알레르기 비염 및 부비동염에 의한 아데노이드의 국소 면역에 대한 고찰 (Local Immunity of Pediatric Adenoid with Allergic Rhinitis & Sinusitis)

  • Background: Chronic rhino-sinusitis and persistent allergic rhinitis is often cited as risk factor for developing adenoid hypertrophy or adenoiditis, but this relationship has not been studied extensively. In this study, we evaluated the mucosal barrier, squamous changes of ciliated epithelium, IgA secretion and BCL-6 expression in adenoids, and adenoid size. Methods: Six children with allergic rhinitis and sinusitis, nine children with only allergic rhinitis, nine children with only sinusitis and six children without any history of allergic rhinitis and sinusitis were enrolled. H-E stain of adenoid for squamous metaplasia, immunohistochemical study of adenoid for IgA and BCL-6, cytokeratin stain for evaluation of mucosal barrier and lateral view X-ray for adenoid size were performed. ANOVA test was used in the analysis and data showing p value of less than 0.05 were considered significant. Results: The number of ciliated cells had tendency to be decreased and squamous metaplasia had tendency to be increased in three experimental groups (p>0.05). Deterioration of mucosal barrier had tendency to be detected in three experimental groups than control group (p>0.05). BCL-6 had tendency to be increased and IgA secretion had tendency to be decreased in three experimental groups (p>0.05). There is no difference in adenoid size between three experimental groups and control group. Conclusion: Despite the expectation that adenoid would be affectecd by allergic rhinitis and rhino-sinusitis, we found no evidence for influence of adenoid immunity.

구강 편평세포 암종에서의 CD44 발현 (CD44 EXPRESSION IN ORAL SQUAMOUS CELL CARCINOMA)

  • The cell surface glycoprotein CD44 is a kind of adhesion molecule, which binds hyaluronic acid, type I collagen and fibronectin. Although there have been numerous reports on the expression and the function of CD44 in lymphocytes and macrophages, very little is known about its distribution and definite role in epithelial tissue, especially in oral epithelial one. The present study was performed to investigate the distribution and expression of the CD44 in human gingiva and squamous cell carcinoma(SCC) arising in human gingiva. And the authors compared CD44 expression with histopathologic grade of SCC. The results were as follows: 1. The CD44 was strongly expressed in granular, spinous and basal layers of normal marginal and attached gingiva, in spinous and basal layers of normal sulcular gingiva, and in all epithelial layers of normal junctional gingiva. 2. In SCC of gingiva, the CD44 was expressed in all but one case. In most of the cases the CD44 was expressed at cell membrane and the degree of expression was relatively strong. 3. In low-grade SCC of gingiva, the CD44 was strongly expressed, especially at the basal and spinous layers of abundantly keratinized cancer nests. In high-grade SCC of gingiva, the CD44 expression tended to be weak but was strong at cells showing individual keratinization. This study suggest that the CD44 expression of normal and cancerous gingival epithelium is associated with the degree of proliferation and differentiation of epithelial cells.

Pathogenesis and prognosis of primary oral squamous cell carcinoma based on microRNAs target genes: a systems biology approach

  • Oral squamous cell carcinoma (OSCC) is the most prevalent head and neck malignancy, with frequent cervical lymph-node metastasis, leading to a poor prognosis in OSCC patients. The present study aimed to identify potential markers, including microRNAs (miRNAs) and genes, significantly involved in the etiology of early-stage OSCC. Additionally, the main OSCC's dysregulated Gene Ontology annotations and significant signaling pathways were identified. The dataset GSE45238 underwent multivariate statistical analysis in order to distinguish primary OSCC tissues from healthy oral epithelium. Differentially expressed miRNAs (DEMs) with the criteria of p-value < 0.001 and |Log2 fold change| > 1.585 were identified in the two groups, and subsequently, validated targets of DEMs were identified. A protein interaction map was constructed, hub genes were identified, significant modules within the network were illustrated, and significant pathways and biological processes associated with the clusters were demonstrated. Using the GEPI2 database, the hub genes' predictive function was assessed. Compared to the healthy controls, main OSCC had a total of 23 DEMs. In patients with head and neck squamous cell carcinoma (HNSCC), upregulation of CALM1, CYCS, THBS1, MYC, GATA6, and SPRED3 was strongly associated with a poor prognosis. In HNSCC patients, overexpression of PIK3R3, GIGYF1, and BCL2L11 was substantially correlated with a good prognosis. Besides, "proteoglycans in cancer" was the most significant pathway enriched in the primary OSCC. The present study results revealed more possible mechanisms mediating primary OSCC and may be useful in the prognosis of the patients with early-stage OSCC.

Analysis of HR-HPV Prevalence among Unvaccinated Busan Women

  • To prevent cervical cancer, human papillomavirus (HPV) bivalent and quadrivalent vaccinations are common, but there is a need for a vaccination system based on the high-risk human papillomavirus (HR-HPV) genotype that differs by region. This study aimed to investigate the prevalence of HPV and the distribution of HR-HPV genotypes in 2,014 women who were not vaccinated against HPV. In this study, HPV DNA testing was performed on 2,014 women not vaccinated against HPV and who visited the Busan Obstetrics and Gynecology Department from September 2020 to July 2021. In addition, liquid-based cytology (LBC) test was performed on 493 cases of HR- HPV genotype infection confirmed by HPV DNA test. The prevalence of HPV among women in Busan was positive in 609 (30.2%) out of 2,014 cases. Among the 609 HPV-positive cases, HR-HPV infection accounted for 493 cases (81.0%), which is a high proportion. Of the total 493 HR-HPV infection cases, liquid-based cytology (LBC) was within normal limits (WNL) in 266 cases (54.0%), atypical squamous cells of undetermined significance (ASCUS) in 97 cases (19.7%), low-grade squamous intraepithelial lesion (LGSIL) in 88 cases (17.8%), and high-grade squamous intraepithelial lesion (HGSIL) in 42 cases (8.5%). Single HR-HPV 52 and 16 accounted for the highest and second highest infection rates, respectively. The high infection rate among women aged 18~39 underscores the need for continuous monitoring. In addition, when there were abnormal findings in the cervical epithelium, HPV 52 was the most common, while in the case of HGSIL, HPV 16 was the most common. The HR-HPV genotypes related to cervical cancer should be continuously collected and monitored for use in health policies, including local and national vaccinations.

