척추 결핵의 전방유합술시 Titanium Mesh Cage의 효과 (The Efficacy of Titanium Mesh Cage in Tuberculous Spondylitis Treated with Anterior Intervertebral Fusion)

  • Objective : The maintenance of the correction of kyphotic deformity is one of the difficult problem in tuberculous spondylitis after anterior debriment and fusion with tricortical bone graft. The goal of this study is to find out the efficacy of titanium mesh cage impacted with autogenous bone chip in tuberculous spondylitis treated with anterior intervertebral fusion. Materials and Method : Twelve patients were treated with anterior intervertebral fusion using titanium mesh cage for tuberculous spondylitis from January 1996 to June 1999. We analized the changes in the correction of kyphotic deformity, changes of ESR and CRP, fusion state and recurrence after anterior intervertebral fusion with titanium mesh cage. Results : Clinical symptoms were improved in all twelve patients without any neurologic complications. The mean kyphotic angle corrected was 7.3 degrees immediately after operation, but the loss of correction of kyphotic angle was 2.2 degrees after 3 months and 2.6 degrees after 6 months. We found that the loss of correction of kyphotic deformity occurred mainly within the first 3 months after surgery. Only one patient, suffered from acute hepatic failure after first operation and had an insufficient anti-tuberculous medication therapy, showed recurrence of tuberculous spondylitis after 6 months. The patient underwent a second operation with posterior fixation procedure with good outcome. The changes of ESR and CRP were not specifically important factor to reveal recurrence of tuberculosis of the spine in our series. Conclusion : The surgical procedure of tuberculous spondylitis using titanium mesh cage with bone chip seems to be an effective procedure to minimize loss of the correction of kyphotic deformity without any aggravating inflammatory change and recurrence with titanium mesh cage, when sufficient debridement and anti-tuberculous chemotherapy are achieved.

외상성 추간판 손상과 의원성 척추장애인 만들기 (Traumatic Disc Injuries and the Iatrogenic Spinal Disability)

  • Objective : Traumatic disc lesion is a lesion with tremendous controversies. The causal relationships of this lesion are not established along with pathogenesis, diagnostic criteria, methods of treatment and the outcome. However, a significant number of patients with back pain after a trauma underwent spinal operations under the diagnosis of traumatic disc lesions. Such an ill-directed operation eventually produce a person with an iatrogenic disability. We present two illustrative cases, and tried to make a preventive method. Methods : We examined the path from mild trauma after a road traffic accident into the iatrogenic disability in two illustrative cases, who requested disability assessment and medical appraisement. We evaluated the reason and background for such an unwanted outcome and tried to find a method to reduce or prevent it by a literature review. Results : These two patients were admitted to the hospital with the diagnosis of lumbar sprain after a road traffic accidents. They eventually underwent spinal surgery under another diagnosis such as traumatic disc herniation or internal disc disruption. They stayed at the hospital for more than six months and finally lost their jobs. They became the disabled at last. Although they complained back pain, they never insisted by themselves that their symptoms were due to the traumatic disc lesion. To prevent such an iatrogenic disability, the doctors should assist them to go-back to the workplace promptly instead of a reckless extension of the treatment period. It may be necessary to evaluate the certificates to extend the treatment period with an additional diagnosis by a medical expert. To reduce the unnecessary long-term admission, a new billing system such as a partial share for the high cost of the treatment by the patients may be needed. Conclusion : It is not the patient but the doctors, who has the responsibility to avoid the unnecessary operations. All treating doctors should try to reduce or prevent such an iatrogenic complication caused by ourselves before we are forced to do so.

The Evolution of Flow-Diverting Stents for Cerebral Aneurysms; Historical Review, Modern Application, Complications, and Future Direction

