Design and Implementation of Real-time ECG Monitoring System for Personal Health Records (개인건강기록을 위한 실시간 심전도 모니터링 시스템 설계 및 구현)

  • In this paper, we propose a real-time ECG monitoring system for personal health records. This study aims to provide services that help patients to monitor their own physical condition and manage their own health records consistently, whereas existing medical services are Medical Institute-Centric model. The system is composed of web server, smart phone, and ECG meter, and web page. Without time and space restraints, It provides us with managing personal health records by performing patient's ECG measurement and real-time monitoring. And also Real-time bidirectional communication between smart phone and web page can be performed rapidly by applying the ECG monitoring with WebSocket Technology that follows HTML5 standard. Through this system, It can handle patient in need immediately.

Utilization of and Satisfaction with Smart Bedside Station System as a Patient-centered Healthcare System (일 병원의 환자중심 지능형 병상 지원(Smart Bedside Station) 시스템의 이용현황 및 사용 만족도)

  • Purpose: The aim of this study was to examine the utilization of and satisfaction with the smart bedside station (SBS) system among users in a hospital. Methods: A cross-sectional descriptive design was used. The participants were 190 patients, 186 family caregivers, and 154 nurses in a hospital. Results: Around 78.1% of patients or family caregivers used the SBS system at least once during their hospital stay. The commonly used items on the SBS system menu were "lab findings", "hospital cost", "today's medication", and the "alarm message". Satisfaction with the SBS system of patients and family caregivers were significantly higher than those of nurses (F=39.88, p<.001). Conclusion: A patient-centered SBS system was a useful system that could increase patient satisfaction and comfort. More specific and technical service contents reflecting the current healthcare system should be added.

How to Sustain Smart Connected Hospital Services: An Experience from a Pilot Project on IoT-Based Healthcare Services

  • Objectives: This paper describes an experience of implementing seamless service trials online and offline by adopting Internet of Things (IoT) technology based on near-field communication (NFC) tags and Bluetooth low-energy (BLE) beacons. The services were provided for both patients and health professionals. Methods: The pilot services were implemented to enhance healthcare service quality, improve patient safety, and provide an effective business process to health professionals in a tertiary hospital in Seoul, Korea. The services to enhance healthcare service quality include healing tours, cancer information/education, psychological assessments, indoor navigation, and exercise volume checking. The services to improve patient safety are monitoring of high-risk inpatients and delivery of real-time health information in emergency situations. In addition, the services to provide an effective business process to health professionals include surveys and web services for patient management. Results: Considering the sustainability of the pilot services, we decided to pause navigation and patient monitoring services until the interference problem could be completely resolved because beacon signal interference significantly influences the quality of services. On the other hand, we had to continue to provide new wearable beacons to high-risk patients because of hygiene issues, so the cost increased over time and was much higher than expected. Conclusions: To make the smart connected hospital services sustainable, technical feasibility (e.g., beacon signal interference), economic feasibility (e.g., continuous provision of new necklace beacons), and organizational commitment and support (e.g., renewal of new alternative medical devices and infrastructure) are required.

Application of Deep Recurrent Q Network with Dueling Architecture for Optimal Sepsis Treatment Policy

  • Sepsis is one of the leading causes of mortality globally, and it costs billions of dollars annually. However, treating septic patients is currently highly challenging, and more research is needed into a general treatment method for sepsis. Therefore, in this work, we propose a reinforcement learning method for learning the optimal treatment strategies for septic patients. We model the patient physiological time series data as the input for a deep recurrent Q-network that learns reliable treatment policies. We evaluate our model using an off-policy evaluation method, and the experimental results indicate that it outperforms the physicians' policy, reducing patient mortality up to 3.04%. Thus, our model can be used as a tool to reduce patient mortality by supporting clinicians in making dynamic decisions.

Development of Authentication Service Model Based Context-Awareness for Accessing Patient's Medical Information (환자 의료정보 접근을 위한 상황인식 기반의 인증서비스 모델 개발)

  • With the recent establishment of a ubiquitous-based medical and healthcare environment, the medical information system for obtaining situation information from various sensors is increasing. In the medical information system environment based on context-awareness, the patient situation can be determined as normal or emergency using situational information. In addition, medical staff can easily access patient information after simple user authentication using ID and Password through applications on smart devices. However, these services of authentication and patient information access are staff-oriented systems and do not fully consider the ubiquitous-based healthcare information system environment. In this paper, we present a authentication service model based context-awareness system for providing situational information-driven authentication services to users who access medical information, and implemented proposed system. The authentication service model based context-awareness system is a service that recognizes patient situations through sensors and the authentication and authorization of medical staff proceed differently according to patient situations. It was implemented using wearables, biometric data measurement modules, camera sensors, etc. to configure various situational information measurement environments. If the patient situation was emergency situation, the medical information server sent an emergency message to the smart device of the medical staff, and the medical staff that received the emergency message tried to authenticate using the application of the smart device to access the patient information. Once all authentication was completed, medical staff will be given access to high-level medical information and can even checked patient medical information that could not be seen under normal situation. The authentication service model based context-awareness system not only fully considered the ubiquitous medical information system environment, but also enhanced patient-centered systematic security and access transparency.

