Factors Related to Increasing Trends in Cigarette Smoking of Adolescent Males in Rural Areas of Korea

  • Objectives: Cigarette smoking prevalence among adolescent males in rural areas of Korea has increased in recent years. The aim of this study was to explore the factors related to increasing trends in cigarette smoking among adolescent males living in rural areas. Methods: The raw data from the Korea Youth Risk Behavior Web-based Survey from 2006 to 2009 were used. Data were analyzed by using the method of complex survey data analysis considering complex sampling design. Multiple logistic regression models were used to explore the factors affecting cigarette smoking. We evaluated the linear time trends in the prevalence of factors that were related to current smoking status and the linear time trends in cigarette smoking in groups stratified by the exposure to each factor using logistic regression models. Finally, we examined the contributions of the factors to the time trends in cigarette smoking by adjusting for each of those factors in the baseline regression models and changes in the adjusted odds ratio by survey year. Results: A statistically significant increasing trend in smoking was observed after adjusting for the factors affecting cigarette smoking. Significant factors related to cigarette use were perceived stress, experience with depression, current alcohol drinking, exposure to secondhand smoke, and academic performance. The factor related to increasing trends in cigarette smoking was academic performance. Conclusions: Stress about academic performance is an important factor affecting the increase in cigarette smoking among adolescent males in a rural area of Korea.

  • Purpose: This study was done to identify the relationship between smoking, drinking and safety behavior in middle and high school students in one Korean province, North Cholla. Method: The study sample included 1,327 randomly selected middle and high school students from S-Gun area in Chonbuk Province. The data were collected using an anonymous questionnaire developed by the Ministry of Health and Welfare (2005). The data were analyzed using the SPSSWIN 15.0 program. Results: The smoking rate in the middle and high school students was 6.9%, and the drinking rate was 24.9%. The rate for seatbelt use was 49.3%, for drunk driving, 8.8%, and for riding in car driven by a drunk driver, 24.0%. The rates for seatbelt use, for drunk driving, and for riding in a drunk driver were higher in smokers and drinkers than in non-smokers and non-drinkers(p<.05). Conclusion: The smoking and drinking status of the adolescents was correlated with their safety behavior practices as they relate to seatbelt use, drunk driving, and for riding in car driven by a drunk driver. Therefore, an elementary school level program to curb student smoking and alcohol consumption is needed to prevent smoking and alcohol consumption in middle and high school students and contribute to improvement in their safety behavior practices.

청소년의 흡연, 음주와 정신건강과의 관계 (The Relationship Between Smoking, Drinking and the Mental Health in Adolescents)

  • Purpose: This study sought to investigate the relationship between smoking, drinking and the mental health of adolescents. Method: The study subjects included 1,092 randomly sampled third-year middle and high school students from N-City and the L-Kun area in Chonbuk Province. The data were collected using an anonymous questionnaire developed by the Ministry of Health and Welfare(2005). The data were analyzed using the SPSSWIN 15.0 Program. Result: The smoking rate in the adolescents was 10.1%, and the drinking rate was 43.2%. The stress recognition rate in normal daily life was 45.5%, the frequency of depressive symptoms was 33.2% and 40.0% of the subjects reported that they wanted to die at some point within the past year. The rate of stress recognition, depressive symptom experience, suicidal planning and attempt in smokers and drinkers was higher than that in non-smokers and non-drinkers (p<.05). Especially, the frequency of suicidal thoughts was higher in the drinkers than in the non-drinkers (p<.05). Conclusion: The smoking and drinking status of the adolescents was correlated with their mental health a sit relates to stress, depression and suicide. Therefore, an effective program for the prohibition of cigarette smoking and alcohol consumption is needed in order to prevent smoking and alcohol consumption in adolescents and improve their mental health status.