  • In spite of the developing endovascular era, large (15-25 mm) and giant (>25 mm) wide-neck cerebral aneurysms remained technically challenging. Intracranial flow-diverting stents (FDS) were developed to address these challenges by targeting aneurysm hemodynamics to promote aneurysm occlusion. In 2011, the first FDS approved for use in the United States market. Shortly thereafter, the Pipeline of Uncoilable or Failed Aneurysms (PUFS) study was published demonstrating high efficacy and a similar complication profile to other intracranial stents. The initial FDA instructions for use (IFU) limited its use to patients 22 years old or older with wide-necked large or giant aneurysms of the internal carotid artery (ICA) from the petrous segment to superior hypophyseal artery/ophthalmic segment. Expanded IFU was tested in the Prospective Study on Embolization of Intracranial Aneurysms with PipelineTM Embolization Device (PREMIER) trial. With further post-approval clinical data, the United States FDA expanded the IFU to include patients with small or medium, wide-necked saccular or fusiform aneurysms from the petrous ICA to the ICA terminus. However, IFU is more restrictive in South Korea than in United States. Several systematic reviews and meta-analyses have sought to evaluate the overall efficacy of FDS for the treatment of cerebral aneurysms and consistently identify FDS as an effective technique for the treatment of aneurysms broadly with complication rates similar to other traditional techniques. A growing body of literature has demonstrated high efficacy of FDS for small aneurysms; distal artery aneurysms; non-saccular aneurysms posterior circulation aneurysms and complication rates similar to traditional techniques. In the short interval since the Pipeline Embolization Device was first introduced, FDS has been firmly entrenched as a powerful tool in the endovascular armamentarium. As new FDS are developed, established FDS are refined, and delivery systems are improved the uses for FDS will only expand further. Researchers continue to work to optimize the mechanical characteristics of the FDS themselves, aiming to optimize deploy ability and efficacy. With expanded use for small to medium aneurysms and posterior circulation aneurysms, FDS technology is firmly entrenched as a powerful tool to treat challenging aneurysms, both primarily and as an adjunct to coil embolization. With the aforementioned advances, the ease of FDS deployment will improve and complication rates will be further minimized. This will only further establish FDS deployment as a key strategy in the treatment of cerebral aneurysms.

  • This study is to investigate the status and problems in the anual physical examination in elementary & secondary schools. The study helps the introduction of health assessment. Two hundreds and forty nurse teachers of elementary & seconcary school in Seoul received in-service traomong fpr 20 daus frp, September 7 to September 27, 1992. Questionnaires were given to the 240 teachers and the number of collected questionnaire is 112. The result of the questionnaire analysis is like the following. First, the answered teachers aged 35~39, 36.6%; experience 6~10 years 37.5%; the number of class 31~40, 28.6%. Among the answered of the school doctors is practicioners. The doctors major subjects are 26.8% of internal medicine and 35.7% of surgery. Second, in elementary & secondary school nurse teachers received in-service training for the primary health assessment. And then 37.5% of schools practiced the health assessment. The term of the health assessment is largely a-year-length, occurately its rate of schools have practiced the assessment reaches 81.0%. The number of health assessment are consist of eyes, nose, ear, throat, skin, spine, heart and other abnormal regions and diseases all over the body. And 83.3% of the rate of the health assessment includes all these contents. The period of the health assessment is 7 to 28 days. Third, the physical examination conducted by school doctors is 91.0%. The method is various; one is 56.6% of the students who checked first by he nurse teacher. The number of 15.1% of the schools is directly checked by the school doctor. The invited medical doctors are divided into two categories. The number of general physician is 61.9%. Contrary to that school doctors are 38.1%. The contents of the medical examination is all the items printed on the medical examination sheet. To follow the medical examination sheet the number of school is 59.6%. Eyes, throat, skin and heart etc. partly medical examination is 40.4%. The rate of schools that used only stethoscope, tongue pressor as the medical instruments reaches 53.4% and 87.1% of elementary & secondary school give the invited doctors the allowance a nurse teachers conference decided. The number of 8.9% schools pay the doctors independent allowance. The medical examomatopm allowance is 200 to 250 won per capita. The rate is 56.1%. Forth, after the medical examination 72.1% of school sends letter to the parents to notice the result of the examination. The number of response from parent is 12.5%. The observation record in secondary school is 70.6% for junior, and 80.0% for senior respectively, and 65.5% is for the elementary school children. To conclude the regular physical examination in elementary & secondary school is very important. Because the students are in the stage of rapid growth and development. Early finding of the students diseases can help to cure with ease. In the light of public health the management of health program in the elementary & secondary school is of consequence.

Development and Characterization of Horse Bone-derived Natural Calcium Phosphate Powders

  • Purpose: This study was to develop an effective process for fabricating biocompatible calcium phosphate powders (CPPs) using horse bones, and to investigate the characteristics of them. Methods: The characteristics of horse bone powders (HBPs) were investigated according to the different osseous tissue types (compact bone and cancellous bone), bone types (spine and tibia), pretreatment methods (cold water, $H_2O_2$, and hot water), sintering time (4, 8 and 12h), and sintering temperature (600, 900, 1100 and $1300^{\circ}C$). In addition, the grinding methods were compared based on the wet grinding (ball mill) and dry grinding (blade grinder) method to make it as powders. Finally, their cytotoxicity and cell viability were checked. Results: Regardless of the types of osseous tissues and bones, HBPs were well fabricated as biocompatible CPPs. It was also found that the pretreatment methods did not influence on the resultants, showing well-fabricated HBPs. Considering the processing time, the hot water method was the most suitable compared to other pretreatment methods. Further, 12h-sintering time was sufficient to remove residual organic compounds. The sintering temperatures greatly affected the properties of bone powders fabricated. The x-ray diffraction (XRD) peak of horse bone sintered at $600^{\circ}C$ was most closed to that of hydroxyapatite (HA). Our bioactivity study demonstrated that the HBPs fabricated by sintering horse bones at $1300^{\circ}C$ showed the best performance in terms of cell viability whereas the HBPs $1100^{\circ}C$ showed the cytotoxicity. Conclusions: Using various types of horse bone tissues, biocompatible CPPs were successfully developed. We conclude that the HBPs may have a great potential as biomaterials for various biological applications including bone tissue engineering.

