비전형적인 가와사키 병 환자에서 발생한 좌주간지 거대 동맥류를 동반한 급성심근경색 (Acute myocardial infarction with a giant left main aneurysm in atypical Kawasaki disease)

  • Kawasaki disease (KD) is an acute vasculitis of small and medium sized arteries. Even many years after onset, aneurysms and stenosis in coronary arteries may lead to an acute myocardial infarction, which is described as atypical or missed KD in childhood. KD is an underlying disease of young adults with acute myocardial infarction. We report on a rare case involving a total occlusion in the proximal left anterior descending coronary artery combined with a giant left main aneurysm in a young adult patient with acute myocardial infarction ascribed to antecedent KD that is undefined but almost certain.

  • Objectives: This study was conducted in order to ascertain the important factors for SMEs in managing the risks of chemicals and to suggest a government role in strengthening the responsibility of small and medium enterprises(SMEs) as employers. Methods: About 100 enterprises were surveyed in Incheon. A questionnaire for employers and walk-through surveys by occupational hygienists were performed at each enterprise. Results: The results showed that most employers thought chemicals were not hazardous but chemical management was needed. When employers determine how to manage chemicals, they rely heavily on personal experience and rather less on information from the government or experts. However, if employers think the chemicals are hazardous, they do more to manage the chemicals. Conclusions: When employers think chemicals are hazardous, risk assessment would be an effective tool to control chemical hazards in SMEs. Employers' position on chemical hazards is very important. Since the government is the initiator, it is the government who develops messages for SMEs, such as "Chemicals are hazardous and management can reduce the risk of chemicals." Governmental messages can play an invaluable role in strengthening the responsibility of SME employers to manage chemical hazards.

중소병원 간호사가 인식하는 행복감, 임상간호인성 및 직무만족도와의 관계에 관한 융합연구 (Convergence study on the relationship between happiness, clinical nursing personality and job satisfaction perceived by small and medium hospital nurses)

  • 본 연구는 간호사의 행복감, 임상간호인성 및 직무만족도와의 관계를 파악하고 직무만족도 영향요인을 확인하여 임상 현장에서 간호서비스의 질 향상과 간호사의 직무만족도 향상을 위한 프로그램의 개발의 기초자료를 제공하고자 시도되었다. 연구의 대상자는 M시 소재 2곳의 병원에 근무하는 간호사로, 자료 수집은 2020년 4월 27에서 5월 11일까지였다. 수집된 자료는 SPSS WIN 23.0 program을 활용하여 분석하였다. 연구결과 대상자의 직무만족도에 미치는 영향요인을 분석한 결과 행복감이 영향력을 나타내었으며, 설명력은 20.4%였다. 결론적으로 간호사의 행복감을 향상시키고 직무만족도를 향상시킬 수 있는 구체적이고 체계적인 프로그램을 개발하는 것이 필요하다고 사료된다.

Microscopic versus Endoscopic Inlay Butterfly Cartilage Tympanoplasty

  • Background and Objectives: Inlay butterfly cartilage tympanoplasty makes the graft easy, and reduces operating time. The present study aimed to investigate the outcomes of microscopic versus endoscopic inlay butterfly cartilage tympanoplasty. Subjects and Methods: In this retrospective study, the outcomes of 63 patients who underwent inlay butterfly cartilage tympanoplasty with small to medium chronic tympanic membrane perforation were evaluated. Twenty-four patients underwent conventional microscopic tympanoplasty and 39 underwent endoscopic tympanoplasty. The outcomes were analyzed in terms of the hearing gain and graft success rate. Results: The surgical success rate was 95.8% in the patients who underwent conventional microscopic tympanoplasty and 92.3% in those who underwent endoscopic tympanoplasty. In both groups of patients, the postoperative air-bone gap (ABG) was significantly lower than the preoperative ABG. There were no significant differences between the preoperative and postoperative ABG values in either group. Conclusions: Endoscopic inlay tympanoplasty using the butterfly cartilage technique appears to be an effective alternative to microscopic tympanoplasty and results in excellent hearing.

종합병원의 경영성과에 영향을 미치는 요인에 관한 연구 (The Study on the Effect of Factors on Management Performance in General Hospital)

  • This research analyzes on the factors and difference of the management performance in general hospital. In details, we provide that personnel expenses, materials costs and administrative expenses have impact on management performance in general hospital and these factors have differences by region and sickbed size. The meaningful results of this study as follow. In the first, personnel expenses ratio influence management performance but materials costs ratio and administrative expenses ratio are only partially impact for the management performance. Secondly, in the management performance by region, there are no significant differences between the two groups of the big city and small and medium-sized cities. In personnel expenses ratio, materials costs ratio and administrative expenses ratio, The materials costs ratio is significant difference but ersonne expenses ratio and administrative expenses ratio have no significant difference. Finally, there's no significant difference between the hospital size and management performance. But this study provide that there is partially a difference between personnel expenses ratio, materials costs ratio and administrative expenses ratio and the hospital size.

