The Influence of Feedback in the Simulated Patient Case-History Training among Audiology Students at the International Islamic University Malaysia

  • Background and Objectives: There is a scant evidence on the use of simulations in audiology (especially in Malaysia) for case-history taking, although this technique is widely used for training medical and nursing students. Feedback is one of the important components in simulations training; however, it is unknown if feedback by instructors could influence the simulated patient (SP) training outcome for case-history taking among audiology students. Aim of the present study is to determine whether the SP training with feedback in addition to the standard role-play and seminar training is an effective learning tool for audiology case-history taking. Subjects and Methods: Twenty-six second-year undergraduate audiology students participated. A cross-over study design was used. All students initially attended two hours of seminar and role-play sessions. They were then divided into three types of training, 1) SP training (Group A), 2) SP with feedback (Group B), and 3) a non-additional training group (Group C). After two training sessions, the students changed their types of training to, 1) Group A and C: SP training with feedback, and 2) Group B: non-additional training. All the groups were assessed at three points: 1) pre-test, 2) intermediate, and 3) post-test. The normalized median score differences between and within the respective groups were analysed using non-parametric tests at 95% confidence intervals. Results: Groups with additional SP trainings (with and without feedback) showed a significantly higher normalized gain score than no training group (p<0.05). Conclusions: The SP training (with/without feedback) is a beneficial learning tool for history taking to students in audiology major.

Survey on the Status and Perceptions, Needs of Non-verbal Autism Spectrum Disorders Intervention of Speech-Language Pathologists (무발화 자폐스펙트럼장애 중재에 대한 언어재활사의 현황과 인식, 요구 조사)

  • The purpose of this study is to examine the status and perceptions, needs of speech language pathologists(SLPs) for the intervention of non-verbal autism spectrum disorders(ASD) through a survey. Among SLPs registered in the Korean Association of Speech-Language Pathologists (KSLP), 116 SLPs participated in this survey. The result is as follows. First, 96.6% of SLP reported that they had been referred for non-verbal ASD, and it was found that SELSI was the most used evaluation tool, and communication ability and social interaction were the most frequently used intervention goals. Second, 86.2% of the SLP said that speech therapy had difficulties, and the reason for the burden was the lack of speech therapy methods. Also, the level of knowledge of speech therapy for non-verbal ASD was low in the treatment area, and the level of confidence responded positively only in communication ability and social interaction. Third, education that was considered necessary within the curriculum was education on treatment methods, and it was found that the improvement points of education other than regular courses were increased education such as expert courses and workshops and activation of supervision. From the results of this study, it is expected that the related curriculum will be expanded and improved in the future.

Using Utterance and Semantic Level Confidence for Interactive Spoken Dialog Clarification

  • Spoken dialog tasks incur many errors including speech recognition errors, understanding errors, and even dialog management errors. These errors create a big gap between the user's intention and the system's understanding, which eventually results in a misinterpretation. To fill in the gap, people in human-to-human dialogs try to clarify the major causes of the misunderstanding to selectively correct them. This paper presents a method of clarification techniques to human-to-machine spoken dialog systems. We viewed the clarification dialog as a two-step problem-Belief confirmation and Clarification strategy establishment. To confirm the belief, we organized the clarification process into three systematic phases. In the belief confirmation phase, we consider the overall dialog system's processes including speech recognition, language understanding and semantic slot and value pairs for clarification dialog management. A clarification expert is developed for establishing clarification dialog strategy. In addition, we proposed a new design of plugging clarification dialog module in a given expert based dialog system. The experiment results demonstrate that the error verifiers effectively catch the word and utterance-level semantic errors and the clarification experts actually increase the dialog success rate and the dialog efficiency.

  • Objectives: This study is a clinical report of a patient with ADHD and learning disorders who is being treated with hyperbaric oxygen, scalp acupuncture, cognitive enhancement therapy and speech-language therapy. Methods: The BASA-R, BASA-M and REVT tests were used for the diagnosis of learning disorders. For the treatment, hyperbaric oxygen therapy, scalp acupuncture, cognitive enhancement therapy and speech-language therapy were all being used. The Raven's matrix tests were compared for between before and after the abovementioned therapies. Results: After the treatment, Raven's matrix test grade improved from 4 to 5. The improvement of the patient's concentration, communication, motion, confidence, and sleep conditions were observed. Conclusions: These therapies including the hyperbaric oxygen therapy are efficient for the treatment of ADHD and learning disorders.

