A home-based exercise program for temporomandibular joint osteoarthritis: pain, functionality, and joint structure

  • Objectives: Osteoarthritis (OA) is the most prevalent and disabling joint disease in the world. Temporomandibular joint (TMJ) exercise is a widely used treatment and could be a beneficial and long-term tool for treating TMJ OA. The present study aims to evaluate the effects of therapeutic exercise in the conservative treatment of TMJ OA. Materials and Methods: A single-group experimental pre-post test was performed. We included patients who met the diagnostic criteria for TMJ OA. Outcome variables were pain intensity (visual analogue scale), functionality (Helkimo index), and structural changes (ultrasound). Follow-up periods were at months 1, 3, and 6. The intervention included a home-based program with thermotherapy, manual therapy, and therapeutic exercise during the entire follow-up period. Results: We included 15 patients and 26 joints, all women with a median age of 57 years (range, 49-62 years). Median change in pain intensity on joint palpation, mouth opening, and at rest at the first month was 47.5 mm, 51 mm, and 60 mm, respectively, and 48 mm, 49.5 mm, and 42.5 mm, at six months (P=0.001). The Helkimo index showed significant improvement in medians from baseline severe dysfunction (17 points) to minimal dysfunction at three and six months (2 points) (P=0.001). Ultrasound showed improved disc position. Conclusion: This study demonstrated significant improvements in pain, function, and joint disc position and represents a valuable tool for the long-term treatment of patients with TMJ OA.

Prevalence, pattern, etiology, and management of maxillofacial trauma in a developing country: a retrospective study

  • Objectives: This retrospective study aims to evaluate the prevalence of maxillofacial trauma in a developing country, along with its pattern, etiology and management. Data for the present study were collected from the Department of Dentistry, ESIC Medical College and Post Graduate Institute of Medical Sciences and Research, Chennai in India. Materials and Methods: The medical records of patients treated for maxillofacial injuries between May 2014 and November 2015 were retrospectively retrieved and analyzed for prevalence, pattern, etiology, and management of maxillofacial trauma. SPSS software version 16.0 was used for the data analysis. Results: Maxillofacial fractures accounts for 93.3% of total injuries. The mean and standard deviation for the age of the patients were $35.0{\pm}11.8$ years and with a minimum age of 5 years and maximum age of 75 years. Adults from 20 to 40 years age groups were more commonly involved, with a male to female ratio of 3:1. There was a statistically significantly higher proportion of males more commonly involved in accident and injuries (P <0.001). Conclusion: The most common etiology of maxillofacial injury was road traffic accidents (RTA) followed by falls and assaults, the sports injuries seem to be very less. In RTA, motorized two-wheelers (MTW) were the most common cause of incidents. The majority of victims of RTA were young adult males between the ages of 20 to 40 years. The malar bone and maxilla were the most common sites of fracture, followed by the mandible. The right side of the zygomatic complex was the predominant side of MTW injury. The majority of the zygomatic complex fractures were treated by conservative management. Open reduction and internal fixation were performed for indicated fracture patients.

  • Objectives: The purpose of this article is to analyze the incidence, demographic distribution, type, and etiology of mandible fractures that were treated by the Department of Oral and Maxillofacial Surgery in Kyung Hee University Dental Hospital from January 2002 to December 2012. Materials and Methods: This was a descriptive and analytic retrospective study that evaluated 735 patients that were treated for mandible fracture. Results: This study included 1,172 fractures in 735 patients. The ratio of male to female patients was 5.45 : 1; the maximum value was in patients between 20 and 29 years (38.1%) and the minimum in patients over 70 years old. The monthly distribution of facial fractures peaked in the fall and was lower during winter. No specific correlation was identified based on the annual fracture distribution. Among the 735 fracture patients, 1.59 fracture lines were observed per patient. The most frequent site was the symphysis, which accounted for a total of 431 fractures, followed by the angle (348), condyle (279), and body (95). The symphysis with angle was the most common site identified in combination with fracture and accounted for 22.4%, followed by symphysis with condyle (19.8%). The angle was the most frequent site of single fractures (20.8%). The major cause of injury was accidental trauma (43.4%), which was followed by other causes such as violence (33.9%), sports-related accidents (10.5%), and traffic accidents (10.1%). Fracture incidents correlated with alcohol consumption were reported between 10.0%-26.9% annually. Conclusion: Although mandible fracture pattern is similar to the previous researches, there is some changes in the etiologic factors.

