하악전돌증 환자에서 악교정수술 후 하악각 및 하악폭경의 변화 (CHANGES IN GONIAL ANGLE AND MANDIBULAR WIDTH AFTER ORTHOGNATHIC SURGERY IN MANDIBULAR PROGNATHIC PATIENTS)

  • The treatment plan for orthognathic surgery must be based on accurate predictions, and this can be produced the most esthetic, functional and stable results. The purpose of this study was aimed to evaluate the amount and interrelationship of the gonial angle and the mandibular width change after the mandibular setback surgery in the mandibular prognathic patients. Twenty patients were selected who received orthognathic surgery after presurgical orthodontic treatment. The patients with skeletal and dental Class III malocclusion were operated upon with bilateral sagittal split ramus osteotomy and mandibular setback. The lateral and posteroanterior cephalometric radiographs were taken preoperatively, postoperative 1 day and 12 months later after the orthognathic surgery, and then the gonial angle and mandibular width were measured. The computerized statistical analysis was carried out with SPSS/PC program. The gonial angle at postoperative 1 day was decreased about $5.3^{\circ}$ than preoperative value and the gonial angle at postoperative 12 months was increased about $1.4^{\circ}$ than postoperative 1 day. So the gonial angle at postoperative 12 months was decreased about $3.9^{\circ}$ than preoperative value. The mean preoperative gonial angle was $125.35^{\circ}{\pm}7.36$, showing significantly high value than normal and mean gonial angle at postoperative 12 months was $121.45^{\circ}{\pm}6.81$, showing value near to normal. The mandibular width at postoperative 1 day was decreased about 1.1 mm than preoperative value and the mandibular width at postoperative 12 months was more decreased about 1.7 mm than postoperative 1 day. So the mandibular width at postoperative 12 months was decreased about 2.8 mm than preoperative value. These results indicate that sagittal split ramus osteotomy in mandibular prognathic patients with high gonial angle is effective to improvement of gonial angle. It is considered to be helpful for maintenance of postoperative stable gonial angle area that detailed postoperative care and follow-up.

  • Introduction. In patients with mandibular prognathism, Bilateral Sagittal Split Ramus Osteotomy(BSSRO) combined with orthodontic treatment reduces oral volume and influences tongue and other surrounding tissues. Purpose of this study was to analyze post-operative tongue position and airway dimension, as well as mandibular changes in vertical, horizontal, and angular dimensions. Materials and methods. Height of dorsum of tongue, width of airway, gonial angle and lower facial height of mandibular prognathic patients who visited Kangdong Sacred Heart Hospital from Jan. 2001 to Dec. 2006 were anaylzed via pre-operative and post-operative cephalograms. T-test was used to compare pre-operative and post-operative measurements. Also, correlations among pre-operative measurements of the patients were analyzed. Results and conclusion. A significant correlation was shown between ANS-Xi-PM area and location of dorsum of tongue in pre-operative patients. A significant superior movement of tongue and decrease of airway width was observed in post-operative patients. Also the upper gonial angle decreased significantly.

50 kHz 체장어군탐지기용 분할 빔 음향 변환기의 지향성 보정 및 위치각 추정 (Estimation of Angular Location and Directivity Compensation of Split-beam Acoustic Transducer for a 50 kHz Fish Sizing Echo Sounder)

  • The most satisfactory split-beam transducer for fish sizing maintains a wide bearing angle region for correct fish tracking without interference from side lobes and lower sensitivity to fish echoes outside of the main lobe region to correctly measure the angular location of free-swimming fishes in the sound beam. To evaluate the performance of an experimentally developed 50 kHz split-beam transducer, the angular location of a target was derived from the electrical phase difference between the resultant signals for the pair of transducer quadrants in the horizontal and vertical planes consisting of 32 transducer elements. The electrical phase difference was calculated by cross-spectral density analysis for the signals from the pair of receiving transducer quadrants, and the directivity correction factor for a developed split-beam transducer was estimated as the fourth-order polynomial of the off-axis beam angle for the angular location of the target. The experimental results demonstrate that the distance between the acoustic centers for the pair of receiving transducer quadrants can be controlled to less than one wavelength by optimization with amplitude-weighting transformers, and a smaller center spacing provides a range of greater angular location for tracking of a fish target. In particular, a side lobe level of -25.2 dB and an intercenter spacing of $0.96\lambda$($\lambda$= wavelength) obtained in this study suggest that the angular location of fish targets distributing within a range of approximately ${\pm}28^{\circ}$ without interference from side lobes can be measured.

