수직액와 피부절개술을 이용한 자연기흉의 치료 (Treatment of the Spontaneous Pneumothorax by the Vertical Axillary Thoracotomy)

  • Forty patients with spontaneous pneumothorax underwent vertical axillarty thoracotomy for surgical bullectomy of spontaneous pneumothorax between June, 1991 and september, 1992. We evaluated the method in terms of postoperative pain, wound complication, days of hospital stay and cosmetic result. It`s concludid that verical axillary thoracotomy provides satisfactory exposure for limited procedures within the thoracic cavity and offers the specific advantages of minimal and cosmetically acceptable results when compared with the thoracotomy methods.

식도 자연 천공 치험 1례 (Spontaneous Rupture of the Esophagus (Boerhaave's Syndrome) -A case report-)

  • Spontaneous esophageal perforation occurred rarely but often lead to a high mortality and morbidity. We had experienced one case of spontaneous rupture of esophagus. A 52 - year old male patient was admitted to our hospital because of the chest pain and massive hematemesis after emetic strain. Esophagogram that taken at private clinic revealed leakage of dye into the right pleural space. Under the diagnosis of the thoracic esophageal perforation, two stage operation was planned because the size of perforation was large and pyothorax was developed on the right side. At first, exclusion and diversion of the esophagus were carried out. After six months, the esophageal reconstruction with left colon was performed.

Internal derangement as a predictor of provoked pain on mouth opening: A magnetic resonance imaging study

  • Purpose: This study investigated the relationship between pain and internal derangement in temporomandibular disorder(TMD) patients using magnetic resonance imaging (MRI). Materials and Methods: This study analyzed 356 TMD patients (712 temporomandibular joints [TMJs]). The inclusion criteria were the presence of spontaneous or provoked pain on one or both TMJs and having undergone MRI. The patients with provoked pain were divided into 3 groups: pain on palpation, pain on mouth opening, and pain on mastication. MRI was performed using a 1.5-T scanner. T1- and T2-weighted parasagittal and paracoronal images were obtained. According to the findings on the T1-weighted images, another 3 groups were created based on internal derangement: normal, disc displacement with reduction, and disc displacement without reduction. The MRI findings were independently interpreted by 2 experienced oral and maxillofacial radiologists at 2 different times. Statistical analysis was performed by the chi-square test using SPSS (version 12.0; SPSS Inc., Chicago, IL, USA). Results: Provoked pain on mouth opening was found to be correlated with internal derangement in TMD patients (P<.05). However, spontaneous pain and provoked pain on palpation or mastication were not associated with internal derangement(P>.05). Conclusion: These results suggest that internal derangement was a significant predictor of provoked pain on mouth opening.

자연 기흉의 정도에 따른 호흡 및 순환 생리의 변화에 관한 고찰 (Changes in Respiratory & Circulatory Physiology Caused by Various Degrees of Spontaneous Pneumothorax)

  • Here analized the chest physiologic changes caused by various degrees of spontaneous pneumothorax in 77 patients admitted in Pusan National University Hospital from Jan. 1991 to Aug.1992. The results were summarized as follow: 1. There were 59 patients of primary spontaneous pneumothorax and 18 of secondary spontaneous pneumothorax. 2. The intrapleural pressure risings were paralled to the increasing sizes of pneumothorax, especiallythe intrapleural pressure changes were significant in large pneumothorax. In the secondary spontaneous pneumothoraces the intrapleural pressure were relatively higher than primary in the same sizes of pneumothorax. 3. The intensity of chest pain was paralled to the increasing sizes and intrapleural pressures of the pneumothorax, but the degrees of dyspnea had no linear interrelationship. 4. The pulse rate, cardiac output, and arterial PO2 started to change from positive intrapleural pressure, and significant changes were noted between 6 to 9 mmHg of intrapleural pressure. But the arterial PCO2 changes had no interrelationship to the degrees of pneumothorax.

