효율적인 환자관리를 위한 링거액 감지기 및 송수신기 설계 (Ringer's solution detector and transceiver design for efficient manage of patient)

  • 본 논문에서는 효율적인 환자관리를 위한 링거액 감지기 및 송수신기를 설계 하고자 한다. 링거액 감지기 및 송수신기는 메인 제어부, 링거액 감지부, 디스플레이 및 경고 발생부, 무선 송수신부, 전원 공급부로 구성된다. 메인 제어부에서는 GC89L591A0-MQ44IP의 CPU를 사용하여 전체 시스템을 제어하고 링거액 감지부는 수광부에 TSL235R-LF 포토다이오드, 투광부에 Water-Clear Type LED를 적용하였다. 또한, 디스플레이 및 경고 발생부는 7-세그먼트와 적색 LED를 적용하였으며 무선 송수신부는 NR-FPCX 모듈을 사용하여 데이터를 송수신하였다. 전원 공급부는 건전지를 사용하며 일반 전원이나 충전식 전원도 사용할 수 있도록 하였다. 따라서, 효율적인 환자관리를 위한 링거액 감지기 및 송수신기 설계에 대한 연구 결과 링거액이 포토다이오드에 의해 감지되었을 때 주파수는 11.95kHz이며, 감지되지 않았을 때는 9.6kHz로 측정되었다. 이때, 링거액의 감지 데이터가 링거액 송신기를 통해 출력되지 않을 때는 "0" 으로, 출력되었을 때는 해당 병실의 호실 정보가 링거액 수신기에 표시됨을 확인 할 수 있었다. 또한, 링거액 감지기의 사용전원은 신호가 감지되지 않을 때에는 Sleep 모드로 전환하여 배터리 절약 모드로 동작하며 링거액 송수신기는 대략 700m이내의 거리에서 무선 송수신이 가능하다.

  • 운동이 신체적, 정신적 건강에 긍정적인 영향을 미친다는 것은 수많은 연구를 통해 일반적인 상식이 되었다. 그리고 규칙적인 운동은 체지방 감소와 혈중지질을 감소시켜 비만을 조절하며, 궁극적으로 인간의 삶의 질을 향상시킨다고 보고되어 있다. 본 연구에서는 선행연구를 통해 수영운동치료 관리를 위한 지표들을 도출하였으며, Fuzzy ANP(Analytic Network Process) 기법을 이용하여 지표들간의 상관관계를 모델링하여 가중치를 측정하였다. 측정된 결과를 가지고, 공간 제약 없이 개인별 맞춤 운동처방을 실시간으로 알려주도록 설계 하였다. 그리고 휴대폰, 스마트폰, 노트북 등 모바일 장치를 통해 실시간으로 질환자별 맞춤 관리가 이루어질 수 있도록 환자관리시스템을 구현하고자 한다.

Remote Monitoring of Patients and Emergency Notification System for U-Healthcare

  • This study proposes a remote monitoring of patients and emergency notification system with a camera and pulse wave sensor for U-Healthcare. The proposed system is a server client model based U-Healthcare system which consists of wireless clients that have micro-controller, embedded-board for patient status monitoring and a remote management server. The remote management server observes the change of pulse wave data individually in real-time sent from the clients that is to be remote-monitored based on the pulse wave data stored by users and divides them into caution section and emergency section. When the pulse wave data of a user enters an emergency situation, the administrator can make a decision based on the real-time image information and pulse rate variability. When the status of the monitored patient enters the emergency section, the proposed U-healthcare system notifies the administrator and relevant institutions. An experiment was conducted to demonstrate the pulse wave recognition of the proposed system.

The Effect of Public Health Center-Based Hypertension School on Hypertension-related Knowledge, Self-efficacy, Anthropometric Value and Blood Pressure

  • This study is to identify the effects of hypertension management program at a community health center on the disease-related knowledge, self-efficacy, anthropometric measurements and blood pressure of a hypertensive patient in local community. This study is a quasi-experimental study using nonequivalence control group no-synchronized design in order to verify the effects of the hypertension management program at a community health center on the hypertension-related knowledge, self-efficacy, anthropometric measurements and blood pressure of a hypertensive patient in local community. The result indicated a significant difference between the experimental group and control group in the scores of hypertension-related knowledge (t=-4.25, p<.001), self-efficacy (t=-4.20, p<.001), systolic blood pressure (t=7.70, p<.001) and diastolic blood(t=5.91, p<.001), body weight(t=2.32, p=.026) and abdominal circumference(t=2.17, p=.036). The hypertensive patients' knowledge and self-efficacy were improved, and their weight and abdominal circumference as well as systolic blood pressure and diastolic blood pressure were reduced. Therefore, it was confirmed that multilateral approaches in terms of physical and psychosocial aspects only targeting hypertensive patients were required for managing hypertensive patients in local community.

