$Site-Specific^{99m}$Tc-Labeling of Antibody Using Dihydrazinoph-thalazine (DHZ) Conjugation to Fc Region of Heavy Chain

  • The development of an antibody labeling method with $^{99m}$Tc is important for cancer imaging. Most bifunctional chelate methods for $^{99m}$Tc labeling of antibody incorporate a $^{99m}$Tc chelator through a linkage to lysine residue. In the present study, a novel site-specific $^{99m}$Tc labeling method at carbohydrate side chain in the Fc region of 2 antibodies (T101 and rabbit anti-human serum albumin antibody (RPAb)) using dihydrazinophthalazine (DHZ) which has 2 hydrazino groups was developed. The antibodies were oxidized with sodium periodate to pro-duce aldehyde on the Fc region. Then, one hydrazine group of DHZ was conjugated with an aldehyde group of antibody through the formation of a hydrazone. The other hydrazine group was used for labeling with $^{99m}$Tc. The number of conjugated DHZ was 1.7 per antibody. $^{99m}$Tc labeling efficiency was 46-85% for T101 and 67∼87% for RPAb. Indirect labeling with DHZ conjugated antibodies showed higher stability than direct labeling with reduced antibodies. High immunoreactivities were conserved for both indirectly and directly labeled antibodies. A biodistribution study found high blood activity related to directly labeled T1 01 at early time point as well as low liver activity due to indirectly labeled T101 at later time point. However, these findings do not affect practical use. No significantly different biodistribution was observed in the other organs. The research concluded that DHZ can be used as a site-specific bifunctional chelating agent for labeling antibody with $^{99m}$Tc. Moreover, $^{99m}$Tc labeled antibody via DHZ was found to have excellent chemical and biological properties for nuclear medicine imaging.edicine imaging.

  • Purpose: Multidrug resistance (MDR) of the cancer cells related to mdr1 gene expression can be effectively treated by selective short hairpin RNA for mdr1 gene (shMDR). Sodium/iodide symporter (NIS) gene is well known to have both reporter and therapeutic gene characteristics. We have co-transfected both shMDR and NIS gene into colon cancer cells (HCT15 cell) expressing MDR and Tc-99m sestamibi and I-125 uptake were measured. In addition, cytotoxic effects of doxorubicin and I-131 therapy were also assessed after transfection. Material and Methods: At first, shMDR was transfected with liposome reagent into human embryonic kidney cells (HEK293) and HCT cells. shMDR transfection was confirmed by RT-PCR and western blot analysis. Adenovirus expressing NIS (Ad-NIS) gene and shMDR (Ad-shMDR) were co-transfected with Ad-NIS into HCT15 cells. Forty-eight hours after infection, inhibition of P-gycoprotein (Pgp) function by shMDR was analyzed by a change of Tc-99m sestamibi uptake and doxorubicin cytotoxicity, and functional activity of induced NIS gene expression was assessed with I-125 uptake assay. Results: In HEK293 cells transfected with shMDR, mdr1 mRNA and Pgp protein expressions were down regulated. HCT15 cells infected with 20 MOI of Ad-NIS was higher NIS protein expression than control cells. After transfection of 300 MOI of Ad-shMDR either with or without 10 MOI of Ad-NIS, uptake of Tc-99m sestamibi increased up to 1.5-fold than control cells. HCT15 cells infected with 10 MOI of Ad-NIS showed approximately 25-fold higher I-125 uptake than control cells. Cotransfection of Ad-shMDR and Ad-NIS resulted in enhanced cytotoxic by doxorubicin in HCT15 cells. I-131 treatment on HCT15 cells infected with 20 MOI of Ad-NIS revealed increased cytotoxic effect. Conclusion: Suppression of mdr1 gene expression, retention of Tc-99m sestamibi, enhanced doxorubicin cytotoxicity and increases in I-125 uptake were achieved in MDR expressing cancer cell by co-transfection of shMDR and NIS gene. Dual therapy with doxorubicin and radioiodine after cotransfection shMDR and NIS gene can be used to overcome MDR.

Transient Abnormalities on Magnetic Resonance Imaging after Absence Seizures (결신발작 이후 확인된 일시적인 자기공명영상 이상 1예)

  • Magnetic resonance imaging (MRI) is recommended for patients with epileptic seizures to rule out an underlying focal lesion. However, abnormalities in idiopathic generalized epilepsy, including childhood absence epilepsy, cannot usually be identified using brain imaging modalities such as MRI. Peri-ictal MRI abnormalities have been most commonly reported secondary to status epilepticus and are rarely observed in patients with focal seizures and generalized tonic-clonic seizures. Transient peri-ictal MRI abnormalities in absence epilepsy are extremely rare. A five-year-old girl presented with a three-day history of absence seizures that persisted despite continued treatment with sodium valproate. Electroencephalography showed bursts of generalized 3-Hz spike-and-wave discharges, during and after hyperventilation. Abnormal cortex thickening in the left perisylvian region was detected on T2-weighted brain MRI, and cortical dysplasia or a tumor was suspected. The patient started treatment with lamotrigine and was seizure-free after one month. The abnormal MRI lesion was completely resolved at the two-month follow-up. We report on a patient with childhood absence epilepsy and reversible brain MRI abnormalities in the perisylvian region. To our knowledge, this is the first report of transient MRI abnormalities after absence seizures. Transient peri-ictal MRI abnormalities should be considered for differential diagnosis in patients with absence seizures and a focal abnormality on brain MRI.

