  • Objectives: Estimating the number of deaths caused by smoking is crucial for developing and evaluating tobacco control and smoking cessation policies. This study aimed to determine smoking-attributable mortality (SAM) in Korea in 2020. Methods: Four large-scale cohorts from Korea were analyzed. A Cox proportional-hazards model was used to determine the hazard ratios (HRs) of smoking-related death. By conducting a meta-analysis of these HRs, the pooled HRs of smoking-related death for 41 diseases were estimated. Population-attributable fractions (PAFs) were calculated based on the smoking prevalence for 1995 in conjunction with the pooled HRs. Subsequently, SAM was derived using the PAF and the number of deaths recorded for each disease in 2020. Results: The pooled HR for all-cause mortality attributable to smoking was 1.73 for current men smokers (95% confidence interval [CI], 1.53 to 1.95) and 1.63 for current women smokers (95% CI, 1.37 to 1.94). Smoking accounted for 33.2% of all-cause deaths in men and 4.6% in women. Additionally, it was a factor in 71.8% of men lung cancer deaths and 11.9% of women lung cancer deaths. In 2020, smoking was responsible for 53 930 men deaths and 6283 women deaths, totaling 60 213 deaths. Conclusions: Cigarette smoking was responsible for a significant number of deaths in Korea in 2020. Monitoring the impact and societal burden of smoking is essential for effective tobacco control and harm prevention policies.

  • Purpose: This study was carried out to investigate the prevalence and the main factors influencing the farmer's Syndrome in farmers in Korea. Method: A survey questionnaire was completed by 238 farmers who lived in 8 agricultural districts in Seosan City. The data were collected from April 10 to May 5, 2002. Results: The prevalence of the farmer's Syndrome was 42.6%, and its prevalence rate in woman was higher than that in man. The prevalence of the farmers syndrome proportionally increased with advanced age. Variables that were positively correlated with the farmer's syndrome were age, duration of farming, days of resting, and the number of the agricultural chemical's poisoning. On the other hand, variables that were negatively correlated with the farmers syndrome were smoking, sleeping hours, economic condition, and level of educational. In a regression analysis, the factors influencing the farmers syndrome were age, sex, sleeping hours, days of resting, and the number of the agricultural chemical's poisoning, which accounted for 25% of the total variance. Conclusion: Based on these study results, older age and woman gender are the remarkable risk factors influencing the farmer's syndrome. Sleeping hours, days of resting, and the number of the agricultural chemical's poisoning were also the significant attributable variables to the farmers syndrome.

  • Purpose: This study examined the prevalence and risk factors of peri-implant disease after at least 7 years of dental implant loading. Methods: A total of 111 patients with 218 dental implants were treated. The follow-up period for all implants was at least 7 years. The patients' dental records were collected and risk factors of peri-implant disease were investigated through logistic regression analysis. Results: The overall implant survival rate was 95.87%, because 9 of the 218 implants failed. The prevalence of peri-implant mucositis and peri-implantitis was 39.7% and 16.7%, respectively. As risk factors, smoking and prosthetic splinting showed significant associations with peri-implantitis (P<0.05). Conclusions: Within the limits of this study, no significant correlations were found between any risk factors and peri-implant mucositis, but a significantly elevated risk of peri-implantitis was observed in patients who smoked or had splinted prostheses in 2 or more implants.

  • This study provides a systematic review of the literature on nipple-sparing mastectomy and necrotic complications in order to estimate the prevalence of necrotic complications and to investigate their significant predictors. A literature search was conducted using the MEDLINE and Ovid databases. A pooled analysis was performed for calculation of the prevalence of nipple-areolar complex (NAC) necrosis, mastectomy flap necrosis, and overall necrotic complications and to evaluate the relationships between necrotic complications and potential risk factors. A total of 44 papers were analyzed. The prevalence of overall necrotic complications was 13.7%, including 7.5% for NAC necrosis and 7.8% for mastectomy flap necrosis. Types of incisions showed significant association with the rates of NAC necrosis and mastectomy flap necrosis. Incisions involving the NAC showed a significantly higher rate of NAC necrosis than those not involving it. The prevalence of NAC necrosis was higher in the autologous tissue reconstruction group than in the prosthesis group. Active smoking and diathermy dissection were significant predictors of both NAC necrosis and mastectomy flap necrosis. The findings of this review suggest that there are several predictors of necrotic complications in nipple-sparing mastectomy. Appropriate patient selection, careful operative planning, and surgical technique refinements may reduce the risk of necrotic complications.