일 농촌지역 고등학생의 금연 변화단계 및 변화과정 (Stages and Processes of Smoking Cessation among High School Students in Rural Community)

  • Purpose: The purpose of this study was to describe stage distribution of current and former smokers and to analyze significant variables that used by adolescents at each stage using the Transtheoretical Model. Methods: A cross-sectional study was performed with 145 students attended to five high schools in one rural area. The survey instruments included stages and processes of change. decisional balance, self-efficacy, and knowledge related to smoking. Results: Among the participants, 47.6% were in the precontemplation and contemplation stages, 20.7% were in the preparation stage, and the other 31.7% were in the action and maintenance stages. Analysis of variance tests revealed that behavioral processes, pros and cons of smoking, and self-efficacy were significantly associated with the stages of smoking cessation, while among 10 processes of change, self-reevaluation, self-liberation, and reinforcement management were significantly different by the stages. Conclusions: The study results suggest that smoking cessation programs should emphasize behavioral strategies, self-efficacy, and decisional balance to help students adopt and maintain smoking cessation.

보건소 금연사업 담당자의 직무분석 (Job Analysis of a Staff who Manage Quit-smoking Policy on Health Centers)

  • Objectives: This job analysis of a staff in charge of quit-smoking policy at public health centers aims at providing fundamental information to establish strategies supporting various quit-smoking. Methods: The job analysis of a staff in charge of quit-smoking policy at public health centers was carried out through DACUM(Development of Curriculum) method from April through May 2006. Three experts had developed job description with staffs in charge of quit-smoking policy at public health centers through two workshops. The survey was practiced for staff in charge of quit-smoking policy at the other public health centers. The characteristics of the staffs such as age, years for working at public health center, years for charging with quit-smoking work, the proportion of responsibility for quit-smoking work, were surveyed. Results: The research has reached the conclusions below. 1. The job description have been developed considering input-process-outcome axis and plan-do-evaluation axis for quit-smoking policy at public health centers. The final job description is composed of 3 missions, 7 accountabilities, 20 sub-work items. 2. The quit-smoking activity mostly focused at direct education and counselling. But planing and evaluation activity for quit-smoking have been under-achieved. 3. The staffs for quit-smoking policy were feel it is easy to educate and counsel to comer to public health centers for quit-smoking. But having the high proportion of responsibility for quit-smoking policy have usually difficult to do that. So they want to education about counselling for smoker. 4. The staffs who worked over the 2 years for quit-smoking policy the public health center have responded that investigate the smoking rate of the jurisdiction community and the problem of the culture about smoking and smoking policy is important. Conclusions: The study helps reinforcing the initiatives of central government for quit-smoking policy at public health centers. Especially staffs want education in technology area for counselling smoker. And they want nationwide supporting for investigating smoking rate and related factors at the local level.

흡연갈망의 신경해부학적 특이성:기능자기공명영상연구 (fMRI Investigation on Cue-induced Smoking Craving:A Case Report)

  • Object:Nicotine dependence is the most common substance abuse disorder. One of the characteristics of nicotine dependence is craving. Regional activation of the brain induced by craving for nicotine was evaluated by using functional magnetic resonance imaging to investigate neuroanatomical site of smoking craving. Method:A smoker who satisfied DSM-IV criteria for nicotine dependence and a non smoker was studied. MRI data were acquired on a 1.5T Magnetom Vision Plus with a head volume coil. Two sets of visual stimuli were presented to subjects in a random manner. One was the film scenes of inducing smoking craving and the other was neutral stimuli not related to smoking. There were two fMRI sessions before and after smoking or sham smoking. Data were analyzed using SPM99. Results:fMRI showed significant activated area in anterior cingulate and medial frontal lobes in the smoker during smoking craving. Right dorsolateral prefrontal cortex and parietal lobes were activated in the control during visual stimulation before smoking. After smoking, there was no brain activation during visual stimulation in both of smoker and non smoker. Conclusion:Metabolic activity of the anterior cingulate and medial frontal lobes increased during craving for smoking. This result suggests that fMRI may be a valuable tool in the identification of neurobiological process of craving.

