Purposes: The purpose of this study was to examine the association between risk behaviors and smoking in Korean adolescents. Methods: This study used data from online survey of youth health behavior in 2017. Data from a total of 54,411 people (27,139 male, 27,272 female) were included in the analysis. chi-square test, simple logistic regression, multiple logistic regression were performed using SAS 9.4. Findings: Multiple logistic regression analysis showed that risk behaviors such as drinking alcohol experience, sexual experience, drug use experience and high caffeine energy drinks intake experience had a significant effect on smoking. Adolescents with drinking experience were more likely to smoking than those who had no experience(OR=8.58, 95% CI: 7.67~9.60). Adolescents with sexual experience were more likely to smoking than those who had no experience(OR=4.47, 95% CI: 3.91~5.11). Adolescents with drug use experience were more likely to smoking than those who had no experience(OR=2.32, 95% CI: 1.63~3.32). Also, adolescents with high-caffeine energy drinks intake experience were more likely to smoking than those who had no experience(OR=1.37, 95% CI: 1.23~1.53). Practical Implications: All the risk behaviors were significantly associated with smoking rates. Results of this study suggest that physicians and health workers in medical institutions and health centers should simultaneously serve education and consultation for the smoking cessation as well as for the prevention of risk behaviors.
Purpose: This study aimed to identify the directionality of the causal relationship and interaction between depression and amount of smoking over time in hardcore smokers using longitudinal descriptive analysis. Methods: Secondary data from the Korean Welfare Panel Study were analyzed using autoregressive cross-lagged modeling. Participants included 342 hardcore smokers who participated in the 8th to 11th waves of the panel study. Results: Analyses revealed that change(s) in depression levels according to time had a significant positive relationship with the total amount of smoking per day (${\beta}=.29$, ${\beta}=.19$, ${\beta}=.17$, p<.001), while change(s) in total amount of smoking per day according to time had a significant positive relationship with depression (${\beta}=.43$, ${\beta}=.50$, ${\beta}=.38$, p<.001). Analysis of the cross-lagged effect between depression and total amount of smoking per day showed that depression at one time point had a significantly positive relationship with the total amount of smoking per day at the next time point (${\beta}=.14$, ${\beta}=.13$, ${\beta}=.13$, p=.021), and that the total amount of smoking per day at one time point had a significant positive relationship with depression at the next time point (${\beta}=.04$, ${\beta}=.04$, ${\beta}=.03$, p=.044). Conclusion: The findings in the present study confirmed a cross-interaction between depression and total amount of smoking per day in hardcore smokers. The present findings could be used to develop appropriate smoking-related interventions.
Background and Objectives: To evaluate the impact of smoking in young adults on the risk of cardiovascular disease (CVD) and the clustering effect of behavioral risk factors such as smoking, obesity, and depression. Methods: A Korean nationwide population-based cohort of a total of 3,280,826 participants aged 20-39 years old who underwent 2 consecutive health examinations were included. They were followed up until the date of CVD (myocardial infarction [MI] or stroke), or December 2018 (median, 6 years). Results: Current smoking, early age of smoking initiation, and smoking intensity were associated with an increased risk of CVD incidence. Even after quitting smoking, the risk of MI was still high in quitters compared with non-smokers. Cigarette smoking, obesity, and depression were independently associated with a 1.3-1.7 times increased risk of CVD, and clustering of 2 or more of these behavioral risk factors was associated with a 2-3 times increased risk of CVD in young adults. Conclusions: In young adults, cigarette smoking was associated with the risk of CVD, and the clustering of 2 or more behavioral risk factors showed an additive risk of CVD.
