Purpose: Management of soft-tissue defect after open tibial fractures includes immediate and repeated debridement, skeletal stabilization, and early soft-tissue coverage with muscle flaps. The purpose of this study was to evaluate the outcome of the free rectus abdominis muscle flap (RA flap) for treatment of open fractures of the tibia and to discuss its advantages compared with the latissimus dorsi muscle flap (LD flap) in poly trauma patients. Materials and Methods: We performed a retrospective review of 5 patients who had a severe (Gustilo IIIb or IIIc) open fracture of the tibia treated with RA flap from May 2003 to March 2006. All were men, and the mean age was 46.6 years (range, $28{\sim}68$). Three patients had combined injuries such as pelvic bone fractures, multiple rib fractures with hemothorax, and contralateral tibial fracture. All patients received RA flap within 7 days after trauma except two with established chronic osteomyelitis. Results: All flaps survived, and there was no marginal flap necrosis. During the follow-up period, there was no evidence of persistent or recurrent osteomyelitis. The size of RA flap ranged from $8{\sim}20\;cm$ in length and $6{\sim}10\;cm$ in width. The average time required for RA flap elevation was 32 minutes, which is shorter than LD flap. Flap elevation could be done in supine position which is essential in poly trauma patients. Conclusion: Although a wide variety of options are available, RA flap is regarded as an optimal method for coverage of soft-tissue defect of the open tibial fracture in poly trauma patients. LD flap is reserved for large sized soft-tissue defect which cannot be covered by RA flap.
The present study investigated the physical changes and reaction products with setting time after mixing of various mineral admixtures such as lime, hydrated lime, gypsum, kaolin, zeolite and diatomaceous earth with four types of cement (portland cement, slag cement, quick lime, hydrated lime) and clay rich sediments in soft foundation. As results, slag cement showed the greater compressive strength than normal portland cement. The mixing experiments with various mineral admixtures and slag cement resulted that gypsum showed the greatest compressive strength. Additionally, we conducted mixing experiments with various mixing ratios of gypsum and slag cement. The experiments showed that the mixing ratio of $30\%$ gypum and $70\%$ slag cement has the greatest compressive strength. Ettringite was produced as a reaction Product. This fact indicates that gypsum effectively promotes hydration reaction and contributed to the greater compressive strength. These experimental results can be used as fundamental data for the stabilization of soft clay foundation.
Recently, the demand for industrial and residential lands are being increased with economic growth, however, it is difficult to acquire the land for development with good ground condition. For efficient and balanced development of land, new development projects are being carried out not only the areas with inland but those with the soft ground as well. As soft grounds have complex engineering properties and high variations such as ground settlement especially when their strength is low and depth is deep, it needs to accurately analyze the engineering properties of soft grounds and find general measurement for stabilization and economic design and management. Prefabricated vertical drain technology is widely used to accelerate the consolidation of soft clay deposits and dredged soil under the preloading and various types of vertical drain are being used with the discharge capacity. Under field conditions, the discharge capacity is changed with various reason, such as soil condition, confinement pressure, long-term clogging and folding of vertical drains, and so on. Therefore, many researcher and engineer recommend the use of required discharge capacity. In this paper, the experimental study were carried out for two different types of vertical drains by utilizing the large-scale model tests and waste lime.

This systematic review and meta-analysis aimed to analyze the effectiveness of maxillary stabilization splint (SS) therapy to reduce headache (HA) intensity and HA frequency in patients with temporomandibular disorders (TMD)-HA comorbidity. Randomized controlled trials (RCTs) using full-arch coverage, hard resin, and maxillary SS therapy were included. Electronic databases, including Cochrane Library, MEDLINE through PubMed, Web of Science, and EMBASE, were searched. The risk of bias was analyzed based on Cochrane's handbook. The search yielded 247 references up to January 28, 2020. Nine RCTs were included at a high risk of bias. The comparison groups included other splints, counseling, jaw exercises, medications, neurologic treatment, and occlusal equilibration. Four studies reported a statistically significant reduction in HA intensity, and five studies reported significant improvement in HA frequency from baseline at 2-12 months in patients with TMD-HA comorbidity treated with a full-arch hard maxillary SS. HA frequency in tension-type HA (TTH) comorbid with TMD diagnoses of myofascial pain (MFP) or capsulitis/synovitis improved significantly with SS than that with full-arch maxillary non-occluding splint (NOS) in two studies. Comparison groups receiving hard partial-arch maxillary splint nociceptive trigeminal inhibition (NTI) showed statistically significant improvements in HA intensity in patients with mixed TMD phenotypes of MFP and disc displacement comorbid with "general HA." Comparison groups receiving partial-arch maxillary resilient/soft splint (Relax) showed significant improvements in both HA intensity and frequency in patients with HA concomitant with MFP. The meta-analysis showed no statistically significant difference in the improvement of pain intensity at 2-3 months with comparison of the splints (partial-arch soft [Relax], hard [NTI], and full-arch NOS) or splint use compliance at 6-12 months with comparison of the splints (partial-arch Relax and full-arch NOS) versus the SS groups in patients with various TMD-HA comorbidities. In conclusion, although SS therapy showed a statistically significant decrease in HA intensity and HA frequency when reported, the evidence quality was low due to the high bias risk and small sample size. Therefore, further studies are required.

