공간적 가중을 이용한 강화 유무에 따른 3주간 수축-이완 중재가 넙다리뒤근 단축 대상자의 유연성과 균형능력에 미치는 효과 (Effects of Three-Week Contract-Relax Interventions with and without Reinforcement Using Temporal Summation for Flexibility and Balance Ability in Young People with Hamstring Shortening)

  • Purpose: The purpose of this study was to examine the effect of three-week contract-and-relax (CR) interventions with and without reinforcement using temporal summation for flexibility and dynamic balance ability in young people with hamstring shortening. Methods: This study was conducted on 20 female college students with hamstring shortening. The participants were divided equally into two groups using stratified randomization: the CR group (CRG) and the CR with reinforcement group (CRRG). All interventions were applied three times a week for three weeks. The passive straight leg raise (PSLR) test and functional reaching test (FRT) were conducted on each participant before and after the three-week intervention. Results: In both groups, PSLR and FRT improved significantly after the three-week intervention compared to before intervention (p < 0.01). The amount of change in PSLR after the three-week intervention was significantly higher in CRRG than in CRG (p < 0.05). Conclusion: Three-week CR interventions with and without reinforcement were effective in improving PSLR and FRT. To improve hamstring shortening, CR intervention with reinforcement may be more useful than CR intervention without reinforcement.

Immediate effects of a neurodynamic sciatic nerve sliding technique on hamstring flexibility and postural balance in healthy adults

  • Objective: In this study, we applied a neurodynamic sciatic nerve sliding technique to healthy adults to elucidate its effects on hamstring flexibility and postural balance. Design: Cross-sectional study. Methods: This study targeted twenty four healthy adults (16 men, 8 women). A neurodynamic sciatic nerve sliding technique was applied 5 times to all subjects' dominant leg. The subjects were asked to sit on the bed while performing cervical and thoracic flexion, as well as knee flexion with ankle plantar flexion. Then, they were asked to perform cervical and thoracic extension and knee extension with their ankle in dorsiflexion and maintain the position for 60 s. For postural balance, we measured postural sway while the subjects maintained a one-legged standing posture using the Good Balance System and measured the hip joint flexion range of motion using a standardized passive straight leg raise (SLR) test. Results: SLR test increased significantly from $79^{\circ}$ before the intervention to $91.67^{\circ}$ after the intervention (p<0.05). Regarding the participants' balance evaluated using the one-legged standing test, the X-speed decreased significantly from 18.61 mm/s to 17.17 mm/s (p<0.05), the Y-speed decreased from 22.28 mm/s to 20.52 mm/s (p<0.05), and the velocity moment was significantly decreased from $89.33mm^2/s$ to $74.99mm^2/s$ after the intervention (p<0.05). Conclusions: Application of the neurodynamic sciatic nerve sliding technique exhibited improved hamstring flexibility and postural balance of healthy adults.

운동유무가 20대 성인여성의 FMS점수에 미치는 영향 (The Effects of Regular Exercise on the FMS Score in 20s Females)

  • Purpose : The purpose of this study is to evaluate the stability and balance of the body, mobility complex exercise group and a student group for FMS tests to compare the differences between the two groups to identify its purpose. Method : The subjects of this study FMS measuring D University Physical Therapy, a student enrolled patients (7:7), and 7 patients total of 14 patients was conducted, compound exercise group weekly stretching 10 minutes, the 24 members who have run a complex exercise, 10 minutes of cool down stretching, 20-minute aerobic exercise were included in the study. Result : The results of this study, according to 1) Deep squat, Inline lunge, Trunk Stability Push-up there was significant difference(p<0.05), compound exercise group came out significantly higher. 2) Hurdle Step, Shoulder Mobility Reaching, Active Straight-leg Raise, Rotary stability in measured target these women flexibility because the test items that did not show a significant difference(p>0.05), the two groups averaged compared to the other items were higher. Complex exercise group, a statistically significant difference was overall average. Conclusion : FMS can not be resolved because of a compensatory mechanism to identify disparities and unbalanced movement patterns can help. Therefore, if the FMS and other tests conducted by splicing, will be utilized more profitably, and you will be able to suggest ways that can have a positive impact injury prevention is added to the side to expand the scope of the physical therapist.

