The Efficiency of Voice Therapy on Various Laryngeal Disorders (각종 후두질환에서 음성치료의 효과)

  • Vocal hyperfunction is considered to be the most significant characteristic in larynx disorders which is found among many patients presenting hoarseness Primarily as chief complaint. In Pusan National University Hospital, we executed the voice therapy to 28 patients being 17 female and 11 male patients who visited the Voice & Speech Therapy Clinic, due to the voice disorder, and then compared and analysed the voice before and after its therapy using acoustic and aerodynamic test. The obtained results were as follows. In the analysis by the local findings, it was improved to 88% in the patients of vocal nodule, 75% in mutational falsetto, 75% in the functional dysphonia, 75% in the vocal cord palsy, 50% in the vocal polyp and 50% in dysphonia plica ventricularis. For the acoustic analysis, Fo, litter, Shimmer and NHR were measured. In the patients of mutational falsetto, Fo, Jitter and NHR were shown to be improved significantly and in the patients of vocal nodule, Shimmer was shown to be improved significantly. In the patients of vocal polyp, Fo was significantly improved. In the patients of vocal cord palsy in litter and NHH were significantly improved. In the patients of dysphonia plica ventricularis, Shimmer and NHR were significantly improved and the patients of functional dysphonia were more improved in Fo, litter and Shimmer. For the aerodynamic analysis, MPT was measured. In particular, it was shown to be improved significantly in the patients of vocal nodule, improved in the vocal polyp, vocal cord palsy, functional dysphonia patients.

Development of the Korean version of ICF e-Learning tool

  • Purpose: The aim of the study was to develop a Korean version of an ICF e-Learning tool (KICF e-Learning tool). Methods: The process of translation and adaptation of the ICF e-Learning tool was followed: two translators developed the Korean versions independently, and a consensus version of the translation was then produced. An expert committee, which was composed of five experts from physiotherapy, occupational therapy, speech pathology, and social welfare, reviewed the consensus Korean version to make a beta version of the tool. A field test was conducted to determine if the Korean version of the tool was easy to understand and suitable to use in ICF learning. Feedback from the field test were used for the final adaptation of the KICF e-Learning tool. Results: One-hundred and twenty-six volunteers (40 males and 76 females) were invited to examine the KICF e-Learning tool. The participants reported various levels of ICF knowledge from none to very good. Forty-eight participants reported no knowledge of ICF. The majority of participants (n=84) reported that Korean terms or expression in the tool were easy to understand and one-hundred fourteen participants would recommend the tool to another person. The Korean cases would be helpful for a Korean audience to study the ICF using the tool. Conclusion: The KICF e-Learning tool was developed and is ready for use by the public for the consistency of ICF education. On the other hand, development of an advanced module will be needed.

Musical Therapeutic Approch for Improving Self-expression and Self-esteem of Persons with Chronic Mental Disorder (음악치료가 만성정신장애인의 자기표현 및 자아존중감 향상에 미치는 효과)

  • Chronic mental-handicapped people are lacking in non-verbal expression such as eye contact, intonation, voice volume, facial expression, and gesture as well as the contents of speech, speak with a monotonous voice, fail to be vivid and clear in voice, and have absence of expression, thereby bringing about difficulty even for social adjustment and about low self-esteem. Accordingly, the purpose of this study was to examine effectiveness for enhancing self-expression and self-esteem by applying music therapy to the chronic mental-handicapped. The subjects were the chronic mental-handicapped who receive rehabilitation service at the community rehabilitation center, and who have over 10 years in the duration of disease. 1he music therapy activity was progressed with totally 14 sessions during 7 weeks with twice a week. This study confirmed t-test that is verification of difference in the mean, in order to examine difference between before and after music therapy in self-expression and self-esteem of the chronic mental-handicapped, and researched into qualitative case. The findings are as follows. First, as a result of score in self-expression scale, the significant improvement was shown after music therapy compared to before music therapy. The significant difference was indicated in verbal self-expression, phonetic self-expression, and non-verbal self-expression, which are its sub-spheres. Thus, the conclusion was obtained as saying that music therapy is effective for enhancing self-expression. Second, as a result of score in self-esteem scale, the significant difference was shown after music therapy compared to before music therapy. Thus, the conclusion was obtained as saying that music therapy is effective for enhancing self-esteem. Through the above results, the music therapy showed effectiveness of self-expression ability and self-esteem in the chronic mental-handicapped at the community rehabilitation center, thereby having been confirmed to be possibly utilized as rehabilitation program for the social skill ability and the social adjustment of the chronic mental-handicapped.

