뇌파 영역에서 수면 발생 과정 (Sleep Onset Period from the EEG Point of View)

  • In accordance with the development of EEG and polysomnography in the field of sleep research, the sleep onset period (SOP) between wakefulness and sleep has been considered an important part for understanding the physiology of sleep. SOP in the transition from wakefulness to sleep is a gradual process integrating various viewpoints such as behavior, EEG, physiology and subjective report. Particularly, based on understanding of EEG changes during sleep, SOP has been regarded as a pattern of topographical change in specific frequency and specific state in EEG. Studies on quantitative EEG (qEEG) and event-related potential (ERP) have suggested that SOP shows the changes of functional coordination at the specific cortical areas in qEEG and the changes of regular patterns in response to environmental stimulation in ERP. The development of sleep EEG and topographic mapping of EEG is expected to integrate various viewpoints of SOP and clarify the neurophysiologic mechanism of SOP further.

청소년의 규칙적 아침식사를 위한 수면 관련 요인 분석 (Association between Regular Breakfast and Sleep-related Factors in Korean Adolescents)

  • Purpose: Breakfast is the most important meal to provide energy for the day. Breakfast is especially important to give enough nutritional support to children and adolescents for their physical growth and sexual development. Sleep-related factors like average sleep duration and wake up time would mostly be associated with regular breakfast. This study aimed to investigate the effect of sleep on regular breakfast consumption in Korean adolescents. Methods: The study used the data from the 12th Korea Youth Risk Behavior Web-based Survey (KYRBS-XII) conducted in 2016 by the Korea Centers for Disease Control and Prevention. The data of 62,820 subjects (middle/high school students) were included in the final analysis. The study examined the factors related to regular breakfast, focusing on weekday average sleep duration and wake up time of middle school students and high school students, respectively. Results: Regular breakfast consumption was shown to have a statistically significant association with high economic status, nutritional education, weekday average sleep duration, wake up time, and subjective sleep satisfaction in the multivariate logistic regression. Regardless of the school level, regular breakfast consumption was significantly associated with early wake up time. As to the effect of weekday average sleep duration on regular breakfast consumption, it showed some different results depending on the school level. Conclusion: Regular breakfast consumption of Korean adolescents was related to weekday average sleep duration and wake up time. Having breakfast regularly was affected by both adequate weekday average sleep duration and early wake up time.

복지관 이용 노인의 건강상태와 건강행위 관련요인 (Health Status and Factors related to Health Behaviors of Older Adults Using a Senior Center)

  • Purpose: The aim of this study was to describe health status and to identify the factors related to health behavior in older adults in South Korea. Methods: A cross-sectional survey was conducted with a convenience sample of 186 older people (mean age =68.2yrs, 65.1% Female) registered at one senior center. Data were collected by self-report questionnaires or through face to face interview. The instruments were the Modified Health Behavior Assessment Scale, Stanford Research Instruments for Chronic Disease, Self-Efficacy, SOF Frailty Index and Quality of life questionnaire. The data were analyzed using t-test, ANOVA, Pearson's correlation, and stepwise multiple regression. Results: 58.6% older adults perceived their health status positively. Education level and economic status were significantly related to health behaviors of older adults. Self-rated health, sleep, stress, quality of life, health distress, depression, and frailty were significantly correlated with the health behaviors of older adults. Frailty, education level, and sleep disturbance were the significant factors predicting the health behaviors. Conclusion: The findings from this study suggest that nurses should take into consideration education level of older adults to promote their health behaviors and health promotion program which focuses on maintaining the quality of sleep and preventing frailty.

철강회사 남성 근로자의 직무스트레스, 우울, 건강관련 행위, A형 성격과의 관련성 (A Study on the Correlation among Occupational Stress, Depression, Health-Related Behavior and Type A Behavior Patterns of Male Workers at a Steel Factory)

  • Purpose: This study investigates the relationship between job stress, depression, health-related behavior and type A behavior patterns among male workers at a steel factory. Methods: The study was done on 171 male workers, using the structured questionnaire. The data were collected in April, 2009 and analyzed by descriptive statistics, t-test, ANOVA, and pearson correlation coefficient. Results: The results showed that 65.5% of workers were under moderate stress, 25.7% high stress and 0.6% extremely high stress. The overall score of these workers' occupational stress was 41.9, lower than the Korean workers' average. Occupational stress was correlated with depression, type A behavior patterns, a very poor physical condition without sufficient sleep. Conclusion: Stress management program is strongly needed for these subjects working over 45 hours a week, with a very poor subjective physical condition, insufficient sleep, type A behavior patterns and signs of severe depression. Further longitudinal study is recommended to show the effect of such worker's stress management program to mitigate job-related stress.