비골 골절에 대한 최근 5년간의 후향적 연구 (A RECENT 5-YEAR RETROSPECTIVE STUDY ON NASAL BONE FRACTURE)

  • Purpose: This study was performed to investigate the incidence, types of fracture, treatment, associated fracture and complications in patients with nasal bone fracture. Materials and methods: Clinical examination, patient's records and radiographic images were evaluated in 230 cases of nasal bone fractures who were treated at the Department of Oral and Maxillofacial Surgery, Chonnam National University Hospital for recent 5 years; from January 2002 to December 2006. Results: 1. The age of patient was ranged from 4 to 77 years (mean age=36.6 years); Males was 75.7% (n=174), and females 24.3% (n=56). 2. The cause of the nasal bone fracture in this study was a fall or slip down (28.8%, n=66), sports accident (26.0%, n=60), fighting (21.3%, n=49), traffic accident (9.6%, n=22), industrial trauma (7.8%, n=18), and the others (6.6%, n=15). 3. For the patterns of fracture, simple fracture without displacement occured in 10.4% (n=24). Simple fracture with displacement without septal bone fracture was found in 49.5% (n=114). Simple fracture with displacement in company with septal bone fracture showed in 32.6% (n=75). Commiuted fracture with severe depression was presented in 7.4% (n=17). 4. The reduction the displaced nasal bone was carried out in 2 to 10 days (mean 6.8 days) after the injury. 5. Nasal bone fracture associated with Le Fort I fracture (6.5%, n=6.5), Le Fort II fracture (7.4%, n=17), Le Fort III fracture (1.3%, n=3), NOE fracture (13.9%, n=32), ZMC fracture (17.4%, 40), maxillary bone fracture (8.3%, n=19), orbital blow-out fracture (15.7%, n=36), frontal bone fracture (1.3%, n=3) and alveolar bone fracture (10.9%, n=25). 6. The major type of treatment method was closed reduction in 90% (n=207), open reduction in 3% (n=7), and observation in 7% (n=16). 7. There were some complications such as ecchymosis, hyposmia, hypo esthesia and residual nasal deformity which are compatible. Open rhino-plasty was conducted for 3 patients who had residual nasal deformity. Conclusions: These results suggest that most of nasal bone fractures are occurred physically active aged groups(age 10-49 years) and could be treated successfully with closed reduction at 7 days after the injury.

  • According to the National Health Insurance Corporation in 2008, there were 17,764,428 physical therapy patients, exceeding 31 percent for the population covered by health insurance. This means that three out of 10 Koreans received physical therapy. And now, 10 years later, due to the aging population and the increase in the sports population, the number of patients with physical therapy is expected to be much more than a decade ago. Among them, many physical therapy patients were orthopedic and neurologic disorder. However, in the medical field applied to physical therapy, it is widely applied across all medical fields, including orthopedics, neurosurgery, pediatrics, gynecology, thoracic surgery and dentistry. It is believed that various cases of patients receiving physical therapy will be secured. as mentioned earlier, there will be a large number of patients with physical therapy treatments, making big data analytics easier. based on this, physical therapy applications are thought to be helpful in the analogy of disease and the development of effective physical therapy and will ultimately promote the development of physical therapy.

치과의사 맞춤형 마우스가드의 제작방법 (Understanding and Practice of Eating-Swallowing Disorder)

  • The mouth guard is a device with elasticity to be installed in the oral cavity. It has a function to reduce the trauma in the oral cavity and to protect the teeth and surrounding tissues from trauma. The purpose of mouth guard is to prevent trauma and concussion. It is mandatory to wear a mouthguard in sports where there is a lot of contact and the possibility of trauma or concussion in the mouth area is high. The mouse guard is divided into a stock type, a mouth formed type, and a custom made type according to the manufacturing method. The custom made type is made on the individual dentition model and has excellent retention because of its excellent fit. Also, the effect of trauma prevention is excellent. It is possible to design and adjust by the dentist and reduce the complaint the athlete has about the mouthguard. In this article, the process of making a dentist-customized mouthguard was described.

