동종이식 연구를 위한 마우스 지방전구세포의 표지 및 분화 방법의 확립 (Differentiation and Labeling of Mouse Preadipocytes for Allogenic Transplantation Study)

  • Due to its safety and softness, autologous fat transplantation has been commonly performed for soft tissue correction. However, the injected fat is absorbed resulting in the reduction of volume of the graft by 40-60% within a few months. Thus, there was an attempt to use adipocytes differentiated from preadipocytes in vitro for transplantation. Differentiated adipocytes were biocompatible and matured with gradual volume increase at transplantation site in clinical study(unpublished data). In addition, they did not induce immune rejection in response to nonself lymphocytes in a mixed lymphocyte reaction(MLR)(unpublished data). The purpose of this study is to differentiate mouse preadipocytes following labeling into adipocytes to establish an animal model for allogenic transplantation. Preadipocytes isolated from inguinal and retroperitoneal fat pad of C57BL/6 mice were proliferated with growth medium by passage 3 and differentiated into adipocytes with different culture conditions after labeled with BrdU. At most suitable conditions, above 90% of preadipocytes were differentiated and BrdU labeling did not affect differentiation rate and function of differentiated adipocytes. These results demonstrate that BrdU-labeled adipocytes resulting from this in vitro differentiation protocol are useful for allogenic transplantation study.

암종으로 오인된 전이 폐포횡문근육종의 세침흡인 세포소견 -1예 보고- (Fine Needle Aspiration Cytology of Metastatic Alveolar Rhabdomyosarcoma Misinterpreted as Carcinoma - A Case Report -)

  • Fine-needle aspiration cytology (FNAC) has been used extensively in the diagnosis of metastatic malignancies. However, metastatic soft tissue sarcomas are often overlooked, primarily due to the low frequency with/ which they occur. Here, we report a rare case of metastatic rhabdomyosarcoma in both cervical lymph nodes, which was detected by FNAC. A 45-year-old woman presented with anosmia, postnasal drip, and sneezing, symptoms which had persisted for 1 month. The patient was found to have a tumorous lesion at the upper portion of the mid-turbinate, with multiple enlarged cervical lymph nodes, and this lesion was examined closely at our facility. FNA cytology smears obtained from both cervical lymph nodes revealed a high degree of cellularity, and displayed cohesive clusters with gland-like spaces, as well as single isolated cells with abundant karyorrhectic debris. The tumor cells exhibited round to oval nuclei containing fine chromatin, occasional small nucleoli, and scanty cytoplasm, or a total lack of cytoplasm. Some of the tumor cells were arranged in multinucleated forms and abundant dense eosinophilic cytoplasms, reminiscent of a rhabdomyoblast. The histological findings on the lymph nodes revealed an outstanding sinusoidal infiltration and a prominent alveolar growth pattern, interspersed with occasional typical rhabdomyoblasts. The immunohistochemical results [desmin (+), myoglobin (+), myogenin (+), pan CK (-), synaptophysin (-), neuron specific enolase (-)] supported a confirmative diagnosis of alveolar rhabdomyosarcoma. Alveolar rhabdomyosarcoma is a representative sarcoma, which typically manifests with nodal metastasis and carcinoma-like clustering. The cytopathologist should remain alert upon encountering unusual morphology, so that the possibility of this condition, although somewhat remote, should not be dismissed or overlooked.

백서에서 흡수성막과 탈회동결건조골을 이용한 두개골결손부의 골재생 (GUIDED BONE REGENERATION OF CALVARIAL BONE DEFECTS USING BIOABSORBABLE MEMBRANE AND DEMINERALIZED FREEZE DRIED BONE IN RATS)

