수술 후 재발된 메니에르병 환자에서의 성상신경절 차단술 -증례 보고- (Stellate Ganglion Block for Postoperative Recurred Meniere's Disease -A case report-)

  • Meniere's disease is a disorder of the membranous labyrinth that is characterized by vertigo, hearing loss, tinnitus and ear fullness. The etiology of Meniere's disease is yet to be described and the management is also very difficult. Fifty-one years old female patient had paroxysm of severe tinnitus and dizziness after medical therapy and surgical intervention during several years. Patient received stellate ganglion block (SGB) and her symptoms were completely relieved. Therefore, SGB could be considered another therapeutic modality for Meniere's disease.

  • Stellate ganglion block is most commonly used in pain clinic because it is an easy procedure and it has broad indications reported that Angina pectoris, tachyarrhythmia and long QT syndrome (LQTS) are indicated. LQTS is a disorder of the abnormalities of cardiac sympathetic innervation and of myocardial repolarization. LQTS is characterized by marked prolongation of the QT interval, often manifestating as syncope, seizures, or sudden death due to polymorphic ventricular tachyarrhythmia known as torsades de pointes. Treatment of symptomatic patients usually begin with beta blocker. The elective treatment of LQTS patients unresponsive to beta blocker is the left cardiac sympathetic denervation. We report a case of LQTS patient who had received stellate ganglion block.

성상신경절 차단후 총경동맥, 액와동맥, 상완동맥, 요골동맥의 혈류속도변화 (Flow Velocity Changes of Carotid, Axillary, Brachial and Radial Artery after Stellate Ganglion Block)

  • Stellate ganglion block (SGB) is applicated frequently to increase the blood flow and to reduce the pain in head, neck and upper extremity. The effects of SGB are able to be estimated by clinical signs and symptoms of Horner's syndrome, skin warmth, anhydrosis, etc. The effects are also estimated by sympathetic function and the blood flow. Blood flow velocities and pulsatility indices of common carotid,d axillary, brachial and radial artery were measured by Doppler flowmeter after SGB with 1% lidocaine at C6 level. Blood velocities of all arteries were increased and pulsatility indices of all arteries were decreased. This results suggest that SGB increase the blood flow of head and upper extremity and Doppler flowmeter is a good indicator of the effects of SGB.

안면신경마비에 대한 성상교감신경절 차단요법 (Treatment of Bell's Palsy by Stellate Ganglion Block)

  • Bell's palsy is not a serious disease and recovery is spontaneous in more half of the patients, but this recovery is long delayed in many instances and the cosmetic effect of an altered facial appearance is psychologically distressing. The etiology is unknown, but Kettle's ischemic hypothesis has been widely accepted. The aim of treatment is to reduce edema and improve circulation to the facial nerve. Stellate ganglion block (SGB) resulted in abolishing cerebral vascular spasm and in increasing cerebral blood flow. Thus, stellate ganglion block is effective in treatment of Bell's palsy. From 1978 to Oct. 1988, we have treated 222 patients (20 patients were recurred cases) with facial palsy by SGB and analysed the effect of SGB. We noticed that SGB was very effective in early repetitive treatment (90% recovery). This study is reported and the literature is reviewed.

The Change in Regional Cerebral Oxygen Saturation after Stellate Ganglion Block

  • Background: Stellate ganglion block (SGB) is known to increase blood flow to the innervations area of the stellate ganglion. Near infrared spectroscopy reflects an increased blood volume and allows continuous, non-invasive, and bedside monitoring of regional cerebral oxygen saturation ($rSO_2$). We investigated the influence of SGB on bilateral cerebral oxygenation using a near infrared spectroscopy. Methods: SGB was performed on 30 patients with 1% lidocaine 10 ml using a paratracheal technique at the C6 level and confirmed by the presence of Horner's syndrome. The blood pressure (BP), heart rate (HR) and rSO2 were measured before SGB and 5, 10, 15 and 20 minutes after SGB. Tympanic temperature of each ear was measured prior to SGB and 20 minutes after SGB. Results: The increments of the $rSO_2$ on the block side from the baseline were statistically significant at 5, 10, 15 and 20 minutes. The $rSO_2$ on the non-block side compared with the baseline, however, decreased at 15 and 20 minutes. The difference between the block and the non-block sides was significant at 15 and 20 minutes. The BP at 10, 15 and 20 minutes was increased and the HR was increased at 10 and 15 minutes. Conclusions: We observed an increment of the $rSO_2$ on the block side from the baseline; however, the $rSO_2$ on the non-block side decreased.

