A Study on the Concept and Mechanism of 'The Pi Controls the blood(脾統血)' ('비통혈(脾統血)'의 개념(槪念)과 기전(機轉)에 관한 고찰(考察))

  • Objectives : This study was done to investigate the formation process of the 'The Spleen controls the blood(脾統血)' concept, to clarify what this concept means and the mechanism of its physiology. Methods : Contents including 'Controlling blood(統血)' and 'Binding blood(攝血)' were searched and analyzed in medical classics. Previous researches were applied. Results & Conclusions : The concept of 'Controlling blood' could be defined as the control of blood movement. This means that it sends blood to where it's needed, and inhibits flow from where it's excessive. 'The Spleen controls the blood' was not used as a physiologic term in early books like Huangdineijing(黃帝內經). It was first used in the 13C, then widely after the 16C. The mechanism of 'Controlling blood' could be classified as the function of 'Production', 'Distribution', and 'Adjustment' of blood. 'Production' of blood can reduce blood fever(血熱) and blood stasis(瘀血), and prevent bleeding. 'Distribution' of blood can reduce the symptoms raised by lack of blood in the five viscera and body. 'Adjustment' of blood means maintaining homeostasis and stability of the human body. Pi can adjust blood flow and prevent blood from being imbalanced.

Study on the Bitonghyul (비통혈(脾統血)에 관한 고찰)

  • Although Bi(脾 Spleen) is a major organ which controls digestion and absorption in Korean medicine, there also introduced another function of bi, which is called 'bitonghyul(脾統血)' that means the spleen controls(holdings) the blood. While the spleen is involved in the function of blood generation through the digestion and absorption of food, 'bitonghyul' is another expression regarding function of the spleen. The meaning of 'bitonghyul' is related with the function of keeping the blood flowing within the blood vessels, hemostasis. Comparing the function of the spleen with the mechanisms of hemostais, it is concluded that 'bitonghyul' is a physiological effects which affect hemostatic mechanisms via absorption of vitamin K.

  • In this paper, we attempted to link the theories of korean medicine and medicine on 'Heart controls the blood vessels (心主血脈)', 'Spleen controls the blood (脾統血)' and 'Liver possesses the blood (肝藏血)' related to blood circulation for the new possibility of theorizing integrative medicine. 'Heart controls the blood vessels (心主血脈)', which is explained as the function of blood that nourishes the five viscera and six bowels under the control of the heart, can be interpreted as the vascular system of the dynamics of blood movement through the cardiovascular system and the function of blood flow dispersing to tissues. 'Spleen controls the blood (脾統血)', which enables the normal production and movement of blood, can be interpreted as the function of vascular endothelial cells such as the role of permeability barrier between plasma and interstitial fluid, regulation of the amount of other substances, mediation of angiogenesis, production of damage response factors, and secretion of substances that regulate coagulation and anticoagulation. The 'Liver possesses the blood (肝藏血)', which regulates blood flow according to metabolic volume, can be interpreted as the function of arterioles that distribute the flow to various organs and tissues and efficiently transfer blood flow to the metabolic needs of individual tissues. The process of theorizing integrated medicine is necessary because it can provide opportunities for the use of multidisciplinary approaches and cooperation, and because it can develop a curriculum that can balance the universality of health care education based on biomedical science and the professionalism of korean medicine doctors.