  • The purpose of this study was to evaluate new bone formation and healing process in rat calvarial bone defects using $BioMesh^{(R)}$. membrane and DFDB. Forty eight rats divided equally into 4 groups of 1 control group and 3 experimental groups. Standardized transosseous circular calvarial defects (8 mm in diameter) were made midparietally. In the control group, the defect was only covered with the soft tissue flap. In the experimental group 1, it was filled with DFDB only, in the experimental group 2, it was covered $BioMesh^{(R)}$. membrane only, and in the experimental group 3, it was filled DFDB and covered with membrane. At the postoperative 1, 2, 4, 8 weeks, rats were sacrificed and histologic and histomorphometric analysis were performed. These results were as follows. In histomorphometric analysis, It showed the greatest amount of new bone formation through experimental in the experimental group 3 (P<0.001). The amount of new bone formation at the central portion of the defect was greater in the experimental group 3 than experimental group 2. $BioMesh^{(R)}$. membrane began to resorb at 1 week and resorbed almost completely at 8 weeks after operation. The collapse of membrane into the defect was observed through the experimental periods in the experimental group 2. In the area of collapsed membrane, new bone formation was restricted. These results suggest that maintenance of some space for new bone to grow is required in the use of $BioMesh^{(R)}$. membrane alone in the defect. It is also thought that use of the membrane may promote new bone growth in DFDB graft.

Effect of Feeding Olive Cake in Complete Diet on Performance and Nutrient Utilization of Lambs

  • Forty-five Najdi ram lambs were equally and randomly allotted to five dietary groups with three replications per diet to evaluate the effect of feeding olive cake in complete diet on nutrient digestibility, growth performance and carcass characteristics. The diets were: a whole-mixed control diet (C) containing 12% wheat straw and four diets; namely O, ON, OU and OUE, where crude olive cake, 3% NaOH-treated olive cake, 5% urea-treated olive cake (U) and ensiled U, respectively were incorporated to replace the wheat straw of C diet. The results showed that the digestibility of DM, CP, EE and NFE and TDN were higher (p<0.05) in OUE diet as compared to the control diet; there were no changes (p>0.05) in the corresponding values between O, ON and OU diets. The nutrient digestibilities, body weight gain, feed efficiency and Longissimus area were significantly (p<0.05) lower in ON diet as compared to other experimental diets. Average daily DM intake and weight gain were higher (p<0.05) in lambs fed O, OU and OUE diets than those fed C diet. The moisture and protein contents in the soft tissue from ON treatment were lower (p<0.05) than those tissues from C, O, OU and OUE treatments. In conclusion, these results indicated that crude olive cake is a better substitute of wheat straw when fed to growing lambs.

키높이 인솔두께에 따른 족부의 생체역학적 특성변화에 대한 연구 (A Study on Changes in Biomechanical Characteristics of the Foot with Respect to Wedge-type Insole Thickness)

  • Recently, functional insoles of wedge-type it is for the young to raise their height inserted between insole and heel cause foot pain and disease. Additionally, these have a problem with stability and excessively load-bearing during gait like high-heel shoes. In this study, we compared the changes in biomechanical characteristics of foot with different insole thickness then we will utilize for the development of the insole with the purpose of relieving the pain and disease. Subjects(male, n = 6) measured COP(center of pressure) and PCP(peak contact pressure) on the treadmill(140cm/s) using F-scan system and different insole thickness(0~50 mm) between sole and plantar surface during gait. Also, we computed changes of stresses at the foot using finite element model with various insole thickness during toe-off phase. COP moved anterior and medial direction and, PCP was increased at medial forefoot surface, $1^{st}$ and $2^{nd}$ metatarsophalangeal, ($9%{\uparrow}$) with thicker insoles and it was show sensitive increment as the insole thickness was increased from 40 mm to 50 mm. Change of the stress at the soft-tissue of plantar surface, $1^{st}$ metatarsal head represents rapid growth($36%{\uparrow}$). Also, lateral moments were increased over the 100% near the $1^{st}$ metatarsal as the insole thickness was increased from 0 mm to 30 mm. And it is show sensitive increment as the insole thickness changed 10 mm to 20 mm. As a result, it was expected that use of excessively thick insoles might cause unwanted foot pain at the forefoot region. Therefore, insole thickness under 30 mm was selected.

