Multiple Angiomyolipoma of the Liver - Report of a Case with Diagnosis by Fine Needle Aspiration Cytology - (간의 다발성 맥관근지방종 - 세침흡인 세포학적 검사로 진단한 1예 보고 -)

  • Angiomyolipoma(AML) of the liver is a rare benign tumor; about 50 cases, almost solitary, have been reported. We present here a extremely rare case of multiple AML in the liver diagnosed by fine needle aspiration cytology(FNAC). Two large masses were found in a 51 year-old woman complaining of abdominal discomfort by computed tomography and several smaller masses are scattered in the liver. FNAC was performed, showing bundles of spindle shaped smooth muscle cells intermingled with mature fat cells. FNAC may be a valuable method for definitive diagnosis of hepatic AML. The diagnosis was further confirmed by histologic examination with immunohistochemical studies.

  • Leiomyomas are common in the myometrial layer of the uterus, though they are rarely found in other reproductive organs. Leiomyomas are benign, hormone-sensitive smooth muscle tumors. They develop during the reproductive years and regress after menopause. Preoperative diagnosis is difficult to make. We present a case of a 54-year-old woman with a vaginal wall tumor. The management in this case included surgical excision. Histologic examination definitively confirmed the diagnosis of a vaginal leiomyoma. We report the details of this case along with a brief review of the literature.

  • Angioleiomyoma of the sinonasal tract is a rare benign tumor. We report a case of angioleiomyoma of the nasal septum in a 51-year-old woman who complained of frequent epistaxis for 3 months. Surgicalexcision was performed. The excised specimen was $0.7{\times}0.5{\times}0.4cm$ in size, well circumscribed, grayish white, rubbery, and soft. Histological examination showed thick-walled blood vessels and smooth muscle cell proliferation. No nuclear atypia or mitoses were present.

  • Intravenous leiomyomatosis of the uterus is a rare tumor that presents with benign histological features. It is characterized by intravenous proliferation of smooth muscle cells originating from the uterus that sometimes extends to the inferior vena cava and the right heart. It may spread elsewhere, usually to the lung. The first case of intravenous leiomyomatosis was described in 1896 by Birsh-Hirshfeld, and only a few cases have been reported since then. Its pathogenesis and optimal treatment have not yet been established. We report a case of metastasizing leiomyomatosis found to have multiple nodular densities in both lower lung fields seven months after myomectomy. In another case the leiomyomatosis was confined to the pelvis after a laparoscopy assisted vaginal hysterectomy, the patient is alive without evidence of disease. Here we provide a detailed report of two cases of intravenous leiomyomatosis of the uterus with a brief review of the literature.

  • Intravenous leiomyomatosis is a rare disease entity of benign smooth muscle invading into the lumen of veins. We describe a case of intravenous leiomyomatosis originating from the uterus, growing in the inferior vena cava, and extending into the right ventricle association with multiple pulmonary metastasis. A 53-year-old woman with chest discomfort and several times attacks of syncope was treated at our hospital. The tumor was successfully removed with moderate hypothermic cardiopulmonary bypass after total hysterectomy with a bilateral salphingo-oophorectomy, and multiple pulmonary metastasis under simultaneous sternotomy and laparotomy was confirmed.

Leiomyoma of the Uterine Cervix in Two Dogs (개에서 발생한 자궁경부 평활근종)

  • Masses of the uterine cervix were detected from two dogs, being a 2-year-old female Yorkshire tarrier and a 12-year-old female Basset Hound. Grossly, the masses measuring 3.O$\times$3.O$\times$4.0 cm and 12.O$\times$l2.O$\times$10.07m were prominent, sharply circumscribed, fleshy tumors in the uterine cervix. Histologically, the masses consisted of smooth muscle cells interwoven in bundles, some of which were cut longitudinally (elongated nuclei) and others transversely. Tumor cell nuclei were ordinarily cigar shaped and had rounded blunt ends in the longitudinal plane. There were low mitotic figures without abnormal ones. From these results, these cases were diagnosed as leiomyoma of the uterine cervix. To our Knowledge, no similar tumors have been reported in the uterine cervix.

Liposarcoma in the lung of a poodle dog

  • A 15-year-old intact female poodle dog was referred to a local animal clinic showing signs of dyspnea. A radiographic examination revealed multiple nodules in the lung. The following day, the animal died and a necropsy examination revealed multiple nodular masses of varying sizes in the lung. Microscopically, the tumor cells were composed of round to polygonal cells resembling adipocytes with little or no collagenous stroma. Most of the cells contained clear cytoplasmic vacuoles with the nucleus at the periphery while the other cells contained varying numbers of smaller vacuoles. The immunohisto-chemical evaluation yielded a positive reaction to S-100 and vimentin. Negative results were obtained for pancytokeratin, smooth muscle actin, desmin, epithelial membrane antigen and CD68. This case was diagnosed as a well-differentiated liposarcoma.

  • Esophageal duplication constitute about 10% of all the mediastinal tumor and relatively rare condition. We have experienced one case of esophageal duplication which was found 2 years previously by radiologic study of chest, as mediastinal mass, in 37 years old male. He had neither clinical manifestations nor physical findings leading to the surgical discovery of the duplication. During the last 2 years, the size k location of the mass were stationary in character. Operative therapy of complete excision performed without surgical complication. On microscopic study, the lining cell of inner wall of cyst. Noted pseudostratified ciliated columnar epithelium with smooth muscle.

Angioleiomyoma masquerading as shoulder pain

  • Angioleiomyoma is a benign soft tissue tumor originating from vascular smooth muscle. We report a case of a 20-year-old student who presented with pain in the right shoulder of 4 years duration. Shoulder movements were pain-free throughout the range of motion except resisted external rotation. Magnetic resonance imaging visualized a well-circumscribed lesion over the infraspinatus tendon. The lesion was surgically removed and sent for histopathological analysis. Morphology and immunohistochemistry results were suggestive of angioleiomyoma. The most common location for such a lesion is the lower limb, with less than 1% being reported in the upper arm, of which an angioleiomyoma of the shoulder is extremely rare.
