소아 아토피성 피부염의 외치법에 대한 고찰 (A Study of External Treatments for the Atopic Dermatitis)

  • Objectives : The aim of this study was to investigate the classification methods of the external treatments of atopic dermatitis for Children. Methods: We surveyed the oriental medical book concerning the atopic dermatitis for Children. Results : 1. The oriental herbal medications for the atopic dermatitis used with Ryuhwang(硫?), WoongHwang(雄黃), KyoungBun(輕粉), WhangBaek(黃栢), SeokGo(石膏) and so on. 2. The frequently used type of external prescription is solid or liquid. 3. The external preparations were frequently painted or washing with to the lesion of dermatitis. For the wet type of atopic dermatitis, the solid prescription were sprinkled on the lesion or irrigated with the liquid. For the dry type, the mixed prescription of the solid and liquid were painted on the lesion.

전산화 단층촬영을 이용한 법랑모세포종의 방사선학적 연구 (A radiologic study of ameloblastoma using computed tomography)

  • Purpose : To reveal what is the distinct differential diagnostic differences between unicystic ameloblastoma and solid or multicystic ameloblastoma. Materials and Methods : 56 cases of ameloblastoma were retrospectively reviewed and evaluated among the patients who had taken CT scans at the department of Oral & Maxillofacial Radiology in Yonsei University Dental Hospital from January 1996 to December 2003. Results : In 56 cases, 21 cases $(37.5\%)$ were unicystic ameloblastoma, 35 cases $(62.5\%)$ were solid or multicystic ameloblastoma. Only 1 case $(4.8\%)$ of unicystic ameloblastoma and 4 cases $(11.4\%)$ of solid or multicystic ameloblastoma were occurred in maxilla. 13 cases $(61.9\%)$ of unicystic ameloblastoma were observed as unilocular, and 8 cases $(38.1\%)$ as lobulated. 5 cases $(14.3\%)$ of solid or multicystic ameloblastoma were observed as unilocular, 13 cases $(37.1\%)$ as lobulated, and 17 cases $(48.6\%)$ as multilocular. Tn the results from the measurements after correction of the buccolingual widths and heights to the mesiodistal lengths, there is a statistically significant difference between unicystic ameloblastoma and solid or multicystic ameloblastoma in ANCOVA test (p<0.05). Hounsfield units in the lesion were $24.9{\pm}8.8\;HU$ in unicystic ameloblastoma, $31.2{\pm}11.5\;HU$ in solid or multicystic ameloblastoma. There is no statistically significant difference (p>0.05). Conclusion : Characteristic differences between unicystic ameloblastoma and solid or multicystic ameloblastoma is that there is higher prevalence of solid or multicystic ameloblastoma that have lobulated or multilocular patterns. To measure the Hounsfield units in the lesion is helpful, but it is not a differential diagnostic point between unicystic ameloblastoma and solid or multicystic ameloblastoma.

Magnetic resonance images of ameloblastoma

  • Purpose: To classify and describe the characteristic features of MRI of some ameloblastoma variants. Materials and Methods: The MR images, CT images, and panoramic radiographs in 5 cases were retrospectively examined as follows. First, the contents of ameloblastomas were devided into two portions of either solid or cystic components on the basis of MR signal intensities. The signal intensity within the solid or cystic portions was classified as homogeneous or heterogeneous. Next, the characteristic internal feature of the lesion on T1W1 or T2WI was described. The signal intensities were classified into low, intermediate, slightly high, high, and strong high signal intensity. Results: Unicystic lesion showed homogeneous high signal intensity (SI) on T2W2 and the rim enhancement of the surrounding area including the mural nodule and the thick wall except the central portion on Gd- T1W1. Solid type revealed heterogeneous and high SI area with strong high SI area on T2W2. On Gd- T1W1, the area corresponding to the low signal spot on T1W1 and the strong high signal spot on T2W1 showed low SI. Hybrid type showed slightly enhanced capsular structures and low SI for the round bony septa and the areas connecting the mixed and cystic lesions on T2Wl and Gd-T1W1. Conclusion: MRI could easily assess the relationship between the mixed and cystic findings in ameloblastoma.