Background: The purpose of this study was to develop a structured and individualized smoking prevention program for adolescents by utilizing a multimedia computer-assisted instruction model and to empirically assess its effect. Method: For the purpose of this study, a guide book of smoking prevention program for middle and high school students was developed as the first step. The contents of this book were summarized and developed into an actual multimedia CAI smoking prevention program according to the Gane & Briggs instructional design and Keller's ARCS motivation design models as the second step. At the final step, the short-tenn effects of this program were examined by an experiment. This experiment were made for middle school and high school students and the quasi experimental design was the pretest - intervention - posttest. The measured data was attitude, belief, and knowledge about smoking, interest in the program, and learning motivation. Result: The results of this study were as follows: First, the guide book of a smoking prevention program was developed and the existing literature on adolescent smoking was analyzed to develop the content of the guide book. Then the curriculum was divided into three main domains on tobacco and smoking history, smoking and health, adolescent smoking and each main domain was divided into sub-domains. Second, the contents of the guide book were translated into a multimedia CAI program of smoking prevention througn Powerpoint software according to the instructional design theory. The characteristics of this program were interactive, learner controllable, and structured The program contents consisted of entrance(5.6%), history of tobacco(30%), smoking and health(38.9%), adolescent smoking(22.2%), video(4.7%), and exit(1.6%). Multimedia materials consisted of text(121), sound and music, image(still 84, dynamic 32), and videogram(6). The program took about 40 minutes to complete. Third, the results on analysis of the program effects were as follows: 1) There was significant knowledge increase between the pre-test and post-test with total mean difference 3.44, and the highest increase was in the 1st grade students of high school(p<0.001). 2) There was significant decrease in general belief on smoking between the pre-test and post-test with total mean difference 0.28. In subgroup analysis, the difference was significantly higher in the 1st grade of high school (p<0.001), low income class (p<0.001), and daily smokers (p<0.01). 3) There was no significant difference in attitudes on his personal smoking between the pre-test and post-test. 4) The interest in the program seemed to lower as students got older. The score of motivation toward this prevention program was the highest in the middle school 3rd grade. Among sub-domains of motivation, the confidence score was the highest. Conclusion: To be most effective, the smoking prevention program for adolescents should utilize the most up-to-date and accurate information on smoking, and then instructional material should be developed so that the learners can approach the program with enjoyment. Through this study, a guide book with the most up-to-date information was developed and the multimedia CAI smoking prevention program was also developed based on the guide book. The program showed positive effect on the students' knowledge and belief in smoking.
Background : The past studies on prediction formulas of pulmonary function parameters in healthy nonsmoking Korean adults have been performed in relatively small number of subjects and the reported results were restricted on a few parameters. Also there was no systematic investigation into the effect of smoking on pulmonary function parameters in smokers who have no respiratory symptoms. Therefore we attempted to establish prediction formulas of pulmonary function parameters and examined the effect of smoking on pulmonary function parameters. Methods We analyzed the result of parameters derived from the forced expiratory spirogram in 1,067 nonsmoking subjects from June in 1990 to December in 1991. They consisted of 306 males and 761 females and had neither respitatory symptoms nor history of respiratory disease. We derived prediction formulas by multiple linear regression method from their age, heights, and weights in each sex. To examine the effect of smoking on pulmonary function parameters, we classified 383 smoking men into three groups according to the past amount of smoking as follows : i.e. group of smokers who have smoked below 10 pack-years, 10-20 pack-years and above 20 pack-years. Regarding each group of past smoking as an independent dummy variable, we analyzed pulmonary function parameters including nonsmoking men as a baseline by multiple linear regression. We evaluated the smoking effect on pulmonary function parameters according to estimated p-value. Result : 1) Prediction formulas for pulmonary function parameters in each sex were derived. 2) The past smoking less than 10 pack-years does not give any effect on pulmonary function parameters. The past smoking of 10~20 pack-years showed significant negative correlation with $FEV_1$/FVC and FEF 25~75%, and the smoking above 20 pack years showed negative correlation with $FEV_1$ and $FEV_1$/FVC. Conclusion : We have got prediction formulas of pulmonary function parameters which is driven from forced expiratory spirogram in nonsmoking Korean adults by multiple linear regression from age, heights and weights of subjects. The past smoking more than 10 pack-years showed negative correlation with some pulmonary function parameters of airflow obstruction.
Follow-up surveys with 700 smoking male adults and 300 nonsmoking male adults were performed before 20-days and after one month, three months and six months since government's price increase enforcement. 572 smokers among 700 and 198 non-smokers among 300 were remained and followed up till the end of the surveys. The cessation rate of smokers are 6.6%(after one month), 10.3%(after three months) and 11.0%(after six months). Smoking cessation ratio of new smoking quilters who considered that price increase as a motive of their giving-up smoking are 76.3%, 81.3% and 65.1%. The smokers estimates of short-run price elasticities from follow-up surveys are -0.6853, -0.6230 and -0.5482 at each survey period. Including non-smokers, estimates of short-run price elasticities from follow-up surveys are -0.3920, -0.3739 and -0.3481 at each survey period. The effect of demand decrease caused by KR\500 price increase stayed with little difference for six months because price elasticities between each survey period showed no much change. Effectiveness and validity of tobacco control by price increase was confirmed through the survey results. Therefore if the government want to attain long term strategic goal to decrease general smoking rate among male adult smokers by 30%, the strong smoking prohibition policy, just like the price increase of December 2004, should be continuously driven.

