Analysis of Korean Nursing Studies Applied for the Transtheoretical Model (Transtheoretical Model을 적용한 국내 간호연구 분석)

  • Purpose: The purpose of this study was to explore the utilization of the Transtheoretical Model(TTM) for Korean nursing studies on health behaviors and identify the strategies for improvement of further studies. Methods: From a main and seven branch journals on the Korean Society of Nursing Science, a total of 26 studies, which had been conducted based upon the TTM, were selected. The selected studies were analyzed from the September of 2006 to the February of 2007 by the criteria; name of journal, year of publication, health behavior, population, sampling, sample size, research design, instrument (translation process, reliability and validity), and statistical analysis. Results: TTM on the Korean nursing studies was mainly applied for smoking cessation or exercise with elderly or middle aged women. The research findings could not be generalized with the limitation of the use of non representative samples. The most frequently used research design was a cross-sectional survey. Some studies had a lack of reliable and valid instruments, so that the internal validity of the study findings may be threatened. Statistically related problems increased type I error and decreased power. Conclusion: Korean nursing studies based on TTM can be the beginning stage in quality and quantity.

Intersectoral Collaboration for Tobacco Policy: Focusing on WHO FCTC (흡연위험요인관리를 위한 부문간 협력: WHO FCTC를 중심으로)

  • Objectives: The purpose of this manuscript was to review Intersectoral Collaboration policies for Tobacco Control. Methods: The author selected the WHO Framework Convention on Tobacco Control and adopted guidelines, and reviewed intersectoral and multisectoral collaboration policy recommendations. Results: There are 11 chapters and 38 articles in the Convention. In the Demand reduction policies included price and non price measures. The author selected a few non price measures for cross sectoral collaboration examples. They are protection from exposure to tobacco emission, education and communication, banning advertising, promotion and sponsorship of tobacco products, and offering treatment to tobacco use cessation. Inter sectoral and multi sectoral approaches could increase effectiveness, and better outcome of the tobacco control policy for implementation of many different articles of FCTC. Conclusions: It is important to give a specific role in structures of different government sectors and infrastructure for intersectoral collaboration. In addition, the role of civil society is very important for implementation of tobacco control policy effectively, and governments have to support the civil society for anti-smoking activities and campaigns.

The Need of Health Education among Chinese Students in Korea (중국인 유학생의 보건교육 요구도)

  • Purpose: This study was conducted in order to survey the needs of health education among Chinese students in Korea. Methods: The subjects of this study were 206 Chinese students in Korea who were attending one of three universities in Chungcheong-do and Seoul. Data were collected from April 1 to October 27, 2011. Results: The subjects' need of health education was 131.53 out of 176. By area, the score was highest in safety accident prevention and emergency care (3.25 out of 4), which was followed by personal hygiene and healthy habits (3.22), family health care management and medical examination (3.15), prevention and management of disease (2.94), environmental health (2.81), moderation in drinking and smoking cessation (2.81), psychiatric and mental health (2.79), and sexual education (2.68). When the need of health education was examined according to the subjects' characteristics, the need of health education was significantly higher in female students. Conclusion: Need of health education among Chinese students studying in Korea was high. To meet Chinese students' need of health education, it is necessary to provide an on-line health education program which is written in bilingual languages (Korean and Chinese) for effective learning.

Population Inquiry Regarding Mammography in Postmenopausal Women in Southern Brazil

  • This study was conducted to analyze the prevalence of non-performance of mammography, and associated factors, among postmenopausal women. This analytical, exploratory, cross-sectional study, of a domicile population inquiry type, was performed in the municipality of Maringa, Parana, Brazil. A total of 456 women were interviewed, aged 45 to 69 years, who presented with natural menopause and cessation of menstruation for at least twelve months. Statistical associations were found between the non-performance of mammography and schooling of less than seven years, paid employment, sedentary lifestyle, smoking, the non-use of hormone replacement therapy, not having consulted a doctor in the previous year, not having consulted a gynecologist, lacking a family history of breast cancer, not having performed the Papanicolaou test, not having performed clinical breast examination, and not having difficult access to health services. After logistic regression analysis, not performing mammography was associated with reports of a fair or bad health status. The study data revealed factors "responsible" for the non-performance of mammography, and the results should contributing to improvement/enhancement of healthy behaviour of Brazilian women in the post-menopausal phase.

Health Promotion Programs in Primary Health Care Posts in Rural Areas and Factors Influencing Service (일 지역 보건진료소 건강증진사업 실태와 관련요인 분석)

  • Purpose: The purpose of this study was to identify health promotion services in rural areas and factors influencing this service. Method: From March to April, 2007, a structured questionnaire on services in 2006 was used to collect data from community health practitioners in all of the Primary Health Care Posts (PHCP) in North Chungchong Province. Collected data were analyzed using SPSS 12.0 Win program. Results: The most frequently offered programs were health gymnastics, walking exercise, bathing and vaccination service, and hypertension management. The main obstacles to these health services were lack of adequate space, insufficient budget, and overwork. The most frequently offered health education programs were education on hypertension, exercise, diabetes, volunteer work, and smoking cessation. The main obstacles to health education were lack of adequate space, insufficient education materials and equipment, and lack of cooperation from the citizens. Improvement and reinforcement of health promotion programs should include support of specialist, development of appropriate methods of service delivery, and education materials, and increase ease in using community resources. Conclusions: The research results show that a new model of health promotion must be developed for efficient health promotion programs in rural PHCP.

