수면중 이갈이에 의해 발생한 턱관절 통증 환자에 대한 FCST 활용 1예 보고 (A Case Report of Sleep Bruxism-induced Temporomandibular Jount (TMJ) Pain Improved by Appliance of FCST)

  • 본 증례에서는 수면 중 이갈이에 의해 발생한 턱관절 통증에 대하여 FCST를 포함한 침치료를 통해 양호한 치료효과가 있는 것을 관찰하였다. 상기 환자는 FCST의 균형측정지 검사상 TMJ의 경미한 불균형 외에 방사선 소견상 구조적인 큰 이상이 없었음에도 이갈이에 따른 심한 근육긴장으로 통증이 발생한 경우였다. 이것을 통해 FCST가 턱관절의 구조적인 문제에만 국한되어 적용되는 것이 아니라 턱관절 주위 근육 긴장의 빠른 회복에도 상당한 효과가 있음을 볼 수 있었다. 또한 야간에 TBA를 착용한 상태로 잠을 자게 되면 그 자체로 이갈이에 대한 예방효과가 있어 현재 턱관절통증 등의 증상이 없는 이갈이 환자에게도 예방적인 목적으로 TBA의 처방이 가능할 것으로 사료된다.

  • Bruxism is nonfunctional jaw movement that includes clenching, grinding and gnashing of teeth. It usually occurs during sleep, but with functional abnormality of brain, it can be seen during consciousness. Oromandibular dystonia (OMD) can involve the masticatory, lower facial, and tongue muscles and may result in trismus, bruxism, involuntary jaw opening or closure, and involuntary tongue movement. Its prevalence in the general population is 21%, but its incidence after brain injury is unknown, Untreated, bruxism and OMD cause masseter hypertrophy, headache, temporomandibular joint destruction and total dental wear. We report a case of successful treatment of bruxism and OMD after brain injury treated with botulinum toxin A and occlusal appliance. The patient was a 59-year-old man with operation history of frontal craniotomy and removal of malformed vessel secondary to cerebral arteriovenous malfomation. We injected with a total 60 units of botulinum toxin A each masseteric muscle and took impression for occlusal appliance fabrication under general anesthesia. On follow up 2 weeks and 2 months, the patient remained almost free of bruxism. We propose that botulinum toxin A and occlusal appliances be considered as a treatment for bruxism and OMD after brain injury.

The Effect of Psychological Factors on Sleep Disturbances in Young Adults

  • Purpose: The purpose of this study was to assess influences of psychological factors on sleep disturbances in young adults through the revised version of the Minnesota Multiphasic Personality Inventory (MMPI-2). Methods: Two hundred and ninety nine college students in Gyeonggi-do completed the MMPI-2 and a questionnaire related to sleep disturbances and collected data were analyzed by logistic regression. Results: The odds of tooth grinding increased significantly with the increase of T-score of hysteria (Hy) scale. The increase of T-score of hypomania (Ma) and social introversion (Si) scale significantly contributed to the risk of insomnia. The occurrence of insomnia increased significantly as T-score of somatic complaints (RC1) and psychoticism (PSYC) scale increased. The odds of tooth grinding increased significantly as T-score of anxiety (ANX) and family problems (FAM) scale increased. The occurrence of insomnia decreased significantly with the increase of T-score of type A behavior (TPA) scale. The increase of T-score of dominance (Do) scale significantly contributed to the risk of tooth clenching. The odds of tooth grinding decreased significantly as T-score of MacAndrew alcoholism-revised (MAC-R) scale increased, whereas it increased significantly as T-score of addiction potential scale (APS) increased. No scale of MMPI-2 significantly affected the occurrence of snoring. Conclusions: T-scores of Hy, ANX, FAM, Do, MAC-R, and APS scales on MMPI-2 affected the risk of bruxism. T-scores of Ma, Si, RC1, PSYC, and TPA scales on MMPI-2 influenced the occurrence of insomnia. The psychometric instrument such as MMPI-2 is helpful in understanding and managing bruxism and insomnia.

교합안정장치를 이용한 이갈이의 치료 (TREATMENT OF BRUXISM USING THE OCCLUSAL SPLINT)

  • 이갈이(bruxism)는 일반적으로 주간의 이악물기(clenching)와 야간의 이갈이(grinding)를 포함하는데 이악물기는 상하악의 정적인 관계에서 대합하는 치열이 힘있게 다물어지는 것을 의미하고 이갈이는 하악의 편심운동시에 상하악이 동적인 관계에서 힘 있게 다물어지는 것을 의미한다. 이갈이의 원인은 아직까지 정확하게 밝혀진 것은 없지만 가장 큰 요인으로 정서적 스트레스를 들 수 있고, 수면장애나 약물, 중추신경장애 등을 원인으로 볼 수 있다. 이갈이의 치료는 근본적인 치료법은 아직까지 없으나 정서적 스트레스를 감소시키고, 교합조정이나 교합안정장치, 약물요법, 물리치료 등을 통해서 이갈이의 증상 및 징후를 치료하는 것이 치료의 목표가 된다. 본 증례는 이갈이를 주소로 내원한 환자에서 교합안정장치를 통하여 이갈이가 감소하는 결과를 얻었기에 보고하는 바이다.

