통증 치료 환자에 대한 임상통계적 고찰 (Clinical Survey of the Patients of Pain Clinic)

  • We had retrospective analysis of 421 patients who were consulted from other departments. From 1994 to 1997, these patients received nerve blocks, intravenous lidocaine infusion, lasers and so on for their pain. From these results, we are gathering information and making some recommandations. The largest percentage of patients were in their fifties with a distribution of 32.8% male and 55.1% female. The most common condition requiring treatment was low back pain 44.6%, followed by cancer pain 19.2%, cervical pain 7.4%, and shoulder pain 4.3%. In case of low back pain, the largest portion was HNP(27%), followed by spinal stenosis(16%), sprain(11%), and postlaminectomy(10%). The most common cacer was colorectal(28.4%) and the next was stomach(19.7%). The most commonly done nerve block was stellate ganglion block 32.3%, followed by lumbar epidural block 24.5% and caudal block 7.2%.

Ramsey Hunt 증후군의 치험 -1예 보고- (Ramsey Hunt Syndrome -A case report-)

  • Ramsey Hunt Syndrome occurs when herpes zoster afters the facial nerve. It causes vesicular eruption of the pinna, external auditory meaturs and ear drum, severe otalgia with associated facial paralysis and vertigo. We experienced a case of Ramsey Hunt syndrome and managed it with repeated sympathetic blocks using a stellate ganglion block. We achieved early resolution of the eruption, relief of pain and prevention of postherpetic neuralgia. We concluded that SGB was effective treatment against Ramsey Hunt Syndrome.

Successful Treatment of Severe Sympathetically Maintained Pain Following Anterior Spine Surgery

  • Sympathetic dysfunction is one of the possible complications of anterior spine surgery; however, it has been underestimated as a cause of complications. We report two successful experiences of treating severe dysesthetic pain occurring after anterior spine surgery, by performing a sympathetic block. The first patient experienced a burning and stabbing pain in the contralateral upper extremity of approach side used in anterior cervical discectomy and fusion, and underwent a stellate ganglion block with a significant relief of his pain. The second patient complained of a cold sensation and severe unexpected pain in the lower extremity of the contralateral side after anterior lumbar interbody fusion and was treated with lumbar sympathetic block. We aimed to describe sympathetically maintained pain as one of the important causes of early postoperative pain and the treatment option chosen for these cases in detail.

  • Prostaglandin E1(PGE1) is a potent vasodilator and is a useful drug for the treatment of occlusive peripheral vascular disease. It has been used systemically via intravenous route or regionally via intraarterial route. We tried intravenous regional administration of PGE1 for the treatment of a patient with occlusive arterial disease involving left fingers. During the 13th injection, the patient complained of severe pain at the injection site during the drug administration. Thereafter, the patient developed painful and severe swelling with blebs on his left hand. Systemic antibiotics were given together with stellate ganglion block of the affected left side. PGE1 was substituted to reserpine, which is subcutaneously injectable, for the second term treatment.

김 찬 신경통증클리닉 환자의 통계고찰 (A Clinical Review of the Patients in the Kim Chan Pain Clinic)

  • Backgrouds: Twent five years have passed since the opening of the first pain clinic in korea, in 1973 at Yonsei University Hospital. The number of pain clinics are gradually increasing in recent times. It is important to plan for future pain clinics with emphasis on improving the quality of pain management. Therefore we reviewed the patients in our hospital to help us in planning for the future of our pain clinic. Methods: We analyzed 2656 patients who had visited our Kim Chan Pain Clinic, accordance to age, sex, disease, and type of treatment block, from July 1996 to August 1997. Results: The prevalent age group was in the fifties, 27.3%, seventy years and older compromised 9.2%. The most common disease were as follows: lower back pain(46.2%); cervical and upper extremities pain(23.1%); trigeminal neuralgia(7.2%); and hyperhydrosis(5.8%) Both nerve blocks and medication were prescribed as treatment. Lumbar epidural block(16.3%) and stellate ganglion block(15.6%) were the most frequent blocks performed among various nerve blocks. Among nerve block under C-arm guidance, lumbar facet joint block(24.4%) and lumbar root block(22.5%) were performed most frequently. Trigeminal nerve block(18.4%), thoracic(17.0%) and lumbar sympathetic ganglion block(11.4%) were next most prevalent blocks performed frequent block. Conclusions: Treatments at our hospital were focused on nerve blocks and medications prescriptions. Nerve blocks are of particular importance in the diagnosis and treatment of chronic pain. However in future, to raise the quality of pain management, we need to fucus on a multidisciplinary/interdisciplinary team approach.

