뇌종양 환자의 양성자 치료 시 Range Compensator의 Smooth Thickness 적용에 따른 선량비교 (Dose comparison according to Smooth Thickness application of Range compensator during proton therapy for brain tumor patient)

  • 목 적 : 양성자 치료 시 사용되는 Range Compensator는 Target의 Distal Margin의 선량에 대해 정상조직에 전달되는 양성자 빔 선량을 보정하는 역할을 한다. 이에 뇌종양 치료에 사용되는 Range Compensator의 Smooth Thickness를 다르게 적용함에 따른 PTV와 OAR의 선량을 비교하여 대상 부위의 선량이 개선되는 것을 확인해 보고자 한다. 대상 및 방법 : 본원에서 양성자 치료를 받은 뇌종양 환자 10명을 대상으로 Eclipse Proton Planning System(Version 10.0, Varian, USA)의 Compensator Editor를 사용하여 Range Compensator에 적용되는 Smooth Thickness를 각각 1회에서 5회까지 순차적으로 적용하였다. 치료계획의 알고리즘은 Proton Convolution Superposition(version 8.1.20 or 10.0.28)을 사용하였고, Smooth Thickness를 단계적으로 적용함에 따른 PTV의 Dmax, Dmin, Homogeneity Index, Conformity Index 그리고 종양주위의 OAR 선량을 비교하였다. 결 과 : Smooth Thickness를 1회에서 5회까지 적용하였을 때 PTV의 최대선량(Dmax)은 최대 4.3%, 최소 0.8%, 평균 1.81% 감소하였으며, 최소선량(Dmin)은 최대 1.8%, 최소 0.2%, 평균 0.82% 증가하였고, 최대선량과 최소선량의 차이는 최대 5.9%, 최소 1.4%, 평균 2.63% 감소하였다. Homogeneity Index는 평균 0.018 감소하였고 Conformity Index는 거의 변화가 없었다. OAR 선량은 Brain Stem에서 최대 1.6%, 최소 0.1%, 평균 0.59% 감소하였으며, Optic Chiasm에서 최대 1.3%, 최소 0.3%, 평균 0.45% 감소하였으나, C와 E환자가 각각 0.3%, 0.6% 증가하였다. 그리고 Rt. Optic Nerve에서 최대 1.5%, 최소 0.3%, 평균 0.8% 감소하였으나, B환자가 0.1% 증가하였다. Lt. Optic Nerve에서는 최대 1.8%, 최소 0.3%, 평균 0.67% 감소하였으나, H환자가 0.4% 증가하였다. 결 론 : 뇌종양 환자의 양성자 치료에 사용되는 Range Compensator의 Smooth Thickness가 단계적으로 적용될수록 Compensator의 해상도가 증가하여 가장 최적화된 양성자 빔 선량을 전달할 수 있다. 이는 PTV에 좀 더 균일한 선량을 조사할 수 있고 또한 OAR에 작용하는 불필요한 선량을 감소시켜 부작용을 줄일 수 있을 것으로 사료된다.

An Upper Bound on the Index of the Smoothest Density With Given Moments

  • For finite discrete distributions with prescribed moments, there is a well-known upper bound on the index of the support. In this paper, we are interested in the smoothest density with prescribed moments among the class of smooth functions. We define an index of continuous distribution through the support and derive an upper bound on the index of the smoothest density. Some examples are given, some of which achieve the upper bound.

Circumferential Alignment of Vascular Smooth Muscle Cells in a Cylindrical Microchannel

  • We report the circumferential alignment of human aortic smooth muscle cells (HASMCs) in an orthogonally micropatterned circular microfluidic channel to form an in vivo-like smooth muscle cell layer. To realize a biomimetic smooth muscle cell layer which is aligned perpendicular to the axis of blood vessel, we first fabricated a half-circular polydimethylsiloxane (PDMS) microchannel by soft lithography using a convex PDMS mold. The orthogonally micro wrinkle patterns were generated inside the half-circular microchannel by stretching-releasing operation under UV irradiation. Upon UV treatment with uniaxial 40 % stretch of a PDMS substrate and releasing process, the microwrinkle patterns perpendicular to the axial direction of the circular microchannel were generated, which could guide the circumferential alignment of HASMCs successfully during cultivation. The analysis of orientation angle, shape index, and contractile protein marker expression indicates that the cultured HASMCs revealed the in vivo-like cell phenotype. Finally, we produced circular microchannels by bonding two half-circular microchannels, and cultured the HASMCs circumferentially with high alignment and viability for 5 days. These results are the first demonstration for constructing an in vivo-like 3D smooth muscle cell layer in the circular microfluidic channel which can provide novel bioassay platforms for in-depth study of HASMC biology and vascular function.

