소프트 콘택트렌즈 착용자의 실태와 문제점 (The State and Problem of the Soft Contact Lens Wearer)

  • 소프트 콘택트렌즈의 착용자에 대한 실태와 문제점에 대한 결과는 다음과 같다. 1. 최초의 콘택트렌즈 착용자의 가장 많은 연령층은 19세이며, 가장 먼저 착용한 연령은 15세이고, 가장 늦게 착용한 연령은 26세이다. 2. 처음 콘택트렌즈의 착용할 때의 시력검사 장소는 대게 안경원이다. 3. 현재 사용하고 있는 콘택트렌즈의 구입처는 대부분 안경원이다. 4. 현재 사용하고 있는 렌즈는 거의 소프트 렌즈이다. 5. 현재 사용하고 있는 렌즈의 생산국은 국내생산이 가장 많고, 모르는 것, 그리고 외국생산품 있다. 6. 콘택트렌즈 착용 후 시력을 알고 있는 사람은 거의 적으며, 특히 양안 시력을 알고 있는 사람은 특히 드물다. 7. 현재 사용하고 있는 소프트 콘택트렌즈의 색깔에 대해서는 10대에서는 무색이 가장 많고, 20대에서는 유색이, 그리고 30대에서는 거의 모르고 있다. 8. 소프트 콘택트렌즈의 장점에 대해서는 10대와 20대에서는 한 개정도, 30대에서는 전혀 모르는 것이 가장 많았다. 9. 소프트 콘택트 렌즈의 단점에 대해서는 10대와 30대에서는 한 개이나, 20대에서는 2개 정도 알고 있는 것이 가장 많았다. 10. 소프트 콘택트렌즈의 착용에 대한 부작용에 대해서는 10대에서는 한가지, 20대와 30대에서는 전혀 모르는 것이 가장 많았다. 11. 소프트 콘택트렌즈의 착용자에 대한 주요 자각증상에 대해서는 모두 7가지가 있으나, 전혀 자각증상이 없는 것이 가장 많고, 다음은 잘모르는 것, 그리고 자각증상이 있는 것에서는 통증이 가장 많았다. 12. 현재 착용하고 있는 소프트 콘택트렌즈의 부작용이 있는 렌즈의 현미경적 표면 조사에서는 렌즈의 파손, 렌즈의 중첩, 이물질의 침착 등이 주로 나타났다. 이상의 연구에서 현재 착용하고 있는 렌즈는 대게 소프트 렌즈이며, 구입처는 안경원이며, 소프트 렌즈에 대한 피검자의 지식과 시력에 대한 지식은 아주 적은 편이다. 따라서 현실적으로 볼 때에 안경사의 소프트렌즈에 대한 관리 및 취급을 할 수 있도록 법적 또는 교육적 지원이 있어야 한다고 생각된다.

배양된 Pseudomonas aeruginosa의 소프트 콘택트 렌즈 표면부착에 대한 미세구조적 연구 (A Study of Ultrastructure on attachment of Soft Contact Lens Surface of Incubated Pseudomonas aeruginosa)

  • 소프트 콘택트렌즈는 취급이 간단하고 착용감이 좋고 미용효과를 가지며 운동과정에서 좋은 부착상태를 가지므로 피검자들은 시력교정으로 많이 사용되고 있다. 그러나 사용의 부주의로 눈에 부작용을 일으킬 수 있다. 본 연구는 배양된 P. aeruginosa의 소프트 콘택트렌즈 표면부착에 대한 미세구조적 상태를 규명하기 위해서 저자들은 주사전자현미경을 이용해서 소프트 콘택트렌즈 표면을 관찰하였다. 샘플 제작과정에서 $OsO_4$의 후고정과 tannic acid에 염색한 것이 가장 좋은 관찰이 되었다. 균의 모양은 막대모양으로 섬모가 형성되어 있었다. 후고정을 하지 않는 샘플에서는 많은 이물질과 인공산물로서 균의 동정이 어려웠다.

토끼 후두내 삽입한 풍선도관에 대한 단기 조직 반응 (Soft Tissue Response of Rabbit Larynx to Implanted Foley Catheter for Short Duration)

  • Objectives : This study was aimed to investigate soft tissue reaction such as inflammation, immune reaction of rabbit larynx to implanted foley catheter. Methods : After 8 rabbits were anesthetized, their thyroid cartilage and trachea were exposed through a skin incision and a 6 French foley catheter was inserted into the thyroid cartilage via cricothyroid membrane and ballooned with normal saline (0.1 mL). The other end of catheters were ligated and cut. The wound was closed keeping catheter under the skin. Two rabbits were used as normal control Larynges were removed for pathologic examination at 4weeks and 8 weeks of the study respectively, Results : Ten rabbits were euthanized for gross and pathologic examination (5 rabbits after 4 weeks and 5 rabbits after 8 weeks). All rabbits survived the study periods and inflammations or foreign body reactions were minimally found on pathologic examinations. Conclusions : Foley catheter could be useful and safe material for vocal fold medialization in rabbit models.

