중소병원 간호사의 병상규모별 간호업무환경과 직무배태성이 이직의도에 미치는 영향 (Effects of Nurses' Practice Environment and Job Embeddedness on Turnover Intention in Medium-small sized Hospital)

  • 본 연구는 중소병원 간호사의 병원규모별 간호업무환경, 직무배태성이 이직의도에 미치는 영향을 확인하기 위함이다. 본 연구는 2015년 8월 10일부터 8월 24일까지 B광역시 소재 200병상 미만 병원에 근무하는 간호사 131명, 200~400병상 병원에 근무하는 간호사 145명을 대상으로 설문지를 이용한 서술적 조사연구이다. 자료분석은 SPSS/WIN 18.0 프로그램으로 분석하였다. 연구결과는 다음과 같다. 첫째, 병상규모 200~400병상 간호사와 200병상 미만 간호사에서 간호업무환경과 직무배태성은 유의한 차이가 없었으나, 이직의도는 유의한 차이가 있었다. 둘째, 두 집단 모두에서 이직의도는 간호업무환경 및 직무배태성과 유의한 음의 상관관계가 있었다. 셋째, 간호사의 이직의도에 영향을 미치는 요인은 200병상 미만 병원에서 직무배태성, 나이, 간호업무환경의 순이었으며, 200~400병상 병원에서 직무배태성, 월급여, 간호업무환경의 순이었다. 현재 중소병원 간호사의 이직률 증가로 인해 간호사의 인력난이 심각한 수준이다. 이에 본 연구결과는 중소병원 간호사의 간호업무환경을 개선하고 직무배태성을 높여 이직의도를 감소시키기 위한 전략을 마련하는데 기초자료로 이용될 수 있을 것이다.

중소의료기관의 외부정도관리 시행과 관련요인 분석 (Analysis of Relevant Factors and External Quality Assessment in Small-Medium Sized Hospital)

  • 국내 의료기관은 임상 검사의 질 향상을 위하여 외부 정도관리를 시행하고 있으나 일차 의료기관의 경우 기관장의 인식부족 및 비용에 대한 부담으로 미시행하고 있는 실정이다. 본 연구는 중소 의료기관을 대상으로 임상 화학 분야 외부정도관리를 시행하고 종합병원 이상의 의료기관 외부정도관리 결과와 비교해 보며, 정도관리에 영향을 주는 관련요인에 대하여 분석해 보았다. 중소 의료기관을 대상으로 3년간 년 1회 임상화학 분야 21개 종목의 외부 정도관리를 실시하고 항목별 분석방법, 변이계수, 의료기관의 특성에 따른 이상치를 분석하였다. 검사 항목별 분석 방법 및 변이 계수는 3년간 특이적 변화는 보이지 않았으며, 중소 의료기관은 임상화학 검사의 분석방법이 다양하고 변이계수가 높게 나타났다. 정도관리에 영향을 주는 이상치(poor value)와 같은 오류 발생은 외부 정도관리를 실시하지 않을수록(p<0.05), 내부 정도관리의 주기가 매일 이루어지지 않을수록(p<0.01), 검사 량이 적은 의료기관 일수록(p<0.01) 많이 발생하는 것으로 유의하게 나타났다. 중소 의료기관의 외부 정도관리에 대한 인식 개선과 중소 의료기관 종사자를 위한 맞춤식 교육이 필요하다 하겠다.

중소병원 간호사의 환자안전 간호활동 영향 요인 (Factors Associated with Patient Safety Care Activity among Nurses in Small-Medium Sized General Hospitals)

  • 본 연구는 중소병원 간호사의 환자안전관리 중요성 인식, 환자안전문화 인식과 조직 의사소통이 환자안전 간호활동에 미치는 영향을 분석하여, 환자안전 간호활동을 증진시키기 위한 보건프로그램 개발의 기초자료를 제공하고자 시행되었다. K시에 위치한 200병상 이상의 중소병원 3곳에서 6개월 이상의 재직기간을 가진 간호사 210명을 편의추출 하였고, 2019년 2월 8일부터 15일까지 자료를 수집하였으며, 202부가 최종분석에 사용되었다. 수집된 자료는 SPSS 21.0을 이용하여 분석하였다. 환자안전관리 중요성 인식은 $3.60{\pm}.49$, 환자안전문화 인식은 $3.39{\pm}.41$, 조직 의사소통은 $3.29{\pm}.45$, 환자안전 간호활동은 $4.08{\pm}.50$이였고 환자안전 간호활동은 환자안전 중요성 인식(r=.597, p<.001), 환자안전문화 인식(r=.626, p<.001) 및 조직 의사소통(r=.559, p<.001)간에 유의한 양적 상관관계가 있는 것으로 나타났다. 환자안전 간호활동에 환자안전관리 중요성 인식(${\beta}=.258$, p<.001), 환자안전 문화인식(${\beta}=.323$, p=<.001), 조직 의사소통(${\beta}=.160$, p=.044)은 유의한 영향을 미치는 것으로 나타났으며(F=53.067, p<.001), 이들 간의 설명력은 43.7%였다. 본 연구를 바탕으로 중소병원 간호사의 환자안전 간호활동을 증진시킬 수 있는 병원 규모별 구체적 특성을 고려한 간호사들의 안전관리 자신감 증진, 비처벌적 환경 조성과 조직의 적극적인 의사소통 형성을 위한 환자안전 간호활동 증진프로그램의 개발이 필요하다.

