Linear accuracy of cone-beam computed tomography and a 3-dimensional facial scanning system: An anthropomorphic phantom study

  • Purpose: This study was conducted to evaluate the accuracy of linear measurements of 3-dimensional (3D) images generated by cone-beam computed tomography (CBCT) and facial scanning systems, and to assess the effect of scanning parameters, such as CBCT exposure settings, on image quality. Materials and Methods: CBCT and facial scanning images of an anthropomorphic phantom showing 13 soft-tissue anatomical landmarks were used in the study. The distances between the anatomical landmarks on the phantom were measured to obtain a reference for evaluating the accuracy of the 3D facial soft-tissue images. The distances between the 3D image landmarks were measured using a 3D distance measurement tool. The effect of scanning parameters on CBCT image quality was evaluated by visually comparing images acquired under different exposure conditions, but at a constant threshold. Results: Comparison of the repeated direct phantom and image-based measurements revealed good reproducibility. There were no significant differences between the direct phantom and image-based measurements of the CBCT surface volume-rendered images. Five of the 15 measurements of the 3D facial scans were found to be significantly different from their corresponding direct phantom measurements(P<.05). The quality of the CBCT surface volume-rendered images acquired at a constant threshold varied across different exposure conditions. Conclusion: These results proved that existing 3D imaging techniques were satisfactorily accurate for clinical applications, and that optimizing the variables that affected image quality, such as the exposure parameters, was critical for image acquisition.

  • Myoepithelioma is composed exclusively of myoepithelial cells. Myoepithelial cells are ectodermally derived contractile cells that can be routinely identified in many normal tissues having secretory function such as major and minor salivary glands, lacrimal gland and sweat gland. Tumors composed exclusively of myoepithelial cells, so-called myoepitheliomas, are rare-less than 1% of all salivary gland tumors. Grossly, these tumors are well demarcated. The external surface is smooth and may be bosselated. The cut surface is white and homogenous. Microscopically, these tumors are surrounded by a thin fibrous capsule. They are composed of benign-appearing spindle- shaped and/or polygonal cells. Mitoses are rare. Frequently these tumors contain myxomatous stroma which is susceptible to alcial blue stain. Clinically, myoepitheliomas present as slow-glowing, painless masses and can not distinguished from pleomorphic adenomas. Treatment is the same as for pleomorphic adenoma, and the surgical excision should include a margin of normal tissue. Although the majority of myoepitheliomas have behaved in a beingn manner, pleomorphism and mitotic activity have been associated with local aggressiveness.

근위부 경골 노출을 동반한 벗겨진 손상의 장딴지 근육 피판을 이용한 조기 피복 치험례 (Early Resurfacing Using Gastrocnemius Muscle Flap Transposition for Degloving Injury with Exposure of Proximal Tibia)

  • Degloving injuries result from the tangential force against the skin surface, with resultant separation of the skin and the subcutaneous tissue from the rigid underlying muscle and fascia. These injuries are associated with extensive soft tissue loss and occasionally with exposure of bone, and they require reconstructive modality for resurfacing and successful rehabilitation that considers the vascular anatomy and the timing of the operation. A 19-year-old male patient was transferred to our facility with degloving injury extending from the lower third of the right thigh to the malleolar area. The tibial bone was exposed to a size of $2{\times}3.5cm^2$ on the upper third of the lower leg at the posttraumatic third day. The exposed soft tissue was healthy, and the patient did not have any other associated disease. At the posttraumatic sixth day, one-stage resurfacing was performed with a medial gastrocnemius muscle flap transposition for the denuded bone and a split-thickness skin graft for the entire raw surface. The transposed gastrocnemius muscle attained its anatomical shape quickly, and the operating time was relatively short. No transfusion was needed. This early reconstruction prevented the accumulation of chronic granulation tissue, which leads to contracture of the wound and joint. The early correction of the gastrocnemius muscle flap transposition made early rehabilitation possible, and the patient recovered a nearly full range of motion at the injured knee joint. The leg contour was almost symmetric at one month postoperatively.