The Quantitative Evaluation of Cardiac Calcification Using 18F-Sodium fluoride PET/CT (18F-Sodium fluoride PET 이용한 심장 석회화 정량평가에 대한 고찰)

  • Purpose Although computed tomography (CT) is used for coronary artery calcification, it is difficult to differentiate between high risk microcalcifications. Studies have shown that $^{18}F$-sodium fluoride ($^{18}F-NaF$) is very useful for the diagnosis of microcalcifications. In this study, we aimed to evaluate the usefulness of $^{18}F-NaF$ PET imaging in quantitative evaluation of calcification. Materials and Methods A total of 45 patients ($67.1{\pm}6.9years\;old$) were injected with 250 MBq of $^{18}F-NaF$ for 1 hour and images were acquired for 30 minutes. All patients underwent CT angiography (CTAngiography, CTA) before the PET scan. The SUVmax of calcification was measured and the background radioactivity of the left atrium was measured to determine Target to Background (TBR) and quantitatively analyzed. High risk group was classified through ROC curve (Receiver Operating Characteristic Curve). Results There were 226 coronary artery calcifications in the cohort and SUVmax was $1.15{\pm}0.39$. Of the 28 patients (62%), 58 were classified as high risk (TBR > 1.25). The remaining 168 were $TBR{\leq}1.25$. Conclusion $^{18}F-NaF$ PET images were available for quantitative assessment of microcalcifications and could be classified into high-risk groups. The combination of angiographic CT and $^{18}F-NaF$ PET may be a new method for early diagnosis of high-risk microcalcifications.

  • We have synthesized uniform nanometer sized magnetite particles using chemical coprecipitation technique through a sonochemical method with surfactant such as oleic acid. Magnetite phase nanoparticles could be observed from X-ray diffraction. Magnetite nanoparticles is surface phase morphology and biopolymer-microspheres for Application Medical. Magnetite nanoparticles coated biopolymer. Atomic Force Microscope (AFM) was used to image the coated nanoparticles. Magnetic colloid suspensions containing particles with sodium oleate, chitosan and $\beta$-glucan have been prepared. The morphology of the magnetic biopolymer microsphere particles were characterized using optical microscope. Magnetic hysteresis measurement were performed using a superconducting quantum interference device (SQUID) magnetometer at room temperature to investigate the magnetic properties of the biopolymer microspheres and magnetite coated biopolymer including magnetite nanoparticles. Magnetic Resonance (MR) imaging was used to investigate biopolymer coated nanoparticles and biopolymer microspheres.

Alkali swelling characteristics of wood elements (목재 구성세포의 알칼리 팽윤 특성)

  • To elucidate the behaviors of alkali swelllng of woods. the dimensional change in cross section of cell elements of four major Korean woods during alkali swelling were examined by an optical microscory, an imaging analysis method and an X-ray diffrartion During alkali swelling, tracheid diameter of Larix kaempferi wood showed greater swelling property than that of Pinus koraiensis wood, and the cell wall swelled highly over 10% sodium hydroxide solution treatment. The radial diameter of vessel elements in earlywood shrunk, but it swelled slightly in tangential direction. When treated with 5% NaOH, the wall thickness of wood fiber increased about three times over the original one. The thickness of cell wall in all elements and the diameter of wood fiber and tracheid showed almost isotropic shrinkage. The diameter of cell elements during the mercerization process decreased, but cell wall thickness Increased. Crystal transformation of cellulose in wood was not occurred by alkali treatments. but relative crystallinity and crystallite width of the woods increased slightly. Consequently, it was demonstrated that the swelling properties of woods were dependant on wood species, cell elements and alkali concentration.

Reactive oxygen species increase neuronal excitability via activation of nonspecific cation channel in rat medullary dorsal horn neurons

