The Effects of Sling Exercise Based with Bobath Concept on the Balance of Spastic Diplegia Cerebral Palsy: Case Report (보바스 개념에 기초한 슬링 운동이 양하지 뇌성마비 균형에 미치는 영향: 단일사례연구)

  • Purpose This study is to investigate the effect of sling exercise on the balance capacity of spastic diplegia cerebral palsy patient based on Bobath concept. Methods A single subject experiment was designed targeting an 8year old child with the rigid bilateral cerebral palsy. The static balance test used the 30 second Rombug test of BT4, and the dynamic balance test used the timed up and go test. Results In the 30second Rombug test of BT4, the child had a smaller median outcome than baseline and withdrawal period. In the TUG test, the walking speed in the intervention period was improved comparing to the baseline and withdrawal period. Conclusion The sling exercise based on the Bobath concept has been proved that it is an effective intervention to improve the static and dynamic balance capacity of patients with rigid bilateral cerebral palsy.

Dual Mode Feedback-Controlled Cycling System for Upper Limb Rehabilitation of Children with Cerebral Palsy

  • Background/Objectives: This paper proposes a dual mode feedback-controlled cycling system for children with spastic cerebral palsy to rehabilitate upper extremities. Repetitive upper limb exercise in this therapy aims to both reduce and analyze the abnormal torque patterns of arm movements in three- dimensional space. Methods/Statistical analysis: We designed an exercycle robot which consists of a BLDC motor, a torque sensor, a bevel gear and bearings. Mechanical structures are customized for children of age between 7~13 years old and induces reaching and pulling task in a symmetric circulation. The shafts and external frames were designed and printed using 3D printer. While the child performs active/passive exercise, angular position, angular velocity, and relative torque of the pedal shaft are measured and displayed in real time. Findings: Experiment was designed to observe the features of a cerebral palsy child's exercise. Two children with bilateral spastic cerebral palsy participated in the experiment and conducted an active exercise at normal speed for 3 sets, 15 seconds for each. As the pedal reached 90 degrees and 270 degrees, the subject showed minimum torque, in which the child showed difficulty in the pulling task of the cycle. The passive exercise assisted the child to maintain a relatively constant torque while visually observing the movement patterns. Using two types of exercise enabled the child to overcome the abnormal torque measured in the active data by performing the passive exercise. Thus, this system has advantage not only in allowing the child to perform the difficult task, which may contribute in improving the muscle strength and endurance and reducing the spasticity but also provide customizable system according to the child's motion characteristic. Improvements/Applications: Further study is needed to observe how passive exercise influences the movement characteristics of an active motion and how customized experiment settings can optimize the effect of pediatric rehabilitation for spastic cerebral palsy.

  • Purpose : The purpose of this study was to evaluate the influence of Respiratory Capacity by Vojta's treatment in children with spastic cerebral palsy. Method : The subjects of this study, children diagnosed with spastic cerebral palsy, 12 patients were picked up, who were agreed with this research and were having hospital care for 4weeks at MH and PR medical centers. Result : In comparison of Respiratory muscle activity and Phonation capacity were significant in the group(p<.01) and MPT and Rectus abd(Rt) were significantly correlated in the pre test and SMR and Ex/oblique (Lt) were significantly correlated in the post test. Conclusion : Therefore, the Vojta's treatment is useful to improve the Respiratory muscle activity and Phonation capacity in children with spastic cerebral palsy.

The Effects of Changing the Respiratory Muscles and Acoustic Parameters on the Children With Spastic Cerebral Palsy (체간 조절을 통한 앉기 자세 교정이 경직형 뇌성마비 아동들의 호흡근과 음향학적 측정치들의 변화에 미치는 효과)

  • The purpose of this study was to investigate the effects postural changes on respiratory muscles and acoustic parameters of the children with spastic cerebral palsy. Nine children with spastic cerebral palsy who required assistance when walking were selected. The ages of the children ranged from 6 to 9 years old. The phonation of the sustained vowel /a/ and the voice qualities of each child such as fundamental frequency($F_0$; Hz), pitch variation (Jitter; %), amplitude variation (Shimmer; %) and noise to harmonic ratio (NHR) were analyzed by Multi-Dimensional Voice Program (MDVP). The muscle activity of three major respiratory muscles: pectoralis major muscle, upper trapezius muscle and rectus abdorminalis muscle, were measured by examining the root mean square (RMS) of the surface EMG to investigate the impact of changes in the adjusted sitting posture of each subject. However, the RMS of pectoralis major muscle showed a significant differences (p<.05). Secondly, there were no significant differences in $F_0$, Jitter and Shimmer between pre and post posture change, but there was a significant difference in NHR (p<.05). The data were collected in each individual; once prior and once after the sitting posture change. The data were analyzed by Wilcoxon signed ranks-test using SPSS version 14.0 for Windows. The findings of this study were as follows; Firstly, the RMS of upper trapezius and rectus abdorminalis muscle were not significant different between pre and post sitting posture changes. From the result, it is concluded that changes in the adjusted sitting posture decreases the abnormal respiratory patterns in the children with spastic cerebral palsy which is characterized by the hyperactivity of the respiratory muscles in breathing. Also, there is increased on the voice qualities in children with spastic cerebral palsy.