Design of S-Band Phased Array Antenna with High Isolation Using Broadside Coupled Split Ring Resonator

  • In this paper, a method of designing a Vivaldi type phased array antenna (PAA) which operates at S-band (2.8-3.3 GHz) is presented. The presented antenna uses broadside coupled split ring resonators (BC-SRRs) for high isolation, wide field of view, and good active S-parameter characteristics. As an example, we design a $1{\times}8$ array antenna with various BC-SRR structures using theory and EM simulations. The antenna is fabricated and measured to verify the design. With the BC-SRR implemented between the two radiating elements, the isolation is shown to be enhanced by 6 dB, up to 23 dB. The scan angle is shown to be within ${\pm}53^{\circ}$ based on a -10 dB active reflection coefficient. The operation of the scan angle is possible within ${\pm}60^{\circ}$ with a little larger reflection coefficient (-7 dB to -8 dB). The proposed design with BC-SRRs is expected to be useful for PAA applications.

Roles of Bearing Angle in Bond Action of Reinforcing Bars to Concrete

  • The ribs of deformed bars can split the cover concrete by wedging action or shear off the concrete in front of the ribs. As slip of deformed bars increases, the rib face angle is flattened by the crushed concrete wedge, which reduces the rib face angle to a smaller bearing angle. The roles of bearing angle are explored to simulate this observation. Analytical expressions to determine bond strength for splitting and pullout failure are derived, where the bearing angle is a key variable. As the bearing angle is reduced, splitting strength decreases and shearing strength increases. When splitting strength becomes larger than shearing strength, the concrete key is supposed to be sheared off and the bearing angle is reduced with decreasing the splitting strength. As bars slip, bearing angle decreases continually so that splitting bond strength is maintained to be less than shearing bond strength. The bearing angle is found to play a key role in controlling the bond failure and determination of bond strength of ribbed reinforcing steel in concrete structures.

비인두암 Split VMAT 치료계획 유용성 평가 (Utility Evaluation of Split VMAT Treatment Planning for Nasopharyngeal cancer)

  • 목 적: 비인두암 (Nasopharyngeal cancer) 방사선 치료 시 Tomotherapy을 이용한 IMRT는 종양 조직에 정확한 선량을 전달하며 정상조직에는 급격한 선량 감소의 효과가 있지만 MU가 높고 Beam On Time이 긴 단점이 있다. 본 연구는 Helical IMRT (Tomotherapy)와 VMAT (Linac : 2Arc), Split VMAT (Linac : 4Arc)의 치료계획을 시행하여 종양 및 정상 조직, 저 선량 분포의 차이를 분석하고 Split VMAT의 효율성을 평가해 보고자 한다. 대상 및 방법: 본원의 비인두암 환자 10명을 대상으로 하였으며 3개 치료계획 (Helical IMRT, VMAT, Split VMAT)을 비교하였다. Helical IMRT 치료 계획은 Precision® (Version 1.1.1.1, Accuray, USA)을 이용하였고 VMAT, Split VMAT 치료계획은 Pinnacle3® (Version 9.10, Philips, USA)을 이용하였다. 선량은 Daily Dose 2.4 Gy (GTV + 0.3 cm) / 2 Gy (CTV + 0.3 cm) 16 Fx으로 Total Dose 38.4 Gy / 32 Gy를 적용하였고 GTV + 0.3 cm (P_GTV)가 V38.4Gy이 95%가 되도록 처방하였다. VMAT은 360° 2Arc, Split VMAT은 좌, 우, 상, 하로 Field를 나누고 360° 4Arc, 6MV를 사용하였다. 평가는 종양 및 정상조직, 저 선량 영역의 차이를 비교하여 치료 계획의 Quality를 평가하였고 Beam On Time을 비교하여 효율성을 평가하였다. 결 과: 대상 환자들의 3개 치료계획 (Helical IMRT, VMAT, Split VMAT)의 평가 항목 평균 값은 P_GTV의 H.I (Homogeneity Index)는 (1.04, 1.11, 1.1), P_CTV의 C.I (Conformity Index)는 (1.03, 0.99, 1.00)이었다. RT Parotid Gland의 평균 선량 (Gy)은 (14.54, 17.06, 14.76), LT Parotid Gland의 평균 선량 (Gy)은 (14.32, 17.32, 15.09), P_Cord (Spinal Cord + 0.3 cm)의 최대 선량 (Gy)은 (20.57, 22.59, 21.06), Brain Stem의 최대 선량 (Gy)은 (22.35, 23.99, 21.68)이었다. 50% isodose curve (cc)는 (1332, 1132.5, 1065.2)이었다. Beam On Time (sec)은 (373.7, 130.7, 254.4)이었다. 결 론: Split VMAT은 두경부치료에 많이 사용하는 Helical IMRT와 비교했을 때 유사한 치료계획 Quality를 보이면서도 저 선량 영역을 줄여주며 Beam On Time이 줄어든 결과를 보여주었고 VMAT보다 좋은 결과를 보였다. Split VMAT은 비인두암 뿐만 아니라 다른 두경부암에서도 효과적이라 사료된다.