Effects of general and oral health on quality of life in the elderly living alone and with family (독거노인과 가족동거노인의 건강 및 구강건강이 건강 관련 삶의 질에 미치는 영향)

  • Objectives: The purpose of this study was to investigate the effects of general and oral health on quality of life in the elderly living alone and with family. Methods: We analyzed data from the $6^{th}$ Korea National Health and Nutrition Examination Survey. Distribution of the elderly living alone and with family based on the general characteristics and general and oral health was analyzed using complex-sample chi-square tests. Multiple logistic regression was used to analyze the factors affecting quality of life by calculating the 95% confidence intervals. Results: In the elderly living alone, the quality of life significantly correlated with restriction of activity, perceived general and oral health status, perceived stress, and speech difficulties. Further, in the elderly living with family, lower quality of life significantly correlated with restriction of activity, perceived health status, walking days per week, life time smoking history, Community Periodontal Index, and chewing and speech difficulties. Conclusions: The elderly are concerned with self-maintenance of general and oral health. Therefore, systematic policies related to health services need to be developed and operated at the national level. It is especially necessary to take social interest in the elderly living alone and a more continuous and professional approach in their health care.

  • In this paper, we propose a technique to improve Out-Of-Vocabulary(OOV) rejection algorithms in variable vocabulary recognition system which is much used in ASR(Automatic Speech Recognition). The rejection system can be classified into two categories by their implementation method, keyword spotting method and utterance verification method. The utterance verification method uses the likelihood ratio of each phoneme Viterbi score relative to anti-phoneme score for deciding OOV. In this paper, we add speaker verification system before utterance verification and calculate an speaker verification probability. The obtained speaker verification probability is applied for determining the proposed variable-confidence threshold. Using the proposed method, we achieve the significant performance improvement; CA(Correctly Accepted for keyword) 94.23%, CR(Correctly Rejected for out-of-vocabulary) 95.11% in office environment, and CA 91.14%, CR 92.74% in noisy environment.

Effects of EAI and VAS on perceptual judgement and confidence rating by listeners for voice disorders (청지각적 평가 방식에 따른 음성장애 심한 정도 판단과 자가 신뢰도에 대한 차이)

  • The purpose of the present study was to evaluate the effect of 7-point interval scale(EAI) and visual analogue scale(VAS) on perceptual judgement and the reliability of severity on voice problems by dysphonic speakers. 30 undergraduate students studying communication disorder were enrolled in the perceptual evaluation. Those listeners judged overall voice severity within the anchored(condition 1) and non-anchored scales(condition 2) for vowel prolongation and reading tasks by 25 speakers with voice disorder. The results of this study showed that the scores by VAS was significantly higher than EAI in both condition 1 and condition 2 for vowel prolongation and reading task. However, the scores by EAI method was higher than by VAS method on voice severity of vowel prolongation (condition 1) and reading task(condition 2). These results suggest auditory-perceptual scaling procedures must be more studied in the aspects of clinical application of voice disorder.

Humanoid Active Audition System Using the Fuzzy Logic System (퍼지를 이용한 인간형 능동 청각 시스템)

  • We propose a humanoid active audition system which detects the direction of sound using just three microphones. Compared with previous researches, this system which has a simpler algorithm and less microphone shows better performance. Moreover, it uses the fuzzy logic system to remove the reflective sound and makes the resultant improvement of confidence. In order to verify our system's performance, we install the proposed active audition system to the home service robot, called hombot II, which has been developed at the KIST(Korea Institute of Science and Technology), and confirm its excellent performance through the experiment.

A Study on the Rejection Capability based on Utterance Verification for Speech Recognition (발화 검증에 의한 음성인식 거절기능 연구)

  • 본 논문에서는 단어독립 음성인식 시스템을 위한 음성인식 거절(rejection)기능에 대해 기술한다. 음성인식 거절 기능은 음성인식기를 제작할 때 정해놓은 인식대상 단어 이외의 단어가 입력되었을 때 그 단어가 인식할 수 없는 단어임을 알려주는 기능이다. 본 연구에서는 단어독립 음성인식 시스템에 적용될 수 있는 발화 검증 방식에 의해 음성인식 거절 기능을 구현하였다. 특히 유사도를 결정함에 있어서 산술평균, 기하평균, 조화평균을 사용하고 각각을 비교하여, 기하 평균을 사용하는 방식이 우수한 성능을 보임을 알 수 있었다. 음성의 신뢰도(confidence score)를 정규화하기 위해서 Sigmoid 함수를 사용하는데 이 함수의 가중치(weight) 상수의 변화에 대해 인식률을 비교함으로써 가장 적절한 가중치 상수값을 결정하였다. 음성인식 테스트 결과에서는 신뢰도 임계치 값을 구하고 이 값을 사용하여 인식률을 계산하였으며, 거절의 오류까지 포함된 음성인식률은 약 76%였다. 이 연구결과는 현재 한국통신에서 시험 서비스 중인 음성인식 증권정보 안내 시스템에 적용될 예정이다.