Comparison of pain relief in soft tissue tumor excision: anesthetic injection using an automatic digital injector versus conventional injection

  • Background: The pain caused by local anesthetic injection can lead to patient anxiety prior to surgery, potentially necessitating sedation or general anesthesia during the excision procedure. In this study, we aim to compare the pain relief efficacy and safety of using a digital automatic anesthetic injector for local anesthesia. Methods: Thirty-three patients undergoing excision of a benign soft tissue tumor under local anesthesia were prospectively enrolled from September 2021 to February 2022. A single-blind, randomized controlled study was conducted. Patients were divided into two groups by randomization: the experimental group with digital automatic anesthetic injector method (I-JECT group) and the control group with conventional injection method. Before surgery, the Amsterdam preoperative anxiety information scale was used to measure the patients' anxiety. After local anesthetic was administered, the Numeric Pain Rating Scale was used to measure the pain. The amount of anesthetic used was divided by the surface area of the lesion was recorded. Results: Seventeen were assigned to the conventional group and 16 to the I-JECT group. The mean Numeric Pain Rating Scale was 1.75 in the I-JECT group and 3.82 in conventional group. The injection pain was lower in the I-JECT group (p< 0.01). The mean Amsterdam preoperative anxiety information scale was 11.00 in the I-JECT group and 9.65 in conventional group. Patient's anxiety did not correlate to injection pain regardless of the method of injection (p= 0.47). The amount of local anesthetic used per 1 cm2 of tumor surface area was 0.74 mL/cm2 in the I-JECT group and 2.31 mL/cm2 in the conventional group. The normalization amount of local anesthetic was less in the I-JECT group (p< 0.01). There was no difference in the incidence of complications. Conclusion: The use of a digital automatic anesthetic injector has shown to reduce pain and the amount of local anesthetics without complication.

Micromorphological and Chemical Characteristics of Cengal (Neobalanocarpus heimii) Heartwood Decayed by Soft Rot Fungi

  • The heartwood of cengal (Neobalanocarpus heimii) is known to have a high degree of decay resistance by virtue of its high extractive content. After 30 years in ground contact an utility pole of this tropical hardwood was found to be degraded only in the surface layers by cavity-forming soft rot fungi. The present work was undertaken 1) to characterize the degradation of cengal heartwood from the aspect of ultrastructure and chemistry and 2) to investigate the correlation between soft rot decay and its extractive microdistribution in wood tissues. The chemical analysis of cengal heartwood revealed the presence of a high amount of extractives as well as lignin. The wood contained a relatively high amount of condensed lignin and the guaiacyl units. Microscopic observations revealed that vessels, fibers and parenchyma cells (both ray and axial parenchyma) all contained extractives in their lumina, but in variable amounts. The lumina of fibers and most axial parenchyma were completely or almost completely filled with the extractives. TEM micrographs showed that cell walls were also impregnated with extractives and that pit membranes connecting parenchyma cells were well coated and impregnated with extractives. However, fungal hyphae were present in the extractive masses localized in cell lumina, and indications were that the extractives did not completely inhibit fungal growth. The extent of cell wall degradation varied with tissue types. The fibers appeared to be more susceptible to decay than vessels and parenchyma. Middle lamella was the only cell wall region which remained intact in all cell types which were severely degraded. The microscopic observations suggested a close correlation between extractive microdistribution and the pattern and extent of cell wall degradation. In addition to the toxicity to fungi, the physical constraint of the extractive material present in cengal heartwood cells is likely to have a profound effect on the growth and path of invasion of colonizing fungi, thus conferring protection to wood by restricting fungal entry into cell walls. The presence of relatively high amount of condensed lignin is also likely to be a factor in the resistance of cengal heartwood to soft rot decay.

