Object-Oriented Stereotactic Radiosurgery Planning System (객체 지향 개념을 이용한 뇌정위 방사선 수술 계획 시스템)

  • In this paper, we present an object-oriented stereotactic radiosurgery planning system, which accepts medical images such as CT and angiography, transforms the coordinates to a reference frame coordinate, calculates dose distributions, and finally displays isodose curves over the images. The user finds an adequate one for radiosurgeries after performing computer simulations on different treatment parameter sets. The object-oriented design concept was fully applied to the system composed of seven manager objects of different classes: a patient information manager, a user-interface manager, a coordinate transformation manager, a blackboard manager, a dose calculation manager, an isodose curve display manager, and a report manager. All the user interactions are carried out through the use of mouse buttons. The performance of the system was verified by four physicians and two medical physicists, and now is being used in two clinical sites.

Vim Thalamotomy for Intractable Rubral Tremor Associated with Midbrain Tumor - Case Report - (중뇌종양에 의한 적핵진전에 대한 Vim 시상핵절제술 - 증례보고 -)

  • This 26-year-old man gradually developed a disabling midbrain tremor involving both distal and proximal part of left upper arm. On neurologic examination, oculomotor palsy, and ataxia of the left arm were noted. Radiologic examination revealed a mass lesion on midbrain tegmentum. He was tentatively diagnosed as brain tumor (presumably germinoma) without stereotactic biopsy. Conventional radiation therapy was given for this lesion. Although there was improvement in the radiologic imaging, his midbrain tremor worsened and became untolerable. The authors performed MR-guided stereotactic Vim-thalamotomy. With macroelectrode stimulation and radiofequency lesioning, his resting, postural and action tremors were almost completely abolished in both distal and proximal part of left upper extremity. Authors consider that Vim thalamotomy is still an effective means of controlling midbrain tremor involving proximal upper limb.

  • In the past 10 years, modern technology has made deep seated obscure lesions visible. With development of computer technology and various stereotaxic techniques, many new procedures. refinement of old procedures, and development of new applications are possible. The authors are intended to provide a detailed description of our experience with the Brown-Roberts-Wells(BRW) stereotactic system in the evaluation and management of 90 patients with intracranial lesions, and to provide cases presentation of various inaccessible intracranial lesions.