Big IoT Healthcare Data Analytics Framework Based on Fog and Cloud Computing

  • Throughout the world, aging populations and doctor shortages have helped drive the increasing demand for smart healthcare systems. Recently, these systems have benefited from the evolution of the Internet of Things (IoT), big data, and machine learning. However, these advances result in the generation of large amounts of data, making healthcare data analysis a major issue. These data have a number of complex properties such as high-dimensionality, irregularity, and sparsity, which makes efficient processing difficult to implement. These challenges are met by big data analytics. In this paper, we propose an innovative analytic framework for big healthcare data that are collected either from IoT wearable devices or from archived patient medical images. The proposed method would efficiently address the data heterogeneity problem using middleware between heterogeneous data sources and MapReduce Hadoop clusters. Furthermore, the proposed framework enables the use of both fog computing and cloud platforms to handle the problems faced through online and offline data processing, data storage, and data classification. Additionally, it guarantees robust and secure knowledge of patient medical data.

A Study on the Safety Perception, Ethical Awareness, and Safety Activities of Nursing Students

  • The purpose of this study is to identify the level of safety perception, ethical awareness, and safety activities of nursing students for patients, and to identify the correlation and impact between them. The research design is a descriptive survey study, and the subject of the study were 197 nursing college students in G City. Safety perception, ethical awareness, and safety activity tools were used for, and the data collection period was from October 17 to 28 in 2022. T-test, one-way ANOVA, Pearson's correlation coefficient, Regression analysis were used to analyze data. The result of the study indicated that the average level of safety perception of nursing students was 3.72 points, the average ethical awareness of patients, professional work, and cooperators perceived by nursing students was 3.04 points, and the safety activities of nursing students were 4.20 points. In the case of safety awareness and ethics awareness, r=.327, a significant positive correlation, in the case of safety awareness and safety activities, r=.399, significant positive correlation, ethics awareness and safety activities as r=.296. And so on these results showed that high safety perception increases safety activities, and high ethical awareness increases safety activities. Therefore, we need practical and step-by-step convergence education to equip nursing students with patient safety nursing capabilities. To this end, a safer environment will be created if the social support network for the systematic application of safety education is well formed.

The Study on Service Design Development for Pharmacy Users: Using Smart Phone Application

  • Objective: This study is to review the current state of services offered to pharmacy users, and develop a service design converging a prescription service and a smart phone application service in order to enhance patients' experience at pharmacies and home. Background: Under the new medical system; separation of prescribing and dispensing drugs, a doctor writes a prescription to their patients and patients have their prescription filled at the pharmacy. As the number of flue, allergy and atopy patients has increased, waiting lines have been longer at pharmacies. Besides, the current medical service system lacks of providing proper information on prescribed pills to patients. There are already services offered during waiting times such as offering free drinks, magazines or suggesting general drugs which you can buy without prescription, however they neither cover the current medical service's shorts nor shorten the waiting time. Method: I researched objective and perceived waiting times reduction methods, the current service status at pharmacies, and the government's policy direction in a medical service. Also, I observed a patient's journey from the hospital to pharmacy and then home. I examined the circumstance at pharmacies, patients' behaviors and their thoughts during their journey, and extracted three main goals to design a service in order to help patients have positive perception during the waiting time; (1) to reduce the perceived time by the way of visualizing time and offering readings about what patients consider necessary, (2) to educate patients what they are into and how to get through, (3) to establish trust among patients, doctors and pharmacists. Based on three goals, I designed a structure and a wireframe for a new service application of smart phones. Results: With a new service design for pharmacy users, users can track their medical record and visit the information about their current medical treatments anytime. Also the service helps patients build reliable relationships with doctors and pharmacists. Conclusion: Experience is not just an activity but series of multiple activities. The serving range of a medical service should not be determined by stakeholders but user's holistic experience. By approaching a service design with a holistic vision, it can enrich not only a temporary experience but also a whole life well being. Application: Since there are already many service applications advising patients about their illness and finding right doctors, this service design is focused on the experience from getting a prescription till feeling better. The next move is to combine those two parts medical services and design an integrated service application. As a prescription is going to be coded in numbers, we might consider to design an un-attended pharmacy which can shorten huge amount of time for filling prescriptions.

