수질 매개변수 추정에 있어서 항공 초분광영상의 가용성 고찰 (Airborne Hyperspectral Imagery availability to estimate inland water quality parameter)

  • 본 연구는 항공 초분광영상을 사용한 수질추정 활용을 검토하고 한강일부분에 대해 가용한 측정자료를 이용하여 초분광영상 기반의 수질추정을 테스트하였다. 원격탐사에 의한 수질추정은 수체에 대한 downwelling과 수체 내에서의 산란과 반사에 대한 관측정보를 이용하는 방법과 원격탐사 센서에 도달하는 upwelling과 수질측정정보와의 선형적 회귀분석을 구하는 방법이 선호된다. 두 방법 모두 유의미한 결과를 도출하지만 수질정보나 산란정보 등 추정에 필요한 보조자료에 의한 영향이 더 클 것으로 판단되었다. 수질 추정 테스트는 팔당댐 하류에 위치한 한강의 일부분에 대해서 적용되었다. AISA eagle 초분광센서로 취득된 자료와 수질관측정보를 선형적 회귀분석을 통한 방법을 적용하였다. 기존 문헌에서 제시된 밴드조합에 대해서 회귀분석한 결과 유의미한 밴드조합으로 $-24.847+0.013L_{560}$의 회귀식을 얻었다 ($L_{560}$은 560 nm 파장에서의 radiance로 $R^2$=0.985). 다중분광영상을 이용했을 경우의 결과와 비교하기 위해서 spectral resampling을 통해 Landsat TM 영상을 생성하여 -55.932 + 33.881(TM, TM3)의 회귀식을 얻을 수 있었다(TM, TM3는 radiance로, $R^2$=0.968). 부유물질 농도는 수질측정지점에서 약 3.75 mg/l 이고, 초분광영상으로 추정된 농도는 약 3.65 mg/l, 시뮬레이션된 TM은 약 5.85 mg/l 로 다중분광영상을 이용했을 경우 과대 추정하는 경향을 보였다. 항공 초분광영상의 활용가치를 높이고 보다 정밀한 값을 추정하기 위해서 영상 전반에 걸친 sun glint 와 같은 영향을 최소화하기 위해 태양고도각을 고려하여 정교한 비행계획을 구성하고 체계적 전처리와 검 보정 체계를 갖출 필요가 있다고 사료된다. 일반적으로 적용된 방법에 따른 테스트로, 대기보정의 정밀성과 부족한 수질측정 샘플자료, 분광밴드의 검색, 적합한 선형회귀모델의 선택, 그리고 정량적 검증방법과 같은 몇 가지 문제점과 제약사항들을 발견할 수 있었다.

자체 제작한 Foxtail Millet Vacuum Cushion의 광자선 피부암 치료 시 유용성 검증 (Evaluation of the Efficiency of the Foxtail Millet Vacuum Cushion in Skin Cancer Radiation Treatment)