골격성 3급 부정교합 환자의 하악골 후퇴술 시행후 안모변화에 대한 3차원적 연구 (Three-dimensional analysis of soft and hard tissue changes after mandibular setback surgery in skeletal Class III patients)

  • 하악 수술로 치료한 골격성 3급 부정교합 환자의 연조직 변화를 3차원적으로 분석하였다. 수술전과 수술후에 CT를 촬영하고, 연조직과 경조직을 각각의 임계값에 따라 segmentation였다. FH.평면, 정중시상면, PNS를 포함하는 전두면을 기준으로 공통 좌표계를 구성하고, 이 좌표계를 기준으로 술전, 술후 영상을 위치시켰다. 술후의 변화를 측정하기 위해 각각의 모형에 대해 전두면에 평행한 grid를 형성하였다. Grid내의 교점에서 골조직과 연조직 모형에 투사하여 만나는 점의 좌표값을 구하고, 이를 바탕으로 술후의 변화를 측정하였다. 하악골 후퇴술시 안모의 변화는 하악골 부분에서만 발생한 것이 아니라, 구각부에서도 관찰되었다. 하악골 부위의 연조직 변화는 대응되는 골조직 이동량에 따른 상대적인 값으로 계산하였다. 정중시상면에서의 변화율은 $77\~102\%$로 나타났다. (p<0.05). 정중시상면 이외의 부분의 변화양상도 이와 유사하였다. 구각부에서의 변화는 하악골의 이동을 대표할 수 있는 점의 이동량에 대한 상대적인 값으로 계산하였다. 정중시상면에서의 변화는 B점을 기준으로 $14\~29\%$이고, Pog점을 기준으로 $17\~37\%$, grid상 83번째 점을 기준으로 $11\~22\%$로 관찰되었다.(p<0.05)

안경용 초음파세척기에 의한 소프트콘택트렌즈의 단백질 침전물 세척효과 (The Effects of Ultrasonic Cleaner for Eye Glasses on Protein Deposits and Parameters in Soft Contact Lens)

  • 목적: 안경용 초음파세척기를 이용하여 소프트콘택트렌즈의 단백질 침전물 세척 효과와 콘택트렌즈의 변수에 미치는 영향을 조사하였다. 방법: 단백질을 오염시킨 etafilcon A 재질의 소프트콘택트렌즈를 다목적용액에 넣어 안경용 초음파세척기와 콘택트렌즈용 초음파세척기를 사용하여 세척하고 대조군의 콘택트렌즈는 다목적용액으로 세척한 후 콘택트렌즈에 남아있는 단백질을 추출하여 정량하였고 콘택트렌즈의 전체지름, 베이스커브, 중심두께, 도수 및 함수율의 변화를 측정하였으며 주사전자현미경(scanning electron microscope)으로 콘택트렌즈 표면을 관찰하였다. 결과: 단백질을 오염시킨 etafilcon A 재질의 콘택트렌즈는 다목적용액에 의해 6.08%의 세척효과를 보였고, 다목적용액에 콘택트렌즈를 넣고 초음파세척기로 세척한 경우에는 시간에 따라 안경용 초음파세척기에서는 23.73~33.92%, 콘택트렌즈용 초음파세척기에서는 0~12.99%의 세척효과를 보였다. 결론: 안경용 초음파세척기로 세척한 콘택트렌즈에서 변수 및 표면 변화는 관찰되지 않았다. 결론: 안경원에서 사용하는 안경용 초음파세척기는 소프트콘택트렌즈의 단백질 침전물 제거에 효과적이고 변수 변화를 유발하지 않아 안경원에서 진단용 소프트렌즈의 세척에 활용될 수 있을 것으로 사료된다.