유치열과 혼합치열기 어린이의 상악 절치부 외상 (TRAUMATIC INJURIES TO THE UPPER INCISORS IN PRIMARY AND MIXED DENTITION)

  • 현대인의 생활양식과 환경이 도시화 및 인공화되고 취미활동이 다양화됨에 따라 외상을 받을 수 있는 요인들이 증가하고 특히 어린이 활동량이 많아져 외상의 기회가 잦아지고 있다. 어린이에서 외상이 발생하는 경우, 어린이의 일상적인 활동 뿐 아니라, 사회성 및 정서 발달에도 영향을 미칠 수 있으며 그 보호자에게도 부정적인 영향을 미칠 수 있기에 외상의 발생 빈도를 줄이고 예방하는 것이 중요하다. 본 연구는 유치열과 혼합치열기에 발생하는 치아 외상을 예방하기 위한 기초 자료를 얻기 위하여, 특히 구강악안면부의 외상시 치아 외상을 가장 많이 받는 상악 유절치와 영구절치를 연구하기 위하여, 2002년 6월부터 2003년 5월까지 연세대학교 치과대학병원 소아치과에 내원한 유치열과 혼합치열기의 신환 8,799명중 상악 절치부 외상을 주소로 내원한 어린이 487명(남자 316명, 여자 171명 )의 상악 절치 955개(유절치 543개, 영구절치 412개)를 조사하여 다음과 같은 결과를 얻었다. 1. 상악 절치부 외상이 호발하는 나이는 유절치에서 $1{\sim}3$세, 영구절치에서 7세였으며 남자대 여자의 비율은 1.85:1이었다. 2. 상악 절치부 외상은 유절치와 영구절치 모두 오후 시간에 많이 발생하였으며, 유절치는 외상 장소로 집이 가장 많았고(44.1%), 영구절치는 길에서 다치는 경우가 가장 많았다(17.5%). 3. 외상의 원인으로 유절치와 영구절치 모두 넘어져 다치는 경우가 가장 많았으며(39.8%, 12.9%), 다음으로 부딪히는 경우가 많았다(22.2%, 6.0%). 유절치에서는 다음으로 떨어져 다치는 경우 교통사고, 폭력과 운동에 의한 외상 순이었으며, 영구절치에서는 폭력, 운동, 교통사고, 떨어져 다치는 순이었다. 4. 조사대상 어린이 1명당 유절치는 1.6개, 영구절치는 2.8개에 외상을 입었다. 5. 유절치와 영구절치 모두에서, 치관 파절 양상은 치수미노출 치관 파절이 많았고, 치주조직 외상 양상은 아탈구가 가장 많았다. 최근 어린이의 단체 생활이나 운동과 취미 활동에 참여하는 나이가 예전에 비하여 점차 낮아지고 있어, 이번 연구 결과와 같이, 외상이 발생하는 나이 역시 지속적으로 낮아질 수 있다. 나이에 따른 어린이의 외상을 받는 장소는 특별한 곳이 아닌, 집이나 학교와 같은 일상의 생활공간이기 때문에 단순한 주의만으로도 외상의 발생을 예방할 수 있을 것으로 사료된다.

COVID-19 팬데믹 상황에서 소아 환자의 구강악안면 외상의 변화 추이: 단일 기관 연구 (The Trend of Change in Oral and Maxillofacial Injuries of Pediatric Patients in the COVID-19 Pandemic: a Regional Emergency Medical Center and Dental Hospital Study)

  • 이 연구의 목적은 코로나 바이러스 감염증(Coronavirus Disease 2019, COVID-19) 시기의 12세 이하 아동에서 발생한 치과 외상의 변화 여부를 분석해 보는 것이다. 세계보건기구에서 COVID-19를 공식적으로 팬데믹으로 선포한 시점인 2020년 3월을 COVID-19의 기시점으로 설정하였다. 2018년 3월부터 2020년 2월까지를 COVID-19 이전 시기의 대상자를 Pre-COVID-19 군, 2020년 3월부터 2022년 3월까지를 COVID-19 이후 시기의 대상자를 COVID-19 군으로 분류하였다. 전자의무기록을 통해 외상과 관련한 정보들을 수집하였다. COVID-19 발생 전후 외상 환자 수는 유의미하게 감소하였다. COVID-19 기간 동안 남·여 비율과, 연령 구간의 분포 순서는 유의한 차이가 없었다. 영구치의 COVID-19 군에서 스포츠로 인한 외상 발생보다는 개인 이동 수단에 의한 외상의 비율이 높아졌다. COVID-19 군에서 치수 침범이 있는 치관 파절 비율이 치수 침범이 없는 치관 파절 비율보다 증가하였고 유의미하였다. 미취학 아동보다 학령기 아동에서 COVID-19에 의한 외상 패턴 변화가 뚜렷하게 관찰되었다. COVID-19와 같은 팬데믹 상황에서는 환경의 변화로 인해 호발하는 진단명이 변할 수 있다는 점을 알 수 있었다.

