외과적 악교절수술에 있어서 측모연조직예측의 재평가에 대한 연구 (REAPPRAISAL OF SOFT TISSUE PREDICTION IN ORTHOGNATHIC SURGERY FOR MANDIBULAR PROGNATHISM)

  • 두부방사선사진을 이용한 안모의 측모예측은 악교정수술에 있어서 가장 중요한 수술전 치료계획의 한부분으로 술후 경조직과 연조직의 변화를 예상하며 술자와 환자간에 술후 결과를 의논하는 대화의 재료로서도 그 중요성이 크다. 그러나, 술후 환자의 최종적인 측모와 수술 직전에 수술 계획에 따라 시행한 술후 측모의 예측과는 차이가 있다. 이러한 수술 전 시행하는 측모 예측의 부정확성을 감소시키기 위하여 새로운 경조직 대 연조직 변화비율 등을 추정하는 많은 연구가 시행되어 왔으나 측모방사선 예측의 부정확도를 조사하고 이러한 부정확성에 영향을 미치는 변수를 확인함이 측모방사선예측의 부정확성을 이해하고 감소시키는 데에 있어서 그 중요성이 크다고 생각된다. 이에 저자들은, 이미 시행되어 수술에 사용된 술후 측모예측기록을 수술직후 두부방사선 사진 및 술후 최소 6개월 추적 방사선 사진에 중첩하여 측모 예측의 부정확도를 조사한 바 총 30명의 환자에서 20명(66.6%)이 부정확도를 보였고 악골 및 치열의 술후 위치 불안정성도 측모 예측 정확군과 부정확군에 모두 19명(63.3%)에서 발생하여 측모 예측의 부정확도 영향을 주는 중요한 요인으로 확인되었다.

  • Facial balance is the primary detevminant of good facial esthetics and is expressed externally by the shape of facial soft tissues. Balance of the facial skeleton is most important in prediction of orthognathic surgery, however, it is not alwags coincided to soft tissue balance because the soft tissue drapes overlying hard tissue varies in thickness and tones from case to case. So, soft tissue facial balance and esthetics also should always be considered in prediction of hard tissue changes preoperatively. The chin has a paramount importance in the overall appearance of the face and facial profile because it may express individual charactor or image. Therefore positional change of the chin must be considered in any cases as the last and important option to give an overall soft tissue balance. Two cases were referred from orthodontists only for anterior segmental of teortomuy of the chin. Pre-operative evaluation showed poor soft tissue chin profiles which were not coincided to hard tissue chin balance. We altered surgical plans to fulfill balancing soft tissue profile and then could improve overall esthetics after surgery.

안면비대칭자의 하악골 악교정수술 후 정면 연조직 변화 예측을 위한 연구 (Prediction of frontal soft tissue changes after mandibular surgery in facial asymmetry individuals)

  • 본 연구는 안면비대칭 환자의 악교정수술 시 경조직 이동에 따른 연조직 변화를 정면에서 평가함으로써 정면 얼굴 이미지 시뮬레이션 프로그램 개발에 도움이 되고자 시행되었다. 안면비대칭이 동반된 하악골 수술 예정 환자 45명을 대상으로 정모 두부방사선규격사진과 얼굴사진(photo)을 술전 및 술후에 각각 같은 각도로 촬영한 후, 술전 및 술후의 방사선사진을 이용하여 경조직 계측점의 변화를, 얼굴사진을 이용하여 연조직 계측점의 변화를 수평 및 수직으로 구분하여 각각 측정한 후 경조직 변화에 따른 연조직 변화를 비교 분석하였다. 연조직 변화와 경조직 변화의 상관성을 살펴본 결과 수평 방향, 수직 방향 모두에서 전반적으로 낮은 상관성을 보였으며, 1 : 1 mean ratio 산출을 위하여 서로 상관성이 가장 높은 경조직 계측점을 연조직 계측점별로 선택한 결과 직하방에 있는 경조직보다는 다소 멀리 떨어져 있는 경조직 계측점이 선택되는 경우가 많이 나타났다. 경조직 변화를 이용하여 연조직 변화를 예측할 수 있는 회귀방정식을 연조직 계측점별로 산출한 결과 연조직 수평변화 예측에 경조직 수직변화도 사용되고 연조직 수직변화 예측에 경조직 수평변화도 사용되었으며, 수평과 수직변화 모두에서 가장 설명력이 높은 방정식은 연조직 menton에서 나타났다. 이상의 연구결과 하부 경조직 계측점과 상부 연조직 계측점의 비율을 이용하는 1 : 1 mean ratio 방법은 불가능한 것으로 나타난 반면 회귀분석을 이용한 연조직 변화 예측은 임상에 도움이 될 수 있는 것으로 나타나 정면 이미지의 경우 컴퓨터를 이용한 시뮬레이션 프로그램이 반드시 필요함을 시사하였다.