일부지역 여대생들의 흡연경험과 구강건강인식도와의 관련성 (A Survey on Related of College Women's Smoking experience in a Certain Area and their Awareness of Oral Health)

  • This study is designed to take a look at college women's understanding of the actual condition of their smoking and the harmfulness of their smoking, to make an analysis of smoking factors affecting oral health and to present basic materials for the development of oral health education programs. Self-administered questionnaires were given to the subjects from May 22 through June 2, 2003. The results were as follows. 1. 71.7 percent of the subjects said that they had no experience. 16.7 percent of them said that they had had experience. 11.6 percent of them said that they are now smoking. The oftener they don't live with their parents, the more monthly money they had, the more smoking experience they had(p<0.001). 2. 37.4 percent of the subjects said that they brush their teeth three times a day. And 52.2 percent of them said that they brush their teeth after smoking. 3. The rate of using dental floss and interdental brushes accounts for 46.5%, and the ratio of having tartar scaled from their teeth accounts for 45.5%. There was no significant difference in accordance with smoking or nonsmoking. 4. The subjects with no smoking experience(76.0%) thought that they had better teeth condition than their counterparts(64.3%). But there was no significant difference. 5. 56.5 percent of the subjects responded that their dental condition was related to their smoking, but 43.5 percent of them said that it was not.

우리나라 성인의 사회경제적 요인이 건강 수준에 미치는 영향: 7기 국민건강영양조사를 중심으로 (The Effect of Socioeconomic Factors on Health Status Among Korean Adults: Based on the 7th Korean National Health and Nutrition Examination Survey)

  • Purpose: This study identified socioeconomic factors affecting the health status of Korean adults. Methods: Secondary data from 12,921 adults aged 19 to 64 old in the 7th Korean National Health and Nutrition Examination Survey were used. The participants' health status was measured using the indicators that included health behaviors (smoking, high-risk drinking, strength exercise, and aerobic physical activity) and health outcomes (metabolic syndrome, and subjective health status). Results: For all health behaviors and health outcomes, gender, age, educational level, and income were common affecting factors. Regarding health behaviors, the employment status was related to smoking, high-risk drinking, strength exercise, and aerobic physical activity. The marital status was related to high-risk drinking, strength exercise, and aerobic physical activity. The household type was related to smoking. The residential area was related to smoking, high-risk drinking, and aerobic physical activity. For health outcomes, the household type was related to obesity, and subjective health status; residential area was related to obesity. Conclusion: This study presented basic data for assessing the differences in health status. The characteristics of the affecting factors to health status should be considered, depending on the health behaviors and health outcomes.

경산지역 대학생의 흡연, 음주 및 식행동에 관한 조사 (Smoking, Alcohol Consumption and Dietary Behavior of College Students in the Kyungsan Area)

  • Alcohol has as much effect on our lives as the different taste for foods that people have all of the world. Recently, the interest about drinking habits has increased with the rise in health problems for college students with poor health related behavior. The purpose of this study was to evaluate alcohol consumption, smoking and eating behavior of college students in the Kyungsan area. This survey was administered through questionnaires, and the subjects were 177 male and 189 female college students. The self-administered questionnaire was composed of questions concerning social-demographic factors, general characteristics, cigarette smoking, alcohol consumption and eating behavior. The data were analyzed by $\chi$$^2$-test and t-test. According to the survey results, almost of all of the survey subjects drank alcohol. The mean alcohol consumption level per day for male students (33.9${\pm}$29.7g) were significantly higher than for the female students (18.5${\pm}$16.5g), and more than 39.6% of the subjects drank alcohol 1-3 days a week. Alcohol consumption changed the students eating habits. Most students had dietary problems such as fast eating, skipping meal and spending too much money on fast food. The survey about eating behavior of the students showed the male students had more problems when compared with female students. Alcohol consumption levels and alcohol dependence showed a significant positive correlation with smoking cigarettes (r=0.386, p<0.001). The correlation coefficient between alcohol consumption and eating behavior was negatively correlated with skipping meals (r=-0.121, p<0.001). However, there were significant and positive correlations with overeating when students were depressed (r=0.130, p<0.05), with eating meals when watching TV or videos (r=0.085) and with spending money on fast food(r=0.235, p<0.235). The results indicate that health related behaviors of college students in the present study were fairly good. More attention should be given to college students and their habits of skipping meal, alcohol drinking and smoking, and the fairly good students can act as a model for correct dietary behavior so as to improve overall student health.