상악골에 발생한 원발성 골내암종 (Primary intraosseous carcinoma occurring in the maxilla)

  • Primary intraosseous carcinoma (PIOC) is a rare odontogenic carcinoma defined as a squamous cell carcinoma arising within a jaw having no initial connection with the oral mucosa, and presumably developing from residues of the odontogenic epithelium. A 56-year-old patient who complained of delayed healing after extraction of upper left central incisor visited our department. The conventional radiographs showed a bony destructive lesion with ill-defined margin and moth-eaten appearance. On the computed tomographic images, the lesion perforated the labial cortex of alveloar bone, elevated the left nasal floor superiorly, and perforated partially both nasal floor. The magnetic resonance images showed low signal intensity at T2 and Tl weighted images at the area and adjacent soft tissue. Histologically, there were irregular epithelial islands with cell atypia, nuclear hyperchromatism, pleomorphism, atypical mitosis. The final diagnosis was PIOC.

Pulmonary Carcinosarcoma within Bronchogenic Cyst: 1례 보고 (Pulmonary Carcinosarcoma within Bronchogenic Cyst)

  • Carcinosarcoma is an unusual and rarest neoplasm in man, consisting of carcinomatous parenchyme and sarcomatous stroma. Two clinical types of carcinosarcoma were present. One type of tumor was centrally located [endobronchial type], infrequent metastasis, and better prognosis than parenchymal type. The other type was peripherally located [parenchymal type], frequent metastasis, and poor prognosis. The histogenesis of carcinosarcoma is many hypothesis, but controversial; 1] sarcomatous degeneration of stroma, 2] intermingling of simultaneously arising carcinoma & sarcoma, 3] multiple primary tumor, 4] blastomatous changes in hamartoma, 5] stromal reaction to squamous cell carcinoma, 6] true & collision carcinosarcoma. In this case, 52 year-old male patient was hospitalized due to intermittent hemoptysis & known pulmonary lesions. Since 1968, chest PA showed round haziness within cyst & multiple cyst on RUL & RLL. Radical pneumonectomy was performed and histopathology showed carcinosarcoma, surrounded by bronchial epithelium. The patient maintain general well-being without clinical evidence of recurrence till now.

  • Benign papillary tumors commonly arise in the upper respiratory tract but are rare in the lower respiratory tract, especially in the bronchi. Solitary benign papillary tumors arising at the bronchi,covered with stratified squamous epithelium and associated with little or no inflammatory cell response,seem to be rarest of all types of benign bronchial tumors. Only six cases of such tumors had been reported in the medical literature over the past 74 years until 1968. The following is a case report of solitary benign papilloma arising at the opening of the left main bronchus which was treated with bronchoscopic removal at Chest Surgery Depariment, N.M.C. on Sept.

임상가를 위한 특집 2 - 구강점막의 궤양성 병소 (Ulcerative Conditions of Oral Mucosa)

  • An ulcer is defined as loss of epithelium. Although many oral ulcers have similar clinical appearances, their etiologies encompass many disorders, including trauma, infection, immunologic disease, and malignant oral cancer. Oral squamous cell carcinoma(SCC) occupying about 90% of oral cancer, usually manifests as unhealed ulcer over 2 weeks. Oral SCC can metastasize to the cervical neck lymph node, and therefore the surgical therapeutic modality for oral SCC could encompass the neck node dissection as well as wide excision for primary lesions, which should leave the post-operative complication of functional damage like dysphagia and facial deformity. Therefore, it is important to discriminate oral SCC from other ulcerative conditions to make a prompt management. The knowledge for the pathogenesis of the ulcerative lesions could help the clinicians to understand the differences of clinical features and to practice an appropriate therapeutics.

  • Dermoid cysts located in the floor of the mouth are very rare developmental keratinizing squamous epithelium lined cysts. Anatomically, they are classified as sublingual (median genioglossal), submental (median geniohyoid) and lateral dermoid cysts, and they can be further classified as epidermoid, dermoid, and teratoid cysts by histology. We report a case of sublingual dermoid cyst in a 16-year-old boy presenting as a large sublingual swelling causing speech and swallowing difficulties and discuss the surgical treatment techniques and histopathological features of this lesion.

Traumatic Epidermal Inclusion Cyst under Anterolateral Thigh Free Flap on Great Toe

  • Epidermal inclusion cyst is a common mass in life. It is covered with a stratified squamous epithelium, thus, there is a granular cell layer adjacent to the keratin-containing cyst lumen. It can be caused by mechanical force, trauma, or a spontaneous event. It can rupture spontaneously or be ruptured by external mechanical forces. Epidermal inclusion cysts that exhibit inflammation or recur should be removed by simple excision. In this case, the patient showed an epidermal inclusion cyst under an anterolateral thigh free flap, which can cause the palpable mass to go unnoticed. First we thought he had neuroma formation after a surgical procedure on his foot. However it was an epidermal inclusion cyst, which was diagnosed by a special pathologist. It is a curious and rare case.
