Objectives: The purpose of this study was to identify the family phenomenon and characteristics by ICNP such as family shape, life standard. area, and development stage. Method: 1. Sample size was 115. 2. Data analysis method included frequency analysis including ratio. Results: 1. On the distribution of the family phenomena by family shape, the nuclear family showed a higher ratio on the distribution in family communication and the unhealthy life style. The nuclear family was related to children andhad a higher ratio on the inadequate care management of the sick member. The extended family showed the highest ratio on the inadequate care management of sick member. 2. On the distribution of the family phenomena by family life standard, the family with medium life standard showed a higher ratio on the inappropriate family coping, whereas the family with lower life standard appeared to have a higher ratio on the inadequate care management of the sick member. 3. On the distribution of the family phenomena by area. the large city area showed the highest ratio on the inappropriate family coping, the medium and small city area appeared to have the highest ratio on the unhealthy life style. The county area showed the highest ratio on the inadequate care management of sick member. 4. On the distribution of the family phenomena by development stage, the family in rearing period showed a higher ratio on the inappropriate family coping and the lack of family intimacy. Families with preschool children showed a higher ratio on the unhealthy life style, and families with school age children showed the highest ratio on the inappropriate family coping. Families with adolescents appeared to have the highest ratio on the disturbance in family communication, and families with launching young adults showed the highest ratio on the inadequate care management of sick member. 5. On the distribution of the family characteristics by family shape, the nuclear family showed a higher ratio on the family characteristics such as less communication chances among family members and neglect of general child rearing, whereas the third generation family appeared to have a higher ratio on the characteristics such as overburden of housewife's role and short of caring among family members. 6. On the distribution of the family characteristics by family life standard, the family of medium life standard showed higher ratio on the family characteristics such as a few communication chance among family and overburden of housewife's role, and the family of lower life standard appeared to higher ratio on the family characteristics such as short of caring among family members. 7. On the distribution of the family characteristics by area. the large city area showed a higher ratio on the family characteristics such as overburden of housewife's role and neglect of general child rearing. The medium and small city area appeared to have a higher ratio on the family characteristics such as less communication chancec among family members. The county area showed a higher ratio on the family characteristics such as short-term care among family members. 8. On the distribution of the family characteristics by development stage, the family with rearing period showed a higher ratio on the family characteristics such as neglect of general child rearing. Families with preschool children showed a higher ratio on the family characteristics such as less communication chancec among family members. Families with school age children showed a higher ratio on the family characteristics such as overburden of housewife's role. This study is to examine current child care support policies and their limitations and to make some suggestions by means of statistics and previous literature. Major findings are as follows: First, As the women have younger children, the effect is more negative. Second, a maternity leave of Korea is 90 days, which is below ILO standard. Moreover, it is not well supported by the companies they work for. Third, the increasing number of men has spent a paternity leave since the pertinent law was enforced in 2001. The rate of spending the leave, however, is not as high as expected, because it has not yet been decided whether the leave would be paid or not. Lastly, the number of the employer-supporting child care center is rather few due to the legal standard of the facility and the expense that a company should cover. Only 46.5% of the companies that has been appointed to obligatorily establish the center now operate the facility. Therefore, child care support policies should be reformed or improved to help reduce married women's child care burden obviously hindering women from being employed, and this will consequently promote their economic activities. It is also urgently required to expand the application of the parental leave in terms of both object and scope. It is important that employers and employees get ready to compromise each other on the wage issue during the leave. In addition, the flexibility in period and form of the parental leave and the connection of working places with local community for better child care service must be taken into consideration.