두부방사선 분석에 의한 상악전치부 후방이동시 연조직 변화 예측에 대한 연구 (SOFT TISSUE PROFILE CHANGE PREDICTION IN MAXILLARY INCISOR RETRACTION BASED ON CEPHALOMETRICS)

  • 본 연구는 성장에 의한 효과를 배제한 교정치료시의 연조직 변화와 치아 및 악골의 수평적, 수직적 움직임과의 상관 관계를 알아보고 예측 가능성을 검토하기 위해 Angle씨 2급 1류 부정 교합 환자 41명을 대상으로 치아 및 악골, 연조직의 수평, 수직이동의 평균 변화량을 측정하고 연조직에 대한 계단식 다중 회귀분석을 시행하였으며 다음과 같은 결론을 얻었다. 1. 경조직 계측항목 중 상악 중절치의 수평이동 및 기울기, 하악 중절치의 수평 및 수직이동, 수평피개량, 수직피개량, interincisal angle, 하악골 위치, 변화, A, B, 경조직 측모 돌출각 등의 항목에서, 연조직 계측항목 중 상순의 두께 및 후방이동, 기울기, 하순의 후방이동 및 기울기, interlabial angle, 비순각, 이순각, Sn, Ss, Si, 연조직 측모 돌출각등의 항목에서 치료 전과 후에 유의성 있게 변하였다(P<0.05). 2. 치료 전 후 유의한 변화를 보인 연조직 계측항목을 분석한 결과 모든 항목에서 경조직의 변화에 대해 영향을 받는 것으로 나타났다(P<0.05). 3. 연조직 회귀방정식에 중절치 수평피개량, 상악 전치 절단연의 후방이동량, 상악 전치 치근의 후방이동량, Pg의 수평변화량이 가장 높은 빈도로 포함됨으로써 이들은 연조직의 수직 및 수평변화에 영향을 많이 미치는 요소로 나타났으며 하악 전치의 수직이동량은 포함되지 않음으로써 연조직 변화와 관련성이 적은 것으로 나타났다. 4. Sn의 수직 및 수평변화는 경조직의 가장 많은 항목과 연관됨으로써 (A, B, Pg(H), overjet, upper incisor angle, Ur, Is(H)) 여러 경조직 변화의 복합적인 영향을 받는 것으로 나타났다. 5. 연조직 계측항목에 대한 상관계수($R^2$)의 범위는 0.2-0.6으로 중등도의 상관정도를 나타내었다.

하악전돌증 환자의 악교정 수술후 시간경과에 따른 안모 연조직 변화에 관한 연구 (A STUDY ON CHANGE OF THE SOFT TISSUE FACIAL PROFILE AFTER ORTHOGNATHIC SURGERY IN PATIENTS WITH THE MANDIBULAR PROGNATHISM)

  • This study was performed to evaluate the change of the soft tissue facial profile after mandibular set back surgery during time intervals. For this study, 33 patient, 8 males and 25 females, were selected and their lateral cephalograms were taken and analyzed periodically. Hard and soft tissue changes during postoperative time intervals, correlation between surgical skeletal changes and postoperative soft tissue changes, and prediction for long-term soft tissue changes were established through varying statistical methods. The results were as follow : 1. There were meaningful changes of anteroposterior skeletal position at 6 months and 2 years after mandibular set back by mandibular ramus osteotomy. Two years postoperatively, there was 30%, 32%, 29% relapse on B point, pogonion, menton each. 2. Two years after the mandibular ramus osteotomy, the relative changes of the soft tissue to their osseous counterparts showed 76% on the lower lip and 91% on the pogonion. 3. The movements of the mandibular landmarks in correlation to anteroposterior position of the lower lip and soft tissue of the chin showed to be effective on a long-term basis. 4. Using surgical changes of pogonion, prediction of changes in soft and hard tissue pogonion was useful and the coefficient of determination was 0.46 each and their reliability decreased 2 years postoperatively. 5. The upper lip position after the mandibular set back surgery was somewhat anterior 2 years postoperatively, but that has no statistical meanings.