  • The caudal subnucleus of the spinal trigeminal nucleus (medullary dorsal horn; MDH) receives direct inputs from small diameter primary afferent fibers that predominantly transmit nociceptive information in the orofacial region. Recent studies indicate that reactive oxygen species (ROS) is involved in persistent pain, primarily through spinal mechanisms. In this study, we aimed to investigate the role of xanthine/xanthine oxidase (X/XO) system, a known generator of superoxide anion ($O_2{^-}$), on membrane excitability in the rat MDH neurons. For this, we used patch clamp recording and confocal imaging. An application of X/XO ($300{\mu}M/30mU$) induced membrane depolarization and inward currents. When slices were pretreated with ROS scavengers, such as phenyl N-tert-butylnitrone (PBN), superoxide dismutase (SOD), and catalase, X/XO-induced responses decreased. Fluorescence intensity in the DCF-DA and DHE-loaded MDH cells increased on the application of X/XO. An anion channel blocker, 4,4-diisothiocyanatostilbene-2,2-disulfonic acid (DIDS), significantly decreased X/XO-induced depolarization. X/XO elicited an inward current associated with a linear current-voltage relationship that reversed near -40 mV. X/XO-induced depolarization reduced in the presence of $La^{3+}$, a nonselective cation channel (NSCC) blocker, and by lowering the external sodium concentration, indicating that membrane depolarization and inward current are induced by influx of $Na^+$ ions. In conclusion, X/XO-induced ROS modulate the membrane excitability of MDH neurons, which was related to the activation of NSCC.

Clinical manifestations of headache in children younger than 7 years

  • Purpose: Headache is a common symptom during childhood. It is usually persistent and requires special care. This study aimed to identify the characteristics of headache in children <7 years of age. Methods: We reviewed 3 years of clinical files on children <7 years of age with a chief complaint of headache. Results: This study included 146 children (66 males, 80 females; mean age, $5.5{\pm}1.0years$). Mean symptom duration was $5.8{\pm}7.9months$. Attack durations were longer than 2 hours in 31 patients, shorter than 2 hours in 70 patients, and unchecked in 45 patients. Attack frequency was $15.1{\pm}10.6$ times per month. Pain locations and characteristics were also variable. Mean pain severity score was $5.1{\pm}2.2$ on the visual analog scale. Of 38 patients who underwent electroencephalography, 9 showed positive findings. Of 41 who underwent brain magnetic resonance imaging, 20 showed positive findings. The diagnoses were migraine (including probable migraine) in 34, tension-type headache in 5, and congenital malformations in 3. Medications were used in 29 patients: acetaminophen in 17, ibuprofen in 8, naproxen sodium in 1, and topiramate or amitriptyline in 3. Conclusion: In children aged <7 years, headache has a relatively benign course, but detailed history taking is needed for more accurate diagnosis.

Studies on the Formation of Pyrophosphate-$^{99m}T_c$ complex

  • An instant labelling technique for lyophilized pyrophosphate with $^{99m}$Tc is described labelling yield of about 90% is obtained at the pH range 3.5-5.5 on reconstitution with sodium pertechnetate-$^{99m}$ Tc solution. The final product is controlled by a modified two dimensional paper chromatography using 85% methanol and 0.85% saline, and biodynamic investigations are performed on white mice. Generally, the less amount of stannous chloride is used. the higher labelling yield is obtained. The molar ratio of pyrophosphate to stannous chloride of 10 : 1 ~ 50 : 1 is sufficient. The more amount of reduced unbound $^{99m}$Tc is injected, the more radioactivity is incorporated in the liver. Thus. the cause of the false bone-imaging is attributable to the presence of reduced unbound $^{99m}$Tc which is known to be well adsorbed to oxidized tin colloidals. The maximum uptake ratio of bone: liver in mice, in weight basis, 35 : 1 is achieved in lime of 60 min. or so. Tile preparation is suitable for clinical investigations on patients with bone diseases.iseases.

Characterization of Rhizophora SPP. particleboards with SOY protein isolate modified with NaOH/IA-PAE adhesive for use as phantom material at photon energies of 16.59-25.26 keV

  • In this work, Rhizophora spp. particleboard phantoms were made using SPI-based adhesives, modified with sodium hydroxide and itaconic acid polyamidoamine-epichlorohydrin (0, 5, 10, and 15 wt%). An X-ray computed tomography (CT) imaging system was used to ascertain the CT numbers and density distribution profiles of the particleboards. The SPI-based/NaOH/IA-PAE/Rhizophora spp. particleboard phantoms with 15 wt% IA-PAE addition level had the highest solid content, flexural strength, flexural modulus, and internal bonding strength of 36.06 ± 1.08%, 18.61 ± 0.38 Nmm-2, 7605.76 ± 0.89 Nmm-2, and 0.463 ± 0.053 Nmm-2, respectively. The moisture content, mass density, water absorption, and dimensional stability were 6.93 ± 0.27%, 0.962 ± 0.037 gcm-3, 22.36 ± 2.47%, and 10.90 ± 0.86%, respectively. The results revealed that the mass attenuation coefficients and effective atomic number values within the 16.59-25.26 keV photon energy region, were close to the calculated XCOM values in water, with a p-value of 0.077. Moreover, the CT images showed that the dissimilarities in the discrepancy of the profile density decreased as the IA-PAE concentrations increased. Therefore, these results support the appropriateness of the SPI-based/NaOH/IA-PAE/Rhizophora spp. particleboard with 15 wt% IA-PAE adhesive as a suitable tissue-equivalent phantom material for medical health applications.