Background: Health care professionals (HCPs) are at high risk of contracting blood-borne infections due to their occupational exposure to blood and body fluids (BBFs). The incidence of these infections among HCPs are higher in low income countries such as Ethiopia. The aim of the study was to investigate the extent of occupational exposure to BBFs and its associated factors among HCPs in Bahir Dar town, Ethiopia. Methods: A cross-sectional study was used from October 1, 2012 to October 30, 2012. Three hundred and seventeen HCPs were included in the study using a simple random sampling technique. The data were collected using a structured questionnaire and analyzed using SPSS version 16. Bivariate and multivariate analyses were used to identify the factors related to exposure to BBFs. Results: Two hundred and nine (65.9%) HCPs were exposed to BBFs in the past year, of which 29.0% were needlestick injuries. Work experience [adjusted odds ratio (AOR) 4.13, 95% confidence interval (CI) 1.56-10.91], inconsistent use of gloves (AOR 1.98, 95% CI 1.04-3.43), and not complying with standard precautions (AOR 1.80, 95% CI 1.00-3.22) were the factors associated with occupational exposure to BBFs. Conclusion: A high proportion of HCPs was exposed to BBFs in this study. Occupational exposure to BBFs was determined by the use of gloves and not complying with standard precautions. Ensuring the availability of gloves, training about standard precautions, and motivation of HCPs to implement standard precautions should be emphasized to avoid such exposures.
Purpose: This study was done to develop standards for dementia care as a baseline for professional nurses to provide a framework for dementia care evaluation Methods: The dementia care standards were developed through a literature review and focus group discussions. According to the Delphi method, the data analysis was conducted using the Content Validity Index (CVI). Results: The final set of 18 standards on dementia care was developed through one round of CVI. The standards included four structural standards: 'Organization of nursing system', 'Operating system', 'Management of human resources', 'Management of material resources', 13 procedural standards: 'Advanced assessment', 'Nursing diagnosis', 'Nursing plam', 'Advanced nursing implementation', 'Evaluation', 'Education', 'Research', 'Consultation', 'Counseling and cooperation', 'Development of specialty', 'Utilizing resources', 'Nursing quality assurance', 'Ethics', and one standard concerning outcome ('Evaluation of nursing tasks in care of patients with dementia'). The final set of 55 criteria on care of patients with dementia was confirmed through two rounds of CVI. The final 171 indicators were confirmed through four rounds of CVI. Conclusion: These dementia care standards provides a framework that allows registered nurses to clarify their roles and tasks in the care of patients with dementia and provides evaluation criteria.
There are high recognitions on the importance of comforts in Elderly living environment, but the circumstance is that studies on seniors facility space itself are approached only in planning level, and studies on lighting environment which is significantly associated with the comfort in the indoor environment of seniors where they actually spend the majority of their time are not that active. This study was intended to deduce cozy bedroom environment to which existing elderly care facility can be improved by using light shelf the lighting system with the advantage of being able to serve both as building sun visor and lighting window simultaneously in order to analyze the interior environment of bedroom space of elderly care facility the indoor space where the aged spend the majority of their life and examine the directions for the improvement of existing building lighting system through remodeling and renovation. In this study, lighting performance analysis was done in a way that the windows of the bedroom unit in existing facility were set in southbound direction based on two standard types and were put under initial simulation with the use of Autodesk Revit 2011, and after the simulation results were converted to Green Building Studio gbXML file to be used in ECOTECT, Daylight Autonomy a dynamic simulation and static natural lighting simulation the existing method of calculating daylight factors were deduced through Ecotect Analysis 2011. In conclusion, exiting standard model was found in such a condition that the daylight factors for both type A and type B were above 5% the proper standard value, and required improvement. In case light shelf the natural lighting system was attached, the daylight factor was improved to proper standard value for type A, and also was improved above existing facility for type B.
National Health Insurance Service(NHIS) provide care-plans for beneficiaries in the long-term care insurance(LTCI) systems that help them use LTC services appropriately. The care-plan includes recommendations for the most adequate type of care (gold standard) for beneficiaries. This study develops a decision-support system to determine the appropriate type of care plan. To develop a model, we used a data set that well-trained assessors in the NHIS investigated as a gold standard for beneficiaries: nursing home care, home-visit care, home-visit bathing, home-visit nursing, or day and night care. The decision-support system was established through a decision-tree model, because it may be easy to explain the algorithm of a decision-support system to working groups and policy makers. Our results might be useful in evidence-based care planning in an LTCI system and contribute to the efficient use of LTC services.
