Background: Lung cancer is one of the most commonly diagnosed cancers and the most frequent cause of cancer-related death worldwide. In Iran, it ranks second and third as the cancer-causing death in men and women, respectively. We carried out this study to find out the demographic, clinical, and histological characteristics and risk factors of lung cancer in a referral tertiary center in Iran. Materials and Methods: A retrospective study was conducted on cases of primary lung cancer based on the results of registered cancer reports of cytological and pathological specimens between March 2001 and March 2012. Demographic variables, clinical manifestations, histology and location of tumors were determined based on the data found in the medical records of each patient. Definite or probable etiologic factors were identified. Data analysis was performed with SPSS version 16 and a p-value ${\leq}0.05$ was considered as significant. Results: A total of 203 patients, with a mean age of 65.7 years (SD=11.2), with primary lung cancer were identified, 81.3% being men. Of the total, 110 cases (54.2%) were living in urban areas. In 53.2% of cases, the site of tumor was on the right side and in 72.9% of cases the lesion was centrally located. The histological types of lung cancer were squamous cell carcinoma (SCC) in 107 cases (52.7%), adenocarcinoma (AC) in 30 cases (14.8%), and small cell carcinoma (SC) in 27 cases (13.3%). Significant correlations between the gender and residence, smoking, and the histological type and location (central or peripheral) of tumor were found. The percentage of smokers was 75.2% in men and 15.8% in women. Conclusions: Smoking was the most important risk factor and squamous cell carcinoma the most common histological type of lung cancer in our study. Male sex and being a smoker was associated with histological types of SCC while being nonsmoker had relationship with adenocarcinoma.
Background: Lung carcinoma is the leading cause of cancer mortality worldwide. Although the 5-year survival rate nearly tripled from 5-15% over the last 25 years, the estimated number of deaths still exceeds 1.3 million annually. The overall 5-year survival of lung cancer is only 10% in Europe and 15% in the United States. The aim of the current study was to determine the long-term survival and the effect of certain prognostic factors on survival of patients with lung cancer in Yazd city, Iran. Methods: In this cross-sectional descriptive study, we retrospectively reviewed hospital records and follow-up data of 148 patients with histological proven lung cancer using the cancer data registered between 1998 and 2005 in the pathology department of Shahid Sadoughi educational hospital, Yazd, Iran. Data were extracted from patient documents that included sex, age, clinical manifestations, histopathological report of the tumor and type of treatment given. Results: Overall survival time in all patients was 8.5 months after diagnosis and there was no significant difference in survival according to sex (p=0.958). Histological analysis revealed that squamous cell carcinoma was the most common histologic type (35%). Kaplan-Meier statistical methods estimated the average survival time for SCC to be better (22.6 months) in comparison with the other types of histology (all of them below 10 months). There was a trend towards significance between type of histology and duration of survival (p=0.08). Conclusion: It is reasonable to expect that early lung cancer detection, and appropriated treatment, may improve surgical morbidity and mortality. Low survival of lung cancer in our center patients show our shortages in screening programs for early diagnosis. Designing studies with larger sample size that take some other variables like staging of patients is now necessary.
Background: Lung cancer is one of the most common cancers in the world and a major cause of death from cancer. One of the important indicators to compare the prevalence and incidence of the disease is a change in the trend. The aim of this study was to investigate the changes in the incidence of lung cancer in Iran. Materials and Methods: This study was conducted based on existing data obtained from a national registry of cancer cases and the Disease Management Center of Ministry of Health in Iran. All cases registered in the country were included during 2003-2008. Incidence rates were reported based on the direct method and standard population of World Health Organization. The study also examined the morphology of common lung cancers. Trends in incidence underwent joinpoint regression analysis. Results: Based on the results of this study, 14,403 cases of lung cancer have been recorded of which 10,582 cases were in men and 3,821 in women. Highest incidence rates were observed in the 80-84 age group. Considerable variation across provinces was evident. In females squamous cell carcinoma (SCC) demonstrated a reduction from 24% to 16% of lesions over the period of study, while adenocarcinoma rose from 21% to 29%. In males a similar reduction in SCC was apparent (42% to 29%, again with increase in AC (13 % to 18%). Conclusions: The results show that the increase in the incidence of lung cancer the trend is that more men than women and in men and may be caused by changes in smoking pattern. The incidence of lung cancer in the North West and West provinces was higher than in other regions.

