Ultrasonographic Utility for Arthroscopic Examination of Knee (슬관절 관절경 검사 시 초음파 검사의 유용성)

  • Purpose: To evaluate the relationship between the real pathology & abnormal finding found by ultrasonography. Without an MRI test being done beforehand, an arthroscopy is done after an ultrasonography to show abnormal lesions during a knee abnormality. Materials and Methods: The subjects were 42 patients out of 49 cases, excluding those with rheumatoid arthritis, septic arthritis and patients suspected with a ligament tear, which were examined by ultrasonography alone before receiving a knee arthroscopy in our hospital from July 2007 to July 2008. In every case, a physical examination, simple X-ray and knee ultrasonography was done. An arthroscopy was performed when there was ultrasonographic abnormal finding. Before the procedure, a MRI test was not performed and when abnormal findings were found by an arthroscopy, an appropriate surgery was done. Results: During the ultrasonographic examination, there were various sized effusions in the suprapatellar pouch. Also, in addition there were eleven cases of medial meniscus abnormalities, sixteen cases of lateral meniscus abnormalities, and two cases of cystic lesions. Throughout the arthroscopic examination, there were 14 cases of medial meniscus abnormalities, 20 cases of lateral meniscus abnormalities, 15 cases of cartilage damages, 9 cases of medial pathologic plica, 2 cases of intra-articular loose bodies, 5 cases of chondromalacia, 2 cases of cyst, and 2 cases of synovitis. When an effusion abnormality was found by the ultrasonography in a suprapatellar pouch, there was a 100% probability of knee pathology. When a medial meniscus abnormality was found with an ultrasonography, there was a 90.9% probability of a real pathology. When a lateral meniscus abnormality was found there was 81.2% probability of a real pathology. Ultrasonography was 100% accurate when it came to cystic lesions. Conclusion: Knee ultrasonography performed before an arthroscopy seems to be a very useful examination method when suspecting intra-articular lesions.

The Fusion Effect of Deep Transverse Stroking, Manual Stretching Exercise and Active Muscle Release Technique on Psoas Major Muslce Thickness and Muscle Tone and Pelvic Angle of Non-specific Low Back Pain Patient (비특이성 허리통증환자 큰허리근의 근두께와 근긴장도, 골반각도에 심부횡적강찰법과 수동신장운동, 능동적근육이완기법이 융합적으로 미치는 영향)

  • The purpose of this study was to investigate the fusion effects and difference them of the deep transverse stroking, manual stretching exercise and active muscle release technique on psoas major muscle thickness and muscle tone, and pelvic angle in non-specific low back pain patients. Psoas major muscle thickness was significantly decreased after the application of the deep transverse stroking $0.19{\pm}0.16cm$ (p <0.05), manual stretching exercise $0.18{\pm}0.14cm$ (p <0.05), and active muscle release technique $0.43{\pm}0.35cm$ (p <0.05). The pelvic angle was significantly decreased after the application of the deep transverse stroking $4.48{\pm}1.63^{\circ}$ (p <0.05), manual stretching exercise $5.36{\pm}2.04^{\circ}$ (p <0.05), and active muscle release technique $7.24{\pm}2.23^{\circ}$ (p <0.05). The Psoas major muscle tone was significantly decreased after application of the deep transverse stroking $0.96{\pm}0.93Hz$ (p <0.05), but manual stretching exercise $0.87{\pm}1.20Hz$ (p> 0.05) and active muscle release technique $0.82{\pm}0.98Hz$ (p> 0.05) there was no significant difference after application. There were no significant differences between the three intervention methods in the pelvic angle and psoas major muscle thickness and tone changes. In order to change psoas major muscle thickness and pelvic angle, three intervention methods should be applied appropriately according to the condition and environment of the patient, and deep transverse stroking is more effective for changing psoas major muscle tone.

Risk Factors of Neurologic Complications After Coronary Artery Bypass Grafting (관상동맥 우회수술후 신경계 합병증의 위험인자)

  • Background: As the early outcome after coronary artery bypass grafting(CABG) has been stabilized, neurologic complication has now become one of the most important morbidity. The aim of this study was to find out the risk factors associated with the neurologic complications after CABG. Material and Method: In 351 patients who underwent CABG, the incidence and features of neurologic complications, with associated perioperative risk factors, were retrospectively reviewed. Neurologic complication was defined as a new cerebral infarction confirmed by postoperative neurologic examination and radiologic studies, or delayed recovery of consciousness and orientation for more than 24 hours after the operation. Result: Neurologic complications occurred in 18 patients(5.1%), of these nine(2.6%) were diagnosed as having new cerebral infarctions(stroke). Stroke was manifested as motor paralysis in four patients, mental retardation or orientation abnormality in four, and brain death in one. Statistical analysis revealed the following variables as significant risk factors for neurologic complications by both univariate and multivariate analyses: cardiopulmonary bypass longer than 180 minutes, atheroma of the ascending aorta, carotid artery stenosis detected by Duplex sonography, and past history of cerebrovascular accident or transient ischemic attack. Age over 65 years, aortic calcification detected by simple X-ray, and intraoperative myocardial infarction were significant risk factors by univariate analysis only. Neither the severity of carotid artery stenosis nor technical modifications such as cannulation of the aortic arch or single clamp technique, which were expected to affect the inciden e of neurologic complications, had significant relationship with the incidence. Conclusion: This study confirmed the strong association between neurologic complications after CABG and atherosclerosis of the arterial system. Therefore, to minimize the incidence of neurologic complications, systematic evaluation focused on atherosclerotic lesions of the arterial system followed by adequate alteration of operative strategy is needed.