Modified Split Flux를 이용한 주광률 계산식 적용에 관한 연구 (Application of the Daylight Factor Formula with the Modified Split Flux)

  • Daylighting has a great effect on people inside the room. It is also closely related to the lighting energy consumption. Daylight factor(DF) is a very important index for evaluation of the daylighting in overcast sky. The objective of this study is to verify the suitability of the Tregenza's Modified Split Flux formula for the calculation of the daylight factor. We compared the daylight factors calculated by two methods; one by a measurement with 1/5 scale model and the other by the Daysim program. We used variables for verification as window wall ratios(WWR) and angles of the sky visible. As a result, daylight factor calculated by Modified Split Flux is similar to the measurement when more daylight enters the inside. And error is significantly increased when the angle of the sky visible is $50^{\circ}$.

The effect of botulinum toxin-A injection into the masseter muscles on prevention of plate fracture and post-operative relapse in patients receiving orthognathic surgery

  • Background: Botulinum toxin-A (BTX-A) injection into muscle reduces muscular power and may prevent post-operative complication after orthognathic surgery. The purpose of this study was (1) to evaluate BTX-A injection into the masseter muscle on the prevention of plate fracture and (2) to compare post-operative relapse between the BTX-A injection group and the no injection group. Methods: Sixteen patients were included in this study. Eight patients received BTX-A injection bilaterally, and eight patients served as control. All patients received bilateral sagittal split ramus osteotomy for the mandibular setback and additional surgery, such as LeFort I osteotomy or genioplasty. Post-operative plate fracture was recorded. SNB angle, mandibular plane angle, and gonial angle were used for post-operative relapse. Results: Total number of fractured plates in patients was 2 out of 16 plates in the BTX-A injection group and that was 8 out of 16 plates in the no treatment group (P = 0.031). However, there were no significant differences in post-operative changes in SNB angle, mandibular plane angle, and gonial angle between groups (P > 0.05). Conclusions: BTX-A injection into the masseter muscle could reduce the incidence of plate fracture.

하악골 비대칭 환자의 양측성 하악골 시상분할 골절단술 후 하악과두의 위치 변화 (POSTOPERATIVE POSITIONAL CHANGE OF CONDYLE AFTER BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY ASSOCIATED WITH MANDIBULAR ASYMMETRY)

  • Purpose: After the surgical correction with sagittal split ramus osteotomy, the position of the mandibular condyle in the glenoid fossa and the proximal segment of the mandible change because of bony gap between proximal and distal segment, especially in case of mandibular setback asymmetrically. In this study, positional changes in the condyle and proximal segment after BSSRO were estimated in the mandibular asymmetry patient by analyzing the in submentovertex view and P-A cephalogram for identification of ideal condylar position during surgery. Patients and Methods: The 20 patients were selected randomly who visit Dankook Dental Hospital for mandibular asymmetry. Bilateral sagittal split ramus osteotomy with rigid fixation was performed and P-A cephalogram and submentovertex view was taken at the time of preoperative, immediate postoperative, 3 month postoperative period. Results: Intercondylar length and transverse condylar angle was increased due to inward rotation of proximal segment and anteromedial rotation of lateral pole of condyle head. The condylar position had a tendency to return to the preoperative state and after 3 months return up to about half of the immediate post-operative changes, and all the results showed more changes in asymmetry patient and deviated part of the mandible. Conclusion: Based on all these results above, surgeon should make efforts to have a precise preoperative analysis and to have a ideal condylar position during rigid fixation after BSSRO.

Split-triplet 분사기를 장착한 액체 추진제 로켓엔진의 연소특성 해석 (Numerical Analysis of Combustion Characteristics in a Liquid Propellant Rocket Engine with Split-triplet Injector Elements)

  • Split-triplet(F-O-O-F)형의 분사기 요소를 장착한 KSR-III 액체 로켓엔진의 성능과 연소장을 고찰하기 위해 수치해석을 수행하였다. 방사형 분사기 배열의 액체 로켓엔진의 수치해석 검증을 위해 2차원 축대칭과 3차원 계산을 수행하여 연소시험 결과와 비교하였다. 2차원 축대칭 계산과 3차원 해석을 통하여 성능 측면에서 오차가 약 3∼5% 정도의 정확도를 유지하며 예측하는 것을 볼 수 있었다. 3차원 해석에서는 연소장을 해석하여 분사기 면의 온도 분포가 연소 시험결과와 정성적으로 일치하는 것을 볼 수 있었다. 또한 충돌각의 감소와 분사기 배열의 직교-방사형으로 변경이 방사형 분사기배열의 국부적인 고온 영역을 감소시키며 성능에도 영향을 미치는 것을 볼 수 있었다. 이러한 해석을 통하여 분사기 배열과 충돌각 선정이 액체 로켓엔진의 성능과 연소장에 영향을 미치는 중요한 요인이 됨을 알 수 있었다.