  • 목 적: 광자선을 이용한 피부암 치료 시 피부 선량의 충족과 환자 셋업의 재현성 유지는 치료의 성과에 있어서 무엇보다 중요한 요소이다. 이에 본 연구는 기존에 시행해 오던 방식이었던 상품화된 고정기구 및 조직 등가물질의 조합에 의한 치료 방식이나 물 수조를 이용한 치료 시 발생하던 단점들을 보완하기 위해 Foxtail Millet Vacuum Cushion (FMVC)의 자체 제작을 실시하고 그 유용성을 검증하여 광자선 피부암 치료의 질을 높이고자 한다. 대상 및 방법: 자체 제작한 FMVC의 광자선 투과도에 따른 절대 선량 및 선량 분포를 분석하기 위해 조직과 밀도가 유사한 고체 팬톰과 볼루스에 대해 비교 측정하였다. 조사야 $10{\times}10$ cm에서 각 대상 물질의 1 cm, 2 cm, 3 cm 두께별로 6 MV 광자선 100 MU를 각각 3회씩 조사하였고, 중심점에 대한 절대 선량 측정을 실시하였으며, Mapcheck을 사용해 조사야 내의 선량 분포를 비교하였다. 선량 분포 비교의 오차범위는 ${\pm}3%$로 정하고 합격, 불합격 여부를 결정하여 FMVC와 비교 분석하였고, FMVC를 사용하여 CT 모의치료를 실시하여 종양용적(Gross Tumor Volume, GTV)에 대한 계획 선량 목표를 $100{\pm}5$%로 하여 방사선 치료 계획을 실시해 그 결과를 분석하였다. 결 과: 중심선에서 측정된 FMVC의 절대선량 평균값은 실험에 사용된 모든 두께에서 고체 팬톰과 볼루스에 대해 0.1~0.2% 내외의 차이를 보여 거의 일치함을 보였고, 선량 분포 역시 모든 두께에서 오차 범위 ${\pm}3%$에 대해 97% 이상 일치하였다. 또한 FMVC를 이용한 방사선 치료 계획 및 임상 적용에서 목표한 피부 선량 획득하였다. 결 론: FMVC는 기존 상용화된 조직 등가 물질과 광자선 투과도에 따른 절대 선량 및 선량 분포가 거의 동일하여 그 역할을 대체함에 있어 부족한 점이 없었다. FMVC는 방사선 치료 계획에서 원하는 피부 선량을 얻어낼 수 있고 기존 보조 기구의 기능을 유지할 수 있어 환자 자세의 재현성 유지와 볼루스의 역할을 동시에 수행할 수 있다. 또한 기존 수조를 통한 치료 방식의 단점인 물 엎질러짐으로 인한 장비 위해 가능성을 줄이고, 몸이 자유롭지 않은 환자의 치료 시 자세와 Gantry angle을 다양하게 구현할 수 있다. 따라서 FMVC는 광자선 피부암 치료 시 매우 유용할 것으로 사료된다.

두부 위치 변화에 따른 파노라마 방사선사진상의 치축각도의 왜곡 (Distortion of tooth axes on panoramic radiographs taken at various head positions)

  • 본 연구는 파노라마 방사선사진에서 두부 위치 변화에 따라 인접 치아장축 사이에 이루는 각도가 어떻게 변화되는지 알아보기 위해 시행하였다. 정상교합자의 모델을 기초로 하여 3차원 solid 모델을 구성하고 이를 신속 조형모델(rapid prototyping, RP)로 제작하였다. 다음 9가지 두부위치들을 5주 동안 일주일 간격으로 재위치시키고 촬영하였다: ideal head position, $5^{\circ}$, $10^{\circ}$ up, $5^{\circ}$, $10^{\circ}$ down, $5^{\circ}$, $10^{\circ}$ right up, and $5^{\circ}$, $10^{\circ}$ right rotation. 파노라마 방사선사진 촬영은 디지털 방사선 촬영기(Planmeca ProMax)를 이용하였고 모니터상에서 직접 인접 치아장축을 표시하고 그 사이 각도를 계측하였다. 그 결과 이상적인 두부 위치와 다양한 두부 위치 변화에 따라 촬영한 파노라마 방사선사진 사이에 인접 치아장축 사이의 각도가 대부분 유의한 차이를 보였다. 수평축(X축)을 중심으로 고개를 든 경우, 각 치아들의 장축이 교합평면을 향하여 수렴하는 경향을 보였고 고개를 숙인 경우에는 치근단측을 향하여 수렴하는 경향을 보였으며, 이는 특히 전치부에서 가장 큰 차이를 보였다. 전후방축(Y축)을 중심으로 좌우측으로 고개를 기울인 경우, 교합평면이 올라간 쪽의 치아장축은 치근단측을 향하여 수렴하는 경향을 보였고 교합평면이 내려간 쪽은 교합평면을 향하여 수렴하는 경향을 보였다. 수직축(Z축)을 중심으로 고개를 좌우로 회전시킬 때, 고개를 돌린 쪽의 측절치와 견치는 치근단측을 향하여 수렴하고 반대측의 측절치와 견치는 교합평면을 향하여 수렴하였다. 이상의 결과들로 볼 때 파노라마 방사선사진 촬영 시 두부의 위치 변화에 따라 치근 평행도가 달라지기 때문에 환자를 올바르게 위치시키는 것이 중요하며, 교정 환자에서 파노라마 방사선사진을 이용하여 치근 평행도를 평가할 때 교합평면의 경사(전후방, 좌우측방)나 악궁의 비대칭 등이 반드시 고려되어야 할 것이다.