지방 중소병원 간호사의 소진, 언어폭력 경험, 사회적 지지가 직무배태성에 미치는 융복합적 영향 (Effect of Burnout, Verbal Abuse Experience, and Social Support on Job Embeddedness in the Small and Medium Sized Hospital Nurses)

  • 본 연구의 목적은 지방 중소병원 간호사의 소진과 언어폭력경험, 사회적 지지가 직무배태성에 미치는 영향을 확인하는 것이었다. 자료는 497명의 중소병원 간호사를 대상으로 2017년 8월 23일부터 10월 30일까지 자가기입식 설문지를 이용하여 수집되었다. 수집된 자료는 t-test, ANOVA, Pearson's correlation coefficients, 위계적 다중회귀분석을 통해 분석하였다. 연구결과, 간호사의 지무배태성에 영향을 미치는 요인으로 소진, 언어폭력 경험, 사회적 지지, 직위가 확인되었다(Adjusted $R^2=.45$). 따라서 간호사의 직무배태성을 높이기 위해서는 간호사의 소진을 낮출 수 있는 업무환경을 조성하고, 상사와 동료의 사회적 지지를 높일 수 있는 지지적인 환경을 조성해 나감으로써 간호사의 직무배태성을 향상시킬 수 있을 것이다. 또한 간호관리자는 간호사를 위한 언어폭력 예방 프로그램을 마련해야 할 것이다.

중소병원의 교육훈련 프로그램 특성 및 조직상황적 특성이 직무전이에 미치는 영향에 관한 연구 (A Study on the influence of job transfer in E&T programs characteristics & organizational situational characteristics of the medium and small-sized hospitals)

  • The purpose of this study was 1) to understand the job transfer in education & training(E&T) programs & organizational situational characteristics, and 2) to analyze the effects of the E&T programs & organizational situational characteristic groups on each of the levels of job transfer. There is intimate relationship between E&T programs characteristics & organizational situational characteristics and job transfer, so we can't think of them separately. With this point of view the most frequently discussed problem is how to apply the theory in actual job, but in reality it's true that the improvement of job transfer can't reach the expectations of the E&T programs. In literature view study, the concepts of job transfer in E&T, the general theory on E&T, and the actual condition and points at issue of the existing E&T practices in the medium and small-sized hospitals were defined. As the results from empirical analysis, we could find followings: First, in evaluation of the job transfer, demographic characteristics (seniority, occupational category, position) variables have a significant different. Second, in degree of the job transfer, E&T programs characteristics(contents of E&T, design of E&T, conditions of E&T, teaching ability of lecturer, methods of E&T) variables have a different influence upon each of the evaluation levels of E&T. Third, support of CEO variables have a different influence on the job transfer according to contents, design, conditions of the E&T programs. Fourth, support of co-works variables have a different influence on the job transfer according to design, conditions, method of the E&T programs. Fifty, support of organizational atmosphere variables have a different influence on the job transfer according to contents, design, conditions of the E&T programs.

  • 19세기 이후 다양한 분야의 과학기술이 거듭 성장하면서 의료분야에서도 보다 혁신적인 기술들이 높은 수준에까지 발전하고 있다. 융복합 기술의 촉진으로 인하여 실버계층을 위한 환자만족도 증진 시스템이 국내 메디컬 프로세스에 도입되고 있지만 기존 대형병원을 중심으로 한정적인 운용성을 유지하고 있으며, 서비스 시스템의 환경 여건 및 높은 경제적 비용으로 인하여 중소병원에서의 활용이 불가피한 실정이다. 그로인해 환자 만족도의 취약성 및 실버계층에 대한 서비스 접근성이 낮아지는 현상이 발생하게 된다. 본 논문에서는 HTK(Hidden Markov Model Toolkit)를 이용하여 일반 사용자는 물론 실버 계층과 같은 취약 계층까지 메디컬 서비스에 대한 사용자 접근성을 높일 수 있도록 하고 중소병원에서의 효율적인 환자 만족 서비스 관리가 가능하도록 하는 안드로이드 기반의 저비용 스마트 진료 정보서비스 시스템(Smart Medical treatment Information Service System)설계 기법을 제안한다.

Preventive Measures Against COVID-19 in Small- and Mid-sized Enterprises from an Early Stage of the Epidemic in Daegu and Gyeongsangbuk-do

  • Background: In the early stage of the coronavirus disease 2019 (COVID-19) epidemic, small- and midsized enterprises (SMEs) may be an important transmission consideration. The study aimed to identify the pattern of COVID-19 prevention measures during the outbreaks in Daegu and Gyeongsangbuk-do at the early stage of COVID-19. Moreover, we investigated whether SME size and past experiences affected the preventive measures implemented in the region. Methods: A survey detailing the general characteristics and implementation of 12 preventive activities was conducted in 122 SMEs in Daegu and Gyeongsangbuk-do. The survey was analyzed by size and operation period. Results: The study subjects consisted of 53 (43.4%) workplaces with 1-5 employees, 50 (40.9%) workplaces with 6-30 employees, and 19 (15.6%) workplaces with 31-49 employees. The lowest three items among those surveyed were 'symptomatic workers to stay home for 3-4 days' (17.2%), 'work remotely' (18.9%), and 'video meetings' (20.5%). There were significant differences in the rate of several preventive measures implemented. The larger sized SMEs, the higher the number of implementations (p < 0.01). The operation period had no significant relationship with the implementation of preventive measures. The same pattern was observed in multiple generalized linear regression with covariate adjustment. Conclusion: Preventive measures among SMEs with fewer than 50 employees were identified. Even within SMEs, a gap in preventive measures according to size was confirmed. To prevent the spread of infection and protect workers' right to health, different support for different sized SMEs is necessary.
