편마비환자에게 플라스틱 단하지 보조기착용 전${\cdot}$후 선자세 균형과 동적 움직임에 미치는 효과 (The immediate effect standing balance and dynamic activity on barefoot, wearing SPAFO and wearing HPAFO in hemiplegic patients)

  • Objective: The purpose of this study were to investigate the standing balance, dynamic activity in hemiplegic patients according to the types of ankle-foot orthosis(AFO) and to determine the most effective type of AFO for gait training. Method: A prospective study was performed for 16 patients with hemiplegia who was able to walk independently. Static balance and dynamic activity were compared in two condition : 1) barefoot and SPAFO, 2) barefoot and HPAFO. Static balance and dynamic activity characteristics were evaluated by Active Balance while they were standing with in two condition AFO and barefoot. Results: There were significant difference in standing balance between barefoot and wearing SPAFO and HPAFO(p<0.05). There were significant difference in dynamic activity balance between barefoot and wearing SPAFO and HPAFO(p<0.05). There were significant difference in gait speed between barefoot and wearing SPAFO and HPAFO(p<0.05). Conclusion: This study showed that wearing SPAFO and HPAFO gave fair amount of improvement to balance and gait ability of hemiplegic patients.

고유수용성신경근촉진법이 만성 뇌졸중환자의 하지기능에 미치는 효과 (The Effects of Proprioceptive Neuromuscular Facilitation on Lower Extremity Functions of Chronic Stroke Patients)

  • Purpose: The purpose of this study is to determine the effects of proprioceptive neuromuscular facilitation on the lower extremity function of chronic stroke patients. Methods: The participants consist of 26 chronic stroke patients. They were randomly assigned to either an experimental group (n=13) or a control group (n=13) and engaged in exercise three times per week for eight weeks. The experimental group engaged in proprioceptive neuromuscular facilitation exercises and the control group engaged in general mat exercises. To measure lower extremity strength, the sit to stand test and the static balance test (standing on one leg) were used. To measure the stability index (SI) and weight distribution index (WDI), a Tetrax Portable Multiple System (Tetrax Ltd, Israel) was used in addition to a static balance test (standing on one leg). Results: : Lower extremity strength and static balance showed a significant improvement in the experimental group (p<.05). The SI and WDI were significantly improved in the experimental group (p<.05) for tests of standing with their eyes open, standing with their eyes closed, and standing on a sponge with their eyes open. Conclusion: Proprioceptive neuromuscular facilitation exercises have confirmed that lower extremity function improves after exercise interventions in chronic stroke patients.

여성노인들의 라인댄스 활동이 정적 직립 균형성에 미치는 영향 (Influence of the Silver Line Dance Effects on the Elderly Balance During Static Standing)

  • 이 연구는 여성 노인들에게 15주간 라인 댄스 운동 프로그램을 실시하게 한 후 자세 제어와 반응 메커니즘을 관찰할 목적으로 균형성 요인을 분석해 낙상 예방을 위한 운동 중재 효과를 규명하고자 했다. 이를 위해 65세 이상 16명이 선정되었다. 균형성 판단 요인은 정적 직립 상태에서 압력 판에서 25초 서있는 동안 압력 판의 6가지(3방향 힘, 3방향 모멘트) 자료를 수집해 이를 바탕으로 좌우 및 전후 압력중심 변동폭, 이들의 압력 중심 속도, 흔들림 면적을 분석했다. 분석한 결과 라인 댄스 활동 후 전후 압력 중심의 변화 범위(p<.01)와 압력 중심의 평균 속도(p<.05)는 크게 감소를 보였다. 또한 흔들림 면적은 라인댄스 후 유의한 감소를 보였다(p<.001). 좌우 방향의 압력중심 범위와 속도는 큰 감소를 보이지 않았다. 앞으로 이와 유사한 연구를 수행하고자 할 때는 운동 중재 효과를 실제 낙상과 관련해 연구할 필요가 있으며, 또한 균형성 판단 요인을 신체계뿐만 아니라 전정계, 감각계 등을 관찰하는 연구가 필요하리라 본다.

서서 일하는 작업에서의 정맥류 발생에 대한 연구 (A Study of Varicose Veins in Prolonged Standing at Work)

  • Recent studies suggest that prolonged standing at work is associated with varicose veins (VV). The objective of this study was to analyze the differences of VV risk in terms of work types in casino dealers and hotel service workers. Symptoms of VV were evaluated using a self-reported questionnaire from 2,092 participants, and muscle fatigue in the lower legs were evaluated using surface electromyography. The shift rate of median frequency in Gastrocnemius was -30.0% among the casino dealers working in prolonged static standing posture, and - 14.3% among the casino dealers sitting during working time. After adjusting age, gender, smoking, and family history of VV, the risk ratio of VV was 3.67(95% CI 2.03-6.59) in group I (prolonged standing and occasional walking at work), 6.29(95% CI 3.48-11.35) in group II (prolonged standing, occasional walking and heavy lifting at work), and 8.07(95% CI 4.51-14.43) in groupIII(prolonged static standing at work). In conclusion, prolonged standing at work may be a work related risk factor of VV.