Enhanced Dynamic Wedge를 사용한 Mapcheck에서의 선량분포 평가 (Evaluation of the dose distribution in Mapcheck using Enhanced Dynamic Wedge)

  • 세기조절방사선치료(intensity modulated radiation therapy ; IMRT)는 주변 정상장기 선량을 최소화하면서 종양에 집중적으로 고선량을 조사할 수 있는 장점으로 인하여 최근 그 사용이 급격히 증가하고 있다. 기능강화 동적쐐기 (enhanced dynamic wedge; EDW) 사용 시 기하학적 변화에 따른 Mapcheck에서의 선량분포를 측정 평가하고자 한다. Clinac ix에서 IMRT를 이용하여 EDW angle($10^{\circ}$, $15^{\circ}$, $20^{\circ}$, $25^{\circ}$, $30^{\circ}$, $45^{\circ}$, $60^{\circ}$), field size(asymmetric field), depth 변화(1,5 cm, 5 cm)에 따른 선량분포를 검증하기 위해 Mapcheck위에 고체팬텀을 올려놓고 6 MV, 10MVX-ray로 100MU를 조사하였다. 6MV, 10MV의 에너지를 사용하여 기하학적변화에 따라 최대선량깊이(1.5 cm)와 5 cm 깊이에서 심부 선량백분율(percentage depth dose; PDD)은 치료계획장치에서 계산한 값의 차이가 최대 0.6%로 1%미만의 값으로 나타났다. 심부선량백분율은 조사야 중앙에서 최대선량깊이의 선량을 100%로 보았을 경우, 2.4%에서 7.2%의 범위에 있다. 또한 심부선량백분율 차이의 최대값은 Y2-OUT 방향에서 4.1%로 나타났고 Y1-IN 방향으로 1.7%로 나타났다. 동적쐐기를 사용하여 환자를 치료할 경우, 조사야 주변부의 불필요한 피폭을 유발하여 산란되는 선량을 줄이기 위하여 기능 강화 동적쐐기를 사용하는 것이 바람직할 것으로 사료된다. 특히 임상에서 환자를 치료할 경우, 쐐기의 toe방향 선량이 heel방향 선량보다 높다는 것을 염두에 두고 치료를 수행해야 할 것으로 사료된다.

머리-목 그리고 어깨의 방사선 치료 시 피부곡면과 고정장치로 인한 피부손상연구 (Skin Damage Sustained During Head-and-Neck and Shoulder Radiotherapy Due to the Curvature of Skin and the Use of Immobilization Mask)