장안모증환자의 술후 안정성 및 연조직변화에 대한 연구 (AN EVALUATION OF POSTOPERATIVE STABILITY AND SOFT-TISSUE CHANGES OF THE LONG FACE SYNDROME PATIENTS)

  • Purpose : The purpose of the present study was to evaluate postoperative stability and soft-tissue osseous relations of the long face syndrome patients. Methods : Twenty-five patients who had undergone bimaxillary surgery to correct long face syndrome at the Pusan National University Hospital were evaluated. The lateral cephalograms of preoperative, 1 week postoperative and at least 1 year postoperative were examined. Results : 1. The facial height of the long face syndrome patients were longer than normal Korean adults. 2. The most common malocclusion type of the long face syndrome patients in Korea was class III. 3. Horizontal postoperative skeletal relapses were $-0.64{\sim}0.80mm$ in the maxilla, and $-0.56{\sim}0.48mm$ in the mandible. 4. Vertical postoperative skeletal relapses were $0.20{\sim}0.56mm$ in the maxilla, and $-0.80{\sim}0.20mm$ in the mandible. 5. Postoperative soft tissue changes in long face syndrome patients were correlated with postoperative skeletal changes. So prediction schemes for postoperative soft-tissue changes were obtainable. Conclusion : It is hard to predict the exact direction and quantity of the postoperative skeletal relapse in long face syndrome patient's orthognathic surgery because of large standard deviation. But soft tissue change is predictable via prediction scheme.

하악골후방이동술 후 골격구조와 연부조직의 변화 (The Change of Bone and Soft Tissue Profile after Sagittal Split Osteotomy of Ramus)

  • Orthognathic surgery for Class III malocclusion requires an elaborate preoperative planning using cephalometries or Mock surgery models which enable the surgeon to anticipate postoperative skeletal changes of maxilla and mandible as well as dentition. After surgery, patient's satisfaction is greatly influenced by appearance of soft tissue change. Therefore, it is imperative to predict a relatively accurate soft tissue change prior to surgery. A 5 year retrospective study was designed to evaluate the soft tissue change after sagittal split osteotomy of ramus(SSRO) for class III malocclusion. Analyses of preoperative and postoperative anthropometric measurements were performed. Patients who were treated only by SSRO for class III malocclusion and could follow up for 6 months were studied. Among them, the patients who had history of cleft palate and lip or hemifacial microsomia were excluded. Soft tissue changes were estimated by using the frontal and lateral photographs. Skeletal changes were observed by measuring amount of set back and angular changes of mandible to the reference line by using cephalometries. Relapses were also measured 6 months after the operation. We could observe skeletal changes were more profound than soft tissue changes concerning amount of set back, but soft tissue changes were also profound in angle. Relapse was more profound in skeleton than soft tissue but the amount was not significant. In spite of the variables which may affect proper assessment of the soft tissue change after skeletal relocation, this study can serve as a guide for exact prediction of the postoperative change of soft tissue and skeleton.

후진 이부성형술의 경조직과 연조직 측모 변화 예측에 관한 연구 (A Study on the Prediction of Hard and Soft Tissue Changes after Setback Genioplasty)