의약분업 이후 약국의 운영현황 분석 (An Analysis of the Changes in Community Pharmacy Operation after the Implementation of the Separation Policy of Drug Prescription and Dispensing)

  • It has been 2 years since the implementation of the separation policy of drug prescription and dispensing. This study analyzes the changes in community pharmacy operation after the implementation of the policy. The main purposes of the analysis are to determine whether the changes in community pharmacy operation have occurred and to evaluate that the changes are consistent with the intention of the policy, if the changes actually have occurred. For the study a survey on 961 pharmacies chosen by stratified sampling method has been performed. Of the 961 sample pharmacies, 438 pharmacies were responded resulting 45.6% response rate. The sample pharmacies are classified by the location that the pharmacy are operating: the pharmacies around large size hospitals, the pharmacies around clinics or medium to small size hospitals and the pharmacies with no hospitals or clinics around. Based on the classification, the number of pharmacies, number of prescriptions processed, the personnel structure, the changes in facility, and other operational characteristics are compared. The results showed that the pharmacies were tended to concentrate around hospitals and clinic since the implementation of the policy. The number of pharmacists per pharmacy was increased, the size of pharmacy was increased and the facilities were improved to accomodate the requirements of the policy. The work hours a pharmacist spent on dispensing drug have increased almost twice, however, there was no corresponding increase in the time spent on patient education and medication history management, indicating a problem in the provision of quality pharmaceutical services. Based on the results, suggestions to minimize the negative effects of the policy are provided.

방사선사의 전직의사와 관련된 요인분석 (Factors affecting the Intention of transfering of Radiology Technologists to Different Institutions)

  • This study attempts to analyze the factors affecting the intention of transferring to another hospitals among radiology technologists. 344 cases were reviewed in 5 university hospitals and 1 general hospital. Self-administered questionaire were given to study the socioeconomic characteristics, working conditions, job satisfaction level, and the reasons for transfer among the technologists. The major findings were as follows : 1. Job position and hospital characteristics had a statistically significant relationship with the intention of transferring to another hospital. 2. Those who were not satisfied with their salaries and promotional opportunity showed a higher tendency towards to transfer. 3. Those who were less satisfied with the opportunity for developing the personal ability and had the negative attitude on their job showed a higher tendency to transfer. 4. Those who did not sustain good relationship with their superiors and co-workers scored high on the tendency to transfer. 5. In the result of mutiple regression, recognition of radiation hazard, job satisfaction, satisfaction with salary levels, job attitude were significantly related to transfer. The above indicate that besides economic incentives, job satisfaction and organizational culture to promote their ability and form a good relationship with organization members were very important to decrease the intention of transfer. Since these results represent only 6 hospitals from a limited area, more hospitals nationwide, especially small and medium-sized institutions where there is a high turnover rate of employment, need to be examined in order to investigate the various factors that affect the intention of transferring.

검사건수에 따른 임상병리사의 인원편성에 관한 설문조사 분석 (Survey on the Relationship between the Number of Medical Technologists and the Medical Test Count)

  • 본 연구는 국내의 진단검사의학과의 검사현황 및 인력배치현황과 검사 건수를 파악하기 위함이다. 지역별, 규모별 임상병리사를 대상으로 설문조사를 실시하여 최종 441부를 분석 자료로 사용하였다. 자료의 분석은 STATA (version 12.0)를 이용하여 Descriptive statistics analysis, Chi-square test, ANOVA test를 실시하였다. 병원 규모별 임상병리사 분포는 의원급 5.8명, 병원급 14.9명, 종합병원 25.8명, 대학병원 45.4명으로 나타났다. 부서별검사건수, 1인 1일 검사건수는 의원, 병원, 종합병원, 대학병원간의 유의한 차이가 있었다. 임상병리사의 1일 검사건수와 1일 적정 검사건수의 차이는 상급병원으로 갈수록 감소하는 것으로 나타났다. 정도관리는 상급병원의 부서별 정도관리 평균 시간은 2시간 이내로 나타났다. 급여만족도, 근무환경만족도, 검사성취도, 복지제도측면에서 의원, 병원급에서 불만족도 다수 차지하는 것으로 나타났다. 이는 선진국형 의료 수준에 맞는 신뢰성 있는 검사의 데이터를 제공하기 위해 임상 병리사의 근무환경에 따른 근무의 만족도와 임상검사의 정확도를 높이기 위해서라도 임상병리사의 처우개선에 초점을 맞추어야 할 것으로 사료된다.