소아에서 발생한 비치성 양성종양에 관한 임상적 연구 (A CLINICAL STUDY OF THE NON-ODONTOGENIC BENIGN TUMORS OCCURRED IN THE CHILDREN)

  • Non-odontogenic tumors can be classified as malignant or benign. Most oral tumors in children are benign. In the Belfast series only 7.5 percent of soft tissue tumors were malignant and Bhaskar(l963) found only 9 percent of 293 oral tumors of all kinds to be malignant. Benign tumors may be classified as epithelial and mesenchymal. The most common tumor of surface epithelium is the squamous papilloma. These are easily recognized clinically as cauliflower-like lesions. Fibrous lesions are very common in children's mouths. Many of these are not true neoplasms but are related to fibrous hyperplasia. Another common oral tumor in children is angiomatous tumors. Hemangioma occurred more frequently than lymphangioma. Cystic hygroma, a cystic subtype of lymphangioma, is a developmental tumor of lymphatic origin. It is a considered to be a relatively rare lesion. About 50 percent of cystic hygroma are present at birth, and most of the remaining 50 percent appear in the early years of life during the period of active lymphatic growth. The preferred treatment for these lesions, except for hemangioma, is complete surgical excision. With proper surgical techniques, recurrence is not expected.

Horseshoe Appliance를 이용한 III급 부정교합의 교정적 접근 (ORTHODONTIC MANAGEMENT OF CLASS III MALOCCLUSION WITH HORSESHOE APPLIANCE)

  • 성장기 아동의 III급 부정교합의 치료에는 많은 악기능 정형장치가 사용되어 왔으며 다양한 형태로 변화하여 발전되어 왔다. Horseshoe appliance는 Schwarz에 의해 소개된 장치로서 elastic의 견인력으로 장치의 free sliding 효과를 통해, 하악와와 과두의 관계를 재정립시켜, 악골 주위의 연조직이나 muscle matrix와 조화되는 새로운 위치에 하악골을 repositioning 시켜 악골관계를 개선시킬 수 있다. 또한 하악의 후하방 회전 효과를 극소화시켜 전안면고경의 증가를 방지할 수 있고, 구외장치에 비해 환자의 협조도가 좋다. 저자들은 전치부 반대교합을 주소로 전북대학교 치과병원 소아치과에 내원한 III급 부정교합 환자에게 Horseshoe appliance를 사용한 결과 하악골에 대한 상대적인 상악골의 전방 성장과 교합평면의 반시계방향 회전, 상악 전치의 순측 경사와 하악 전치의 설측 경사로, 3개월 내에 전치부 반대교합이 해소되는 효과를 얻었다.

경부 심부 감염 19예에 대한 임상적 고찰 (A Clinical Study of 19 Cases of Deep Neck Infection)

  • 경부 심부 감염은 경막과 경강에 발생하는 염증으로 항생제 요법으로 그 빈도가 상당히 감소 되었지만 여전히 생명의 위협을 초래할 수 있는 질환이다. 저자들은 최근 본원 이비인후과에서 치험한 경부심부감염환자 19례를 대상으로 임상적 고찰을 하여 다음과 같은 결과를 얻었다. 1 연령별 발생 분포는 2세에서 61세까지 전 연령층에서 고르게 발생하였으며, 평균연령은 39세이었고, 남녀비는 남자 13(68%)례, 여자 6(32%)례 이었다. 2.발생부위는 악하강이 ll례(58%), 측인두강이 7례(31%), 인두후강이 1례(5%)이었다. 3.원인균은 12례의 균배양검사상 용혈성연쇄상구균 6례(50%), 포도상구균 3례(25%)가 검출 되었고 3례(25%)에서는 균이 검출되지 않았다. 4.특기할 합병증 없이 전례가 치유되었다. 그중 13례(68%)에서는 항생제요법을 병행한 수술적 절개배농으로, 6례(32%)에서는 보존적 요법으로 치유되었다.

염증성 근섬유모세포종의 증례보고 (Inflammatory myofibroblastic tumor : A case report)

  • The Inflammatory myofibroblastic tumor (inflammatory pseudotumor) is a rarely occurring soft tissue lesion of unknown etiology. It can be of any location, but commonly it is found in lungs. It has been considered as a nonneoplastic reactive inflammatory lesion, but nowadays, confusion and dispute about its character is increasing due to its high recurrence rate and metastasis. We present a patient who had been diagnosed with an inflammatory pseudotumor in the right maxilla area, 1 year before visiting our hospital. After that, her pain and swelling did not resolved and she visit our hospital. On radiographic examination, aggressively infiltrative growth of the lesion with destruction of adjacent bony structure was noted. We found unusual aggressiveness of the inflammatory myofibroblastic tumor of the head and neck region. Because the typical behavior of the inflammatory myofibroblastic tumor is not defined yet, we recommend the surgical excision of the lesion and close follow-up.