  • Objectives: This study examined the case of a 69-year-old man with a history of stroke and Alzheimer's disease who had been diagnosed with probable-rapid eye movement sleep behavior disorder (probable-RBD). Methods: The patient was treated with herbal medicine (Ukgan-san, Ukgansangayonggolmoryeo-tang, and powdered Gamisoyo-san extract), Western medicine (clonazepam, antiplatelet, psychotropic agents, antihypertensive drugs, and others), and acupuncture. Their effects were evaluated by the frequency and severity of sleep-related behavioral symptoms. Results: After treatment, the observed frequency and severity of sleep-related behavior decreased. Conclusion: The results suggest that using traditional Korean medicine with clonazepam can be effective in the treatment of patients with probable-RBD.

우리나라 청소년에서 수면시작시간과 우울감의 상관관계: 청소년 건강행태온라인조사를 바탕으로 (Relationship between Sleep Timing and Depressive Mood in Korean Adolescents: Based on the Korea Youth Risk Behavior Web-based Survey)

  • Purpose: The purpose of this study was to investigate the relationship between sleep timing and depressive mood in Korean adolescents. Methods: The study analyzed the data from the 2007~2015 Korea Youth Risk Behavior Web-based Survey. A total of 541,693 students in grades 7~12 were included in the final analysis. Multivariable logistic regression was used to examine their sleep timing and depressive mood, adjusted for sex, grade, region, socioeconomic status, academic performance, alcohol, smoking and physical activity. Sleep duration and sleep quality were also included in our model to identify whether or not the effect of sleep timing on depression is mediated by sleep duration or sleep quality. Results: The prevalence of depressive mood was 32.7% and the mean sleep timing was 12:13 AM. After adjustment for eligible covariates, the association between sleep timing and depressive mood showed a J-shaped curve. Adolescents who slept at 8 pm~10 pm were 39% more likely to be depressive (OR = 1.39, 95% CI 1.30~1.40) and at 3 am~ 4 am were 67% more likely to be depressive (OR=1.67, 95% CI 1.64~1.70) than adolescents who slept at 11 pm~12 am. These associations persisted after being adjusted for sleep duration and sleep quality. Conclusion: Sleep timing was related to depression in adolescents, independent of sleep duration and sleep quality. It appears that there is a certain sleep timing beneficial to mental health of adolescents.

일 도시의 초등학교 학생의 수면습관과 행동, 정서, 주의력, 학습과의 관계 (Differences in Sleep Patterns are Related to Behavior, Emotional Problems, Attention and Academic Performance in Elementary School Students of a South Korean Metropolitan City)

  • Objectives: The aim of this study was to investigate the sleep patterns of South Korean elementary school children and whether the differences in sleep patterns were related to behavior, emotional problems, attention and academic performance. Method: This study included a community sample of 268 boys and girls from fourth-, fifth- and sixth-grade classes in a South Korean metropolitan city from November to December 2010. The primary caregivers completed a questionnaire that included information on demographic characteristics, as well as the Child's Sleep Habit Questionnaire (CSHQ), the Korean version of Child Behavior Checklist (K-CBCL), the Korean version of the Learning Disability Evaluation Scale (K-LDES), the Korean version of ADHD Rating Scale (K-ARS) and the Disruptive Behavior Disorder Scale (DBDS). We conducted analyses on the CSHQ individual items, between the subscales, on the total scores and on the K-CBCL, the K-LEDS, the K-ARS and the DBDS. Results: Based on the findings from the CHSQ, the subjects had significantly higher scores for bedtime resistance ($9.18{\pm}2.17$), delayed sleep onset ($1.32{\pm}0.62$), the sleep duration ($4.19{\pm}1.52$) and daytime sleepiness ($14.10{\pm}3.55$) than the scores from the previous reports on children from western countries. The total CHSQ score showed positive correlations to all subscales of the K-CBCL : withdrawn (r=0.24, p<.005), somatic complaint (r=0.24, p<.005) and anxious/depressive (r=0.38, p<.005). Bedtime resistance was associated with oppositional defiant disorder (r=0.15, p<.05) and a positive correlation was demonstrated between sleep anxiety and the oppositional defiant disorder score (r=0.13, p<.05), night waking and the conduct disorder score (r=0.16, p<.05). Delayed sleep onset was related with low performance on the K-LDES with respect to thinking (r=-0.17, p<.05) and mathematical calculation (r=-0.17, p<.05). Conclusion: The results of this study reconfirm Korean children's problematic sleep patterns. Taken together the results provide that the reduced sleep duration and disruption of sleep pattern can have a significant impact on emotion, behavior, performance of learning in children. Further studies concerning more diverse psychosocial factors affecting sleep pattern will be helpful to understanding of the sleep health in Korean children.