  • The purpose of this study was to evaluate the influence of respiratory capacity(forced vital capacity), EMG of rectus abdominal muscle, phonation by respiratory muscle strengthening exercise in children with spasticity cerebral palsy. 24 children with spasticity cerebral palsy was randomized in 2 groups, respiratory muscle strengthening exercise and contro group. In the exprimentral groups, respiratory muscle strengthening exercise for 30minutes duration 3 time per week for 8weeks were respectively preformed, Control group was not performed. Before and after experiments, respiratory capacity(forced vital capacity), EMG of rectus abdominal muscle and phonation was measured in all children. In comparison of difference before and after experiment, the respiratory capacity(forced vital capacity) of respiratory muscle strengthening exercise group was significantly increased than the control group(P<.05), rectus abdominal muscle EMG of the respiratory muscle strengthening exercise group was significantly increased more than the control group(P<.05) and MPT of the respiratory muscle strengthening exercise group was significantly increased more than the control group(P<.05). We found that the respiratory muscle strengthening exercise is useful to improve the respiratory capacity and phonation in children with spasticity cerebral palsy.

  • Purpose: This study intends to investigate if visual perception training program has influence on writing intelligibility and visual perception ability of the children who have spastic cerebral palsy. Methods: The subjects were 12 children of 4-8 years old. Classifying them into 2 groups - cerebral palsy children group(6) and normal children group(6), visual perception training program was conducted to the cerebral palsy children group twice a week for 4 weeks. Korean alphabet writing test and Korean-version development test on visual perception(DTVP-2) were used for the test. Result: First, the cerebral palsy children group had significantly lower scores on visual perception than the normal children group. Second, Two groups didn't show significantly difference between before and after the visual perception training program. Third, The normal children group significantly increased at three parts without the visual perception training program. Forth, The cerebral palsy children group significantly increased at the only a part after the visual perception training program Conclusion: For the visual perception ability improvement of cerebral palsy children, it is necessary to systematic and continued intervention. And the study about the effect of the visual perception training program application according to by type characteristic of the cerebral palsy child wishes to proceed.

The Effects of Insole Supporting Medial Longitudinal Arch while Walking in Spastic Cerebral Palsy with Pes Planus (안쪽 세로 발바닥 활을 지지한 인솔의 착용이 평발을 가진 경직성 뇌성마비 아동의 보행에 미치는 영향)

  • PURPOSE: The object of the present study is to investigate the effects of the insole supporting medial longitudinal arch while walking in spastic cerebral palsy with pes planus. METHODS: Ten spastic bilateral cerebral palsy children with pes planus participated in this study. The insole were custom-made for the individual child. Muscle activity was measured by surface EMGs attached on tibialis anterior (TA), gastrocnemius (GA), vastus medialis oblique (VMO), biceps femoris long head (BF). temporal-spatial parameters such as velocity, step length, stride length, stance time, toe angle were collected while the subjects walked on the GAITRite system. RESULTS: The results of the present study were summarized as follows: 1. Muscle activities in mean EMGs while walking: Left VMO, Right TA, Left BF and GA revealed significant reductions after applying insole. 2. Muscle activities in peak EMGs while walking: Left TA and BF demonstrated reductions significantly after applying insole. 3. There were improvements in temporal-spatial gait parameters with insole: velocity, both step length, Right stride length and Right toe angle were increased(p<.05). CONCLUSION: Therefore the current study demonstrated that insole supporting the medial longitudinal arch would be effective on gait of the spastic cerebral palsy with pes planus.

Effects of Therapeutic Massage Program on Range of Motion, Activities of Daily Living, Strength in Children with Spastic Cerebral Palsy (경직형 뇌성마비아동의 마사지 프로그램 적용이 근력, 관절가동범위 및 일상생활활동에 미치는 효과)

  • Purpose. The purpose of this study was to determine the effect of massage program on muscle strength (MMT), range of motion (ROM) and activities of daily living (ADL) in children with spastic cerebral palsy. Methods. The child welfare center of J city were measurement goniometer by range of motion and evaluation of activities of daily living scale and muscle strength scale. The massage program was applicated with 40 min, 2 times per a week and total 8 weeks. Massage program constituted surface of effleurage, petrisage, deep part of effleurage, friction in regular sequence. Result. For muscle strengths were significantly differenced after 8 weeks. For range of motion were dose dependent increased different two groups. And activities of daily living scale were significantly difference between massage group and non-massage group. Conclusion. In this study, the therapeutic massage program showed a positive effect for muscle strength, range of motion and activities of daily living in spastic cerebral palsy.

  • Thumb adduction is an abnormal pattern typically noted in children with spastic cerebral palsy. This abnormal pattern can limit hand function, specifically in the type and quality of prehension pattern used and in the coordination of release. This ABAB single-subject research was designed to examine the effects of short thumb opponens splint on hand function in cerebral palsy. The subject was a 4 years and 8 months old boy with right upper extremity spasticity. The child was fitted with a short thumb opponens splint, which was worn for 8 hours per day during the daytime. Two different measures were used: (a) prehension component scores; (b) Bruininks-Oseretsky test of motor proficiency. Data was collected three times a week for 10 weeks. Visual analysis of data indicate that after the application of a short thumb opponens splint, improvements were noted in the prehension pattern and fine motor functional task. The results of this study suggest that short thumb opponens splint may prove efficaciousness in the treatment of the child with cerebral palsy.

Three Cases of Developmental Delay Due to Cerebral Palsy Treated with Korean Medicine (뇌성마비로 인한 발달지연 아동의 한방치료 3례)

  • These are three case reports on developmental delay that were treated with Korean medicine more than one year. A child with spastic cerebral palsy was treated only with acupuncture in one case, and in another case, two children with cerebral palsy were treated with both acupuncture and Korean herbal medication: Gami-Dossi-pyengwisan and Gami-Guibiondam-tang. After more than a year of treatment period, all three children showed improvement of general condition and motor function, measured by Gross Motor Function Measure, compare to average of children with cerebral palsy. According to this result, Korean medicine treatment can be helpful in improvement of spasticity and developmental delay due to cerebral palsy. Further studies with more cases and well-designed randomized controlled trials should be performed to establish proper guideline of Korean medicine treatment for cerebral palsy.