개흉술을 시행하였던 자발성 기흉의 임상적 고찰 (An Evaluation of the Thoracotomy in Spontaneous Pneumothorax)

  • This study elvaluated clinically the surgical results of the thirty-eight patients who had exploratory thoracotomy for spontaneous pneumothorax between Jan. 1989 and Nov. 1992. Thirty three, or 86.8%, of the patients were male. The most frequent age of the spontaneous pneumothorax requiring thoracotomy was between fifteen years and twenty years. Sudden onset dyspnea and chest pain, which developed in thirty-five patients[89.5%], were the major chief complaints. Bleb and Bulla located in the both upper lobes were the most frequent causes of spontaneous pneumothorax for exploratory thoracotomy. There was no operative death. Postoperative morbidity included three cases of residual air space in the apical pleural space, tow cases of atrial fibrillation, and one each case of reoperation for bleeding, mediasitinitis, and sternal osteomyelitis. All postoperative complications were treated without any serious problems. This study suggests that early exploratory thoracotmy is desirable for patients with prolonged air-leak, massive air-leak, or multiple blebs and giant bullae on the computed tomography of the chest.

  • 자발성 종격동기종은 기저질환이나 외상의 병력이 없이 발생할 수 있고 드물게는 운동중에 발생할 수 있다. 저자들은 23세 남자 환자에서 태권도중에 발생한 자발성 종격동기종 1예를 경험하였기에 이를 문헌고찰과 함께 보고하는 바이다.

흉요추 이행부 원인에 의한 후장골릉 부근 요통 -증례 보고- (Pain Around the Posterior Iliac Crest of Thoracolumbar Origin -Case report-)

  • Pain around the posterior iliac crest area is usually attributed to disorders of the lower lumbar or lumbosacral spine. However, low back pain arising from the thoracolumbar region is common and it is very similar to low back pain of lumbosacral origin. Low back pain of thoracolumbar origin is clinically distinguished from other nonspecific low back pain syndrome. It is characterized by symptoms localized at one posterior iliac crest innervated by posterior branch of $T_{12}$ spinal nerve. Patients never complain of spontaneous pain at the thoracolumbar junction. Only localized tenderness over involved segments of thoracolumbar junction can be noted. We report two cases of posterior iliac crest pain of thoracolumbar origin which was relieved by the treatment on the thoracolumbar junction.

Painful legs and moving toes syndrome in a 16-year-old girl

  • Painful legs and moving toes (PLMT) syndrome is characterized by spontaneous movements of the digits and pain in one or both lower extremities. Of the reported cases, a majority of the patients was female, and the mean age of onset was 58 years. Only one pediatric case has been reported so far. Herein, we report the first adolescent case of PLMT in Korea. A 16-year-old girl complained of tingling pain in the left leg and involuntary movement of the ipsilateral great toe one month after a second untethering surgery. Three years ago, she had undergone untethering surgery to correct lipomeningomyelocele at the S2 level of the conus medullaris. At that time, she was diagnosed with polyradiculopathy at the left L5 level with axonal involvement. We diagnosed her with PLMT syndrome and prescribed gabapentin. Her symptoms diminished within a day. Complete relief from involuntary movement of the toe was achieved within four months. PLMT is a rare syndrome but it should be considered in the differential diagnosis of children and adolescents with limb pain and spontaneous movement in their toes.

Referred Pain in Right Arm from Abdominal Wall Pseudoaneurysm

  • Pseudoaneurysm of the abdominal wall is a possible but very rare clinical entity. It is a known complication of surgery, trauma, or arterial puncture, but it is rarely spontaneous. Even though it can usually present with a wide range of local symptoms, it can cause referred pain via spinal cord, which is cross-excited with afferent sympathetic nervous system. We report a case of right arm pain which was referred from a small abdominal pseudoaneurysm like a referred pain from gall bladder. This rare entity should be considered in the differential for pain management in case that the pain does not resolve with medication or interventional pain management.

Spontaneous Lead Breakage in Implanted Spinal Cord Stimulation Systems

  • Spinal cord stimulation (SCS) has become an established clinical option for treatment of refractory chronic pain. Current hardware and implantation techniques for SCS are already highly developed and continuously improving; however, equipment failures over the course of long-term treatment are still encountered in a relatively high proportion of the cases treated with it. Percutaneous SCS leads seem to be particularly prone to dislocation and insulation failures. We describe our experience of lead breakage in the inserted spinal cord stimulator to a complex regional pain syndrome patient who obtained satisfactory pain relief after the revision of SCS.