수면 이갈이 습관을 가진 성인 및 성장기 아동 부정교합자의 낮시간 이악물기시 나타나는 교근 및 측두근의 근활성도에 관한 연구 (Electromyographic activity of masseter and temporal muscle at daytime clenching in adult and adolescent sleep bruxers)

  • Objective: To compare the electromyographic activity of masseter and temporal muscles between bruxers and nonbruxers in adult and adolescent patients. Material and Methods: The samples composed of the surface electromyography (EMG) recordings obtained from the orthodontic patients. Sixty-eight patients who had bruxsim habit (43 female and 25 male) were divided into four groups according to their age and gender. Control groups consisted of 79 patients who had not bruxism habit. EMG of the masseter muscle and anterior temporal muscle were recorded before treatment in clenching at maximum intercuspation. The ratio of temporal and masseter muscle activity (T/M ratio) was compared between bruxers and nonbruxers in adult and adolescent patients. Results: EMG of masseter muscle and temporal muscle were significantly higher in adult male bruxism group than control. T/M ratio in adult male bruxism group was significantly lower than in adult male nonbruxism group. However, there was no significant difference in T/M ratio between adolescent bruxism group and adolescent nonbruxism group. Conclusions: The balance in the activity of the masseter and temporal muscles may not differ between bruxers and nonbruxers during adolescent periods. However, in adult period, the masseter muscle activity against temporal muscle is greater in males with bruxism habit compared to non-bruxer.

이갈이의 진단 및 치료 (The Diagnosis and Treatment of Bruxism)

  • 이갈이는 이를 갈거나 악무는 것을 포함하는 부기능적인 행위를 말하며 광범위하게는 주간 및 야간에 나타나는 것을 모두 포함한다. 이갈이의 원인으로는 기본적으로 말초성 요인과 중추성 요인으로 나누어 살펴볼 수 있는데 현재까지의 연구 결과를 종합하여 볼 때 중추성 요인이 주로 작용하는 것으로 알려져 있다. 이러한 이갈이는 교모, 치경부 미세파절, 교근 비대, 교근 및 측두근의 통증, 턱관절의 통증 및 움직임의 제한, 치아 및 수복물 파절, 치수염, 외상성 교합 등을 유발 할 수 있고 특히 이악물기의 경우 협점막 압흔이나 협점막 백선 또는, 혀의 측면에 압흔 등을 유발한다. 이갈이의 정확한 진단을 위해 구강내 장치, 근전도, 수면다원검사 등을 이용하며 미국수면장애학회의 경우 이갈이의 임상 진단 기준을 제시하고 있다. 그러나 아직까지 이갈이의 명확한 원인이 밝혀져 있지 않기 때문에 치료법의 선택은 신중하여야 한다. 이갈이가 중등도 이상이고 임상 증상이나 징후를 유발하는 경우에 이갈이를 관리하기 위한 방법으로는 위험 요인의 조절, 구강내 장치, 보툴리눔 독소 주사, 약물 치료, 바이오피드백 등이 있으며 구강내 장치 요법이 현재로서 가장 합리적인 방법이다. 이갈이 치료와 교근비대에 대한 심미적인 개선을 같이 원하는 환자에서는 보툴리눔 독소 주사 요법이 이용될 수 있다.

Relationships between Intermittent Locking History and Self-Reported Bruxism in Temporomandibular Joint

  • Purpose: To evaluate aggravating factors of intermittent locking among temporomandibular joint using magnetic resonance imaging (MRI) and diagnostic criteria for temporomandibular disorder (DC/TMD) diagnosis. Methods: A retrospective analysis was conducted of 35 patients with intermittent locking history but normal intra-articular findings between September 2012 and June 2015 in Kyung Hee University Dental Hospital. A standardized DC/TMD assessment was performed on subjects with MRI findings. Clinical findings were assessed on the basis of maximum mouth opening (active & passive), self-reported habits, patients' age, gender, systemic diseases at the initial visit. First, chi square test was used to examine differences with variables and then risk factors for intermittent locking were assessed using multivariate logistic regression. Results: Self-reported bruxism was strongly associated with intermittent locking history. Conclusions: The new DC/TMD protocol is intended for use within any clinical setting and supports the full range of diagnostic activities from screening to definitive evaluation and diagnosis. Self-reported sleep bruxism has been associated with a higher likelihood of intermittent locking. Comorbidity is therefore a factor that must be assessed. It is necessary to consider the amount of contact of the teeth and the duration.