고유수용성 신경근 촉진법의 통합 패턴이 정적 균형에 미치는 영향 (The Effects of the Combined Patterns of Proprioceptive Neuromuscular Facilitation on Static Balance)

  • Purpose : The purpose of this study was to investigate the effects of the combined patterns of PNF(proprioceptive neuromuscular facilitation) on the static balance ability. Methods : The measurements of the static balance ability were completed by 10 subjects for 6 weeks, from October to November 2007. The combined patterns of PNF were carried out by means of self-exercising suggested by Dietz, which were designed as four cases: two positions (standing and quadruped) for both patterns(sprinter and skater), respectively. The exercises were practiced once a day, 3 times a week in same condition. By using the GOOD BALANCE system, assessment of the static balance ability was taken at before and after exercise from 6 positions: normal standing, one leg left and one leg right standing when eye open and close, respectively. For each case, the experimental data was obtained in 3 items: mean X speed, mean Y speed and velocity moment. Results : The results of this study were as follows : 1. There were statistically significant differences of Mean X speed, Mean Y speed and Velocity moment between the before and the after exercise in the case of normal standing when eye open and close(NSEO and NSEC), respectively. 2. There was statistically significant difference of Mean X speed between the before and the after exercise in the case of one leg left standing when eye open(OLLEO). In this case, however, the statistically significant differences were not found in both terms of Mean Y speed and Velocity moment. 3. There were statistically significant differences of Mean X speed and Mean Y speed between the before and the after exercise in the case of one leg left standing when eye close(OLLEC). In this case, however, the statistically significant difference was not found in term of Velocity moment. 4. There were statistically significant differences of Mean X speed, Mean Y speed and Velocity moment between the before and the after exercise in the case of one leg right standing when eye close(OLREC). 5. There was statistically significant difference of Mean X speed between the before and the after exercise in the case of one leg right standing when eye open(OLREO). In this case, however, the statistically significant differences were not found in both terms of Mean Y speed and Velocity moment. 6. There were statistically significant differences of total Mean X speed, total Mean Y speed and total Velocity moment between the before and the after exercise. Conclusions : The above results from this study indicated that the combined patterns of PNF have improved the static balance ability. However the used self-exercise can be applied to normal people, i.e., the exercise is difficult to apply into clinical patients. The further study should be focused at development of various modified forms of the combined patterns of PNF in keeping up the improvement effect of this exercise.

The Influence of Foot Position on Standing Balance on an Unstable Base of Support

  • The purpose of this study was to compare the balance ability at different foot positions using K A T 2000 (Berg, Inc, vista, CA.1994). Thirty (male 15, female 15) normal subjects participated in this study. All subjects were tested at a one leg position or a two leg position that were toe-in $25^{\circ}$, toe-out $25^{\circ}$, and $45^{\circ}$. The starting position was where the subject crossed their arms across the chest and flexed knees slightily. The results of each test were displayed on a screen in a score format, which indicated balance index. These collected data were analyzed by using one way ANOVA, and t-test. The results of this study were as follows: When changing the angle of the foot in the one-foot and two-foot standing positions, there was no statistically significant difference, but the balance performance with the foot rotated by $25^{\circ}$ was better than that with the foot rotated by $45^{\circ}$. When changing the direction of the foot in the one-foot and two-foot standing positions, there was no statistically significant difference, but the balance performance with the foot rotated laterally was better - except for the case when the foot was medially rotated by $25^{\circ}$ in the right-foot static standing position. When the feet were medially rotated by $25^{\circ}$ in the two-foot static standing position, and were medially rotated by $25^{\circ}$ and $45^{\circ}$ in the one-foot static standing position, the balance performance of females was better than that of males. In this study, it was found that the balance performance of the subjects changed when the position of the foot was shifted on an unstable base of support. However, there was little correlation between balance performance and the height, weight and foot length of the subjects. It is necessary to conduct a follow-up study targeting various age groups and those with various diseases using an unstable platform or applying different physical or visual conditions, such as the length of the legs. Physical therapists need to consider the position of the foot in clinical settings for a better balance training or assessment.