중소 정형외과병원 간호사의 환자안전문화인식, 환자안전지식과 환자안전간호수행 (Perceptions of Patient Safety Culture, Safety Care Knowledge and Activity among Nurses at an Orthopedic Hospital)

  • Purpose: The purpose of this study was to identify the factors affecting the perceptions of patient safety culture, safety care knowledge, and safety care activity among nurses at orthopedic hospitals. Methods: Data were collected during Feb. 16 and Feb. 26, 2017, from 195 nurses of 9 small to medium sized orthopedic hospitals. Questionnaires about patient safety culture, safety care knowledge and activity were used. Data were analyzed by descriptive statistics, t-test, ANOVA, $Scheff\acute{e}$ test, Pearson's correlation coefficient, and stepwise multiple regression. Results: The safety care activity was positively correlated with perceptions of patient safety culture (r=.50, p<.001) and knowledge (r=.48, p<.001). Factors that had influence on the safety care activity were the patient safety culture (${\beta}=.30$, p<.001), age (${\beta}=.27$, p<.001), and knowledge of the safety activity (${\beta}=.21$, p=.004). The patient safety care activity was explained 36.6% by those factors. Conclusion: To enhance the patient safety care activity, it should be provided the environment and open communication for the perceptions of patient safety culture and the in service education program for safety care knowledge.

조직의 공정성, 조직문화 개선 그리고 서비스 접점이 병원 경영성과에 미치는 영향 (The Effect of Organizational Fairness, Organizational Culture Improvement, and Service Encounter on Hospital Management Performance)

  • The purpose of this study was to investigate the effect of fairness, culture, and service points on organizational management to maximize the ability of organizational members. The subjects of the survey were selected from major hospitals, small and medium hospitals. A total of 500 questionnaires were distributed, but 404 (80.8%) were used for the final analysis. The SPSS WIN 20 program was used to analyze the collected data. The following conclusions were obtained. First, in this study, organizational fairness did not show any general difference, and fairness was obtained with relatively high scores. In terms of improving organizational culture, size of hospital there is a statistically significant difference. the larger the organization, the more the organizational culture needs to be improved. Organizational fairness were significant differences in service encounter points and size of hospital. Second, the correlation between fairness, organizational culture improvement, service contact point and management performance showed a very positive correlation. Third, fairness has no effect on business performance. However, in this study, organizational culture improvement and service contact points are statistically significant and positive (+) influence on dependent variable management performance. Therefore, in order to improve the organizational culture of the hospital, it is necessary for the CEO to the vision, core, to discuss and make a reasonable plan. In order to actively cope with rapidly changing hospital environment, it will be necessary to positively support the of new medical market and the increase of market share.

Knowledge, Perceptions, and Self-reported Performance of Hand Hygiene Among Registered Nurses at Community-based Hospitals in the Republic of Korea: A Cross-sectional Multi-center Study

  • Objectives: To assess the nurses' hand hygiene (HH) knowledge, perception, attitude, and self-reported performance in small- and medium-sized hospitals after Middle East Respiratory Syndrome outbreak. Methods: The structured questionnaire was adapted from the World Health Organization's survey. Data were collected between June 26 and July 14, 2017. Results: Nurses showed scores on knowledge ($17.6{\pm}2.5$), perception ($69.3{\pm}0.8$), self-reported HH performance of non-self ($86.0{\pm}11.0$), self-reported performance of self ($88.2{\pm}11.0$), and attitude ($50.5{\pm}5.5$). HH performance rate of non-self was $Y_1=36.678+0.555X_1$ (HH performance rate of self) (adjusted $R^2=0.280$, p<0.001). The regression model for performance was $Y_4=18.302+0.247X_{41}(peception)+0.232X_{42}(attitude)+0.875X_{42}(role model)$; coefficients were significant statistically except attitude, and this model significant statistically (adjusted $R^2=0.191$, p<0.001). Conclusions: Advanced HH education program would be developed and operated continuously. Perception, attitude, role model was found to be a significant predictors of HH performance of self. So these findings could be used in future HH promotion strategies for nurses.

병원경영자의 병원경영진단 지표에 관한 인식과 요구도 (A Study on the Recognition and Needs of Hospital Management Diagnoses Indicators)

  • Objectives: The purpose of this study was to provide hospital management diagnosis status and basic data required for the future development of hospital management diagnosis program. Methods: We conducted a questionnaire survey on administrative staff of manager level or over of medical institutions in B metropolitan city. Results: As a result of analyzing the relative influence of the needs by hospital management diagnosis indicator, the effect of financial analysis indicator, patient treatment record indicator and medical revenues indicator were high in the medical institutions with number of beds of 100 beds or over and general hospital level or over both on a hospital level and on an individual level. Conclusions: Since the existing laws or systems are centered on large major hospitals, the management environment is very unfavorable for small and medium hospitals as can be seen from the results of this study. Therefore, the government should improve the transparency and rationality of the hospital management environment in Korea through regulation and system reforms that can be applied to all medical institutions.
