclusion: Specific nursing interventions to decrease the stress of primary family caregivers of serious ill elderly patients in an intensive care unit are needed. Additionally, more effective and systematic activation of a long-term medical insurance system for seriously ill seniors is considered necessary to mediate the burden of primary family caregivers.
Purpose: This study was performed to examine the frequency of irritable bowel syndrome(IBS), perceived stress, and mental health among community-dwelling women. Methods: This study conducted a survey to collect data on 869 women in Seoul and Gyeonggi-do. The measurements included the Rome III criteria(to diagnose IBS), the Global Assessment Recent Stress(GARS scale to evaluate perceived stress), and symptom check list 90R(SCL-90R to evaluate psychological distress). Data were analyzed using SPSS 15.0 program by frequency, $x^2$-tests, t-test, and analysis of variance(ANOVA). Results: The frequency of IBS in women was 15.1%. Eighty-six women(65.6%) had mixed constipation and diarrhea subtype of IBS. Compared to the women without IBS, those with IBS reported significantly higher scores on GARS, all subscales of GARS, SCL-90R, all subscales of SCL-90R, and the global severity index(GSI). No significant differences were observed among stress, mental health, and the IBS subtypes. Conclusion: Nursing interventions targeting women with IBS should be developed in order to reduce the associated stress and psychological distress.
Objective Trichotillomania is a relatively common illness whose neurobiology is poorly understood. One treatment for adult trichotillomania, n-acetyl cysteine (NAC), has antioxidative properties, as well as effects on central glutamatergic transmission. Preclinical models suggest that excessive oxidative stress may be involved in its pathophysiology. Methods Adults with trichotillomania provided a blood sample for analysis of compounds that may be influenced by oxidative stress [glutathione, angiotensin II, ferritin, iron, glucose, insulin and insulin growth factor 1 (IGF1), and hepcidin]. Participants were examined on symptom severity, disability, and impulsivity. The number of participants with out-of-reference range oxidative stress measures were compared against the null distribution. Correlations between oxidative stress markers and clinical measures were examined. Results Of 14 participants (mean age 31.2 years; 92.9% female), 35.7% (n=5) had total glutathione levels below the reference range (p=0.041). Other oxidative stress measures did not have significant proportions outside the reference ranges. Lower levels of glutathione correlated significantly with higher motor impulsiveness (Barratt Impulsiveness Scale sub-score) (r=0.97, p=0.001). Conclusion A third of patients with trichotillomania had low levels of glutathione, and lower levels of glutathione correlated significantly with higher motor impulsiveness. Because NAC is a precursor for cysteine, and cysteine is a rate limiting step for glutathione production, these results may shed light on the mechanisms through which NAC can have beneficial effects for impulsive symptoms. Confirmation of these results requires a suitable larger follow-up study, including an internal normative control group.

Objective:The differences of various neurocognitive functions, including attention, memory, motor function, and higher cognitive function were compared between PTSD patients and normal control subjects. Also, correlation with PTSD symptom severity and neurocognitive functions were evaluated between PTSD patients and normal control subjects. Method:We assessed the neurocognitive functions by computerized neurocognitive test(CNT) batteries. The visual continuous performance test(CPT) and digit span test, finger tapping test and Wisconsin card sorting test(WCST) were executed. The Impact of Event Scale-Revised(IES-R) was used in the evaluation of the severity of PTSD. Result:The PTSD patients showed significantly impaired neurocognitive performance in all of the items, compared with normal control subjects. The relation between impairment in neurocognitive functions and symptom severity showed significant correlations. Conclusion:These results imply that PTSD patients have impaired neurocognitive functions concerning with specific brain areas, especially the frontal area. For the thorough evaluation of further neurocognitive functions, more detailed evaluation items of neurocognitive functions and brain imaging studies are necessary in the future study.