  • 곡면 형태의 피부표면의 방사선량을 방사선크롬 필름과 열형광 선량계를 이용하여 측정하고자 한다. 또한 고정 장치의 사용으로 인한 고에너지 방사선의 피부보존효과의 감쇠를 정량적으로 측정하여 Monte-Carlo 프로그램으로 계산한 값과 비교하고자 한다. 머리-목 그리고 어깨의 곡면 형태를 모의하여 만든 11 cm 직경의 원통 팬텀에 $40{\times}40\;cm^2$의 조사야, SAD 100 cm, 6 MV의 방사선을 쪼였다. 또한 관련된 치료 상황과 유사한 조건으로 만들기 위해 그물망 형태의 고정 마스크를 원통형 팬텀에 씌워서 실험하였다. 원통 팬텀의 원둘레 주위를 따라 $0^{\circ}$에서 $360^{\circ}$까지의 피부선량곡선을 구하였다. 방사선크롬 필름을 이용하여 구한, 정면 입사위치($0^{\circ}$)에서의 피부선량은 최대값 깊이($D_{max}$) 방사선량의 47%, 접선 각도인 $90^{\circ}$에서는 61%로 측정되었다. 1.5 mm의 고정마스크를 씌운 경우 $0^{\circ}$ 입사지점에서는 59%, $80^{\circ}$에서는 78%였다. TLD를 통한 결과는 고정마스크를 씌운 경우 $0^{\circ}$ 입사지점에서는 66%, $80^{\circ}$에서는 80%였고 필름의 경우와 유사한 형태를 보였다. 고정 마스크를 머리-목 그리고 어깨 부위에 부착시켜서 치료를 하는 경우에 접선 부근 각도에서의 피부선량이 치료선량과 거의 같은 값을 보였다. 곡면 부위의 피부에는 고정성을 잃지 않는 범위 안에서 보다 더 얇고 더 구멍이 많이 뚫린 고정마스크를 사용해야 과도한 피부선량을 줄일 수 있을 것으로 사료된다.

세기변조방사선치료의 품질관리를 위한 이온전리함 매트릭스의 유용성 고찰 (Feasibility of MatriXX for Intensity Modulated Radiation Therapy Quality Assurance)

  • 목 적: 세기변조방사선치료 시 선행되는 품질관리에서 선량분포와 절대선량 값을 비교하여 2차원 이온전리함의 유용성을 알아보고자 한다. 대상 및 방법: 본 실험은 선형가속기(CL 21EX, Varian, Palo Alto, USA)의 6 MV 광자선을 이용하여 비인두암 환자를 대상으로 하였다. 세기변조방사선치료를 위한 품질관리에는 2차원 이온전리함 배열(MatriXX, Wellhofer Dosimetrie, Germany)을 사용하였다. 비인두암 환자의 치료를 위해 역방향치료계획을 시행하였다. 고체팬톰에 삽입된 MatriXX를 전산화단층 촬영하여 환자 치료계획과 동일한 플루언스로 하이브리드(갠트리 각도 0$^\circ$) 치료계획을 실시하였다. 실험적 선량분포의 측정은 하이브리드 치료계획과 동일한 기하학적 조건으로 MatriXX와 고체팬톰을 이용하여 측정하였다. 선량분포에서 고선량 저변위(High Dose Low Gradient: HDLG) 점을 선정하여 절대선량을 비교하였으며, 선량분포 일치성분석을 위해 감마 인덱스 히스토그람($\gamma$-index, Dose difference: 3%, Distance to agreement: 3 mm)을 이용하여 정량화하였다. 결 과: 감마 인덱스 히스토그람 분석결과 인정허용범위(${\gamma}{\leq}$1) 비율을 모든 조사면에서 각각 확인하였다. 갠트리 각도 0$^\circ$에서 95.08%, 55$^\circ$에서 97.52%, 110$^\circ$에서 96.28%, 140$^\circ$에서 98.2%, 220$^\circ$에서 97.78%, 250$^\circ$에서 96.64%, 305$^\circ$에서 92.7%로 나타났다. HDLG에서 절대선량은 $\pm$3% 이내의 일치도를 보였다. 결 론: MatriXX를 이용한 세기변조방사선치료의 품질관리는 필름이나 단일 이온전리함을 이용한 일반적인 세기변조방사선치료 품질관리에 비해 시간, 인력을 최소화하면서 보다 효율적인 접근이 가능하였으며, 선량분포일치성 분석 및 절대선량확인에 매우 유용한 장치임을 알 수 있었다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.