의료서비스 경험데이터의 측정 및 가공에 관한 연구 -환자중심성 실현 관점에서- (A study on the measurement and processing of medical service experience data - From the perspective of realizing patient-centeredness -)

  • 본 연구는 기존의 서비스디자인 방법론의 소수 데이터를 기반으로 휴리스틱 기반의 연구 한계를 극복하고, 의료서비스의 질적 수준 관리에서 핵심가치로 주목받고 있는 경험데이터의 객관적 측정 및 가공에 관한 모델을 개발하기 위한 연구이다. 이론적 배경에서 의료서비스에서 경험의 중요성, 경험데이터의 측정 및 가공, 환자중심성 실현에 관한 문헌연구를 진행하였다. 이와 같은 문헌 및 이론적 배경 연구결과를 기반으로 다음과 같이 4가지 연구변수에 대하여 조작적 정의하고, 통계적으로 검정을 진행하였다. 가설 H1은 3가지 요인 관점의 경험데이터 측정이 페르소나 모델링에 주는 영향이고, 가설 H2는 페르소나 모델링이 서비스청사진 시각화에 주는 영향, 가설 H3은 서비스청사진 시각화는 환자중심성 실현에 주는 영향, 가설 H4는 페르소나 모델링이 환자중심성의 실현에 주는 영향이다. 요인분석, 신뢰성 분석, 상관분석의 데이터 기초검정 후 회귀분석기법으로 검증한 결과 4가지 가설은 모두 채택되었다. 결론적으로 병원에서 좋은 의료진과 의료장비만을 갖춘다고 해서 그 가치를 인정받기 어려운 시대이기에 환자들에겐 의료진과 의료장비의 효용보다 지속해서 어떠한 의료서비스 경험을 얻고 있는지가 더 중요하다는 의미를 파악할 수 있었다. 서비스 경제의 시대에서 병원서비스 경쟁력의 핵심은 매력적인 경험을 제공하느냐가 병원의 진짜 실력이 되기에 본 연구 주제인 경험데이터의 측정과 가공이 환자중심성의 실현과 스마트병원 구현에 중요한 의미가 있을 것이다.

가족 돌봄제공자의 말기 교모세포종 환자 돌봄경험-갈등과정에 대한 내러티브 분석 (Caring for Dying Patient with Glioblastoma Multiforme: A Narrative Analysis of the Caring Experience of Family Caregiver)

  • Purpose: The purpose of this narrative study was to understand how family caregivers interpreted themselves life during caring for dying patients with gliobalstoma, and how they integrated these experiences into their personal biographies. Methods: Three family caregivers were recruited for the study. Data were collected through a series of audio-taped unstructured interviews and conversations with participants. The interviews and observation were conducted between October and November, 2011. Data were analyzed using psychosocial analytical methods that combined case based, in-depth staged analysis of narratives. Results: The life experiences of the family caregivers with a dying family member were summarized as, in their own voices, 'the repetition of gliobalstoma,' 'a smart patient,' 'being obsessed with rehabilitation treatment,' 'the frustration from nothing but just looking at the suffering of the patient,' and 'a stubborn son'. Conclusion: Caregiving was characterised by various roles and life changes from the moment of diagnosis. Family caregivers of brain tumor reported experiences similar to those described by caregivers of people with other cancers. What differed for this group was the rapidity of change and the need for immediate information and support to assist with caring for a person with high-grade glioma.

위치 추적 센서 기반의 IOT 헬스케어 서비스 관리 모델 (An Efficient IoT Healthcare Service Management Model of Location Tracking Sensor)

  • 전 세계적으로 사물인터넷(IoT) 기술이 주목을 받으면서 사물 인터넷 기반의 헬스케어, 스마트 시티, 농업, 국방 등의 다양한 서비스 개발이 진행되고 있다. 그러나, IoT를 적용한 헬스케어 서비스는 환자의 생체정보가 제3자에게 유출되어 환자의 생명을 위협하는 상황이 발생할 수 있는 문제가 존재한다. 본 논문에서는 사물 인터넷 기반의 헬스케어 서비스를 제공받는 환자의 생체정보를 제3자에게 유출되지 않으면서 센싱된 데이터 및 자원을 이용하여 치료/행정 처리의 시간 및 절차를 간소화하기 위한 위치추적 센서 기반의 IoT 헬스케어 서비스 관리 모델을 제안한다. 제안 모델은 환자의 위치 정보를 이용하여 병원내 의료진들이 환자의 위치를 실시간으로 확인하고 응급상황이 발생했을 경우에도 신속하게 대응할 수 있다. 또한, 병원 내 의료장비에도 위치추적 센서를 부착해 치료에 필요한 장비들의 위치도 즉각적으로 확인 가능하기 때문에 의료서비스의 시간 및 절차를 최소화할 수 있는 장점이 있다.