Propose: The purpose of this study was to help families decrease and alleviate the burden on family care-givers taking care of elderly patients. Method: Data was collected by a questionnaire from 100 family members who were registered in the department of home health care nursing at 4 hospitals of H University Medical Center from September 20 to October 25, 2005. The collected data was analyzed using Mean and Standard Deviation, Pearson Correlation Coefficient, t-test and One-Way ANOVA with the Duncan's test, and Stepwise multiple regression. Result: The average burden on family care-givers of elderly patients with chronic diseases was 3.31. The social burden was the highest(M=3.68), the lowest was the emotional burden(M=2.95). In ADL of elderly patients with chronic diseases, all 10 questions showed an average point above 2.50. The dependency level of going up and down the stairs was the highest(M=2.88). Conclusion: This research is necessary for the application of a plan in the social support system in order to reduce the burden on family care-givers who are taking care of elderly patients with a chronic disease.
The purpose of this study was to evaluate appropriateness of home health care travel cost. For the evaluation, investigated the operating costs of vehicles used by home care nurses and then were components of home care nurses's annual salaries. Travel costs were then calculated based on actual travel expenses of home health care service. Actual data of 23 hospital-based home care agencies between July, 2002 to December, 2002 were collected for the analysis of the travel costs. The results of this study are : 1) For home visit, 65% of home care agencies turned out to be using only hospital owned cars, and 17.1% be depending purely on home care nurses' cars. On average, 1.9 cars used for home visit. 2) Out of 89 agencies, 23 agencies responded to the travel cost survey. Total maintenance cost of a car per month was 381,457 won. 3) Average per visit personal expenses of home care nurses during travel time turned out to be 7,124won assuming 8 working hours per day, 4 visits per day, and 30 minuets of travel time for each visit. 4) Total home health care travel cost per visit was 12,069 won, which was the sum of actually paid travel cost of 4,945 won and personal expenses during travel time of 7,124. In conclusion. we reckon that current compensation price of home care nurses' travel is inappropriate because total home health care travel costs of 12,069 won per visit turned out to be 2.1 times of currently prevailing standard compensation price of 5,830 per visit.
Purpose: The purpose of this study was to develop an evidence-based dysphagia nursing care protocol for nursing home (NH) residents in Korea. Methods: The protocol was developed based on international guidelines and literatures. After testing content validity by experts, the protocol was applied to the intervention group (n=35) for 4 weeks at one NH in December, 2011, whereas the control group (n=34) received routine care. Results: The protocol was composed of one-page algorithm and detailed guidelines. Algorithm pathway was organized in 3 parts, including screening dysphagia risk, grouping by dysphagia risk level, and assigning nursing care into each group. The nursing care included positioning, oral care, meal time care, diet modification, providing exercise and maneuver, and checking dysphagia signs and symptoms. The experts verified the content validity. Protocol was revised to fit NH practice after the participant observation. Clinical validity was established upon evaluating usefulness, appropriateness, and convenience of the protocol by NH nurses. Dysphagia risk of the intervention group was significantly decreased. Conclusion: Developed protocol will improve the quality of dysphagia care in nursing homes as it can serve as a consistent and integrated standard for nursing care of residents with dysphagia.
In this paper, I examined the terms of letter of credit and standard for examination of documents. In connection with the test of standard, I reviewed the bank's reasonable care, inconsistence and linkage between documents, non-documentary conditions and the treatment of discrepant documents. Traditionally credit law is founded on two principles such as the standards of strict of compliance and substantial compliance, but these standards are not consistency. Moreover although the Uniform Customs and Practice for Documentary Credits(UCP) introduces a new standard for examination of documents by incorporating international banking practice, the standards for documentary compliance have created ambiguity. Accordingly, I suggest that the standard requires strict compliance between party concerned in letter of credit transactions but only requires international standard banking practice in customer relationship. Therefore the standard and the specific guideline should be prescribed in the next version of the UCP.