Background: Yanting is one of high risk areas for esophageal cancer and the screening program was therefore initiated there. This study was aimed to investigate the dietary behaviors on the risk of esophageal squamous cell carcinoma (ESCC), among the individuals with normal and abnormal esophagus mucosa. Materials and Methods: A frequency matched case-controls study was proposed to estimate the different distribution of dietary behavior between individuals of control, esophagitis and cancer groups. Cancer cases were selected from hospitals. Esophagitis cases and controls were selected from screening population for ESCC. Health workers collected data for 1 year prior to interview, in terms of length of finishing a meal, temperature of eaten food and interval between water boiling and drinking. Chi-square, Kruskal-Wallis tests and unconditional logistic regression model were used to estimate differences and associations between groups. Results: Compared with controls, length of finishing a meal ${\geq}15mins$ was related to a reduced OR for cancer (OR=0.46, 95%CI, 0.22-0.97) and even compared with cases of esophagitis, the OR of cancer was reduced to 0.30 (95%CI, 0.13-0.72). The OR for often eating food at a high temperature was 2.48 (95%CI 1.06, 5.82) for ESCC as compared with controls. Interval between water boiling and drinking of ${\geq}10mins$ was associated with lower risk of cancer: the OR was 0.18 compared with controls and 0.49 with esophagitis cases (p<0.05). Conclusions: Length of eating food ${\geq}15mins$ and interval between water boiling and drinking ${\geq}10mins$ are potentially related to reduced risk of esophageal SCC, compared with individuals with normal and abnormal esophageal mucosa. Recommendations to Yanting residents to change their dietary behaviors should be made in order to reduce cancer risk.
Background: Inverted papilloma(IP) of the nasal cavity and paranasal sinuses is a benign neoplastic condition that can be associated with squamous cell carcinoma (SCC). Several studies have indicated an etiologic role for viruses in the development of inverted papilloma. And it is necessary to find out the significance of a biologic markers such as p53, c-erbB-2 to predict the malignant potential. The purposes of this study are to detect HPV in inverted papilloma of the nasal cavity and paranasal sinus, to examine role of HPV as an etiological agent, to examine the relationship between HPV subtype and malignant transformation of inverted papilloma, and to investigate the relation between expression rate of p53, c-erbB-2 and HPV in recurrent or malignant transformation cases. Material and Methods: Thirty two cases of inverted papilloma(IP) in the nasal cavity and paranasal sinuses were reviewed and classified into 3 groups; simple IP, IP with dysplasia group, IP with squamous cell carcinoma group. Paraffin embedded achival tissue was used in this study. The HPV was detected by in situ hybridzation (ISH) using HPV type 6/11, 16/18, 31/33/35 DNA probes. Expression of p53 and c-erbB-2 was examined by immunohistochemical staining. Results: 1) The HPV was detected in 6(19%) out of 32 cases. 2) The HPV 6/11 was dectected in 4 out of 21 cases of simple IP, HPV 16/18 in 1, HPV 31/33/35 in lout of 8 cases of IP with dysplasia respectively. 3) The positive expression of p53 was 13 cases out of 32 cases; 2 out of 21 cases of simple IP, all of 8 cases of IP with dysplasia and 3 cases of IP with squamous cell carcinoma 4) The positive expression of c-erbB-2 was in 24 out of 32 cases; 16 out of 21 cases of simple IP, 6 out of 8 cases of IP with dysplasia, 2 out of 3 cases of IP with squamous cell ca. 5) The recurrence of IP occurred in lout of 6 cases of positive for HPV, in 4 out of 26 cases negative for HPV. 6) The recurrence of IP occurred only in positive cases for p53. 7) The recurrence of IP occurred in 4(17%) out of 24 cases positive for c-erbB-2, in 1(13%) out of 8 cases negative for c-erbB-2. Conclusion: The p53 expression was associated with Inverted papillomas exhibiting evidence of malignant transformation. Also, there was a correlation between the p53 expression and recurrence.
Background: To evaluate the performance of Siriraj liquid-based cytology (LBC) for cervical neoplasia screening after increasing use of this technology. Materials and Methods: Cytological reports of 103,057 Siriraj-LBC specimens obtained in 2007-2009 were compared with those of 23,676 specimens obtained in 2006. Results: Comparing with the year 2006, the 2007-2009 patients were slightly older ($43.4{\pm}12.yr$ vs $42.7{\pm}12.2yr$, p <0.001), and their specimens had much lower proportion of unsatisfactory slides (OR=0.06, 95%CI 0.04-0.09) with comparable detection rates (3.96% vs 3.70%, p=0.052) but different proportions of various cytological abnormalities (p<0.001). The 2007-2009 Siriraj-LBC had a negative predictive value (NPV) for cervical intraepithelial neoplasia 2+ (CIN2+) of 97.6% and an overall positive predictive value (PPV) of 43.9%. The PPV for CIN2+ varied with types of abnormal cytology, from 13.7% to 93.8% in atypical squamous cells of undetermined significance (ASCUS), low-grade squamous intraepithelial lesion (LSIL), atypical squamous cells cannot exclude HSIL (ASC-H), high-grade squamous intraepithelial lesion (HSIL), atypical glandular cells (AGC), to squamous cell carcinoma (SCC), respectively. The PPVs for CIN2+ in ASCUS and LSIL were comparable, but the PPV for CIN1 was higher for LSIL than for ASCUS (41.63% vs 16.32%). Conclusions: Siriraj-LBC has demonstrated a stable detection rate and NPV for CIN2+ of >95% since the first year of use. The comparable PPVs for CIN2+ of ASCUS and LSIL suggests that these two conditions may undergo similar management; other cytological abnormalities need immediate evaluation.