일부 대학생의 체중상태와 건강행위에 간한 연구 (A study on Body Weight and Health Behavior of the College Students)

  • This study was done for the purpose of investigating college students' body weight and health behavior. The subjects for this study were 1421 students obtained by a convenient sampling from one junior college located in Mok-Po. The data were analyzed by self-reporting questionnaire from June 18 to 28, 1996. The data were analyzed by descriptive statistics with SAS program. The results were as followed; Factors of comparatively high contribution to overweight were male students, perception they were healthy. smoking, alcohol drinking, irregular sleep and eating pattern, sleep for 9 hours over, regular exercise, no snack, diet control for weight loss. Factors of comparatively high contribution to underweight were female students. perception they were unhealthy. no smoking and alcohol drinking. irregular sleep and eating pattern. high rate of snack, sleep for 9 hours over. In conclusion overweight and underweight were correlated with seven health habits' and we suggest health education related to seven health habits.

Nasal Breath in the Lateral Position for Sleep Apnea: a Retrospective Case Series

  • Objectives: This was a retrospective case series about the clinical effect of integrated approach with behavior therapy for obstructive sleep apnea (OSA). Methods: Medical records of twelve patients with OSA who were treated with behavior modification including nasal breathing with oral appliances and sleep in lateral position, oral administration of herbal medicines and acupuncture treatment between January and September of 2013 were reviewed. Apnea-hypopnea index (AHI), day time sleepiness, apneas and hypopneas counts during sleep, risk indicator (RI), oxygen desaturation index, average saturation during sleep, lowest desaturation, lowest saturation, snoring events ratio and number of desaturations (%) were assessed before and after treatments with the ApneaLink device, which is a portable diagnostic apparatus for monitoring airflows of the patient's breath at home. Results: After an average 62.67 (SD 37.16) days of treatment, AHI (from 17.67, 12.79 to 8.75, 8.25, p=0.007), RI (from 22.00, 13.26 to 12.09, 8.03, p=0.004), oxygen desaturation index (from 17.33, 12.17 to 8.17, 7.86, p=0.005), and number of desaturations (from 7.00 times, 9.79 to 0.92 times, 1.39, p=0.044) showed significant improvement. Daytime sleepiness improved from 6.5 (3.2) to 3.8 (1.8) but there was no significant difference after treatment (p=0.17). No adverse events related to treatment were observed during participation in the treatment. Conclusion: From this case series, we found that behavior modification with herbal medication and acupuncture may be effective for improving sleep apnea without serious adverse events. Future randomized controlled trials with larger sample size will be necessary for concrete evidence on the benefit of this integrated treatment for OSA.

파킨슨병의 중증도에 따른 수면 장애 (Sleep Disturbances in Patients with Parkinson's Disease according to Disease Severity)

  • Background: Sleep-related disturbances and sleep disorders are common in Parkinson's disease (PD) and have a great impact on daily life of PD patients. This study was done to find the sleep characteristics and sleep disturbing factors in PD patients according to disease severity through clinical interview and polysomnographic (PSG) study. Methods: Fifty patients with PD (22 males, age $60.6{\pm}6.4$, Hoehn and Yahr (HY) stage $2.7{\pm}1.0$) were recruited and thoroughly interviewed about their sleep. PSG was performed on the patients taking routine antiparkinsonian medications. Patients were grouped into mild and moderate/severe group according to HY stage, and the results were compared between each group. Results: Ninety-four percent of total patients had one or more sleep-related disturbances based on the interview or PSG. On interview, the moderate/severe group complained more insomnia and REM sleep behavior disorder (RBD) than mild group. In PSG findings, the moderate/severe group showed lower sleep efficiency, longer sleep latency, REM sleep latency, waking time after sleep onset, and higher prevalence of RBD. Conclusions: In this study, most patients with PD had sleep disturbances. Clinical interview and PSG findings revealed deterioration of sleep quality along the disease severity. Our results suggest that sleep disturbances in PD patients are prevalent and warrant clinical attention, especially to the patients with advanced disease.