특발성 안면 통증 환자에서 수면 이갈이의 임상적 특징 (Clinical characteristics of sleep bruxism patients with idiopathic facial pain)

  • 목적: 특발성 안면 통증 환자에서 수면 이갈이의 임상적 특징을 알아보기 위함이다. 연구 재료 및 방법: 특발성 안면 통증을 호소한 210명의 환자 중 수면 이갈이가 존재하는 28명의 환자의 성별, 나이, 통증의 기간, 위치 및 강도, 통증에 영향을 받은 부위를 포함한 차트 분석을 시행하였다. 결과: 특발성 안면 통증이 있는 수면 이갈이 환자는 남성(14.3%)보다 여성(85.7%)이 많았다. 평균 연령은 48.9세였으며 가장 흔한 안면 통증 부위는 우측 상악 치아(28.6%)와 하악 치아(25.0%) 부위였다. 2개의 치아에 통증이 발생한 경우가 가장 많았으며(50.0%), 1개 치아(28.6%), 3개 치아 이상(21.4%) 순으로 통증이 나타났다. 평균 통증 강도는 visual analogue scale (범위: 0 - 10) 기준 5.9였다. 20명의 환자에서 통증은 자발통의 양상으로 나타났으며(71.4%), 평균 통증 기간은 24.4개월이었다. 결론: 특발성 안면 통증 환자에서 수면 이갈이가 있는 환자들의 임상적 특징은 다양한 특발성 안면 통증 환자의 진단에 유용할 수 있으며, 상기 통증을 줄이기 위해 시행하는 불필요한 치료를 감소시키는데도 효과적이다. 보다 체계화된 진단 방법 및 개발을 위한 치료 지침을 위해 후속 연구는 충분한 기간 및 다수의 환자를 대상으로 진행될 필요가 있을 것이다.

대구지역 일개 초등학교를 대상으로 한 소아 수면문제에 대한 예비연구 (Preliminary Study of Children's Sleep Problems in an Elementary School in Daegu)

  • Objectives: The purpose of this study was to investigate the prevalence of sleep problems in Korean elementary school children and the differences in sleep problems elated to their school grade and gender. Methods : One public elementary school in Dae-gu, Korea as randomly selected. And, parents who agreed to participate this study were asked to complete a modified version of Tucson Children's Assessment of Sleep Apnea screening questionnaire (TuCASA). Results: The most common sleep problem was 'falling asleep in vehicle (44.2%)' and the prevalence rate of this problem differed by school grade. The prevalence rate of falling asleep while watching TV before 8 P.M. (8.0%), daytime fatigue (21.9%) and nocturnal enuresis during the past 6 months (5.3%) were significantly differed by school grade. The prevalence rate of falling asleep while doing home work, falling sleep during a lesson, snoring and bruxism were 12.6%, 1.0%, 26.7% and 13.2% respectively. Although these rates did not show any differences between grades, snoring as more in boys and daytime fatigue was more in girls. Conclusion: This study demonstrates that many elementary school children may have various sleep problems. Lower grade elementary schoolers had sleep problems which related to euro-developmental factors, whereas higher grade elementary schoolers had sleep problems related to sleep deprivation and stress. Finally, more female schooler showed signs of fatigue or sleepiness than male schoolers.

Effects of soft occlusal appliance therapy for patients with masticatory muscle pain

  • Background: The options for stabilization appliance therapy for masticatory muscle pain include soft occlusal and hard stabilization appliances. A previous study suggested that hard stabilization appliance therapy was effective for patients with local myalgia who developed long facets on their occlusal appliances. The objective of this study was to identify patients in whom a soft occlusal appliance should be used to treat masticatory muscle pain by analyzing the type of muscle pain present and patient factors that influenced the effectiveness of this treatment. Methods: The study included 42 patients diagnosed with local myalgia or myofascial pain according to the Diagnostic Criteria for Temporomandibular Disorders Diagnostic Decision Tree. The analysis of patient factors included variables believed to be associated with temporomandibular disorders. First, a temporary screening appliance was used for 2 weeks to assess each patient for bruxism during sleep. Soft appliance therapy was then started. For each patient, the effectiveness of the appliance was evaluated according to the intensity of tenderness during muscle palpation and the treatment satisfaction score at one month after starting treatment. Results: Data from 37 of the 42 patients were available for analysis. Twenty-five patients reported satisfaction with the appliance. In logistic regression analysis, the odds ratio for reduction of facet length was 1.998. Nineteen patients showed at least a 30% improvement in the visual analog scale score. The odds ratio for local myalgia was 18.148. Conclusion: Soft appliance therapy may be used in patients with local myalgia. Moreover, patients who develop short facets on the appliance surface are likely to be satisfied with soft appliance therapy. Soft appliance therapy may be appropriate for patients with local myalgia who develop short facets on their occlusal appliance.