돌발성 난청의 치료를 위한 성상신경절 차단 (Stellate Ganglion Block for the Treatment of Sudden Sensoryneural Hearing Impairment)

  • 돌발성 난청 환자의 혈관 확장치료로서 histamine을 사용한 군과 SGB를 사용한 군을 회고적으로 비교한 결과 Histamine군에서는 19예 가운데 10예(52.8%), SGB군 30예 중 17예(56.1%)에 있어서 2~3주 이내에 개선을 보였다. SGB는 금기증이 거의 없으며 수기가 간단하고 부작용이 경미하므로 돌발성 난청의 치료에 있어서 histamine 치료의 좋은 대치 방법이 될 것으로 사료된다.

편측 성상신경절 차단에 의한 혈압 맥박 및 상지 피부온도의 변화 (Blood Pressure, Pulse Rate and Temperature Changes of the Ipsilateral Upper Extremity after Unilateral Stellate Ganglion Block)

  • 성상신경절차단이 혈역학적 변화 및 차단부위의 피부온도에 마치는 영향을 알아보기 위하여 두경부와 어깨 및 상지에 통증을 호소하는 환자 45명 (남자 16명, 여자29명)을 대상으로 편측 성상신경절차단을 시행해 보았던 바 다음과 같은 결과를 얻었다. 1) 혈압의 변화 : 우측 성상신경절차단과 좌측 성상신경절차단의 어느 경우에서도 수축기 및 이완기 혈압에 마치는 영향은 거의 없었다. 2) 맥박수의 변화 : 우측 성상신경절차단후 30분과 60분에 각각 의의있는 맥박수의 감소를 나타내었으나 (p<0.05), 좌측 차단후에는 통계학적으로 의의있는 맥박수의 감소를 나타내지 않았다. 3) 차단측 상지피부온도의 변화 : 우측 성상신경절차단후에는 5분부터, 좌측차단후에는 10분부터 의의있는 피부온도의 상승 (p<0.01)을 보이기 시작하여 차단후 30분에 양측 모두에서 피부온도가 최대로 상승되었다가 60분에는 30분치에 비해 다소 하락하였다. 4) 시술환자 45명중 4명에서 성상신경절차단의 징후는 나타났으나 차단측상지의 피부온도 상숭은 동반되지 않았다. 이상의 결과로 미루어보아 제 6 경추 전결절에서 1.0% lidocaine 10 ml로 편측 성상신경절차단시에는 심순환계에 특별한 영향을 마치지 않았으며 또한 상지질환의 치료목적시에는 반드시 차단즉 상지피부온도의 상승을 확인하여야만 효과있는 차단이 이루어졌다고 판정할 수 있을 것으로 사료된다.

경추 추궁절제술후 발생한 Causalgia (Causalgia After Cervical Discectomy)

  • 49세 여자 환자가 경부 추간판 탈출증의 치료를 받기위하여 전신 마취하에 우측의 추궁 절제술과 전방 추체간 접합술을 받고난 즉시 회복실에서 부터 양측 상지에 작열통이 발생하였다. 환자는 양측 상지에 전형적인 작열통의 증상인 allodynia, 통각 과민(hyperalgesia), 통각 이상 과민증 (hyperpathia)을 보였으며 특히 우측 상지에는 심한 부종을 보이고 있었다. 환자는 일련의 성상 신경절 차단과 disposable $Daymate^{TM}$ portable elastomeric infusor를 이용한 연속적 상완신경총 차단으로 작열통이 완치되었다.