  • PURPOSE: This study examined the prevalence and specific risk factors in males aged 40-49 years with the coexistence of age-related loss of skeletal muscle mass and obesity (CALSMO). METHODS: The current study analyzed the data obtained from a cross-sectional study involving a sample of 1,218 men who resided in the community and fell within the age range of 40 to 49 years. Multiple risk factors were examined: age, height, weight, body mass index, waist circumference, skeletal muscle mass index, smoking and drinking habits, systolic and diastolic blood pressure, fasting glucose levels, and triglyceride and cholesterol levels. All data were analyzed via complex sampling analysis. RESULTS: The coexistence of age-related loss of skeletal muscle mass and obesity in males was 2.94% (95% CI: 2.06-4.17). The clinical risk factors were low height, high weight, body mass index, waist circumference, skeletal muscle index, systolic blood pressure, diastolic blood pressure, and fast glucose (p < .05). CONCLUSION: The study identifies the prevalence and risk factors for CALSMO among adults in the community. These findings contribute to the existing literature on CALSMO and highlight potential risk factors associated with CALSMO development in males aged 40-49 years.

  • Background: Southeast Asia has many people who work in the agriculture sector. Not many stakeholders pay special attention to the health of farmers, even though they are exposed to various types of hazards. One of the most common health complaints among farmers is related to work-related musculoskeletal disorders (WMSDs). This study aims to assess the prevalence of WMSDs and factors associated with them among farmers in Southeast Asia. Methods: A literature search on PubMed, ScienceDirect, Scopus, and EBSCO was conducted. Articles were included if they studied ergonomic problems among farmers from 11 countries in the Southeast Asia region and were published during 2015-2022. The Critical Appraisal Skills Program was used to evaluate the quality of the articles. The search process and retrieval process reflected PRISMA's recommendation. Result: There were 14 studies found from 3 countries that had published articles in PubMed and ScienceDirect, including Thailand [8], Indonesia [4], and Malaysia [2]. The prevalence of WMSDs in Thailand, Indonesia, and Malaysia was 78,31%, 81,27%, and 88,39%, respectively. Common factors associated with WMSDs include age, sex, smoking habits, drinking alcohol habits, working period (years), type of work, awkward position, non-ergonomic equipment, repetitive movements, and lifting heavy loads. Conclusion: The prevalence of WMSDs among farmers in Southeast Asia is considerably high. Effective intervention is essential for reducing the prevalence and protecting workers' health and well-being.

  • Objectives: Smoking is known to be associated with nephropathy in patients with diabetes. The distinct effects of smoking before and after diabetes has been diagnosed, however, are not well characterized. We evaluated the association of cigarette smoking before and after a diagnosis of diabetes with the presence of diabetic nephropathy. Methods: We analyzed data from the 2011-2013 editions of the Korea National Health and Nutrition Examination Survey. A total of 629 male patients diagnosed with diabetes were classified as non-smokers (90 patients), former smokers (225 patients), or continuing smokers (314 patients). A "former smoker" was a patient who smoked only before receiving his diagnosis of diabetes. A "continuing smoker" was a patient who smoked at any time after his diabetes had been diagnosed. Diabetic nephropathy was defined as the presence of albuminuria (spot urine albumin/creatinine ratio ${\geq}30mg/g$) or low estimated glomerular filtration rate ($<60mL/min/1.73m^2$). Multiple logistic regression models were used to assess the independent association after adjusting for age, duration of diabetes, hemoglobin A1c, body mass index, systolic blood pressure, medication for hypertension, and medication for dyslipidemia. Female patients were excluded from the study due to the small proportion of females in the survey who smoked. Results: Compared to non-smokers, continuing smokers had significantly higher odds ratio ([OR], 2.17; 95% confidence interval [CI], 1.23 to 3.83) of suffering from diabetic nephropathy. The corresponding OR (95% CI) for former smokers was 1.26 (0.70 to 2.29). Conclusions: Smoking after diagnosis of diabetes is significantly associated with the presence of diabetic nephropathy in the Korean male population.