진행성 식도암의 고식적 식도삽관술 (palliative intubation for advanced esophageal cancer)

  • Dysphagia is common in patients with cancer of the esophagus. The rate of resectability of the lesion is low, and the majority of the patients require palliation to relieve the dysphagia. Celestin tube intubation was performed in patients with unresectable carcinoma of the esophagus, of one with malignant bronchoesophageal fistula. Dysphagia and respiratory symptoms were relieved and the patients became able to eat semi-solid food and fully ambulatory.

초음파 진단기의 설정 파라미터가 영상의 질에 미치는 효과 (Effects of Ultrasonic Scanner Setting Parameters on the Quality of Ultrasonic Images)

  • 초음파 진단기의 설정은 영상의 질에 영향을 준다. sonographer는 최적의 영상을 얻기 위해 초음파 영상의 질에 영향을 주는 설정 변수에 대한 효과를 이해해야 한다. 본 연구에서는 4가지 대표적인 영상 조절 변수 즉 TGC (Time Gain Control), 이득 (Gain), 주파수 (Frequency), DR (Dynamic Range)를 고려하였다. 초음파 영상의 질은 LCS (Law Contrast Sensitivity) 관점에서 정량적으로 비친 평가하였다. 실험은 임상용 초음파 진단기 (SA-9000 PRIME, Medison, Korea)를 사용하여 초음파 평가 팬텀 (539, ATS, USA)의 LCS 타겟을 영상화하였다. 영상 조절 변수의 설정을 변화하면서, 각 설정에 대한, 6개의 LCS 영상 (+15 dB, +C dB, +3 dB, -3 dB, -6 dB, -15 dB)을 취득하고, 영상에 대한 LCS픽셀 값을 계산하였다. 실험결과 TGC가 최대, Gain은 중간에서 최대사이, 주파수가 Pen모드, DR이 40-66 dB일 때 타겟 영상 (LCS)의 평균 픽셀 값이 높았다. 모든 타겟 영상에서 DR이 40 dB일 때에 LCS가 좋은 영상을 얻었다. 본 결과는 임상에서 잘 발견되는 solid lesion (+15, +6, +3 dB 타겟과 유사) 또는 cystic lesion (-15, -6, -3 dB 타겟과 비슷)이 있는 mass 평가 시 적절한 영상조절 변수 설정에 유용한 정보를 제공할 것으로 기대된다.

갑상선 유두암의 슬개골 전이 (A Case of Patella Metastasis of Papillary Thyroid Carcinoma)

  • A 73-year-old man presented with a chief complaint of progressive left knee pain for two months. He had a history of total thyroidectomy and central lymph node dissection due to papillary thyroid carcinoma three months ago. MRI images revealed a solid mass in the left patella. A solid mass demonstrated low signal on T1 weighed image, and high signal on T2 weighed image. And whole body bone scan showed focal photon defect in same lesion of left patella. The histologic result of left knee lesion was adenocarcinoma, consistent with metastatic papillary thyroid carcinoma. Although patellar metastasis of papillary thyroid carcinoma is very rare, when knee pain and radiologic abnormality are noted, differential diagnosis of metastasis is necessary.

$^{99m}Tc$-MDP 골스캔에서 냉소로 나타난 전이 암의 분석 (Cold Lesions in $^{99m}Tc$-MDP Bone Scans of Patients with Skeletal Metastases)

  • The present study was purposed to evaluate the incidence and the characteristics of metastatic "cold" lesions in $^{99m}Tc$-MDP bone scans of adult patients with solid malignancies. There were 29 cold lesions in 24 patients. The incidence of cold lesions was about 1% of total cases of bone scans for the patients with malignancy, or 2.5% of cases with bone metastases. Th primary sites of malignancies were lung (four cases), uterine cervix (three cases), kidney, nasopharynx, thyroid, urinary bladder, prostate, lymphoma (two cases each other), liver, breast and others (one case each other). But the relative incidence of cold lesion in lung cancer and breast cancer was low. The most frequent site of cold lesion was spine, and pelvis, skull and rib were followed. The incidence of cold lesion was related to the regional incidence of bone metastases. The size of the cold lesions was greater than that of the hot. There were six cases of single cold lesion without any other abnormalities and two cases of cold lesion which were initially hot. So it should be considered that bone metastases might be presented as cold lesions in bone scan.