수종 치조제 이물성형재의 조직반응에 관한 연구 (A STUDY ON TISSUE RESPONSES OF VARIOUS ALLOPLASTIC MATERIALS FOR RIDGE AUGMENTATION)

  • Loss of teeth causes the inevitable reduction of residual ridge. Among the various methods solving this problem, hydroxyapatite proved to be useful for correction of ridge defect and irregularity. The purpose of this study is to evaluate the tissue responses of two types Of hydroxyapatites and resin polymer. Calcitite 2040 (Calcitek Inc.), Interpore 200 (Interpore Int.), and HTR polymer (HTR Sciences) were implanted into the jaw of an adult dog. The procedure was designed to obtain the results of 1 week, 2 week, 4 week, and 12 week-intervals. And after 12 weeks from the first operation day, the dog was sacrificed and evaluated histologically by light microscope. The results were as follows : 1. The mucosa was healed after two weeks. 2. After 1 week, there were acute inflammatory cells, but diminished after 2 weeks and were not seen after 12 weeks. 3. The hydroxyapatites implanted in soft tissues were surrounded by fibrous connective tissue. And some foreign body giant cells were found. 4. Calcitite and Interpore particles implanted subperiosteally were surrounded by newly formed bone after 12 months. And direct contact between bone and particles was noted. 5. The HTR particles implanted in soft tissues were encapsulated by fibrous connective tissues. The sample where the particles contacted directly to bone could not obtained by some probable insufficiencies of surgical technique or care of the animal. And the residue of HTR particles was digested by macrophage.

설기저부로 전이된 소세포 폐암종 1예 (A Case of Small Cell Lung Carcinoma which Metastasizes to Base of Tongue)

  • Metastatic tumors of the oral cavity are uncommon. Most of them are located in the mandible while only small percentage are found within the soft tissues. The common sites of metastases to the soft tissue of oral cavity were the tongue and gingiva and among the tongue, the base of tongue was most common. An explanation of the predilection may be due to a richer vascular supply within base of tongue. The authors have recently experienced a case of small cell lung carcinoma with base of tongue metastasis in a 81-year-old male who had foreign body sensation at swallowing and muffled voice for 2 months. So, we present this case with the review of literatures.

Removal of a broken needle using three-dimensional computed tomography: a case report

  • Inferior alveolar nerve block obtained maximum anesthetic effect using a small dose of local anesthetic agent, which also has low a complication incidence. Complications of an inferior alveolar nerve block include direct nerve damage, bleeding, trismus, temporary facial nerve palsy, and etc. Among them, the major iatrogenic complication is dental needle fracture. A fragment that disappears into the soft tissue would be hard to remove, giving rise to a legal problem. A 31-year-old woman was referred for the removal of a broken needle, following an inferior alveolar nerve block. Management involved the removal of the needle under local anesthesia with pre- and peri-operative computed tomography scans.

하인두에 발생한 지방육종 1예 (A Case of Hypopharyngeal Liposarcoma)

  • Liposarcoma is one of the most common malignancies originating from human soft tissue. Because most of liposarcoma occur in the lower extremities or retroperitoneum, there are few reports about liposarcoma of head and neck region, including larynx and pharynx. Histologically, there are four types of liposarcoma, and prognosis and recurrence rate are different according to histologic subtype. Wide excision is the treatment of choice and liposarcoma hardly respond to primary radiotherapy. A 56-year-old man presented with voice change and foreign body sensation in laryngopharynx. Hypopharyngeal liposarcoma was diagnosed by surgical biopsy and performed totallaryngopharyngoesophagectomy and gastric pull-up. Here we report our experience on this case with review of literature.

Pseudotumor and Subsequent Implant Loosening as a Complication of Revision Total Hip Arthroplasty with Ceramic-on-Metal Bearing: A Case Report

  • Pseudotumors are not uncommon complications after total hip arthroplasty (THA) and may occur due to differences in bearing surfaces of the head and the liner ranging from soft to hard articulation. The most common causes of pseudotumors are foreign-body reaction, hypersensitivity and wear debris. The spectrum of pseudotumor presentation following THA varies greatly-from completely asymptomatic to clear implant failure. We report a case of pseudo-tumor formation with acetabular cup aseptic loosening after revision ceramic-on-metal hip arthroplasty. The patient described herein underwent pseudotumor excision and re-revision complex arthroplasty using a trabecular metal shell and buttress with ceramic-on-polyethylene THA. Surgeons should be aware of the possibility of a pseudotumor when dealing with revisions to help prevent rapid progression of cup loosening and implant failure, and should intervene early to avoid complex arthroplasty procedures.

Abrupt Hard Mass Caused by the Push of an Ice Cream Stick after Gastric Perforation in a Dog

  • A neutered male Maltese, 11-year-old, presented for hard mass at right flank suddenly. Patient vomited and had anorexia before the presentation, but it is improved after. On blood tests, there were no remarkable findings. On physical examination, firm mass ($2.9{\times}2.6cm$, firm) was detected. No remarkable finding was shown in radiography except for right upper-medial abdominal subcutaneous soft tissue mass. Punch biopsy was performed for histopathologic examination. During the punch biopsy, ice-cream stick ($11.5{\times}1.2cm$) appeared from the hole on right flank. Additionally gastric perforation was detected on ultrasonography. Emergency surgery was performed for the perforation. On histiopathologic examination of the mass, marked, diffuse, neutrophilic and mild eosinophilic dermatitis/cellulitis with no infectious agents was observed.

Osseous metaplasia of the palate: a case report

  • Osseous metaplasia is defined as the formation of lamellar bone inside soft tissue structures where bone normally does not exist. It results from the transformation of non-osseous connective tissue into mature bone. This condition is rare in the oral and maxillofacial region. We report a case of osseous metaplasia of the maxilla, a rare benign tumor in an uncommon region. A 60-year-old male patient visited our clinic complaining of foreign body sensation and asymptomatic swelling on the right palatal side. However, he did not experience pain and reported no local trauma that he could remember. Intra-oral examination revealed an exophytic lesion on the right palatal portion. On computed tomography, there was a round hard-tissue mass approximately 2 cm in diameter on the right palate area. The mass was biopsied and diagnosed as an osseous metaplasia. We review the clinical, radiographic, and histologic features and common causes of osseous metaplasia and report a rare case of osseous metaplasia of the palate.