  • Smart-healthcare service is a health medical service which aims to provide an active consumer-directed service for both wellness and preventative health management upon the basis of rapid diffuse of smartphone which has been occurring recently. On this, this paper suggests a tele auscultation medicine smart-healthcare system based on digital stethoscope which is able through the use of the smart phone which is easy to approach anywhere. The suggested system consists of the digital stethoscope for patient's self auscultation and smart phone, the PC for doctor's examination, and the web server to connect those devices. A telemedicine can be performed between a doctor and a patient by the program which exchanges examine data and auscultation data among the devices. To sum up, by the system suggested in this paper, an easily and conveniently usable health medical service on an everyday basis could be provided without spatial and temporal limits.

  • Due to the development of technology and medical care following the 4th industrial revolution, the medical paradigm is shifting towards patient-centered medical services. Based on the development of smart home technology, the residential environment is changing into a residential space that cares for and heals the lifestyles and the healthcare of families. As lifestyle changes, the concept of supporting smart home care based on the residential environment is making it possible to build a smart home IoT service design with enhanced accessibility and convenience for medical appointments and well-being lifestyle care. This paper is a study on user-centered health care smart home IoT service design suitable for family members based on the health care, beauty care, exercise care, and customized diet care beyond the conventional concept of health care monitoring. Based on the analysis, this paper proposes a personal care coordinate smart home service design in a human-centered wellness clinic care smart home service design environment. Human-centered wellness clinic smart home IoT service design is meaningful in presenting a vision for research on smart home service design that links hospital-linked and care-linked service industries, which should be considered from the smart home construction planning stage.

Using Intervention Mapping to Develop a Community-based Disease Self-management Support Program for Patients with Osteoarthritis (Intervention Mapping을 적용한 지역사회기반의 골관절염 자기관리지지 프로그램 개발)

  • Purpose: The purpose of this study was to apply intervention mapping (IM) to develop a community-based disease self-management support program for patients with osteoarthritis. Methods: IM was applied as follows: i) a needs assessment has been carried out by a literature review, survey and interview with osteoarthritis patients; ii) on the basis of the needs assessment, identification of expected outcomes and change objectives for the target population; iii) selection of theory-based methods and practical applications to influence self-management and the determinants of behavior; iv) design of the intervention by developing activities and materials such as osteoarthritis self-management guide and smart patient pocket book. The activities were integrated into an existing healthcare activities; v) implementation and evaluation plan has been developed. Results: The program is aimed at improving health status through activating patients by a patient-centered and tailored intervention for patients with osteoarthritis; consists of 8 sessions with coaching and cognitive emotional psychological skills; includes smart patient, communication, osteoarthritis, medication adherence, pain control, depressive mood control, physical activity and healthy diet. Conclusion: The IM is a systematic and feasible method for developing the program. The next step is to evaluate the impact of the intervention on activation, and health status.

  • In this study, a dementia patient care monitoring system is suggested that uses a wearable type bluetooth beacon to prevent them from going missing. This system shows whether the patients stay within the manageable area and sends a warning message to their monitoring managers' or guardians' smart devices when they leave it. The feature of the system is that it provides the service based on indoor location that makes the beacon worn by dementia patients continuously transmit their location information to the managing server through the smart terminal installed indoors or in hospitals and that enables the monitoring managers or the guardians to receive messages sent from the server that tell the patients' whereabouts through their smart devices. The system suggested in this paper is believed to be a system that effectively contributes to the prevention of the dementia patients' going astray from the hospitals and facilities where they are taking care.

  • The aim of this research is to study and develop enabling technologies for home healthcare device with ubiquitous network. The motivation of this paper is to enable healthcare in home, to development the device for smart home health care. To achieve the aim, we must develop the prototype platform based on home gateways, distributed context user interface based on UPnP and support for information sharing with high speed power line communication and mobile infra-structures. And IPv6 is the base technology of this platform. In this paper, we concern that physical health, mental health and medical emergencies is all of home healthcare. With the smart device, we evaluate the connectivity, automatic information extraction and private data exchange and event driven message. The result of this paper is demonstration of smart device for ubiquitous communication in a healthcare application such as patient monitoring device and several information services. In conclusion, home healthcare will support more healthy and easy living for a human.