Surgery of a Solid Hemangioblastoma at the Cervicomedullary Junction

  • The surgical removal of solid deep-seated hemangioblastomas remains challenging, because treatment of these lesions is often complicated by severe bleeding associated with the rich vascularity of this tumor, and by severe neural tissue injury associated with the difficulty of en bloc resection, especially when the tumor is located at the cervicomedullary junction. Therefore, preoperative embolization of deep-seated solid hemangioblastomas may play an important role in successful surgical removal by reducing major bleeding and neural tissue damage. A 24-year-old woman, 28-weeks pregnant, was admitted to our hospital for the evaluation of quadriparesis, and brain magnetic resonance imaging[MRI] revealed intra-axial mass lesion in the cervicomedullary junction. After delivery, her neurologic symptoms became aggravated, and we decided to operate. Preoperative angiography revealed a hypervascular tumor in the posterior fossa, and embolization of the main feeding artery using gelfoam and microcoil, resulted in marked reduction of tumor vascularity. She underwent a midline suboccipital craniotomy involving the removal of the arch of C-1. The tumor was totally removed through a midline myelotomy, and at her 6-month follow-up she walked independently. We report on the combined use of the preoperative embolization of feeding vessels and subsequent operative resection in a patient with a solid hemangioblastoma at the cervicomedullary junction immediately after delivery.

음낭 질환의 초음파 및 씬티그라피에 관한 방사선학적 연구 (Correlation of Ultrasound and Scintigraphic Findings in Scrotal Disease)

  • Recently ultrasound and scintigraphic study are most useful modality for diagnosis of scrotal diseases. They are able to determine the nature, anatomic extent of the lesion, and possible to differential diagnosis of the scrotal lesion. The ultrasound and scintigraphic findings were analyses in 59 patients of confirmed scrotal disease at Seoul Red Cross Hospital from June 1986 to Aug. 1989. The results were as follows. 1) Most patients were below the 30 years old age (69.4%), and most prevalent age was $11\sim30$ Yrs. in trauma, $31\sim40$ Yrs. in inflammation, and $51\sim60$ Yrs. in mass lesion of scrotum. 2) The diagnostic accuracy of scrotal disease was 94.9% in ultrasound and 72% in scintigraphic study. The ultrasound study showed high accuracy in all kind of scrotat disease, but scintigraphy showed only high detection rate in inflammatory disease. 3) Scrotal ultrasound is first choice of study for the diagnosis of scrotal lesion, and useful determine the cystic or solid lesion and anatomic extent of disease. However it can't be differentiate the acute epididymitis with acute testicular torsion. 4) Scrotal scintigraphy is useful study for the diagnosis of acute painful scrotal disease, and showed specific findings in acute epididymitis and acute torsion for differential diagnosis. 5) In consideration of age, history and clinical findings of patient, appropriate study will be chosen firstly in ultrasound and scintigraphy, and combined all of findings of these studies will be helpful for the accurate diagnosis and adequate treatment of scrotal disease.

도축 한우의 복강내 지방괴사증 발생과 병리조직학적 연구 (An abattoir survey and pathologic study of abdominal fat necrosis on Korean indigenous cattle)

  • The bovine fat necrosis is often seen as an incidental lesion in the adipose tissues of the abdominal cavity. Most of affected animals, however, have been detected at the slaughter house or routine necropsy. The purpose of this study was to examine the occurrence and distribution of abdominal fat necrosis in Korean native cattle, and its pathologic features. Postmortem inspection at an Chonju abattoir during a three-month period in 1997 detected at necrosis lesions in 67(6.6%) of the 1,012 animals received for slaughter. The occurrence was mainly in alimentary tracts and perirenal. Both sexes were affected, but the lesion were predominantly occurred in female. Gross lesions were white or yellowish in color and formed hard lumps irregular in shape ranging from small nodules to large solid masses. On the cut surface, lesions were occasionally seen chalky calcified granules and some of the lesions contained oil-like fluid. The surface of irregular shaped masses constricted kidneys and intestinal loops. Microscopically, the masses were encapsulated by thickened fibrous tissue, which infiltrated deeply and divided them into many irregular lobules. Initial lesions presented mild inflammatory cell and fibrous proliferation. It became fibroplasia in progressive lesions and resembled chronic lesion, eventhough no grossly apparent inflammatory reaction. The hard consistency of masses, due to fibroplasia, can compress the intestines, urinary organs, and reproductive organs.