  • Objectives : Cotinine, the major metabolite of nicotine, is a useful marker of exposure to tobacco smoke and self-reporting of smoking status is thought not to be reliable. This study aimed to evaluate the agreement between the smoking self-report among adolescents and the urinary cotinine test. Methods : The study subjects were 1226 middle and high school students in Hanam city, who were selected by stratified random sampling. The self-report about smoking behavior was compared with urine cotinine value measured with PBM $AccuSign^{\circledR}fi$ Nicotine(Princeton BioMeditech Corporation, USA). The percentage agreement, kappa and 95% confidence interval(CI) were calculated. Results : The overall percentage agreement was 88.6%, and those for boys, girls, middle school, general school and vocational school students were 87.3%, 90.1%, 93.7%, 85,5%, 90.7%, and 78.4%, respectively. The overall kappa index was 0.46(95% CI=0.39-0.54)for overall, .and those for boys, girls, middle school, general school and vocational school students were 0.56(95% CI=0.48-0.65), 0.20(95% CI=0.07-0.32), 0.21(95% CI=0.09-0.34), 0.55(95% CI=0.47-0.64), 0.42(95% CI=0.33-0.52), and 0.48(95% CI=0.36-0.60), respectively. Conclusion : The percentage agreement was relatively high but the kappa values very low for girls, and middle school students. Though the prevalence bias can be influenced by these results, the self-report was not a sufficient tool for the evaluation of adolescents' smoking status, especially in girls or middle school students.

The subjects were 710 people (325 males and 385 females) in rural area, Young-Duk Gun, Kyungbuk, Korea. The data were collected by medical students trained for this study during 4 days from 31 August, 1996. Questionnaire contents were general social characteristics, period of the agriculture, smoking history, alcohol-drinking history, some psychosomatic symptoms and self-evaluation for health. The results obtained are as follows : 1. Positive rates of the some psychosomatic symptoms and self-evaluation for health were higher in female than in male and were significantly increased as the as increased(p<0.05). 2. Smoking rate was 56.9% in male and 4.9% in female. In positive rates of the some psychosomatic symptoms and self-evaluation for health in male, 'insomnia' was only significantly lower in ex-smoker than in smoker and non-smoker(p<0.05). Other items were significantly not different. 3. Prevalence rate of alcohol drinker was 54.8% in male and 17.1% in female and the alcoholism was 8.3% in male and 0.8% in female. Positive rate of the some psychosomatic symptoms and self-evaluation for health in male were significantly not different. 4. The scores of self-evaluation for health was divided into two groups under two point and over three. And the analysis of multiple logistic regression shows that there were significant relationship with age and sex. This study suggests that some psychosomatic symptoms and self-evaluation for health were significant relationship with age and sex but not smoking and alcohol-drinking.

  • This study examined the status of and factors associated with drinking experience, smoking experience, and internet addiction among elementary school students. Cross-sectional data were collected by questionnaire from 1,061 children among the fifth and sixth grades in public elementary schools in Gyeonggi province. The prevalence of drinking and smoking experience, internet addiction of participants was 22.7%, 2.9%, and 4.2% respectively. Multiple logistic regressions showed that the likelihood of drinking and smoking experience, and internet addiction was commonly higher with those who was a male and had higher aggression level. The likelihood of drinking experience was lower with those who had higher family support, while was higher with those who had higher friend support. That of internet addiction was lower with having higher friend and teacher support. Prevention strategies to prevent drinking, smoking, and internet addiction among elementary school students should consider student's gender, control aggression, and enhance social support in family and school environments.