Background: Northern Iran counts as one of the highest prevalence regions for esophageal cancer (EC) worldwide. This study was designed to assess the epidemiologic aspects of EC in north central and northwest Iran over a 10 year period. Materials and Methods: The Guilan cancer registry study (GCRS) is a population-based cancer registry study featuring retrospective (1996-2003) and prospective (2004-2005) phases. A detailed questionnaire based on WHO standards for cancer registratration was applied to gather the required information. Two trained physicians coded information using ICD-O-3 in close coordination with an expert pathologist. Results: A total of 19,936 cases of malignancy (mean age $55.4{\pm}18.0$ years, range: 1-98 years) were registered, including 1,147 cases (670 males, 447 female; mean age: $64.0{\pm}11.5$ years) of EC. In 1996 the male/female ratio among patients with EC was 1.25 which increased to 1.53 in 2005. The lower third of the esophagus still remained the most common site of tumors. The average age-standardized rate (ASR) was 6.9 and 4.1 per $10^5$ men and women, respectively. In 1996, the ASRs were 7.2 and 5.2 per $10^5$ men and women which decreased to 6.9 and 4.1 per $10^5$ in 2004-2005. Squamous cell carcinoma (SCC) was the most prevalent histological subtype of EC accounting over 80% of cases. Conclusions: However the prevalence of adenocarcinoma (ADC) showed an increase to 18.4%. Guilan province may be considered a relatively low incidence region for EC.
연구배경 : 원발성 악성기관종양은 매우 드문 질환으로 모든 호흡기 종양의 0.1% 정도 차지하는 것으로 되어 있으며 매우 드물게 보고가 있어 왔다. 원발성 악성기관종양을 의심할 만한 특이 증상, 신체검사 소견이 없으며 흉부 X-선 상에서도 정상으로 보이는 경우가 많아 상당 기간을 폐쇄성 폐질환 또는 기관지 천식 등으로 치료 받게되어 기관종양에 대한 의심을 하지않을 경우 진단에 어려움이 많으며 적극적인 치료시기를 놓치는 수가 많다. 방 법 : 1981년 1월부터 1995년 12월까지 연세의료원, 영동세브란스병원과 원주기독병원에 입원하였던 환자중 원발성 악성기관종양으로 진단된 17예를 대상으로 후향적 연구를 하였으며 기관내 종양의 발병위치, 흡연력과 호흡기증상 등 입상양상, 내원 당시 흉부 X-선 판독 결과, 내원 후 시행한 객담 세포진 검사 및 환자가 처음으로 의료기판에 방문해서 부터 원발성 악성기관종양으로 진단되기까지의 기간, 내원 당시 초진진단명 및 치료에 따른 생존기간을 관찰하였다. 결 과 : 50세를 기준으로 하여 종양의 발생 빈도를 보면 편평상피세포암의 9예중 8예(88%)가 50세 이상에서, 경우 6예중 5예(83%)가 50세 미만에서 발생하였다. 편평상피세포암의 경우 전 예가 남자에서 관찰 되었으며 전 예에서 흡연력이 있었다. 원주종 6예 중 여자가 5예 이었다. 기관내 발생 위치는 상부 $\frac{1}{3}$이 17예 중 8예(47%)로 제일 많았다. 임상 양상은 호흡곤란이 13예(76%)로 제일 많았으며 그 다음으로는 천명음, 기침, 가래 등의 순이었다. 내원 당시 흉부 X-선 판독 결과 15예중 1예에서만 기관종양으로 판독되었으며 객담 세포진 검사에서는 12예중 1예에서만 악성 종양세포가 관찰 되었다. 진단 되기까지의 기간은 원주종의 경우 평균 24.9개월이었고 편평상피세포암의 경우 5개월 이었다. 내원 당시 진단으로는 천식이 8예(47%)로 제일 많았으며 기관종양을 의심하였던 경우는 4예(23%) 이었다. 결 론 : 원인이 불분명한 객혈 및 쉰 목소리, 흡기성 천명음, 체위 변동에 따른 호흡곤란의 악화, 기관지 천식의 치료에 반응이 없는 경우 등의 비특이적인 기관지 천식의 임상양상을 갖는 경우 기관의 폐쇄성 병변을 의심하여야 하며 흉부 X-선 촬영시 높은 관전압의 사용과 지관지경등 적극적인 진단수기를 시행하여야 하며 원발성 악성기관종양으로 진단되면 가능한 한 근치적 절제술을 시도하고 근치적 수술이 어려운 경우 60Gy 이상의 외부 방사선 치료를 시행함이 바람직 할 것으로 생각된다.