Purpose: The present study aimed to investigate the effects of corneal eccentricity and shape on the rotational pattern of toric soft lens by the postural change of lens wearers. Methods: The corneal eccentricity of 41 eyes (aged 20s) having -1.0 D with-the-rule corneal astigmatism (WRCA) was measured, and then toric soft lenses were fitted with the amount of total astigmatism. In lying and straight postures, the rotation of toric soft lenses was recorded by a camera attached to slitlamp and analyzed. Results: Most toric soft lens designed with accelerated stabilization rotated to the temporal direction, which was the lying position direction, regardless of corneal eccentricity, and some lenses rotated to the nasal direction for high corneal eccentricity and corneal type of asymmetric bowtie. There was no correlation between the amount of rotation and corneal eccentricity right after of contact lens wearing in straight and lying posture, however, the amount of rotation was the greater for the cornea with the higher eccentricity after the subjects laying down for some period. The speed of lens rotation started to decrease after the subjects laying down, but the speed was not different according to corneal eccentricity difference. The amount of lens rotation for symmetric and asymmetric bowtie-typed corneas increased more than it for oval-typed cornea, and it was same even with time elapsing. The speed of lens rotation in lying posture was the slowest in asymmetric bowtie-typed cornea compared with other corneal types. Conclusions: From the present study, it was revealed that the rotational pattern of toric soft lens was affected by corneal eccentricity and corneal shape when the wearer's posture changed. Thus, it should be considered for the development of the fitting guideline and the design of toric soft lens.
A Korean water deer was rescued after being hit by a motor vehicle. Clinical and radiographic examination revealed a grade III open fracture of the right metacarpus with severe soft tissue injury. The water deer underwent closed reduction of the fracture, which was stabilized by the application of the Ilizarov method of 4-ring circular external skeletal fixation (CESF) as a minimally invasive technique. The water deer was successfully released into the wild. CESF was advantageous for the stabilization of open comminuted fracture and the management of a severely contaminated wound in a Korean water deer. CESF can be used successfully in wild deer with good tolerance and minimal complications.

Purpose: The purpose of this study is to evaluate the effectiveness of internal fixation method of $Medpor^{(R)}$ implant with $BioSorb^{TM}FX$ screw which is used for prevention of enophthalmos in posteriorly extended large orbital floor fracture. Methods: From Jun. 1997 to Dec. 2007, 21 patients who were diagnosed with posteriorly extended large orbital floor fractures were classified into two groups. One group(n=11) had undergone reduction surgery with regular $Medpor^{(R)}$ sheets without any fixation method, while the other group(n=10) had their $Medpor^{(R)}$ sheets fixed with the $BioSorb^{TM}FX$ screws. The two groups were evaluated by comparison of their enophthalmos degree and effectiveness. Results: In the non-fixation group, six patients had enophthalmos preoperatively and three of them showed persistent enophthalmos postoperatively. In postoperative CT examination, displacement of $Medpor^{(R)}$ implant with soft tissue impaction into maxillary sinus was observed in the patients. In the screw fixation group, three patients had enophthalmos preoperatively, but none of them suffer from complication such as residual enophthalmos, soft tissue impaction, muscle entrapment or optic nerve compression postoperatively. Conclusion: Internal fixation method of $Medpor^{(R)}$ implant with $BioSorb^{TM}FX$ screw on the medial surface of orbital floor provides firm stabilization of implants and surrounding soft tissues and can be an effective option especially when postoperative implant displacement or malposition was expected.
Excavation of underground space in soft ground implicate to the structure, such as subsidence. As a result, it has been acting as a serious risk to the stability of the roads and facilities. Therefore, in order to stabilize the soil stabilization and reinforcement of the structure, we have been using a number of methods and injecting material. In this study, we compared and analyzed the amount of subsidence regarding the ground reinforcement during underground excavation in soft ground by performing model test. And three-dimensional numerical analysis was performed using FLAC 3D. The subsidence was simulated numerically according to the tunnel excavation. The subsidence results of the model tests and numerical analyzes were relatively consistent. Thus comparing the ground subsidence by varying the reinforcement area on the numerical analysis was analyzed. As a results, three-dimensional numerical simulation could be regarded to simulate better on the ground subsidence by various kinds of underground excavation and it can be used as a material of subsidence prevention methods.

The purpose of this paper is to study on the construction of roadbed with environmental friendly soil amendment agent. The special amendment agent used in this study is mainly composed of inorganic metal salts such as sodium chloride, magnesium chloride, potassium chloride, calcium chloride,, thus is friendly to the environment, and has a function of soil-cement-agent solidification. Various components of this agent weaken the negative function of humic acid and decompose humic acid itself. Then, the calcium cation of the cement can now be made contact directly to the soil surface. The project of local road demonstration of roadbed construction with special soil treatment agent was peformed in Northeast Thailand on August 1999 by the sponsor of Highway Department of Thailand. A series of field experiments including unconfined compressive strength were carried out to investigate the physical and mechanical characteristics of solidified roadbed treated by this solidifying agent. The results of this research showed that the roadbed using poor soil could be efficiently constructed by treatment of this amendment agent.

With the propagation and spread of the new regeneration energy and increase in electricity demand, power systems tend to be decentralized, and accordingly, the use of a power system stabilizer tends to expand for the stabilization of the distribution system. Thus, typical power system stabilizer, Static Var Compensator (SVC) is developed on a variety of topologies. In addition, the trend of technology leads from SVC to Static Synchronous Compensator(STATCOM) technology development. Recently, to overcome STATCOM's conversion losses and economic disadvantages, studies of a hybrid method using STATCOM and SVC in parallel have actively been conducted. This study proposes a new Soft-Step Switching method to limit inrush current problematic in Thyristor Controlled Capacitor (TCC) method in SVC function. In addition, to reduce Statcom's capacity, groups of reactive power compensation reactor and condenser for SVC were designed.