원주세브란스기독병원 응급실로 내원한 0 - 15세 어린이의 치과적 외상에 관한 후향적 분석 (Retrospective Study of Traumatic Dental Injuries among Children Aged 0 - 15 Years in Wonju)

  • 본 연구는 원주세브란스기독병원 응급실에 내원한 소아 환자들의 나이, 성별, 외상 발생 장소, 외상의 원인, 외상의 위치, 외상 후 경과시간, 응급실 내원시간, 처치의 유형 등을 파악하기 위해 시행되었다. 2011년 3월부터 2015년 10월까지 치과적 외상으로 인해 원주세브란스기독병원 외상 센터에 내원한 0 - 15세 사이 841명의 환자들의 정보를 수집하였다. 외상은 남아에서 호발하였으며, 호발하는 나이는 0 - 3세였다. 6세 미만에서는 집에서 외상이 가장 호발하고, 넘어지는 것이 외상의 가장 주된 원인이었다. 하지만 6세 이상에서는 외상의 장소로 집이 크게 줄어들었으며, 외상의 원인으로 넘어지는 것이 줄어들고 스포츠와 기타 원인이 매우 크게 증가하였다. 나이 대 별로 외상의 원인과 장소에서는 유의한 차이를 보였다. 외상의 위치로는 입술과 상악 절치가 가장 많은 빈도수를 보였다. 응급실을 내원하는 시간대로는 18 - 24시(53.3%)에 내원하는 환자가 가장 많았으며, 0 - 6시(4.6%)에 내원하는 환자는 가장 적었다. 외상 후 경과 시간은 1시간 이내가 51.5%, 1 - 2시간이 26.8%, 2 - 3시간이 11.5%로, 3시간 이내에 내원하는 환자가 89.8%에 달했다. 응급에서 행해지는 처치의 유형으로는 경과관찰을 하는 경우가 가장 많았으며, 가장 많이 행해지는 술식은 봉합술이었다. 본 연구를 통해 소아의 치과적 외상과 응급실 방문은 환아의 성별, 나이, 상황에 따라 역학적인 특징을 보인다는 것을 살펴보았다. 외상의 양상은 어린이의 사회, 발달, 생리적인 요인이 종합되어 나타난다.

교합균형이 자세 중심(重心)에 미치는 영향에 관한 연구 (Effect of occlusal balance on center of gravity in body)

  • Suppose that dental occlusion is related to body posture. We want to find out that improving occlusal balance may affect vibration and distribution of C.O.P. in which way, by measuring change of posture and center of gravity (center of pressure, C.O.P.) which plays important role in measuring balance sensation. Total 11 students at Kyung Hee dental college students, 4 females and 9 males (age: 23-30) participated in this test, who have normal occlusion (Angle's classification I), no TMJ problems. All of the participants have no tooth loss except 3rd molar, no prosthesis over single tooth restoration, no orthopedic problems which affect balance sensation, and no otorhinolaryngological problems. First, we registrated bite by centric relation, and then fabricated stabilization splint that is increased 3.5mm vertical dimension around premolar region. By F-scan (Tekscan Inc., Boston, Mass), we measured discrepancy of average contact pressure of left and right foot. And we also measured discrepancy of vibration of C.O.P(center of pressure). before setting stabilization splint and after wearing stabilization splint at intervals of 1 week, 2 weeks, 3 weeks after. In normal human beings, improved occlusal balance by stabilization splint leads to decrease of vibration of C.O.P. (P<0.05). One week after wearing stabilization splint, vibration of C.O.P. decreased reliably (P<0.05), two weeks after wearing stabilization splint, vibration of C.O.P. decreased similarly comparing to before wearing and one week after wearing. (P<0.05) After two weeks and three weeks, however, it was hard to find reliability. (P>0.05) Difference between average contact pressure of right and left foot also decreased. (P<0.05) We could find decrease after one week of wearing stabilization splint (P<0.05) and two weeks after, the decrease was more reliable than one week after. (P<0.05) After two weeks and three weeks, however, it was hard to find reliability. Improvement of occlusal balance leads to decrease of vibration of C.O.P. and decrease of difference between right and left average contact pressure.