The evaluation of methodological quality of meta-analysis studies in speech language pathology using AMSTAR (AMSTAR에 기반한 국내 언어치료 분야 메타분석 논문의 방법론적 질평가)

  • Although research using meta-analysis is increasing in the field of rehabilitation science, not all meta-analytical papers are of the same quality. In particular, although meta-analysis is a research method with the highest level of evidence, it may be possible to derive distorted conclusions or alternatives by simply integrating representative values without considering heterogeneity among individual studies. This study analyzed the current status of meta-analysis papers on the subject of language arbitration published in Korea from January 2010 to June 2019, and used A Measurement Tool to Assess the Methodological Quality of Systematic Review (AMSTAR). As a result of evaluating the methodological quality of the final five papers, the average of 7.4 points out of 11 points was found above average. In order to raise the qualitative level of Meta-analysis in speech-language pathology in the future, it is necessary to include verification of publication bias and specification of conflicts of interest.

The Study of Speech Rate in Normal-Speaking Adults and Children (정상 성인 및 아동의 구어속도에 관한 연구)

  • The purpose of this study was to establish preliminary data on the speech rates in groups of normal speaking adults and children. The results of the present study are intended to serve as clinical measurement guidelines for diagnosis, assessment, treatment planning, and therapy progresses of stuttering. Thirty-one adults (16 females, 15 males), aged 18-30 years and thirty normally developing children (15 females, 15 males), aged 8-10, participated in the study. The subjects' reading of the Stroll (Jeong, 1994) passage and l-minute portion of talking about the daily routine were sampled. The adult speakers had rates of $308.29\pm22.57$ syllables per minute (SPM) or $108.06\pm6.17$ words per minute (WPM) during reading, and $252.87\pm40.86$ SPM and $92.26\pm17.12$ WPM during talking. The children had rates of $176.67\pm33.65$ SPM or $64.07\pm12.62$ WPM during reading, and $149.30\pm33.14$ SPM and $56.60\pm11.36$ WPM during talking. The results of t-tests for reading and talking tasks in adults showed that SPM in reading (t=2.211, p< .05) and WPM in talking (t=-2.284, p< .05) differed significantly by the gender. To answer the questions whether the rate is different across children' s gender and age, a two-way ANOVA was performed. Both SPM and WPM in reading tasks were significantly different between groups of children aged 8 and 10 (p< 01), In speaking tasks, both SPM and WPM were significantly different between groups of children aged 8 and 10, and between 9 and 10.

The Factors Affecting Job Satisfaction in Speech-Language Pathologists

  • The propose of this study was to investigate the factors affecting job satisfaction in Speech-Language Pathologists and to identify the relationships among job satisfaction self-efficiency and job stress. The participant of this study were 145 Speech-Language Pathologists. The results of present study are as follows. The mean score for job satisfaction was 3.62. job satisfaction was found to have a positive correlation with self-efficiency, and a negative correlation with job stress. The influencing factors impacting on job satisfaction were self-efficiency and job stress. This factors explained that job satisfaction was 46.8% of the variance. Conclusion: This study is expected be useful to find ways to improve subjects' job satisfaction by increasing their self-efficiency, decreasing their job stress and developing intervention programs as basic data.

Change in lip movement during speech by aging: Based on a double vowel (노화에 따른 발화 시 입술움직임의 변화: 이중모음을 중심으로)

  • This study investigated the change in lip movement during speech according to aging. For the study, 15 elderly women with an average of 69 years and 15 young women with an average of 22 years were selected. To measure the movement of the lips, the ratio between the minimum point and the maximum point of movement when pronouncing a double vowel was analyzed in pixel units using image analysis software. For clinical utility, the software was produced by applying an automated algorithm and compared with the results of handwork. This study found that the range of the width and length of lips in double vowel tasks was smaller for the elderly than that of the young. A strong positive correlation was found between manual and automated methods, indicating that both methods are useful for extracting lip contours. Based on the above results, it was found that the range of the lips decreased when ignited as aging progressed. Therefore, monitoring the condition of lip performance by simply measuring the movement of lips before aging progresses, and performing exercises to maintain lip range, will prevent pronunciation problems caused by aging.