수장 다한증 환자에서 성상신경절 차단의 효과 (The Effect of the Stellate Ganglion Block on Palmar Hyperhidrosis)

  • We evaluated the effects of the stellate ganglion block(SGB) on the palmar hyperhidrosis. Ten patients of the palmar hyperhidrosis were taken right and left SGBs, 15 times on each side, total of 30 times, with 1% mepivacaine HCl 5 ml, with no discrimination on sex and age. Although there was a little decrease in the frequency of perspirations on 2 patients after the 15th block, no difference was noted after the overall 30th block at them. None of all 10 patients was satisfied symptomatically and no evidence of decreasing perspiration was found. Conclusively it seems that SGB with 1% mepivacaine HCl 5 ml is not an adequate therapy on the palmar hyperhidrosis even though it diminishes perspiration transiently.

  • Stellate ganglion block(SGB) is frequently performed in pain clinics. Facial palsy produces asymmetry of the face, lagophthalmos and incomplete eyelid closure. Exposure keratitis and eyeball pain can be induced by prolonged lagophthalmos. SGB was performed on a 51-year-old female patient who had exposure of keratitis and eyeball pain due to facial nerve palsy after an operation for chronic otitis media. After 31 stellate ganglion blocks, eyeball pain and keratitis nearly subsided. SGB is an effective method to treat lagophthalmos caused by facial nerve palsy.

편도선절제술후 성상신경절 차단이 제통효과에 미치는 영향 (The Effect of Stellate Ganglion Block in Controlling of Pain after Tonsillectomy)

  • Background : The tonsillar region is innervated by the sensory components of the glossopharyngeal nerve(IX) which communicates with certain part of cervical sympathetic ganglion. Some authors suggest stellate ganglion block(SGB) is effective for treatment of recurrent tonsillitis. The goal of this study was to evaluate the effect of SGB in controlling pain after tonsillectomy. Methods : Forty five patients, evaluated to ASA class 1 and 2, scheduled for tonsillectomy under general anesthesia were randomly assigned to 1 of 3 groups (group I: control; group II: SGB with 1% lidocaine 5 ml; group III: SGB with 2% lidocaine 2.5 ml plus 0.5% bupivacaine 2.5 ml), SGB was performed at the end of surgery. Postoperative pain was assessed with Numeric Rating Scale(NRS) NRS assessment was made 30, 60, 90 min, 2 h, 4 h, 24 h, 48 h after tonsillectomy. Results : Pain scores, after 30 min in group II, III and 60, 90 min in group III were significantly lower than group I(p<0.05). Conclusions : We found SGB was effective in controlling pain after tonsillectomy but further studies are required needs to prolong duration of relief..

성상 신경절 차단이 기관내 삽관에 따른 심혈관계 반응에 미치는 영향 (The Effect of Stellate Ganglion Block on Endotracheal Intubation)

  • The impact of cardiovascular changes occurred by endotracheal intubation is risky for patients with ischemic heart disease, or intracranial pathologic conditions typically impairing cerebral autoregulatory mechanisms. Therefore, multiple approaches have been utilized to limit the impact of intubation and reduce damage of central nervous and cardiovascular systems. These approaches include modifications in intubation technique to diminish circulatory stimuli and pharmacologic modifications of either the sensory afferent path or the circulatory response itself. We tried the stellate ganglion block, a kind of sympathetic block, for the same purpose and evaluated the results. The results of study are as follows, 1) Blood pressure and heart rate increased significantly after intubation as compared with preintubation in both control group and SGB group. 2) The difference of the two groups is not found. We conclude the above method does not control cardiovascular consequences of endotracheal intubation.

성상신경절 차단후 발생한 반대측 호너 증후군 (Contralateral Horner's Syndrome after Stellate Ganglion Block -A case report-)

  • 대상포진후 신경통으로 체성신경 차단과 함께 성상신경절 차단을 반복적으로 받아오던 64세 여자환자에서 열세번째 우측 성상신경절 차단후 반대측인 좌측에 호너씨 증후군이 발생되었다. 그 원인은 명확하지 않지만 시술 도중 환자머리의 좌측회전으로 인한 해부학적 위치변화와 술자의 부정확한 지표선정 및 주사바늘의 안쪽방향등으로 약제가 중앙선을 넘어 주입된 것으로 추정할수 있다.