Pattern of Tobacco Use and its Correlates among Older Adults in India

  • Purpose: We examined tobacco use pattern and its correlates among older adults. Materials and Methods: We used data of 9,852 older adults (${\geq}60$ years) (men 47% mean age 68 years) collected by the United Nations Population Fund on Ageing from seven Indian states. Logistic regression analysis was used to assess the correlates of tobacco use. Results: Current use of any form of tobacco was reported by 27.8% (men 37.9%, women 18.8%); 9.2% reported only smoking tobacco, 16.9% smokeless tobacco only and 1.7% used both forms. Alcohol users (OR:5.20, 95% CI:4.06-6.66), men (OR:2.92, CI :2.71-3.47), those reporting lower income (OR:2.74, CI:2.16-3.46), rural residents (OR 1.34, CI 1.17-1.54) and lower castes (OR:1.29, CI:1.13-1.47) were more likely to use any form of tobacco compared to their counterparts. Conclusions: Tobacco cessation interventions are warranted in this population focusing on alcohol users, men, those from lower income, rural residents and those belonging to a lower caste.

Management of Diabetic Foot Ulcer (당뇨병성 족부 궤양의 치료)

  • In patients with diabetic foot, ulceration and amputation are the most serious consequences and can lead to morbidity and disability. Peripheral arterial sclerosis, peripheral neuropathy, and foot deformities are major causes of foot problems. Foot deformities, following autonomic and motor neuropathy, lead to development of over-pressured focal lesions causing the diabetic foot to be easily injured within the shoe while walking. Wound healing in these patients can be difficult due to impaired phagocytic activity, malnutrition, and ischemia. Correction of deformity or shoe modification to relieve the pressure of over-pressured points is necessary for ulcer management. Application of selective dressings that allow a moist environment following complete debridement of the necrotic tissue is mandatory. In the case of a large soft tissue defect, performance of a wound coverage procedure by either a distant flap operation or a skin graft is necessary. Patients with a Charcot joint should be stabilized and consolidated into a plantigrade foot. The bony prominence of a Charcot foot can be corrected by a bumpectomy in order to prevent ulceration. The most effective management of the diabetic foot is ulcer prevention: controlling blood sugar levels and neuropathic pain, smoking cessation, stretching exercises, frequent examination of the foot, and appropriate education regarding footwear.

Influence of Aging on Activities of Health Promotion Behavior in Elderly Men (연령 증가가 남성 고령자의 건강증진행위 실천에 미치는 영향)

  • Objectives : This study aimed to evaluate the factors influencing health promotion behaviors in elderly men. Methods : We used data from the 6th Korea National Health and Nutrition Examination Survey. We analyzed difference between sociodemographic characters, health statuses, and health promotion behaviors by age. Logistic regression analysis was used to investigate the relation between health promotion behaviors and sociodemographic factors by age. Results : Health promotion behaviors related to aging were weight control effort, health screen examination, smoking cessation, and influenza vaccination. Socioeconomic statuses of elderly men declined from the preliminary age, and health status was influenced by the reduction of social role. Health promotion behaviors such as weight control, health screen examination, and performance of aerobic activities were decreased in men of advanced aged. Conclusions : The health promotion behaviors of elderly men differed significantly with age.

  • This paper presents proportional odds cure models to allow spatial correlations by including spatial frailty in the interval censored data setting. Parametric cure rate models with independent and dependent spatial frailties are proposed and compared. Our approach enables different underlying activation mechanisms that lead to the event of interest; in addition, the number of competing causes which may be responsible for the occurrence of the event of interest follows a Geometric distribution. Markov chain Monte Carlo method is used in a Bayesian framework for inferential purposes. For model comparison some Bayesian criteria were used. An influence diagnostic analysis was conducted to detect possible influential or extreme observations that may cause distortions on the results of the analysis. Finally, the proposed models are applied for the analysis of a real data set on smoking cessation. The results of the application show that the parametric cure model with frailties under the first activation scheme has better findings.

Analysis of Worksit Health Promotion Programs (우리나라 사업장의 건강증진 프로그램 유형 분석 - 사업장 건강증진운동 우수사례집을 중심으로 -)

  • Purpose: The purpose of this study is to identify the types of worksite health promotion programs. Method: Data were collected from the excellent 35 cases chosen at the contest for worksite health promotion held by Korea Occupational Safety and Health Agency. Result: Out of all the health promotion programs, the exercise program recorded 35.0%, the nutrition program 29.4%, the smoking cessation program 28.0%, and the alcohol reduction program 7.6%. The major element of worksite health promotion programs were awareness raising intervention. Behavior change intervention and supportive environment intervention occupied a small portion of the health promotion programs. Evaluation of health promotion programs was made mainly by indicators of health behavior change and clinical symptom. Yet economical indicator was not used at all. Conclusion: Use of various evaluation indicator and development of various interventions including behavior change and supply of supportive environment are required to encourage worksite health promotion program.