  • Purpose: The purpose of this setback genioplasty study is to develop a prediction method for the calculated osteotomy angle using horizontal and vertical changes as well as to evaluate the proportion of hard and soft tissue changes. Methods: Twelve patients who had received setback genioplasty with other maxillofacial surgery were examined. Three lateral cephalograms were taken just before surgery, immediately after surgery, and 3 months later surgery. A reference line was established to the reference point of the inner most point of the lingual symphysis cortex, incisor tip, and 2nd molar cusp tip. Measuring was conducted from pogonion (Pg), menton (Me), labrale inferius (Li), Mentolabial fold, soft tissue pogonion (Pg'), and soft tissue menton (Me') to the reference lines. Results: In setback genioplasty, the skeletal Pg moved posteriorly 5.07 mm. The ratios of soft tissue to hard tissue movement were 36% posteriorly and 62% inferiorly at Pg', 67% posteriorly and 104% inferiorly at Me', and 34% anteriorly and 164% posteriorly at Li. In reduction & setback genioplasty, skeletal Pg moved posteriorly 4.63 mm and skeletal Me moved superiorly 3.63 mm. The ratios of soft tissue to hard tissue movement were 76% posteriorly and 18% superiorly at Pg', 68% posteriorly and 42% superiorly at Me', and 44% anteriorly, 124% posteriorly at Li. The calculated mean slope angle, based on ${\Delta}H/{\Delta}V$ ratio, was 61.25 and the measured mean slope angle was 60.17. Thus, the calculated and measured slope angles have a similarity. Conclusion: In setback genioplasty, soft tissue moves posteriorly and inferiorly. In particular, at the Me' and Pg', the inferior movement of the soft tissue is greater than the posterior movement. Also, the predictable results (measured slope angle) after operation can be achieved by the calculated slope angle. Thus, the relationship of soft and hard tissue changes must be considered as the results are predictable.

하악전돌증 환자의 실물 측모사진을 이용한 악교정 수술후 연조직변화 예측에 관한 연구 (THE PREDICTION OF POSTSURGICAL SOFT-TISSUE PROFILE CHANCES ASSOCIATED WITH SURGICAL CORRECTION OF THE PROGNATHIC MANDIBLE BY STANDARDIZED FACIAL PHOTOSURGERY)

  • This study was designed to test the possibility of using a standardized lateral facial photographs as a clinical tool which produce the prediction of postsurgical soft-tissue profile changes associated with surgical correction in skeletal CIII patients. The number of the patients involved in this study were 27 in total, including 11 male patients and 16 female patients. A practical method to the utilization of presurgical photo prediction for mandibular prognathic patients has been presented. To predict postoperative facial appearance, montage photographs were superimposed on standard facial reference photos taken preoperatively. Within the limitations of its technology, postoperative predictions generated by this method were of sufficient accuracy, especially mandible and chin area, for clinical use. In addition, they provide valuable communication and diagnostic information which may be used in formulating treatment plan in cases requiring corrective orthognathic surgery. But, the lip changes were somewhat exaggerated by photo prediction. Consequently, the photo prediction seems suitable for planning profile changes in orthognathic surgery that include mobilization of one main mandibular fragment. Futher investigations are needed to determine whether changes of soft-tissues and hard-tissues are sufficiently reproducible so that more meaningful predictive values can be established.

하악골 후방이동시 중안면부 연조직의 변화양상에 대한 임상통계학적 연구 (A Clinical Study of Soft Tissue Changes of the Midface after Mandibular Setback Surgery)

  • Prediction of the soft tissue changes following hard tissue movement is very important from the esthetic view point for patients who have orthognathic surgery. There are many cephalometric analysis of facial bone and soft tissue on the lower lip and chin region but few soft tissue analysis on the midface after mandibular setback surgery. This study was performed to obtain whether the mandibular posterior movement has influence on the midface and the predictable ratio of post-operative measurement values of the soft tissue changes following mandibular setback surgery. Fifteen patients (8 males and 7 females) who had undergone mandibular setback surgery were selected and analyzed the soft tissue movement on the upper lip and the cheek region. Post-operative changes of the soft tissue measurements after mandibular surgery were examined on pre- and post-operative cephalometrics and the ratio of changes were analyzed after drawing the reference line on the face with the barium sulfate solution. The reference lines were perpendicular to the intercanthal line from infraorbital foramen and lateral canthus. The results obtained were as follows : 1. There were tendancy of anterior movement of soft tissue adjacent the nose after mandibular surgery 2. There were incerased tendancy of the amount of anterior movement from the nasal crease to the cheek region. 3. The amount of anterior movement of the soft tissue was larger below the palatal plane compared with above the palatal plane in the cheek region. 4. The upper lip length was increased and moved posterior direction after mandibular setback surgery 5. The lower lip was moved posterior direction by posterior movement of the mandibular structure 6. Soft tissue of the midface around the nose moved anterior direction after mandibular setback surgery but there was no correlation between the amount of mandibular setback and the amount of the soft tissue changes