This study was carried out to investigate several effects of smoking and alcohol-drinking on the health of rural residents. The subjects were 710 people (325 males and 385 females) in rural area, Young-Duk Gun, Kyungbuk, Korea. The data were collected by medical students trained for this study during 4 days from 31 August, 1996. Questionnaire contents were general social characteristics, period of the agriculture, smoking history, alcohol-drinking history, some psychosomatic symptoms and self-evaluation for health. The results obtained are as follows : 1. Positive rates of the some psychosomatic symptoms and self-evaluation for health were higher in female than in male and were significantly increased as the as increased(p<0.05). 2. Smoking rate was 56.9% in male and 4.9% in female. In positive rates of the some psychosomatic symptoms and self-evaluation for health in male, 'insomnia' was only significantly lower in ex-smoker than in smoker and non-smoker(p<0.05). Other items were significantly not different. 3. Prevalence rate of alcohol drinker was 54.8% in male and 17.1% in female and the alcoholism was 8.3% in male and 0.8% in female. Positive rate of the some psychosomatic symptoms and self-evaluation for health in male were significantly not different. 4. The scores of self-evaluation for health was divided into two groups under two point and over three. And the analysis of multiple logistic regression shows that there were significant relationship with age and sex. This study suggests that some psychosomatic symptoms and self-evaluation for health were significant relationship with age and sex but not smoking and alcohol-drinking.
Background: The lifetime risk of lung cancer incidence due to radiation for nonsmokers is overestimated because of the use of the average cancer baseline risk among a mixed population, including smokers. In recent years, the generalized multiplicative (GM)-excess relative risk (ERR) model has been developed in the life span study of atomic bomb survivors to consider the joint effect of radiation and smoking. Based on this background, this paper discusses the issues of radiation risk assessment considering smoking in two parts. Materials and Methods: In Part 1, we proposed a simple method of estimating the baseline risk for nonsmokers using current smoking data. We performed sensitivity analysis on baseline risk estimation to discuss the birth cohort effects. In Part 2, we applied the GM-ERR model for Japanese smokers to calculate lifetime attributable risk (LAR). We also performed a sensitivity analysis using other ERR models (e.g., simple additive (SA)-ERR model). Results and Discussion: In Part 1, the lifetime baseline risk from mixed population including smokers to nonsmokers decreased by 54% (44%-60%) for males and 24% (18%-29%) for females. In Part 2, comparison of LAR between SA- and GM-ERR models showed that if the radiation dose was ≤200 mGy or less, the difference between these ERR models was within the standard deviation of LAR due to the uncertainty of smoking information. Conclusion: The use of mixed population for baseline risk assessment overestimates the risk for lung cancer due to low-dose radiation exposure in Japanese males.

Purpose: The purpose of this study was to develop and evaluate a school-based peer leader centered smoking prevention program. Methods: Non-equivalent control group with a pre/post-test design was used. Students (n=174) in two boys' junior high schools located in D city, Korea participated with 85 being selected for the experimental group and 89 for the control group. Five sessions were given to the experimental group and a 50 minute lecture to the control group. Knowledge, attitude, nonsmoking intention, and nonsmoking efficacy were measured for the both experimental and control group at two weeks before the program and one month after the program was completed. Data were analyzed using ${\chi}^2$-test, Fisher's exact test, independent t-test and paired t-test with the SPSS 21.0 program. Results: The experimental group showed higher overall knowledge, negative attitude toward smoking, and higher non-smoking intention and efficacy. After receiving the school based peer leader centered smoking prevention program scores for attitude toward smoking and nonsmoking efficacy increased in the experimental group were higher than in the control group. Conclusion: The school-based peer leader centered smoking prevention program needs longitudinal evaluation, but from this study, there is an indication that this program can be used with junior high school students and effectively change students' attitude toward smoking and promote nonsmoking efficacy.
Background: Many chronic diseases are associated with the lifestyle such as smoking, alcohol drinking and exercise. Attention is increasingly paid on the effect of exercise for the management of chronic disease these days. Objective: The purpose of this study was to evaluate the status of the smoking habit, alcohol drinking and physical exercise of the patients with chronic diseases. Methods: Total 793 persons(normal:422, chronic disease patients:371) in a hospital were questioned regarding the habits of smoking and alcohol drinking, and a practice of physical exercise in 1999. The patients with chronic illness were divided into three groups(Group I included the patients of hypertension and diabetes mellitus. Group II includes respiratory disease and cancer. Group III included liver disease). The rate of exercise, the rate of smoking cessation and the rate of abstinence were assessed. In logistic regression analysis with each independent variables in each disease groups (I, II and III, odds ratio for the presence of disease was controlled for age and education. Results: The rate of exercise, the rate of smoking cessation and the rate of abstinence was 31.5%, 27.3% and 9.5% in the patients with chronic disease, respectively. In control group, those were 31.5%, 21% and 2.1%, respectively. In logistic regression analysis with exercise as independent variable, odds ratios(95% CI for age and education were significantly high, with smoking cessation, odds ratio for age was high and with abstinence, odds ratio for disease with high in all disease groups. Conclusion: It was suggested that an effort for proper changes of lifestyle related to disease such as smoking, alcohol drinking and exercise should be intervened in the patients with chronic diseases.