The Effects of Changing the Respiratory Muscles and Acoustic Parameters on the Children With Spastic Cerebral Palsy (체간 조절을 통한 앉기 자세 교정이 경직형 뇌성마비 아동들의 호흡근과 음향학적 측정치들의 변화에 미치는 효과)

  • The purpose of this study was to investigate the effects postural changes on respiratory muscles and acoustic parameters of the children with spastic cerebral palsy. Nine children with spastic cerebral palsy who required assistance when walking were selected. The ages of the children ranged from 6 to 9 years old. The phonation of the sustained vowel /a/ and the voice qualities of each child such as fundamental frequency($F_0$; Hz), pitch variation (Jitter; %), amplitude variation (Shimmer; %) and noise to harmonic ratio (NHR) were analyzed by Multi-Dimensional Voice Program (MDVP). The muscle activity of three major respiratory muscles: pectoralis major muscle, upper trapezius muscle and rectus abdorminalis muscle, were measured by examining the root mean square (RMS) of the surface EMG to investigate the impact of changes in the adjusted sitting posture of each subject. However, the RMS of pectoralis major muscle showed a significant differences (p<.05). Secondly, there were no significant differences in $F_0$, Jitter and Shimmer between pre and post posture change, but there was a significant difference in NHR (p<.05). The data were collected in each individual; once prior and once after the sitting posture change. The data were analyzed by Wilcoxon signed ranks-test using SPSS version 14.0 for Windows. The findings of this study were as follows; Firstly, the RMS of upper trapezius and rectus abdorminalis muscle were not significant different between pre and post sitting posture changes. From the result, it is concluded that changes in the adjusted sitting posture decreases the abnormal respiratory patterns in the children with spastic cerebral palsy which is characterized by the hyperactivity of the respiratory muscles in breathing. Also, there is increased on the voice qualities in children with spastic cerebral palsy.

Linguistic Features of Spontaneous Speech Production in Normal Aging, Alzheimer's Disease (정상 노인과 알츠하이머성 치매 환자의 자발화 산출에서의 언어적 특징)

  • Detecting probable Alzheimer's disease (AD) at an early stage is crucial in slowing the progression of the disease and initiating drug therapy for more effective symptom management. Therefore, this study aimed to identify linguistic features that allow us to distinguish between patients with AD and normal controls. This paper reports on characteristics of spontaneous speech in subjects in three stages of AD (questionable, mild, moderate) compared with education- and age-matched normal controls. Four components of speech were measured in Korean native speakers with AD and normal aging: speech tempo, hesitation (measured in seconds), rate of articulation errors, and rate of grammatical errors. The results revealed significant differences in most of these speech components among the four groups, including significant differences between normal controls and the questionable AD group in the areas of speech tempo and rate of grammatical errors. Phonological? articulatory ability was preserved in questionable AD, and grammatical ability was preserved in questionable and mild AD. Subjects with moderate AD were severely impaired in grammatical ability. Prospective assessments of spontaneous speech skills using a dialogue and picture-description task are useful in detecting the subtle, spontaneous speech impairments that AD causes even in its early stage.

Comparison of Initial Therapeutic Effects of Voice Therapy and Injection Laryngoplasty for Unilateral Vocal Cord Paralysis Patients (일측 성대마비 환자에 대해 음성치료와 성대주입술의 초기 치료 효과 비교 연구)

  • Background and Objectives : The purpose of this study was to classify patients with unilateral vocal fold paralysis according to their fixed location and to analysis the effects of two treatment methods by early voice therapy and injection laryngoplasty. Materials and Methods : Twenty patients who were classified as full abduction and slight abduction according to the position of paralysis were treated injection laryngoplasy, and 23 patients were treated by voice therapy. Twenty patients were treated injection laryngoplasy and 23 patients were treated voice therapy. Results were evaluated by acoustic analysis, electroglottography, cepstrum analysis before and after therapy. The voice therapy was conducted by improving the larynx movement and glottal contact, whilst removing hypertension of the supraglottic and use the breathing. Results : Significant improvement was found in the acoustic parameter, cepstrum parameter, and EGG before and after treatment in both groups. There was no significant difference between the two groups when compared before and after treatment to compare the effects of injection laryngoplasty and voice therapy. Conclusion : The initial treatments for unilateral vocal cord paralysis are injection laryngoplasty and voice therapy. however, there is no precise standard about which method should be applied first. Therefore, in this study, we tried to classify patients according to their paralysis position and then apply two methods. The results of this study suggest that voice therapy and Injection laryngoplasty at the initial stage is a very useful method to improve voice quality of vocal fold paralysis and improve laryngeal function.