Effects of Electric Stimulation with Static Stretching on Hamstrings Flexibility

  • Purpose: Flexibility has been considered one of the most important goals in rehabilitation. This study aimed to investigate the effect of transcutaneous electrical nerve stimulation (TENS) with the static stretching technique on the flexibility of hamstrings. Methods: Twenty-four subjects (15 men, 9 women) with limited hamstrings flexibility received three different intervention sessions in random order. The treatment sessions included static stretching (SS), static stretching with motor-level TENS (SS with motor TENS) and sensory-level TENS (SS with sensory TENS). All sessions of SS were performed in the straight-leg raise position for 30 seconds followed by rest for 15 seconds, in repetitions for 10 minutes using a belt. The TENS groups underwent TENS stimulation ($40{\mu}s$, 100 Hz) during the stretching for 10 minutes. Outcome measures were evaluated according to active knee extension (AKE) and recorded before the session and at 0, 3, 6, 9, and 15 minutes after the session. Results: There was significant improvement in hamstrings flexibility within all groups (p<0.05). SS with TENS (both sensory and motor) maintained significant increases in knee extension range of motion until 15 minutes post-treatment. In contrast, the SS-only group maintained significantly increased hamstrings flexibility only until 6 minutes post-treatment (p<0.05). Conclusion: Improvement in hamstrings flexibility with SS with TENS was maintained longer than with SS-only intervention. Electrical stimulation with stretching may be more effective than SS alone for increased duration of maintained hamstrings flexibility.

좌골신경가동화기법이 만성 요통 환자의 통증과 슬관절 신전근력에 미치는 영향 (Effects of Sciatic Nerve Mobilization Technique on Perceived Pain and Knee Strength in Patient With Chronic Low Back Pain)

  • The purpose of this study was to determine the effect of sciatic nerve mobilization technique on perceived pain, straight leg raise test (SLR), and strength of knee extensor, location of symptoms (LOS) in patients with chronic low back pain. 22 patients with chronic low back pain were recruited for this study. The subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with 11 patients in each group. All patients received a routine physical therapy (hot pack and transcutaneous electrical nerve stimulation). The mobilization technique of the sciatic nerve was performed for 10 min in the case of the EG subjects. Outcome measurements included the level of the perceived pain, SLR, and strength of the knee extensor, LOS. The measurements were recorded 3 times: before the intervention, after the intervention, and at 1 hour of follow up. The two groups did not significantly differ with regard to the level of perceived pain, SLR, and strength of the knee extensor, LOS before the test (p>.05). In the case of the EG subjects, all the variables measured after the intervention significantly differed from those measured before the intervention (p<.05). However, in the case of the CG subjects, a significant difference was noted only with regard to the level of perceived pain (p<.05). The findings indicate that sciatic nerve mobilization technique exerts a positive effect on the control of subjective symptoms and knee strength in patients with chronic low back pain. Further studies are required to generalize the result of this study.

복부 동시수축 형태에 따른 하지 근활성도 비교 (Comparison of Lower-Limbs Muscle Activity according to the Abdominal Co-contractive Activation)

  • 본 연구는 복부 동시수축 형태에 따라 하지의 근활성도에 어떤 영향을 미치는지에 대해 알아보고자 하였다. 연구대상자는 허리와 하지에 통증이 없고, 근골격계에 이상이 없는 건강한 20대 여성 30명을 대상으로 진행하였다. 복부 이완(Rest), 복부 당기기 기법(ADIM), 복부 브레이싱 기법(ABM)을 유지하는 각 자세에서 무릎의 굽힘없이 발을 능동적으로 바닥으로부터 20cm 올린 상태(ASLR)로 약 10초간 유지하도록 하여 하지의 5개 근육(중간볼기근, 엉덩긴모음근, 넙다리곧은근, 넙다리두갈래근, 앞정강근)의 근활성도를 측정하였다. 연구결과, Rest에서 ASLR을 유지하였을 때보다 ADIM 수축형태로 ASLR 유지할 때 하지의 모든 근육에서 근활성도가 높게 나타났다. 또한 Rest에서 ASLR을 유지하였을 때보다 ABM 수축형태로 ASLR을 유지하였을 때 중간볼기근을 제외한 나머지 근육에서 모두 근활성도가 높게 나타났다. ADIM과 ABM을 비교한 결과 중간볼기근과 앞정강근은 ADIM 수축형태에서, 엉덩긴모음근, 넙다리곧은근, 넙다리두갈래근은 ABM 수축형태에서 통계적으로 유의하게 높은 근활성도가 나타났다. 본 연구의 결과로 체간중심 안정화 운동이 필요한 요통 환자의 선제적 복부 수축형태로서 뿐만 아니라 하지의 선택적 근력강화가 필요한 환자들에게 개별화된 방법으로써 근거를 제시할 수 있을 것이다.

  • Objectives: This study is designed to compare clinical symptoms between Symptom-giving pelvic girdle relaxation and Separation of the pubic symphysis. And report the clinical effectiveness of Traditional Korean Treatment on two case. Methods: Two patients who delivery in one week ago had pain that located symphysis and around. Treatment was acupuncture, Placenta Herbal Acupuncture and herbal medicine. The progress of symptom and aggressiveness of pain and disability were measured by Visual Analogue Scale, McGill pain Questionnaire-short form, Oswestry Disability Index and Active Straight Leg Raise test. Results: After treatment most two patients' pain and disabilty were improved. At first case, width of symphysis pubis were increased in normal range. Conclusions: These cases shows that Traditional Korean therapy might be effective in decreasing symptoms on separation of symphysis pubis during delivery and Symptom-giving pelvic girdle relaxation.