The Effects of Static Hamstring Stretching on Hip Motion and Lumbo-Pelvic Kinematics

  • Background: Static hamstring stretching exercises have been widely used to improve flexibility of the hamstring muscles. However, few studies have examined the influence of standing static hamstring stretching (e.g., jack-knife stretching) on movements of the lumbopelvic-hip complex. Objectives: To examine the short-term effects of jack-knife stretching on movements of the lumbopelvic-hip complex. Design: Case series. Methods: Fourteen participants with hamstring tightness (8 male, 6 female) were recruited. Participants performed jack-knife stretching for 150 s. Before and after stretching, participants performed the finger-to-floor distance (FFD), sit and reach (SRT), active knee extension (AKE), passive straight leg raising (PSLA), and active straight leg raising (ASLR) tests as well as pelvic tilt while standing to identify the effects of stretching. Results: There were significant improvements in the FFD, SRT, AKE, PSLA, and ASLR tests after stretching. However, pelvic tilt angle while standing did not significantly change. Conclusion: Jack-knife stretching can be a useful exercise to improve flexibility of the hamstring muscles, but not pelvic alignment while standing.

안구운동 프로그램 적용이 노인의 정적 균형에 미치는 영향 (The Influence of Eye Movement Program Applied to Elderly People on Static Balance)

  • Background: To evaluate the influences of Eye movement program applied to elderly people on static balance. Methods: The subjects consisted of fourteen elderly people. The subjects were divided into two group. The control group (n=7) received no exercise and/or stimulation. The Eye movement program training (E/P) group (n=7) performed balance training using word card and batom. E/P group were accomplished during 4weeks (5 day/week, 60 min/day). All tests were completed before and after experiment. The static balance ability was measured by normal standing when eye open and close on good balance system, respectively. For each case, the experimental data were obtained in 3 item; mean X speed, mean Y speed and velocity moment. Results: The result of this study were as follows: 1. In E/P group, the statistically significants were shown on only mean Y speed in the case of normal standing when eye open (p<.05), but the statistically significants were not shown on mean X, Y speed and velocity moment in the case of normal standing when eye close (p>.05). 2. In control group, the statistically significants were not shown on all posture (p>.05). 3. There was a statistically significant difference on the mean Y speed and velocity moment in the case of normal standing when eye open between control group and E/P group (p<.05). Conclusions: The above results revealed that balance training using Eye movement program were partly effective for improving the static balance ability.

탄력밴드를 이용한 고관절 외전근 근력강화운동이 정적 균형에 미치는 영향 (The Effect of Hip Abductor Strengthening Exercise using Elastic Band on Static Balance)

  • Purpose : To evaluate the effects hip abductor strengthening exercise using elastic band on static balance. Methods : The subjects consisted of twenty healthy people. The subjects were divided into two group. The control group(n=10) received no exercise and/or stimulation. The elastic band strengthening exercise(E/E) group(n=10) performed hip abductor for strengthening exercise using elastic band. E/E group were accomplished during 6weeks(3day/week, 30-40min/day). All tests were completed before and after experiment. The static balance ability was measured by normal standing when eye open and close on GOOD BALANCE System, respectively. For each case, the experimental data were obtained in 3 items: mean X speed, mean Y speed and velocity moment. Results : The results of this study were as follows; 1. In E/E group, the statistically significants were shown on Y speed and velocity moment in the case of normal standing when eye open and X speed and velocity moment in the case of normal standing when eye close(p<0.05). 2. In control group, the statistically significants were not shown on all posture(p>0.05). 3. There was a statistically significant difference on the X speed and velocity moment in the case of normal standing when eye close between control group and E/E group(p<0.05). Conclusion : The above results revealed that hip abductor strengthening exercise using elastic band were partly effective for improving the static balance ability.

노인에서 임상적 균형평가 지수들의 유용성 : 힘판을 이용한 균형평가와의 상관관계 (The Usefulness of Clinical Balance Tests in Elderly : Correlation of Balance Evaluation using by Forceplate)

  • Purpose : The purpose of this study was to investigate the usefulness of clinical balance tests through the correlation of balance evaluation using by forceplate in elderly. Methods : Thirty nine healthy elderly subjects (14 males, 25 females) participated in the study. The subjects were evaluated with clinical balance tests [(Berg balance scale (BBS), Functional reach test (FRT), Tinetti's performance oriented mobility assessment (POMA), and one leg standing (OLS)]. Static balance evaluation was assessed by using forceplate. Center of pressure (COP) parameters were obtained using it as total path distance, total sway area, X mean frequency and Y mean frequency for 20 seconds in the following conditions: (1) comfortable standing with eyes opened and closed, (2) uncomfortable standing with eyes opened and closed. After static balance evaluation tested, dynamic balance evaluation was assessed. COP parameters were error distance and area during sine curve trace. COP parameters were movement time, error distance, and maintained time in the circle during COP movement task. Results : Clinical balance tests showed statistically significant correlation between static and dynamic balance evaluations. Among the clinical balance tests, the BBS, POMA, and OLS showed significant correlation with to assess the balance ability of elderly in clinical setting both evaluations. Conclusion : Clinical balance tests can be recommended in clinical setting because of low costs and simplicity.