Evaluating the effective spectral seismic amplification factor on a probabilistic basis

  • All contemporary seismic Codes have adopted smooth design acceleration response spectra, which have derived by statistical analysis of many elastic response spectra of natural accelerograms. The above smooth design spectra are characterized by two main branches, an horizontal branch that is 2.5 times higher than the peak ground acceleration, and a declining parabolic branch. According to Eurocode EN/1998, the period range of the horizontal, flat branch is extended from 0.1 s, for rock soils, up to 0.8 s for softer ones. However, from many natural recorded accelerograms of important earthquakes, the real spectral amplification factor appears to be much higher than 2.5 and this means that the spectrum leads to an unsafe seismic design of the structures. This point is an issue open to question and it is the object of the present study. In the present paper, the spectral amplification factor of the smooth design acceleration spectra is re-calculated on the grounds of a known "reliability index" for a desired probability of exceedance. As a pilot scheme, the seismic area of Greece is chosen, as it is the most seismically hazardous area in Europe. The accelerograms of the 82 most important earthquakes, which have occurred in Greece during the last 38 years, are used. The soil categories are taken into account according to EN/1998. The results that have been concluded from these data are compared with the results obtained from other strong earthquakes reported in the World literature.

Sharing a Large Secret Image Using Meaningful Shadows Based on VQ and Inpainting

  • This paper proposes a novel progressive secret image-hiding scheme based on the inpainting technique, the vector quantization technique (VQ) and the exploiting modification direction (EMD) technique. The proposed scheme first divides the secret image into non-overlapping blocks and categorizes the blocks into two groups: complex and smooth. The blocks in the complex group are compressed by VQ with PCA sorted codebook to obtain the VQ index table. Instead of embedding the original secret image, the proposed method progressively embeds the VQ index table into the cover images by using the EMD technique. After the receiver recovers the complex parts of the secret image by decoding the VQ index table from the shadow images, the smooth parts can be reconstructed by using the inpainting technique based on the content of the complex parts. The experimental results demonstrate that the proposed scheme not only has the advantage of progressive data hiding, which involves more shadow images joining to recover the secret image so as to produce a higher quality steganography image, but also can achieve high hiding capacity with acceptable recovered image quality.

두경부 세기견조방사선치료계획 최적화 조건에서 주요 인자들의 영향 분석 (Analysis of the major factors of influence on the conditions of the Intensity Modulated Radiation Therapy planning optimization in Head and Neck)

  • 목 적 : 최적화 알고리즘에 적용되는 최적화 인자들의 영향을 고려하여, 가장 적합한 인자 값을 도출함으로써 이상적인 치료계획을 쉽게 설계할 수 있도록 하고자 한다. 대상 및 방법 : 본 연구의 세기조절방사선치료에서 선량계산 알고리즘은 PBC(Pencil Beam Convolution)이고, 최적화 알고리즘은 DVO(Dose Volume Optimizer 10.0.28)이다. 두경부 환자의 세기조절방사선치료에서 치료계획용적의 처방선량은 동시에 2.2 Gy와 2.0 Gy가 될 수 있도록 하였다. 치료계획은 6 MV, 7개의 조사야로 역선량계산방법으로 수립하였다. 최적화 알고리즘 인자는 용적선량-조건강도(Priority, Constrain), 선량부 드럼강도(Smooth)로 선정하고, 각 인자들의 변화량에 따른 치료계획의 영향을 분석하였다. 용적선량-조건강도는 기준 조건강도를 정하고, 비율은 같지만 절대 값은 다른 최적화 과정을 실시하였다. 또한 조건강도의 절대 값에 변화에 따른 치료용적과 주변 정상장기들을 평가하였다. 선량부드럼강도는 기준 조건의 단순 변화와 용적선량-조건강도와 관련시킨 변화를 치료계획에 반영시켰다. 치료계획은 처방선량지수(Conformal Index, CI), 처방선량포함지수(Paddick's Conformal Index, PCI), 선량균질지수(Homogeneity Index, HI)와 각 장기의 평균선량으로 평가하였다. 결 과 : 용적선량-조건강도의 비율을 동일하게 하고 절대 값을 변화 시켰을 때 CI값은 다르지만, PCI는 $1.299{\pm}0.006$, HI는 $1.095{\pm}0.004$, D5%/D95%는 $1.090{\pm}1.011$으로 처방선량에 대한 영향은 유사하였다. 이하선의 평균선량은 용적선량-조건강도의 절대 값이 40, 60, 70, 90으로 증가될 때, 67.4, 50.3, 51.2, 47.1 Gy로 감소하였다. 각각의 치료계획에서 선량부드럼강도를 증가시켰을 때, PCI는 $1.338{\pm}0.006$로 증가된 값을 보였다. 결 론 : 용적선량-조건강도는 절대적인 값보다 각 조건의 비율에 따라 최적화 알고리즘에 영향을 주었다. 절대 값이 다르더라도 같은 비율을 유지하면 유사한 치료계획이 수립되었다. 성공적인 치료계획을 수립하기 위해 특히 보호해야할 정상장기의 용적선량-조건강도는 치료용적의 용적선량-조건강도의 50%이상 되어야한다. 선량부드럼강도는 용적선량-조건강도에 따라 비례하여 증가하거나 감소하여야 한다. 단순히 절대 값으로 적용하면 용적선량-조건강도는 그 조건을 충분히 만족시키지 못한다.