CBCT를 이용한 하악 정중설공의 해부학적 평가 (Anatomical Characteristics of the Mandibular Median Lingual Foramen: the Assessment of the CBCT)

  • 정중설공은 하악 전치부에 존재하는 해부학적 구조물로서, 구강저의 혈행을 담당하는 설하 동맥이 통과한다. 임플란트 시술 시 설하 동맥의 손상으로 인한 대량 출혈을 예방하기 위해서는 CBCT를 통한 정중설공의 주의 깊은 관찰이 필요하다. 본 연구의 목적은 하악 정중설공의 해부학적 특성인 출현빈도, 위치, 직경과 그 수를 성별과 나이에 따라 CBCT 이미지를 이용하여 평가하기 위함이다. 2008년 11월부터 2011년 5월까지 전남대학교 치과병원을 내원한 62명의 CBCT 이미지(남성 41명, 여성 21명; 평균 연령 43세; 연령 분포 9~85세)를 평가하였다. 하악 전치부 CBCT에서 관찰되는 정중설공의 출현빈도, 수, 위치, 수직적 거리와 직경을 계측하였다. 62명의 환자(100%) 모두에서 설극 상방에 1개 이상의 정중설공을 관찰할 수 있었고, 56명(90.32%)은 다수의 정중설공을 가지고 있었다(2개 64.52%, 3개 25.8%). 42명(66.13%)의 환자에서 하악 양 중절치 사이의 정중설공을 관찰할 수 있었다. 치조정에서 설극까지의 평균 높이는 24.21 mm 였고, 하악골 하연에서 정중설공까지의 평균높이는 14.53 mm, 하악골에서의 상대적인 높이는 0.45였다. 설공의 평균 직경은 0.93 mm였다. CBCT를 통해 정중설공의 출현빈도, 위치, 직경과 그 수를 쉽게 확인할 수 있다. 전치부 임플란트 시술 시 대량 출혈의 합병증을 예방하기 위해서는 임플란트 식립 전 CBCT를 촬영하여 개개인의 정중설공의 해부학적 평가하는 것이 필요할 것이다.

인공추간판 슬라이딩 코어의 곡률반경 변화가 종판의 접촉압력에 미치는 영향 (Effect of the Radius of Curvature on the Contact Pressure Applied to the Endplate of the Sliding Core in an Artificial Intervertebral Disc)

  • 척추관 협착증은 감압술과 융합술 그리고 인공추간판 치환술 등이 있으며, 2000년대 중반부터는 인공추간판 치환술이 널리 시술되고 있다. 인공추간판의 연구는 추간판의 자유도 및 추체의 굴곡-신전, 측굴전, 축회전에 대한 해석이 핵심기술이지만, 시술 후 수년이 경과하여 발생하는 피로파손이 새롭게 큰 문제점으로 대두되고 있다. 따라서 인공추간판 연구는 슬라이딩 코어의 피로특성 및 내구성 향상에 집중되어야 한다. 본 연구에서는 세계적으로 가장 많이 사용되는 인공추간판 제품(SB Charit$\acute{e}$ III)을 기초로 유한요소모델을 제작하고, 슬라이딩 코어의 곡률반경과 마찰계수의 변화가 von-Mises 응력과 접촉압력에 미치는 영향을 평가하였다. 이와 같은 결과를 바탕으로 새로운 인공추간판 모델들 (Model-I, -II, -III)을 제안하고 일정수명 후 발생할 수 있는 슬라이딩 코어의 피로파손 거동에 대해 SB Charit$\acute{e}$ III의 결과와 비교 평가하였다.