  • 이 시험은 도입남양재 원목에 부착 또는 서식하는 유해병해충의 방제를 목적으로 실시하는 해수수침소독에 있어 농약의 소요량기준을 설정함을 목표로 실시한 것으로써 결과를 요약하면 다음과 같다. 1. 우리나라에 도입되는 남양재 원목의 평균 길이는 11.12m이었으며 직경은 69.55cm이었다. 2. 원목의 전재적에 대한 부상재적은 평균 $20.82\%$ 이었다. 3. 원목의 전재적에 대한 부상표면적의 비의 값은 2.3356($95\%$ 신뢰수준)이었다. 4. 원목의 전재종 대 부상표면적에 대한 표본추출오차는 1본선내 원목간의 오차(0.2087)가 10본선간의 오차(0.2952)보다 적었다. 5. 원목 $1m^2$당 농약의 적정소요량은 수동식살포기를 이용하는 경우 0.368cc, 그리고 동력분무기를 이용하는 경우는 0.295cc이었다. 6. 예를 들어 남양재 원목 $5,000m^3$을 도입하여 동력분무기로 해수수침소독을 실시하는 경우 농약소요량은 다음과 같이 계산된다. $$5,000\;M^3\times2.333=11,678\;M^2$$ $$11,678M^2\times0.295=3,445cc$$.

Removal of mid-frequency error from the off-axis mirror

  • Manufacturing of lens and mirror using Diamond Turning Machine (DTM) offers distinct advantages including short fabrication time and low cost as compared to grinding or polishing process. However, the DTM process can leave mid-frequency error in the optical surface which generates an undesirable diffraction effect and stray light. The mid-frequency error is expected to be eliminated by mechanical polishing after the DTM process, but polishing of soft surface of ductile aluminum is extremely difficult because the polishing process inevitably degrades the surface form accuracy. In order to increase its surface hardness, we performed electroless nickel plating on the surface of diamond-turned aluminum (Al-6061T6) off-axis mirrors, which was followed by the 6-hour-long baking process at $200^{\circ}C$ for improving its hardness. Then we polished the nickel plated off-axis mirrors to remove the mid-frequency error and measured polished mirror surfaces using the optical surface profilometer (NT 2000, Wyko Inc.). Finally, we ascertained that the mid-frequency error on the mirror surface was successfully removed. During the whole processes of nickel plating and polishing, we monitored the form accuracy using the ultra-high accurate 3-D profilometer (UA3P, Panasonic Corp.) to maintain it within the allowable tolerance range (< tens of nm). The polished off-axis mirror was optically tested using a visible laser source and a pinhole, and the airy pattern obtained from the polished mirror was compared with the unpolished case to check the influence of mid-frequency error on optical images.

The effect of bonded resin surface area on the detachment force of lingual bonded fixed retainers: An in vitro study

  • Objective: The aims of this study were to evaluate the relationship between the detachment force and bonding resin surface are and to determine the resin bonding surface area that would provide adequate bonding strength with minimum resin volume. Methods: One hundred and sixty human premolars were randomly divided into 4 groups of 40 teeth each. The diameter of the resin surface area in each group was as follows: group 1, 1.5 mm; group 2, 2.5 mm; group 3, 3.5 mm; and group 4, 4.5 mm. Respond Dead Soft straight (length 0.0175 inch) was used to fabricate the retainers, and $Transbond^{TM}$ XT was used to fix the retainers to the tooth surfaces. A pair of teeth was embedded in acrylic blocks for each specimen. Thus, each group comprised 20 samples. Fixed retainers were bonded to the teeth, and vertical force was applied at the middle of wire. The force was measured using a universal testing machine. Results: The mean value of detachment force was the highest for group 4 ($102.38{\pm}2.92N$), followed by group 3 ($63.54{\pm}2.21N$), group 2 ($51.95{\pm}1.61N$), and group 1 ($24.14{\pm}1.38N$). Conclusions: The detachment force of lingual fixed retainers was significantly affected as the area of the resin bonding surface increased. Considering the minimum bonding strength of brackets, a resin bonding surface area with a diameter of 3.5 mm would provide adequate bonding strength.