Cutaneous smooth muscle tumors in 3 dogs

  • Cutaneous leiomyomas (leiomyosarcomas) are smooth muscle tumors that occur single or as multiple lesions. They usually arise from the arrector pili muscles (piloleiomyomas) and less commonly from the muscle of veins (angioleiomyomas). This report describes histologic and immunohistochemical features of one cutaneous piloleiomyoma and two angioleiomyosarcomas. Three 7-12-year-old female dogs were presented with single or double cutaneous nodules. Histologically, the neoplastic masses were composed of densely or loosely arranged interlacing bundles. The neoplastic cells were ovoid to elongate, and had eosinophilic cytoplasms and perinuclear cytoplasmic vacuolation. Nuclei were central to eccentric, cigar shaped, oval to elongate. In two cases, high mitotic index in high power field, multifocal necrosis and local invasion were also noted. Masson's trichrome and van Gieson staining revealed muscle origin tumors in these cases. Immunohistochemically, the tumor cells were strongly positive for smooth muscle actin. In our best knowledge, this is the first report of cutaneous smooth muscle tumors in dogs in Korea.

A Comparison of Smooth and Microtextured Breast Implants in Breast Augmentation: A Retrospective Study

  • Background The number of cosmetic and reconstructive surgeries that use breast implants is increasing in Korea. Recently, it has been reported that breast implant-associated anaplastic large-cell lymphoma is related to textured breast implants, and interest in classification according to the texture of breast implants is increasing. However, there is currently no clear and unified classification. In particular, the definition of "microtextured" is highly varied. In this study, we retrospectively investigated and analyzed the clinical outcomes of smooth and microtextured breast implants. Methods A retrospective chart review of all patients who underwent breast augmentation surgery with smooth and microtextured silicone gel implants between January 2016 and July 2020 was performed. We retrospectively analyzed implant manufacturer, age, body mass index (BMI), smoking status, incision location, implant size, follow-up period, complications, and reoperation rate. Results A total of 266 patients underwent breast augmentation surgery, of which 181 used smooth silicone gel implants and 85 used microtextured silicone gel implants. Age, BMI, smoking status, implant size, and follow-up period were not significantly different between the two groups. Similarly, complications and reoperation rates were not significantly different between the two groups. Conclusion It is important to provide information regarding the clinical risks and benefits of breast implants to surgeons and patients through a clear and unified classification according to the texture of the breast implant.

Nanocomposite 이용한 NFR Media 커버층 특성연구 (Nanocomposite Cover-layer for NFR Media)

  • [ $TiO_2$ ] nanoparticles are added into UV curable resin to increase the refractive index of the cover-layer which is laminated on the media for cover-layer incident NFR. High refractive index is required for the cover-layer operating for the solid immersion lens optics with high effective numerical aperture. The eyepattern could be achieved from the cover-layer coated 20 GB ROM disc in which the refractive index of the cover-layer was 1.75, but the gap servo was unstable due to the rough surface of the cover-layer. Even though the light loss due to the nanoparticles is not serious, the rough microstructure is developed by adding the nanoparticles in the organic binder material. To achieve smooth surface for the stable gap servo, some special techniques should be added, for example the solubility of the nanoparticles should be enhanced by the optimization of the surface of the nanoparticles.

Rat의 자궁벽에 발생된 stromal sarcoma의 관찰 (Observations of a endometrial stromal sarcoma in the rat uterus wall)

  • The present observation was focussed mainly on the histopathological and immunohistochemical finding of the tumor developing from the outside of rat uterus wall. In gross finding, this tumor were a oval form mass and 0.75cm and 1cm in greatest demension. The sectioned surface showed uniform, solid, gray-white. In histopathological finding, the tumor showed the form of lymphoid nodule-like structure and was composed of the cells with karryorrhexis, karyolysis and necrosis in the central zone, mature cells in the paracentral zone, and concentric layers of compressed, flattened and atrophied smooth muscle fibers by the proliferating of the cells closely resembling endometrial stromal ells in the peripheral zone and a few smaller nodule-like structure in certain areas of the peripheral zone. In immunohistochemical observations, the proliferating cells presents only among the concentric layers of the smooth muscle fibers in the peripheral zone. Labeling index of proliferating cells in the peripheral zone was $4.34{\pm}1.28%$. These tumor cells did not metastasized in another orgns and labeling index was low, So it was concluded that this tumor was benign stomal sarcoma.