척수종양의 방사선 치료 (External Beam Radiotherapy for Primary Spinal Cord Tumors)

  • 1969년부터 1983년까지 버지니아 의대부속병원에서 방사선 치료를 받은 34예의 원발성 척수암중, 조직검사 또는 아절제술을 시행한 후 수술후 방사선 요법으로 치료받은 32예 (Astrocytoma 16, ependymoma 16)에 대한 치료실적을 보고하고자 한다. 29예에서는 $45\~55Gy$를 5내지 6주간에 조사 받았으며 나머지 3예에서는 40Gy이하의 선량을 조사받았다. 50Gy 이상 조사된 6예는 모두 척수가 조사야에 포함되었다. 본 연구의 최저 추적 기간은 5년이었다. 방사선 치료 실패 후 재수술로 치료된 3예를 포함하여, 모든 예에서 생존율은 5년과 10년에서 각각 $73\%\;와\;50\%$이었다. 각각의 투병생존율은 $60\%\;와\;32\%$이었다. 45Gy 이상의 선량을 받은 29예에서는 5년 무병생존이 $63\%$이었다. 13예에서 치료실패를 경험했으며 치료실패는 조사야에서만 관찰되었다. Ependymoma가 astrocytoma보다 통계적으로 유의하게 무병생존율이 높았다 $(45\%\;:\; 89\%(p<0.05))$, 원발부위에 있어 경추가 포함된 예에서 그렇지 않은 예보다 생존율이 저하되었다. 원발부위의 cauda equina포함여부는 예후에 영향이 없었다. 최고 20년간 추적결과 방사선 치료에 의한 신경성 합병증은 관찰되지 않았다. 본 연구는 원발성 척수암의 치료에 있어서 방사선 치료가 안전하고 효과적임을 확인하였다.

다분절 경추 유합 및 내고정 수술결과 (Multi-Level Anterior Interbody Fusion with Internal Fixation in Cervical Spine)

  • Objective : The purpose of the present study was to examine neurologic changes, fusion rate and degree of kyphosis from the surgical results of those patients who underwent multi-level anterior interbody fusion and internal fixation. Methods : Among 63 cases of the patients who received multi-level anterior interbody fusion and internal fixation in 5 years between 1995 to 1999 at the neurosurgery department, we performed a retrospective study in 52 cases that could be followed up with dynamic view imaging ; the results were compared and analyzed. The analysis was based on the results of history taking, physical findings and radiologic findings, and Odom criteria were used to classify those cases with neurologic changes. Results : Among those 52 cases in whom the follow-up was possible for at least a year and dynamic view imaging was possible, bone fusion was seen in 93% of the trauma cases and 95% in the non-trauma cases and overall bone fusion was observed in 94% of the cases. Bone fusion was seen in 93% of the autobone cases, 95% of the allobone cases, and 94% of the Mesh cases. Radiologic changes were observed by comparing the lateral view after surgery ; kyphosis was seen in 53% of the autobone cases, in 70% of the allobone cases, and in 35% of Mesh cases ; in 45% and 60% of the non-trauma cases and trauma cases, respectively ; and in 55% of the 2 level fusion cases and 46% of the 3 level fusion cases. Neurologic changes classified according to Odom criteria showed excellent result in 48% of all the cases, good in 23%, fair in 4%, and poor in 25%. Conclusion : Even those cases with multi-level fusion, a high fusion rate could be obtained by performing anterior interbody fusion and internal fixation ; those cases with kyphosis were related more with the presence or absence of posterior compartment injury rather than the fusion level ; and those trauma cases showed not much difference in the fusion rate compared with non-trauma cases but had a high possibility of kyphosis.

자가간호프로그램이 요추간판제거술 환자의 운동이행 자기효능감, 자가간호지식, 자가간호이행에 미치는 효과 (Effects of Self-care Program on Exercise performance Self-Efficay, Self-care Knowledge, Self-care Performance in Patients with Lumbar Discectomy)

  • 본 연구는 자가간호프로그램이 요추간판제거술 환자의 운동이행 자기효능감, 자가간호지식, 자가간호이행에 미치는 효과를 확인하기 위한 실험연구이다. 대상자는 D광역시 소재 일개 척추전문병원 입원환자 중 미세현미경 요추간판제거술을 받은 환자로 실험군 26명, 대조군 27명이다. 수집된 자료는 SPSS 25프로그램을 이용하여 평균, 백분율, 표준편차, t-test, χ2-test, t-test, repeated measures ANOVA로 분석하였다. 연구결과 자가간호프로그램 중재를 받은 실험군은 제공받지 않은 대조군에 비해 시간이 경과함에 따라 보조기 관리지식(p=.001)과 일상생활관리지식점수(p=.005)가 더 높아 지지되었다. 또한 자가간호프로그램을 제공받은 실험군은 제공받지 않는 대조군보다 보조기 관리이행도(p=.011), 일상생활 이행도(p=.007), 유해생활 습관관리 이행도(p=.011)가 높아 지지되었다. 따라서 자가간호프로그램은 요추간판제거술 환자의 수술 전후 적용을 통해 운동이행 자기효능감, 자가간호지식 및 자가간호이행도를 향상시켜 보다 빠른 회복을 도울 수 있는 프로그램이라고 할 수 있다.