Effects of applying nerve blocks to prevent postherpetic neuralgia in patients with acute herpes zoster: a systematic review and meta-analysis

  • Background: Postherpetic neuralgia (PHN) is a common and painful complication of acute herpes zoster. In some cases, it is refractory to medical treatment. Preventing its occurrence is an important issue. We hypothesized that applying nerve blocks during the acute phase of herpes zoster could reduce PHN incidence by attenuating central sensitization and minimizing nerve damage and the anti-inflammatory effects of local anesthetics and steroids. Methods: This systematic review and meta-analysis evaluates the efficacy of using nerve blocks to prevent PHN. We searched the MEDLINE, EMBASE, Cochrane Library, ClinicalTrials.gov and KoreaMed databases without language restrictions on April, 30 2014. We included all randomized controlled trials performed within 3 weeks after the onset of herpes zoster in order to compare nerve blocks vs active placebo and standard therapy. Results: Nine trials were included in this systematic review and meta-analysis. Nerve blocks reduced the duration of herpes zoster-related pain and PHN incidence of at 3, 6, and 12 months after final intervention. Stellate ganglion block and single epidural injection did not achieve positive outcomes, but administering paravertebral blockage and continuous/repeated epidural blocks reduced PHN incidence at 3 months. None of the included trials reported clinically meaningful serious adverse events. Conclusions: Applying nerve blocks during the acute phase of the herpes zoster shortens the duration of zoster-related pain, and somatic blocks (including paravertebral and repeated/continuous epidural blocks) are recommended to prevent PHN. In future studies, consensus-based PHN definitions, clinical cutoff points that define successful treatment outcomes and standardized outcome-assessment tools will be needed.

유방암 감염성 림프부종 환자에서 성상신경절 블록이 미치는 영향 (The Effect of Stellate Ganglion Block on Breast Cancer-Related Infectious Lymphedema)

  • 유방암에 의한 림프부종은 유방암 수술 이후 생기는 치료하기 힘든 부작용 중 하나로 여겨져 왔다. 환자들은 림프부종에 의한 신체적인 기능저하뿐만 아니라 삶의 질 저하로 인한 정신적인 우울감을 호소한다. 최근의 연구에서는 봉와직염이 유방암에 의한 림프부종의 위험성을 현저히 높인다는 결과가 있었고, 봉와직염 자체가 유방암에 의한 림프부종을 일으키는 위험 인자로 제시되어 왔다. 본 증례에서는 1달간의 성상신경절 블록으로 유방암에 의한 염증성 림프부종 환자의 증상 완화 및 팔 둘레의 현저한 감소에 대해 기술 하고자 한다. 우리는 환자의 팔 둘레를 4군데에서 측정하였는데 각각 팔꿈치를 기준으로 위 아래 5 cm, 10 cm에서 측정이 이루어졌다. 또한 통증 점수(NRS) 와 breast cancer questionnaire (LBCQ) 점수를 외래 방문마다 측정하였다. 두 번의 연속적인 성상신경절 블록 이후 환자의 통증 점수 및 팔 둘레는 감소하였고, 치료 중간에 봉와직염이 재발하여 다시 입원하여 항생제 치료와 병행하여 연속적인 성상신경절 블록을 시행하였고 환자는 시술의 결과에 대해 매우 만족하였다. 특히 환자는 통증 및 붓기의 완화와 어깨 관절의 움직임 개선에 큰 만족을 보였고, 결과적으로 환자의 삶의 질을 높이는데 기여하였다. 본 증례는 유방암에 의한 염증성 림프부종에 대한 성상신경절 블록의 효과에 대한 연구로, 스테로이드를 사용하지 않는 성상신경절 블록이 치료하기 힘든 염증성 림프부종 환자의 대체적이면서 보조적인 치료 방법으로 제시 될 수 있다는 것을 보여준다.