뒤통수밑근 억제기법과 조합기법이 넙다리뒤근 단축 대상자의 넙다리뒤근 유연성에 미치는 영향 (Effect of suboccipital muscle inhibition and combination technique on the flexibility of hamstring in individuals with shortened hamstring)

  • Purpose: The purpose of this study is to compare changes in the flexibility of hamstring muscles which are relaxed on suboccipital muscle through suboccipital muscle inhibition and combination technique. Methods: Thirty sample subjects (16 male and 14 female) were randomly divided into an experimental group (n=15) and a control group (n=15). 1. Suboccipital muscle inhibition: Suboccipital muscles of the patients were placed on a bed. The occipital region was placed on a therapist's fingers, and the posture was maintained for three minutes before remeasurement. 2. Treatment with combination technique (body bolster and wooden pillow): Wooden pillows were placed where the participants could relax the posterior arch of the atlas; additional body bolsters were also placed to avoid high pressure, and the treatment was maintained for three minutes before remeasurement. Results: There was a statistical difference in finger floor distance (FFD) and straight leg raise (SLR) results for those treated with SMI (P<0.05). There was no statistical difference in FFD and SLR results for those treated with the combination technique (P>0.05). Conclusion: Between the two intervention methods; the suboccipital muscle inhibition technique was more effective in increasing the flexibility of subjects with a shortened hamstring than was the combination technique.

치료적 운동이 축구 특이적 골반 앞 기울임을 가진 유소년 축구선수 엉덩관절의 관절가동범위, 도수근력, 기능적 움직임 및 방사선학적 평가에 미치는 영향: 증례보고 (Effects of Therapeutic Exercise on Hip Joint Range of Motion, Manual Muscle Test, Functional Movement Screen and Radiological Evaluation in a Youth Football Player with Football-specific Anterior Pelvic Tilt: A Case Report)

  • PURPOSE: This study was conducted to determine the effects of therapeutic exercise on range of motion (ROM), the manual muscle test (MMT), functional movement screen (FMS) and radiological evaluation in a youth football player with football-specific anterior pelvic tilt (APT). METHODS: The subject of this case report was a 12-year-old youth football player, who presented with football-specific APT. Therapeutic exercise consisted of hamstring stretch, prone hip extension, abdominal crunch, bridging with isometric hip abduction, plank exercise with posterior tilt and posterior pelvic tilting exercise using a swiss-ball for 40 min/day, twice a week for 8-weeks. ROM, MMT, FMS (deep squat, hurdle step, inline lunge, shoulder mobility, active straight-leg raise, rotary stability and trunk stability push-up) and radiographs (lumbar lordotic and sacral horizontal angle) were analyzed before and after week 8 of therapeutic exercise. RESULTS: The ROM, MMT, and FMS increased and the lumbar lordotic angle and sacral horizontal angle improved after 8-weeks of therapeutic exercise. CONCLUSION: The results of this case report suggest that therapeutic exercise improves ROM, MMT and radiography associated parameters in youth football players with football-specific APT. These findings have clinical implications for therapeutic exercise in youth football players with football-specific APT.

넙다리뒤근이 단축된 성인에게 시행된 근에너지기법과 도구를 이용한 연부조직가동술이 관절가동범위, 근력, 근 두께에 미치는 영향 (The Effect of Muscle Energy Technique and Instrument Assisted Soft Tissue Mobilization in Adults with Shortened Hamstring on the Range of Motion, Muscle Strength and Muscle Thickness)

  • Background: This study aimed to investigate the effects of muscle energy technique (MET) and instrument assisted soft tissue mobilization (IASTM) on knee extension ROM, knee extensor/flexor strength and muscle thickness immediately and after 24 hours. Methods: A total of 30 subjects participated in this study. The participants were assigned to either MET (n=15) or IASTM (n=15). 90-90 straight leg raise, knee extensor/flexor muscle strength, muscle thickness test were measured before, immediately after and 24 hours after the intervention. Results: Both groups significantly improved knee extension ROM on immediate (MET 10.7°, IASTM 10.21° increased) and after 24 hours assessment (MET 5.61°, IASTM 5.47° increased)(p<.05). In the MET group, knee extension and flexion muscle strength increased immediately after intervention (p<.05). In the IASTM group, knee extension muscle strength increased and knee flexor muscle strength decreased immediately after intervention (p<.05). Furthermore, both groups showed a pattern of returning to the initial strength after 24 hours. In both groups, no significant difference in muscle thickness immediately and after 24 hours was observed (p>.05). Conclusion: According to the results of the present study, MET and IASTM technique showed lasting effectiveness in flexibility of shortened hamstring immediately after and in 24 hours after the intervention. In both groups, MET increased muscle strength and increased ROM, while IASTM decreased muscle strength and increased ROM, with no change in muscle thickness.
